Smartphone Applications for Remote Monitoring of Patients After Transsphenoidal Pituitary Surgery: A Narrative Review of Emerging Technologies.

OBJECTIVE: Almost all postoperative assessments for pituitary patients are performed in clinical settings under the supervision of medical providers. With the emergence of telemedicine, however, there are opportunities to monitor these patients remotely. The potential for use of such technologies is inconsistently described in the brain tumor literature, especially for patients with pituitary adenomas. METHODS: In this comprehensive narrative review, we present the literature for the use of mobile applications (apps) for monitoring of postoperative symptomatology that is specific to patients

Less loop diuretic use in patients on sacubitril/valsartan undergoing remote pulmonary artery pressure monitoring.

AIMS: Control of pulmonary pressures monitored remotely reduced heart failure hospitalizations mainly by lowering filling pressures through the use of loop diuretics. Sacubitril/valsartan improves heart failure outcomes and increases the kidney sensitivity for diuretics. We explored whether sacubitril/valsartan is associated with less utilization of loop diuretics in patients guided with haemodynamic monitoring in the CardioMEMS European Monitoring Study for Heart Failure (MEMS-HF). METHODS AND RESULTS: The MEMS-HF population (n = 239) was separated by the use of sacubitril/valsartan (n = 68)

Time until diagnosis of clinical events with different remote monitoring systems in implantable cardioverter-defibrillator patients.

BACKGROUND: Remote monitoring (RM) is an established technology integrated into routine follow-up of patients with an implantable cardioverter-defibrillator (ICD). Current RM systems differ according to transmission frequency and alert definition. OBJECTIVE: The purpose of this study was to compare the time difference between detection and acknowledgment of clinically relevant events between 4 RM systems. METHODS: We analyzed time delay between detection of ventricular arrhythmic and technical events by the ICD and acknowledgment by hospital staff in 1802 consecutive patients followed with R

Improved survival in patients enrolled promptly into remote monitoring following cardiac implantable electronic device implantation.

PURPOSE: Guidelines advocate remote monitoring (RM) in patients with a cardiac implantable electronic device (CIED). However, it is not known when RM should be initiated. We hypothesized that prompt initiation of RM (within 91 days of implant) is associated with improved survival compared to delayed initiation. METHODS: This retrospective, national, observational cohort study evaluated patients receiving new implants of market-released St. Jude Medical™ pacemakers (PM), implantable cardioverter defibrillators (ICD), and cardiac resynchronization therapy (CRT) devices. Patients were assigned t

Use of Remote Monitoring Is Associated With Lower Risk of Adverse Outcomes Among Patients With Implanted Cardiac Defibrillators.

BACKGROUND: We examined the association between the use of remote patient monitoring (RPM) of implantable cardioverter defibrillators (ICD) and all-cause mortality and rehospitalization among patients undergoing initial ICD implant. METHODS AND RESULTS: A limited data set was constructed from Boston Scientific ALTITUDE Registry and National Cardiovascular Data Registry ICD Registry between January 2006 and March 2010. Vital status was determined using the Social Security Death Master File. All-cause mortality up to 3 years was compared in patients who used RPM with those who did not use RPM.

eHealth Literacy and Participation in Remote Blood Pressure Monitoring Among Patients With Hypertension: Cross-Sectional Study.

BACKGROUND: The ability to participate in digital health services such as remote blood pressure monitoring needs digital skills and knowledge known as eHealth literacy (e-HL). However, e-HL is rarely studied among those participating in remote blood pressure monitoring (RBPM). OBJECTIVE: This study assessed e-HL levels among participants with hypertension and determined the e-HL domains that predict participation in RBPM. This study provides important focus areas to increase RBPM participation. METHODS: This study was a quantitative, cross-sectional survey of people with hypertension in the

Remote Continuous Vital Sign Monitoring of Scoliosis Surgery Patients on General Wards: A Cost-Effectiveness Analysis.

BACKGROUND: Scoliosis surgery patients often require continuous postoperative monitoring in intensive care (ICU) or high-dependency units (HDU). We implemented a 24-hour remote continuous monitoring service for high-risk inpatients (HIVE) to allow monitoring in general wards. This study aimed to evaluate the cost-effectiveness of the HIVE service. METHODS: We compared scoliosis surgery patients admitted pre- and postimplementation of the HIVE service and applied multivariable regression to adjust for differences in baseline characteristics. The primary outcome was incremental cost per ICU hou

Safety and utility of CardioMEMS device for remote pulmonary artery monitoring in paediatric Fontan patients: a case series.

BACKGROUND: Patients with single ventricle congenital heart disease who undergo total cavo-pulmonary anastomosis (Fontan surgery) suffer from elevated pulmonary artery pressure (PAP), which leads to multiple adverse sequelae. Traditionally, the Fontan pressures are assessed via invasive haemodynamic catheterization that exposes these medically fragile patients to the risks of vascular injury and anaesthesia. While the CardioMEM remote PAP monitor has been extensively used in adults with heart failure, the safety of this device has not been established in children. CASE SUMMARY: We report safe

Long term follow-up of patients with ventricular high rate events detected on remote monitoring of pacemakers.

AIMS: Previous data suggest ventricular high rate episodes (VHREs) on pacemakers are frequent and not associated with overall mortality on short term follow up. We sought to determine whether VHREs are associated with mortality, device upgrade, or change in ejection fraction on long term follow up. METHODS: A single center, retrospective study was performed on 542 patients with permanent pacemakers followed between 2011 and 2013. Follow-up was extended to 2017 for determination of long term outcomes. "True" VHREs were defined as episodes adjudicated to be due to non-sustained ventricular tach

The MOnitoring Resynchronization dEvices and CARdiac patiEnts (MORE-CARE) randomized controlled trial: phase 1 results on dynamics of early intervention with remote monitoring.

BACKGROUND: Remote monitoring (RM) in patients with advanced heart failure and cardiac resynchronization therapy defibrillators (CRT-D) may reduce delays in clinical decisions by transmitting automatic alerts. However, this strategy has never been tested specifically in this patient population, with alerts for lung fluid overload, and in a European setting. OBJECTIVE: The main objective of Phase 1 (presented here) is to evaluate if RM strategy is able to reduce time from device-detected events to clinical decisions. METHODS: In this multicenter randomized controlled trial, patients with mode

A Remote Patient Monitoring System With Feedback Mechanisms Using a Smartwatch: Concept, Implementation, and Evaluation Based on the activeDCM Randomized Controlled Trial.

BACKGROUND: Technological advances allow for recording and sharing health-related data in a patient-centric way using smartphones and wearables. Secure sharing of such patient-generated data with physicians would enable close management of individual health trajectories, monitoring of risk factors, and asynchronous feedback. However, most remote patient monitoring (RPM) systems currently available are not fully integrated into hospital IT systems or lack a patient-centric design. OBJECTIVE: The objective of this study was to conceptualize and implement a user-friendly, reusable, interoperable
