Spinal deformity after selective dorsal rhizotomy in ambulatory patients with cerebral palsy.

Thirty-four patients with ambulatory spastic diplegia (ages 10-19.8 years) who were part of a prospective study of selective dorsal rhizotomy (SDR) had standardized radiographs before and after SDR. Follow-up ranged from 5 to 11.6 years after surgery. Two different surgical approaches were used: laminectomy (14 patients) and laminoplasty (20 patients). Radiographs were measured for coronal and sagittal balance. Thirty patients had a spinal deformity at long-term follow-up compared with 10 patients before surgery. Seventeen patients (50%) developed lumbar hyperlordosis greater than 60 degrees.

Percutaneous trigeminal nerve radiofrequency rhizotomy guided by computerized tomography fluoroscopy. Technical note.

The authors describe a new procedure for percutaneous trigeminal radiofrequency rhizotomy. Computerized tomography fluoroscopy is used for guidance of the rhizotomy needle insertion through the foramen ovale. Ten patients were treated using this method, and in each case the target was reached with a single puncture. The potential benefits of this method are presented.

Relationship of intraoperative electrophysiological criteria to outcome after selective functional posterior rhizotomy.

At British Columbia's Children's Hospital, the criteria used in selective functional posterior rhizotomy (SFPR) evolved in three distinct phases. In Phase 1 the electrophysiological criteria for abnormality included a low threshold to a single stimulation, a sustained response to 50-Hz stimulation, and spread outside the segmental level being stimulated. In Phase 2 the electrophysiological criteria were unchanged, but fewer L3-4 nerve roots were cut. In Phase 3, fewer L3-4 nerve roots were cut, as in Phase 2, but based on the results of posterior nerve root stimulation in nonspastic controls,

Long-term results after glycerol rhizotomy for multiple sclerosis-related trigeminal neuralgia.

Percutaneous retrogasserian glycerol rhizotomy (PRGR) was used during an 11-year interval in 53 patients with typical trigeminal neuralgia associated with multiple sclerosis. All patients had failed extensive medical trials prior to PRGR. Long-term (median follow-up, 36 months) complete pain relief (no further medication) was achieved in 29 (59%) of 49 evaluable patients. Eight patients (16%) had satisfactory pain control but required occasional medication. Twelve patients (25%) had initial unsatisfactory results with inadequate pain relief; nine underwent alternative surgical procedures. Sixt

Selective sacral rhizotomy in children with high pressure neurogenic bladders: preliminary results.

Recent successful introduction of selective rhizotomy in the management of lower extremity spasticity in patients with myelodysplasia has prompted us to use it as a means of managing high pressure neurogenic bladders occasionally encountered in myelodysplastic patients. During the last 1 1/2 years 8 children have undergone selective sacral rhizotomy in an attempt to avert urinary diversion or bladder augmentation. Patient age ranged from 6.5 to 18.5 years. The level of the respective lesions was evenly distributed throughout the spine. At spinal surgery each patient had an electrode placed in

Human trigeminal root evoked potentials during differential retrogasserian thermal and chemical rhizotomy.

Human trigeminal root evoked potentials have been recorded using signal averaging techniques during radiofrequency trigeminal rhizotomy and lidocaine blocks in patients with trigeminal neuralgia. Both short and long latency trigeminal root potentials have been recorded which appear to represent fast- and slow-conducting fiber activity respectively. Long latency trigeminal root potentials appear in recordings at the noxious threshold as perceived by the awake patient and are relatively selectively abolished in a reversible fashion by lidocaine block and irreversibly by radiofrequency heat. Eval

A prospective gait follow-up study 30 years after selective dorsal rhizotomy.

OBJECTIVE: Selective dorsal rhizotomy (SDR) is a neurosurgical procedure to reduce spasticity in the lower extremities of children with cerebral palsy (CP). The aim of this study was to evaluate gait 30 years after SDR in a prospectively studied cohort to provide clinicians and parents/caregivers with information about the long-term outcomes of this surgery. METHODS: This cohort was assessed preoperatively with 2D gait analysis in 1985 and followed up at 1 and 3 years postoperatively (short-term outcomes) and at 10, 20, and 30 years postoperatively (long-term outcomes). In the current study,

Staged bilateral microsurgical rhizotomy for treatment of bilateral glossopharyngeal neuralgia: illustrative case.

BACKGROUND: Glossopharyngeal neuralgia (GPN) is a rare disorder characterized by severe, episodic pain along the sensory distribution of cranial nerve (CN) IX. Bilateral GPN presents unique challenges for consideration of microsurgical treatment due to potential risks of postoperative dysphagia and cardiac arrhythmias associated with resection of bilateral CN IX/X. Here, the authors describe successful staged bilateral microsurgical rhizotomy of CN IX and upper CN X rootlets to treat bilateral GPN. OBSERVATIONS: A 54-year-old female presented with right GPN and underwent a right retrosigmoid

Sacral Selective Dorsal Rhizotomy for the Treatment of Neurogenic Bladder: Surgical Technique.

