Ventilation-perfusion effects of negative-pressure ventilation: insights from an experimental rat model.

BACKGROUND: Mechanical ventilation typically utilizes positive-pressure ventilation (PPV), which fundamentally differs from physiological pressure conditions. In contrast, negative-pressure ventilation (NPV) more closely mimics physiological pressure conditions; however, its impact on ventilation-perfusion matching remains unclear. Therefore, we compared PPV and NPV in terms of their effects on ventilation-perfusion matching and determined the consequences of increasing end-expiratory pressure (EEP). METHODS: Anesthetized rats (n = 9) were ventilated using PPV at a positive EEP of 0, 3, 6, an

The role of high flow oxygen therapy in acute respiratory failure.

Acute respiratory failure represents one of the most common causes of intensive care unit admission and oxygen therapy remains the first-line therapy in the management of these patients. In recent years, high-flow oxygen via nasal cannula has been described as a useful alternative to conventional oxygen therapy in patients with acute respiratory failure. High-flow oxygen via nasal cannula rapidly alleviates symptoms of acute respiratory failure and improves oxygenation by several mechanisms, including dead space washout, reduction in oxygen dilution and inspiratory nasopharyngeal resistance, a

Noninvasive Options.

Noninvasive ventilation (NIV) has assumed a central role in the treatment of selected patients with acute respiratory failure due to exacerbated chronic obstructive pulmonary disease or acute cardiogenic pulmonary edema. Recent advances in the understanding of physiologic aspects of NIV application through different interfaces and ventilator settings have led to improved patient-machine interaction, enhancing favorable NIV outcome. In recent years, the growing role of NIV in the acute care setting has led to the development of technical innovations to overcome the problems related to gas leaka

The effect of hypo-, normo-, and hypercapnia induced by mechanical ventilation on intrapulmonary shunt.

The effects of hypo-, normo- and hypercapnia on the variations in arterial oxygenation and their indices in critical patients with acute respiratory failure (ARF) receiving mechanical ventilation are studied. It is a prospective and randomized study carried out in multidisciplinary ICU. Fifteen ARF patients, intubated and mechanically ventilated, were studied within the first 48 h of evolution. Three stages were delimited: I) 30 min after the beginning of anaesthesia; II) 30 min after adding 30 cm of dead space (VD); III) 30 min after replacing the previous VD with VD of 60 cm. Ventilation par

[Awake prone positioning in critically ill patients: mechanisms, evidence and future directions].

Awake prone positioning (APP), as an essential therapeutic strategy for acute hypoxemic respiratory failure, has demonstrated significant clinical value, especially during the coronavirus disease 2019 (COVID-19) pandemic. This article systematically summarizes three physiological mechanisms underlying the oxygenation improvement associated with APP, namely enhanced ventilation in gravity-dependent regions, reduction of dead space, and optimization of the ventilation-perfusion ratio. Furthermore, we comprehensively analyze clinical evidence regarding the role of APP in reducing intubation rates

The effect of nasal patency and anatomy during asymmetrical nasal high flow.

Nasal high flow (NHF) with the asymmetrical cannula interface (AI) is increasingly used as respiratory support across a broad range of patients. By incorporating one unilaterally oversized prong, the AI enhances dead-space clearance and increases airway pressure. This study investigated the effects of nasal patency and anatomical variability of the upper airways during NHF delivered with the AI. Twenty healthy adults underwent three 3-h study periods, whereby heated, humidified air was delivered at 30 L/min via either an AI, a symmetrical interface (SI), or a control condition without NHF. Determinants of Effect of Extracorporeal CO2 Removal in Hypoxemic Respiratory Failure.

BACKGROUND: Dead space and respiratory system elastance (Ers) may influence the clinical benefit of a ventilation strategy combining very low tidal volume (VT) with extracorporeal carbon dioxide removal (ECCO2R) in patients with acute hypoxemic respiratory failure. We sought to evaluate whether the effect of ECCO2R on mortality varies according to ventilatory ratio (VR; a composite variable reflective of dead space and shunt) and Ers. METHODS: Secondary analysis of a trial of a strategy combining very low VT and low-flow ECCO2R planned before the availability of trial results. Bayesian logisti

Physiologic effects of alveolar recruitment and inspiratory pauses during moderately-high-frequency ventilation delivered by a conventional ventilator in a severe lung injury model.

BACKGROUND AND AIMS: To investigate whether performing alveolar recruitment or adding inspiratory pauses could promote physiologic benefits (VT) during moderately-high-frequency positive pressure ventilation (MHFPPV) delivered by a conventional ventilator in a porcine model of severe acute respiratory distress syndrome (ARDS). METHODS: Prospective experimental laboratory study with eight pigs. Induction of acute lung injury with sequential pulmonary lavages and injurious ventilation was initially performed. Then, animals were ventilated on a conventional mechanical ventilator with a respirato

Carbon Dioxide Inhalation-Risks for Health or Opportunity for Physical Fitness Development?

Background: Carbon dioxide (CO2) is traditionally regarded as a metabolic by-product; however, growing evidence indicates that it plays an active regulatory role across multiple physiological systems. Acute hypercapnia elicits respiratory, cardiovascular, metabolic, immune, and neurocognitive responses, some of which may transiently influence exercise performance. This narrative review summarizes current evidence on CO2 inhalation in healthy individuals and critically evaluates whether controlled hypercapnia may serve as a targeted stimulus in sport and exercise contexts. Methods: A narrative

Versatility of High-Velocity Nasal Insufflation (HVNI) in respiratory weaning: A case series demonstrating three distinct clinical objectives.

