[Replantation of amputated extremities (author's transl)].

After some short remarks about the organization of microsurgery in Australia, the microsurgical technique of retransplantation of dissected extremities and the conservation methods are described in detail. In 175 out of 208 cases, retransplantation of extremities was successful, i.e. certain functions were reestablished. Results were better in children than in adults. The most important factors in successful treatment are the conservation methods, skilled technique, and postoperative care, which may last up to 1 year. One retransplantaion center with 4 experienced surgeons per 5,000,000 popula

Systematic Review of Replant Salvage and Cost Utility Analysis of Inpatient Monitoring After Digit Replantation.

PURPOSE: Digit replantation is a high-stakes procedure that has been shown to be cost-effective, especially for multiple-digit replantation. However, it is associated with prolonged lengths of stay (LOS) for monitoring and attempts at salvage. The cost-effectiveness of prolonged inpatient stays presumes that this is necessary and inherent to the replantation. We hypothesized that prolonged monitoring of replanted digits, in the hope of possible salvage after primary failure, is cost-ineffective due to the low rates of vascular compromise and salvage after replantation. METHODS: Using previous

Replantation of digits and hands: analysis of surgical techniques and functional results in 71 patients with 86 replantations.

Defining replantation as the restoration of a completely amputated part as opposed to simply restoring circulation to an incompletely severed part, the results of replantation of 86 completely amputated parts in 71 patients performed from January, 1970, to December, 1975, were studied. Twenty-eight, or 32.5 percent, were the result of sharp severances of the part; localized crushing accounted for 56, or 65.1 percent. Two were classified as degloving injuries. Twelve amputations were transmetacarpal, six were at the metacarpophalangeal joints, 14 through the proximal phalanx, 15 at the proximal

Operative timing and surgical complexity in hand trauma: a multicenter analysis of replantation and non-replantation centers in Germany.

BACKGROUND: Hand injuries are common traumatic conditions that often require specialized surgical care. Differences in hospital structures and levels of specialization may influence the timeliness and complexity of treatment. This study evaluates quality of care across different hospital types, focusing on time-to-skin-incision and surgical management of finger and hand injuries. MATERIALS AND METHODS: This retrospective multicenter study is based on data from the HandTraumaRegister of the German Society for Hand Surgery (HTR DGH). A total of 16,726 surgically treated cases with documented fi

Psychiatric Morbidity After Digital Replantation Versus Amputation.

BACKGROUND: The purpose of this study was to compare the psychological outcomes following digital amputations, successful digital replantation surgeries, and failed digital replantation surgeries. METHODS: The TriNetX database was queried to identify 19,238 patients who had undergone primary digital amputations, 383 patients who had successful digital replantation surgeries, and 253 patients with failed digital replantation surgery that required revision amputation. We then queried the database for new psychiatric diagnoses and for new psychotropic medication prescriptions within 3 years afte

Major limb replantation: Current status and perspectives.

BACKGROUND: Major limb replantation is a demanding microsurgical procedure aiming to restore form and function after traumatic amputation. Although survival has improved since the 1960s, ischemia, infection, and complex reconstruction continue to limit outcomes. Emerging preservation and reconstructive strategies may help expand indications and improve results. METHODS: For this scoping review, MEDLINE, Embase, and the Cochrane Library were searched from inception through December 2024 using predefined MeSH terms related to major limb replantation. Adult case series and reports were included;

A comparison of ear amputations replantation techniques.

BACKGROUND: The management of traumatic ear amputations remains a rare and difficult occurrence for surgeons. This is due to the fact that the chosen replantation technique must ensure the best vascular supply and the surrounding tissues have to be preserved as to not jeopardize a future auricular reconstruction in the event of replantation failure. OBJECTIVE: This study aimed to review and synthesize the literature about the different surgical techniques described to date in the management of traumatic ear amputations (partial or total). MATERIALS AND METHODS: Relevant articles were searche

Functional outcome after digit replantation versus amputation.

BACKGROUND: The success of digit replantation is mainly based on survival rates. The functional outcome as well as the recovery of sensibility are essential parameters for judging the outcome after digit replantation but have been poorly assessed in previous studies. PATIENTS AND METHODS: Forty-eight patients with 56 complete traumatic digit amputations occurring between 2008 and 2013 returned for a follow-up examination, the earliest being 6 months postoperatively. Each patient's range of motion, fingertip-to-table distance, fingertip-to-palm distance, grip and pinch strengths, static two-po

Successful Microsurgical Scalp Replantation Utilizing Loupe Magnification.

Total scalp avulsion is a rare injury that poses a unique reconstructive challenge. Microsurgical replantation is considered the first-line choice; yet anastomotic efforts may be strained to find suitable recipient and target vessels. Concomitant injuries may also delay or hinder operative intervention. These complex scenarios are difficult to navigate and necessitate multidisciplinary input to optimize outcomes. As such, the authors present an illustrative case report of a total scalp avulsion injury that underwent successful microsurgical replantation. This report highlights the novel use of

Leech Therapy Following Digital Replantation and Revascularization.

