Respiratory failure is the most common cause of death in children with neuromuscular disease (NMD). This article provides a conceptual framework for understanding failure of the respiratory pump in these children. Pump failure can be attributed to faulty control of respiration, to mechanical defects, and to respiratory muscle fatigue. In most patients with NMD, respiratory drive is intact. Mechanical defects can be due to either an abnormal pump or an excessive respiratory load on which the pump must operate. In patients with NMD, the weak respiratory muscles cannot generate normal respiratory

Multicore myopathy is a rare congenital myopathy that can cause progressive weakness, but it has not been recognized previously to have respiratory consequences. This study describes two patients who developed respiratory failure because of respiratory muscle weakness. Both patients had low vital capacities without evidence of airway obstruction, and CO2 retention. Physical examination found that neck accessory muscles and abdominal muscles were very weak. In inspiration the abdomen expanded, but the rib cage contracted. Detailed studies were done in one patient with magnetometers, esophageal

Simple lung function tests have been used to evaluate respiratory symptoms in human immunodeficiency virus (HIV) infected individuals. Abnormalities of simple lung function tests, in particular decreases in the transfer factor of the lung for carbon monoxide (TL,CO) have been described in patients with acquired immune deficiency syndrome (AIDS) who have respiratory disease. Early studies showed marked reductions in TL,CO in patients with pneumocystis pneumonia but other forms of pulmonary infection or neoplasm also resulted in reductions in TL,CO values. Lung function studies in larger numbers

UNLABELLED: Ankylosing Spondylitis (AS) presents with both musculoskeletal and cardiorespiratory pathophysiological manifestations. Inspiratory muscle training (IMT) may be a useful intervention to address deficits in respiratory and functional status.
CASE DESCRIPTION: A 25-year-old male with AS initially sought treatment for low back and right hip pain, but 7 weeks of IMT was also provided due to abnormal respiratory performance.
OUTCOMES: At baseline, the patient presented with a resting respiratory rate (RR) of 14.5 breaths/minute, tidal volume (TV) of 0.76 L, minute ventilation (VE) of

Physical efficiency (PhE), maximal calorific capacity (MCC) and Pneumocalorimetric index which conditions the function of the respiratory apparatus can be described by the following equation: PhE = 59.9 . MCC + 33. An error of PhE calculated with the above mentioned technique doesn't exceed 12,5%. The use of pneumocalorimetric enables to assess the functional state of the cardiorespiratory system by determining PhE, labour-intensiveness of the method is 5.1 minutes per a test.

The function of respiratory muscles, and mainly inspiratory muscles, is impaired in COPD patients. Most of these impairments are essentially due to pulmonary hyperinflation that puts these muscles in a disadvantageous situation. The main consequence of this dysfunction is respiratory muscle fatigue that may cause shortness of breath, exertion intolerance, and hypoventilation with onset of hypercapnic respiratory failure. This function may be measured at the pulmonary function laboratory by means of unspecific (spirometry, pulmonary volumes) or specific tests (maxim respiratory pressures [MIP -

Asthma is a multifactorial disease with heterogeneous clinical and pathophysiological phenotypes. Classical symptoms of asthma are wheeze, cough and breathlessness, and occasionally chest tightness. These symptoms are common to many respiratory and non-respiratory conditions, and misdiagnosis of asthma is frequent. There is no definitive test for the diagnosis of asthma, but a combination of a suggestive medical history, physical examination, lung function tests, and additional investigations will maximize the chances of making a correct diagnosis. Despite clear statements in international ast

BACKGROUND: Wildfire smoke contains numerous hazardous air pollutants which pose serious health risks to humans. Despite this, there has been a limited focus on the assessment of the acute physiological and longer-term respiratory effects of wildfire exposure on firefighters and other emergency workers. Therefore, we undertook a systematic review of the evidence about the respiratory impacts of occupational wildfire smoke exposure among wildfire fighters (WFF).
METHODS: Eligible studies from Medline, Embase and Scopus databases were included if they described the relationship between wildfire

