Randomized trial of salbutamol in acute bronchiolitis.

To test whether nebulized salbutamol (albuterol) is safe and efficacious for the treatment of young children with acute bronchiolitis, we enrolled 83 children (median age 6 months, range 1 to 21 months) in a randomized, double-blind clinical trial. Participants received two treatments at 30-minute intervals of either nebulized salbutamol (0.10 mg/kg in 2 ml 0.9% saline solution) or a similar volume of 0.9% saline solution placebo. Outcome measures were the respiratory rate, pulse oximetry, and a clinical score based on the degree of wheezing and retractions. Patients in the salbutamol arm had

Refractory congestive heart failure after ribavirin in infants with heart disease and respiratory syncytial virus.

Although treatment with ribavirin has been known to be associated with a decreased mortality in infants with congenital heart disease (CHD) who have respiratory syncytial virus (RSV), few data are available regarding morbidity. We reviewed records of 10 consecutively hospitalized infants with CHD during a recent RSV epidemic. Despite the presence of left-to-right shunt in each patient, symptoms of RSV were respiratory at presentation. After ribavirin, decreased respiratory symptoms were found in 8 infants but in 2 assisted ventilation were required 1 and 3 days after admission. Congestive hear

Economic and long-term benefits of ribavirin therapy on respiratory syncytial virus infection.

Ribavirin is a broad-spectrum antiviral agent. Administered as an aerosol, it has been shown to be clinically effective in improving the signs and symptoms of viral bronchiolitis in infancy, particularly cases due to respiratory syncytial virus (RSV). This paper reviews the evidence for economic and/or long-term clinical benefits from using ribavirin in the acute illness. There are data to suggest that use of ribavirin may lead to a reduction in therapeutic interventions and duration of hospital stay, with associated savings in hospital costs. Ribavirin reduces the production of RSV-specific I

Parainfluenza virus type 2 haemagglutinin-neuraminidase glycoprotein characterized with monoclonal antibodies.

Thirteen monoclonal antibodies (MAbs) were prepared against human parainfluenza virus type 2 (PIV2). These MAbs reacted with the haemagglutinin-neuraminidase glycoprotein with an Mr of 84K. The MAbs defined one antigenic site which could be divided into five epitopes. A correlation between haemagglutination inhibition (HI) and neutralization activity could be seen although one MAb, which recognized a distinct epitope, showed neutralization and no HI activity to PIV2. The reactivity of the MAbs was tested against Sendai virus, parainfluenza virus type 3, simian virus 5 (SV5), mumps virus, Newca

Genetic characteristics of human parainfluenza viruses 1-4 associated with acute lower respiratory tract infection in Chinese children, during 2015-2021.

Human parainfluenza viruses (HPIVs) are a significant cause of acute lower respiratory tract infections (ALRTIs) among young children and elderly individuals worldwide. The four types of HPIVs (HPIV1-4) can cause recurrent infections and pose a significant economic burden on health care systems globally. However, owing to the limited availability of complete genome sequences, the genetic evolution of these viruses and the development of vaccines and antiviral treatments are hampered. To address this issue, this study utilized next-generation sequencing to obtain 156 complete genome sequences o

Immunohistochemical evaluation of lymphocyte subpopulations in sheep pneumonia caused by RSV, PIV3, and their coinfections.

Respiratory syncytial virus (RSV) and parainfluenza virus type 3 (PIV‑3) are important pathogens causing respiratory infections in humans and animals worldwide. This study aimed to investigate the presence of RSV and PIV‑3 in lambs with pneumonia, including single and mixed infections, and to evaluate the role of different lymphocyte types in immunopathogenesis. The material consisted of lung samples from 100 lambs (aged 4-12 months) with pneumonia, collected between July and November 2024. PCR and immunohistochemistry (IHC) were used to detect viral presence, while hematoxylin-eosin (HE) stai

[Prokaryotic expression of the nucleocapsid protein of caprine parain-fluenza virus type 3 and establishment of a double-antigen sandwich ELISA method].

This study aims to establish a double-antigen sandwich ELISA method for detecting serum antibodies against caprine parainfluenza virus type 3(CPIV3), thereby supporting the clinical diagnosis of CPIV3 infection. According to the reference nucleocapsid (N) gene sequence (NCBI accession number: MT756864.1) published in NCBI, the N gene was synthesized and inserted into pET-30a to construct the recombinant plasmid pET-30a-N. The N protein was expressed in Escherichia coli and purified by Ni2+-affinity chromatography via its His tag. Reactivity of the purified N protein was verified by indirect EL

Caprine parainfluenza virus type 3 N protein promotes viral replication via inducing apoptosis.