High-Velocity Nasal Insufflation (HVNI) is an advanced form of high-flow nasal therapy that delivers heated, humidified oxygen at high velocities through a small-bore nasal cannula, generating efficient extrathoracic dead space washout, mild positive end-expiratory pressure (PEEP), and enhanced patient comfort. While Non-Invasive Positive Pressure Ventilation (NiPPV) remains the reference treatment for acute hypercapnic respiratory failure, its use is frequently limited by patient intolerance. This case series presents three critically ill patients in whom HVNI was deployed during the respirat

The pathophysiology of 'happy' hypoxemia in COVID-19.

The novel coronavirus disease 2019 (COVID-19) pandemic is a global crisis, challenging healthcare systems worldwide. Many patients present with a remarkable disconnect in rest between profound hypoxemia yet without proportional signs of respiratory distress (i.e. happy hypoxemia) and rapid deterioration can occur. This particular clinical presentation in COVID-19 patients contrasts with the experience of physicians usually treating critically ill patients in respiratory failure and ensuring timely referral to the intensive care unit can, therefore, be challenging. A thorough understanding of t

Flow and volume dependence of respiratory system mechanics during constant flow ventilation in normal subjects and in adult respiratory distress syndrome.

Seven control subjects and seven patients with adult respiratory distress syndrome (ARDS) were artificially ventilated and flow, volume, and tracheal pressure were monitored. Respiratory system resistance (Rrs,max) was partitioned into its homogeneous (Rrs,min) and uneven (Rrs,u) components. Respiratory system elastance (Ers) was also measured. In both groups Ers did not vary with different inspiratory flows and volumes, but was significantly higher in ARDS. With increasing volume (isoflow maneuvers), Rrs,max and Rrs,u increased but Rrs,min remained unaltered in ARDS. In control patients, howe

The influence of body position on pulmonary function in low birthweight babies.

The influence of the prone and supine body positions on the respiratory rate, arterial pH, pO2, pCO2 and serum bicarbonate level; on alveolar pO2 and pCO2; and on alveolar/arterial gradients for carbon dioxide and oxygen, were compared in 10 low birthweight infants. THE ALveolar pCO2 was higher in the prone, while pO2 was higher in the supine position, possibly as a result of ventilation/perfusion imbalance with increased physiological dead space in the supine position. There were no significant differences between the other parameters studied. Since aspiration of feed by the low birthweight i

Altitude-induced periodic breathing optimises respiratory efficiency during sleep in young healthy males.

OBJECTIVE: To assess the impact of acute hypoxia-induced periodic breathing on respiratory efficiency in young healthy males. METHODS: 20 healthy male (median (IQR) age: 24 (22 to 25) years; body mass index: 22.5 (20.9 to 24.9) kg/m2)-no sleep-disordered breathing in normoxia-underwent polysomnography in normobaric hypoxia simulating 3500 m altitude. Measurements included oesophageal pressure-time curve, airflow and exhaled fraction of carbon dioxide (CO2). Inspiratory pressure-time product (iPTP, index of respiratory muscle effort) was calculated from oesophageal pressure. Physiological dead

[Facial or nasal mask pressure support ventilation in managing acute exacerbation of chronic respiratory failure in chronic obstructive pulmonary diseases].

11 COPD patients (age: 65 +/- 9 Yrs) with acute exacerbation of chronic respiratory failure (PaCO2 11.3 +/- 1.1kPa) were treated with mask pressure support ventilation, another 10 similar patients (age: 68 +/- 12 Yrs) served as control. BiPAP ventilator was used with the following modifications: (1) Non-rebreathing valve set-in proximal to mask; (2) 5 LPM oxygen flow delivered into mask to reduce the dead space effect. Mask ventilation was given 2-3 hours every time and 1-2 time daily for 7 days. Synchrony and airway patency were specially monitored. The results suggested that mask ventilation

[The broncholytic effect of carboanhydrase inhibitors in subjects with healthy lungs (author's transl)].

Carboanhydrase inhibitors are said to have a positive effect on respiratory insufficiency. We examined this, especially regarding the bronchial system. Actions and reactions of the human bronchial system can be followed by measurement of the anatomical dead space breath by breath. We have developed a new method which was tested by classical broncho-effective substances. Our experiments show in case of two men with healthy lungs that the three carboanhydrase inhibitors acetazolamide, methazolamide, and dichlorphenamide have a pronounced broncholytic effect. The effect is greater than that of fe

Postural and respiratory functions of the pelvic floor muscles.

AIMS: Due to their contribution to modulation of intra-abdominal pressure (IAP) and stiffness of the sacroiliac joints, the pelvic floor muscles (PFM) have been argued to provide a contribution to control of the lumbar spine and pelvis. Furthermore, as IAP is modulated during respiration this is likely to be accompanied by changes in PFM activity. METHODS: In order to evaluate the postural and respiratory function of the PFM, recordings of anal and vaginal electromyographic activity (EMG) were made with surface electrodes during single and repetitive arm movements that challenge the stability

Mean airway pressure and response to inhaled nitric oxide in neonatal and pediatric patients.

Inhaled nitric oxide (iNO) can improve oxygenation and ventilation-perfusion (V/Q) matching by reduction of shunt (Qs/Qt) in patients with hypoxemic lung disease. Because the improvement in V/Q matching must occur by redistribution of pulmonary blood flow, and because high airway pressure (Paw) increases physiologic dead space (Vd/Vt), we hypothesized that high Paw may limit the improvement in V/Q matching during iNO treatment. iNO 0-50 ppm was administered during mechanical ventilation. Mechanical ventilator settings were at the discretion of the attending physician. Qs/Qt and Vd/Vt were deri