BACKGROUND: Pulmonary function tests (PFT) are a frequent component of heart transplant evaluation. In cardiac surgery abnormal PFTs, especially reduced DLCO, have been associated with poor outcomes. We sought to evaluate the impact of pretransplant PFTs on post-transplant pulmonary outcomes and patient survival.
METHODS: Among the 652 adult heart transplant recipients between January 1, 2010 and July 31, 2021, 462 had PFTs and constituted the patient cohort. Obstructive ventilatory defects (OVD), restrictive ventilatory defects (RVD), and reduced DLCO were defined according to established cr

Respiratory muscle endurance training reportedly has beneficial effects on whole-body endurance performance. We produced a novel high-intensity interval (HII) protocol and characterized the associated physiological responses and respiratory muscle fatigue. Peak oxygen uptake of respiratory muscle (V̇O2peakRM) was estimated during the respiratory incremental test. The HII session consisted of five 3-minute hyperpnea periods at 100%V̇O2peakRM interspersed with 2-minute periods at 40%V̇O2peakRM (25 min total). The high-intensity continuous (HIC) session involved a single time-to-end bout of hyper

INTRODUCTION: Respiratory failure is a major cause of death in patients with Osteogenesis Imperfecta. Moreover, respiratory symptoms seem to have a dramatic impact on their quality of life. It has long been thought that lung function disorders in OI are mainly due to changes in the thoracic wall, caused by bone deformities. However, recent studies indicate that alterations in the lung itself can also undermine respiratory health.
OBJECTIVES: Is there any intrapulmonary alteration in Osteogenesis Imperfecta that can explain decreased pulmonary function? The aim of this systematic literature re

BACKGROUND: Perinatal and infantile hypophosphatasia (HPP) are associated with respiratory failure and respiratory complications. Effective management of such complications is of key clinical importance. In some infants with HPP, severe tracheobronchomalacia (TBM) contributes to respiratory difficulties. The objective of this study is to characterize the clinical features, investigations and management in these patients.
METHODS: We report a case series of five infants with perinatal HPP, with confirmed TBM, who were treated with asfotase alfa and observed for 3-7 years. Additionally, we revi

The purpose of this review is to provide practitioners and clinical investigators with an update on methods of assessing respiratory function in young children. The importance of this topic is presented in light of the natural history of asthma, as well as maturational changes that occur early in life in terms of airway development. Models of disease are cited to support the concept that injury of the mammalian airway early in postnatal life might have far-reaching consequences in terms of control of airway caliber and responsiveness. The methods currently available to measure respiratory func

When breathing ambient air at rest at 3,658 m altitude, Tibetan lifelong residents of 3,658 m ventilate as much as newcomers acclimatized to high altitude; they also ventilate more and have greater hypoxic ventilatory responses (HVRs) than do Han ("Chinese") long-term residents at 3,658 m. This suggests that Tibetan ancestry is advantageous in protecting resting ventilation levels during years of hypoxic exposure and is of interest in light of the permissive role of hypoventilation in the development of chronic mountain sickness, which is nearly absent among Tibetans. The existence of individu

Respiratory sensation was studied in European low-landers at an altitude of 4382 m after a helicopter flight in order to investigate the acute and prolonged effects of high altitude hypoxia. At rest the ability to detect four inspiratory resistive loads can be used to create a sensitivity index P(A) without taking the response bias (B) into account, based on the Sensory Decision Theory. In 6 subjects respiratory sensitivity increased significantly under acute and prolonged hypoxia after 3-4 days of high altitude exposure. The respiratory sensitivity increased with the hyperventilation and mout

Techniques and methods required for accurate and reproducible measurements of transdiaphragmatic pressure (Pdi), were studied in seven-week and five-month-old calves. Since reliable pleural pressure measurement in cattle has already been described, separate experiments were conducted to compare retrodiaphragmatic pressures measured (i) with balloon-catheters and needle catheters, (ii) before and after eructation and (iii) in the ruminal and in the peritoneal cavities. Whereas respiratory-induced pressure changes were equally transmitted throughout the rumen content, a marked anteroposterior da