Venous congestion after digital replantation or revascularization threatens digit survival in the immediate postoperative period. External bloodletting, including leech therapy, provides a central role in salvage of the congested finger. Although there have been previous studies describing the initiation of leech therapy for digits experiencing venous insufficiency, few published articles and no consensus guidelines have discussed the weaning of leeches in the postoperative period. We review the current evidence behind leech therapy and offer a treatment algorithm based on available data and e

Lower limb replantation: 27 years follow up.

Replantation of lower extremity is a very complex and difficult procedure. There are still a lot of controversies about indications, even numerous scoring systems are now available that can facilitate the surgeon's decision. We present the functional results of a replanted below-knee amputation in an elderly patient, 27 years after the injury and discuss the indication for replantation.

Successful microsurgical replantation of an amputated penis.

Penile amputation is an uncommon injury for which immediate surgical replantation is warranted. Microsurgical replantation is the "standard" method for penile replantation. Early replantation yields a high success and low complication rate. We report a case of a 34-year-old male who presented with amputation at the proximal penile shaft which was successfully replanted using microsurgical techniques. Minor skin necrosis was noted post-operatively which was debrided and covered with skin graft. Follow-up at 6 months showed satisfactory cosmetic appearance, normal voiding, return of sensations a

Venous Bridge Arterial Grafting for Thumb Replantation.

BACKGROUND: The purpose of this study is to describe a novel technique using an interposition vein graft for thumb replantation in these severe avulsion injuries. METHODS: From 2002 to 2012, 8 patients underwent interposition venous bridge grafting from the dorsal radial artery at the anatomic snuffbox to the ulnar digital artery of the thumb. All patients had a traumatic thumb amputation with a severe injury to the ulnar digital artery. The technique began with bony stabilization; the radial artery at the anatomic snuffbox is exposed, vein graft harvested, and microsurgical end-to-side (prox

Retention and Healing Outcomes after Intentional Replantation.

INTRODUCTION: Intentional replantation is an alternative to tooth extraction and prosthetic replacement when conventional endodontic treatment modalities are unfeasible or contraindicated. This study assessed tooth retention and healing after intentional replantation and explored predictors of these outcomes. METHODS: Data of intentional replantation procedures performed between March 2000 and December 2010 were collected prospectively, excluding teeth with preoperative periodontal and root defects. A cohort of 159 teeth was followed up for 0.5-12 years. Retention and healed status without co

Intentional extraction and replantation: the last resort.

The importance of the first molar in the development of occlusion is well-known. The difficulties when first molar is not there during the eruption of the second molar and the premolar don't have to be explained much. The construction of a space maintainer in the area of missing first molar is difficult when seven and five are not erupted. The article highlights a case where a grossly decayed first molar with repeated endodontic failures was treated by intentional extraction and replantation as a last resort. A follow-up along with indications and considerations are documented.

Cross-over limb replantation: a case report.

Following traumatic bilateral arm amputation in a 46-year-old woman, orthotopic replantation was not possible. The patient underwent replantation of the left forearm to the right proximal forearm. At two years she has a sensate hand, with good recovery of her long flexors of the hand and wrist extensors. DASH score is 93.9 and Chen's score is III (medium). We demonstrate a high level of function compared to the contralateral prosthetic limb. There have only been four similar cases described. Although a rare circumstance, a cross-limb replant can provide a level of function and independence sup

Intentional replantation of periodontally compromised hopeless tooth.

Aesthetic considerations have influenced the management of dental maladies in varying degrees for many years. Even single tooth mal-alignment makes the patient to approach a dentist. Intentional replantation is a procedure in which an intentional tooth extraction is performed followed by reinsertion of the extracted tooth. Many authors agree that it should be reserved as the last resort to save a tooth after other procedures have failed or would likely to fail. The main reason of failure in replanted teeth is root resorption, specifically ankylosis or replacement resorption. Although the succe

Penile replantation, complication management, and technique refinement.

We describe a case of complete guillotine-type penile amputation at the proximal penile shaft. The blood flow was established 10 h after trauma. Circulation in the replanted penis was quite good but there was progressive prepuce necrosis after the hematoma. Cosmetic and urinary outcome was good 6 weeks later. The repair of deep dorsal penile vessels helps in corpus tissue healing and glans circulation. The blood supply from the corpus tissue is sufficient for the survival of the replanted penis even when the repaired dorsal vessels were occluded. Surgical pitfalls in replantation procedures an

Microsurgical replantation of total upper eyelid avulsion.

We present a unique case in which a dog bite resulted in total full-thickness upper eyelid avulsion in a young Caucasian female. The eyelid was replanted using meticulous microsurgical techniques reanastamosing the arterial supply. To our knowledge, this is the first reported case of a successful microvascular replantation of a completely avulsed upper eyelid. The details of the case and the photographic clinical improvement are described and discussed.

[Long-term follow-up after leg replantation].

Leg replantation is generally considered to yield good results, particularly since the widespread use of progressive lengthening procedures. This optimism must be weighted against the very long and difficult operative program, the social and occupational implications, and the functional outcome of the replanted limb. We report a case of below knee leg replantation in a young patient whose operative program lasted 29 months. Occupational activities were interrupted for three years. The clinical result was satisfactory but with 14 degrees residual valgus suggesting a potential risk of osteoarthr