BACKGROUND AND OBJECTIVES: Selective dorsal root rhizotomy (SDR) has been traditionally used to alleviate spasticity in patients with cerebral palsy. Its application to modulate bladder dysfunction in neurogenic spastic bladder, however, remains underexplored. We present single-level conus medullaris approach for sacral SDR as a technically feasible and potentially effective strategy for treating neurogenic bladder in a selected patient with neurogenic spastic bladder. METHODS: This instrumentation report details the surgical technique of sacral SDR targeting the S2-S5 dorsal rootlets by a mi

Preoperative gait pattern as predictor of gait changes following selective dorsal rhizotomy.

BACKGROUND: Selective dorsal rhizotomy (SDR) can improve gait in children with spastic paresis. However, outcomes vary, and selecting individuals who will benefit from the procedure remains challenging. How pre-SDR gait pattern predicts post-SDR gait has not previously been investigated. RESEARCH QUESTION: This study aims to compare how different gait patterns change after SDR to gain insight into the potential relevance of gait patterns as a selection criterion for SDR. METHODS: Kinematic and kinetic data of nineteen children with bilateral spastic paresis due to cerebral palsy or hereditar

Cervicothoracic ventral-dorsal rhizotomy for treatment of brachial hypertonia in cerebral palsy.

PURPOSE: Cervicothoracic ventral-dorsal rhizotomy (VDR) is a potential treatment of medically refractory hypertonia in patients who are not candidates for intrathecal baclofen, particularly in cases of severe upper limb hypertonia with limited to no function. A longitudinal cohort was identified to highlight our institutional safety and efficacy using cervicothoracic VDR for the treatment of hypertonia. METHODS: Retrospective data analysis was performed for patients that underwent non-selective cervicothoracic VDR between 2022 and 2023. Non-modifiable risk factors, clinical variables, and ope

Partial sensory rhizotomy in recurrent trigeminal neuralgia. Our experience and literature review.

BACKGROUND AND OBJECTIVE: Trigeminal Neuralgia (NT) is a common pathology in Neurosurgery. It can be classified as idiopathic or secondary to other pathologies, such as Multiple Sclerosis (MS). Several surgical treatments have been described, some of them being replaced by more modern techniques. Partial sensory rhizotomy (PSR), described by Dandy is a technique replaced by other techniques due to its permanent side effects. We present our experience with this technique in patients with recurrent NT. METHODS AND MATERIALS: A retrospective review is carried out on five patients who underwent s

Endoscopically-Assisted Percutaneous Trigeminal Rhizotomy for Trigeminal Neuralgia: A Cadaveric Feasibility Study.

Objective  Trigeminal neuralgia (TN) is a debilitating syndrome characterized by paroxysmal facial pain in one or more divisions of the trigeminal nerve. The etiology and treatment paradigms are still controversial. The endoscopically-assisted procedure has not yet been described in percutaneous procedures for TN. The aim of this study was to assess the utility and feasibility of endoscopic-assisted percutaneous approaches for trigeminal rhizotomy in TN. Methods  This study comprised eight cadaveric sides heads that underwent an endoscopically assisted percutaneous approach using Hakanson's an

Can fifth nerve mapping guide rhizotomy for recurrent trigeminal neuralgia? Case report.

INTRODUCTION: Microvascular decompression (MVD) is usually considered the first-line treatment for trigeminal neuralgia (TN) when medical treatments fail. Recurrence is rare and best treatment option is controversial. MVD was proposed as a feasible and effective technique for recurrent TN by many authors. Nevertheless, in a substantial number of cases, not any impingement or deterioration are found intraoperatively and partial selective rhizotomy is then advised. The rhizotomy site is mostly guided by anatomical landmarks, but variations due to scarring and adhesions are common pitfalls in the

Morphology of the trigeminal ganglion: anatomical structures related to trigeminal radiofrequency rhizotomy.

BACKGROUND: Trigeminal neuralgia is the most common example of craniofacial neuralgia. Its etiology is unknown and is characterized by severe episodes of paroxysmal pain. The trigeminal ganglion and its adjacent anatomical structures have a complex anatomy. The foramen ovale is of great importance during surgical procedures such as percutaneous trigeminal rhizotomy for trigeminal neuralgia. OBJECTIVE: We aimed to identify the anatomical structures associated with the trigeminal ganglion and radiofrequency rhizotomy on cadavers and investigate their relationship with the electrodes used during

A Cadaver Study Confirming the Location of Dye after Trigeminal Ganglion Rhizotomy.

BACKGROUND: Radiofrequency thermal rhizotomy demonstrates an excellent treatment outcome with a high success rate among patients of intractable trigeminal neuralgia. The triangular plexus which is an immediate retrogasserian portion of the trigeminal root is suggested as the best place of lesioning during radiofrequency thermal rhizotomy. However, the anatomy of the triangular plexus has been relatively unrecognized, while the anatomical study related to therapeutic procedure is scarce. OBJECTIVE: The purpose of this study is to confirm with gross and microscopic finding of the trigeminal sys

Selective intradural dorsal rhizotomy for persistent radicular leg pain: a contemporary series.

BACKGROUND: Lumbar disc surgery for radicular leg pain is one of the most frequently performed spine procedures. In approximately 20% of patients poor outcome is achieved. The most complex cases have persistent leg pain without residual nerve root compression. Treatment for refractory cases is limited to medical pain management, spinal cord stimulation (SCS), and dorsal root ganglion stimulation. For the latter two, fair to good results are obtained in only 50% of patients and costs and complication rates are high. An alternative surgical procedure is selective intradural dorsal rhizotomy (SID