Caprine parainfluenza virus type 3 (CPIV3) is one of the most important viral respiratory pathogens of goat. Accumulating evidence demonstrates that apoptosis is a cellular mechanism for the host response to pathogens, and it participates in regulating viral replication. However, there is little study on CPIV3-induced host cells apoptosis. In this study, primary goat tracheal epithelial (GTE) cells were established as a cellular model that is permissive to CPIV3 infection. Then, we showed that CPIV3 infection induced apoptosis in GTE cells, as determined by morphological changes, flow cytometr

Phylogenetic and antigenic analysis of bovine parainfluenza virus type 3 isolated in Japan between 2002 and 2019.

Bovine parainfluenza virus type 3 (BPIV3) is one of the most important viral respiratory pathogens of cattle. In addition to the classical BPIV3 genotype A (BPIV3a), new genetic groups, genotype B (BPIV3b) and C (BPIV3c), have been identified and isolated in certain parts of the world. The present study aimed to investigate the genetic and antigenic characteristics of BPIV3 circulating in Japan. Seventy-three BPIV3 field strains were isolated from nasal samples of cattle between 2002 and 2019. Phylogenetic analysis of the phosphoprotein and hemagglutinin-neuraminidase genes showed that the iso

Sendai virus as a backbone for vaccines against RSV and other human paramyxoviruses.

Human paramyxoviruses are the etiological agents for life-threatening respiratory virus infections of infants and young children. These viruses, including respiratory syncytial virus (RSV), the human parainfluenza viruses (hPIV1-4) and human metapneumovirus (hMPV), are responsible for millions of serious lower respiratory tract infections each year worldwide. There are currently no standard treatments and no licensed vaccines for any of these pathogens. Here we review research with which Sendai virus, a mouse parainfluenza virus type 1, is being advanced as a Jennerian vaccine for hPIV1 and as

Disturbance in respiratory mechanics in infants with bronchiolitis.

The passive flow-volume and partial forced expiratory flow-volume techniques were used to assess pulmonary function in 14 spontaneously breathing infants with acute respiratory syncytial virus bronchiolitis. Two additional infants were studied while paralysed and ventilated. During the acute stage of the illness there was a significant reduction in forced expiratory flow rates and an increase in respiratory resistance. Although the mean thoracic gas volume for the group was increased, five infants did not compensate for their airways obstruction by hyperinflation. Curvilinear passive flow-volu

Seasonal variation in respiratory syncytial virus infections in children in Benin-City, Nigeria.

The seasonal variation in respiratory syncytial virus (RSV) infections in children was examined in Benin City. Nasopharyngeal washes were obtained from children under 3 years hospitalised for acute lower respiratory infections during two seasons - rainy season (June-August) and dry season (December-February). RSV surface antigen was identified by ELISA. 54% of patients in the rainy season were ELISA positive for RSV compared to 8.8% during the dry season. Clinical features in the RSV infected patients were not different between the two seasons and were similar to that reported from temperate c

Experimental pneumonia in gnotobiotic calves produced by respiratory syncytial virus.

A bovine isolate of respiratory syncytial virus (RSV), when inoculated intranasally into eight gnotobiotic calves produced significant macroscopic lesions of the lung (2-25% consolidation) but failed to produce any clinical signs of disease. The microscopic lesions comprised proliferative and exudative bronchiolitis with accompanying alveolar collapse and infiltration by mononuclear cells of the peribronchiolar tissue and alveolar walls. Virus was recovered from the nasopharynx between days 2 and 11 after infection with peak titres between days 4 and 7. Demonstration of viral antigen by immuno

Management of respiratory failure in infants with acute viral bronchiolitis.

Fifteen infants with acute viral bronchiolitis required mechanical ventilation. Infants were all aged less than 12 weeks and all had evidence of atelectasis or pneumonia on chest x-ray films. Respiratory syncytial virus was identified by immunofluorescence in 14 of 15 patients. Intubation and mechanical ventilation were initiated to reduce the work of breathing rather than to treat hypoxemia alone. Additional therapy included continuous positive airway pressure or positive end-expiratory pressure, sedation, aminophylline, diuretics, fluid restriction, and early feeding.

Bronchiolitis: a clinical review.

Bronchiolitis is an acute viral infection primarily caused by respiratory syncytial virus, affecting children under 2 years of age with a peak prior to six months. The clinical picture results from an inflammatory process of the small bronchi and bronchioles. Infants present with tachypnea and wheezing often accompanied by respiratory distress and hyperinflation. Supportive care is essential, children respond slowly to hydration and careful observation. Respiratory distress requires hospitalization with treatment including oxygen and ventilator support as indicated.
