Bacillus Calmette-Guérin vaccination and clinical characteristics and outcomes of COVID-19 in Rhode Island, United States: a cohort study.

Coronavirus disease 2019 (COVID-19) has resulted in a global pandemic, and there is limited data on effective therapies. Bacillus Calmette-Guérin (BCG) vaccine, a live-attenuated strain derived from an isolate of Mycobacterium bovis and originally designed to prevent tuberculosis, has shown some efficacy against infection with unrelated pathogens. In this study, we reviewed 120 consecutive adult patients (≥18 years old) with COVID-19 at a major federally qualified health centre in Rhode Island, United States from 19 March to 29 April 2020. Median age was 39.5 years (interquartile range, 27.0-5

A Pre-Post Assessment of Blood Glucose Control Following Pharmacist-Led Professional Continuous Glucose Monitoring in Rhode Island Primary Care Practices.

BACKGROUND: Professional continuous glucose monitoring (proCGM) can improve glycemic control and support medication and lifestyle adjustments in people with diabetes. In 2022, the Care Transformation Collaborative of Rhode Island launched a quality improvement initiative to implement pharmacist-led proCGM services in primary care practices. This project aimed to determine the effects of a pharmacist-led proCGM service on glycemic control, overall and by practice site, compared to baseline hemoglobin A1C (A1C) level. A secondary aim was to assess care team members' views of the program impact o

Evaluation of Naloxone Uptake Disparities Among Harm Reduction Clients in Rhode Island: A Deeper Dive Using Disaggregated Race and Ethnicity Data.

INTRODUCTION: Although opioid overdose deaths decreased between 2022 to 2024, racial and ethnic disparities persisted during this time. The goal of this analysis was to explore disparities in naloxone uptake by racial and ethnic identity and harm reduction supply preference. This article builds upon prior work and disaggregates race and ethnicity categories that were previously aggregated due to small numbers. METHODS: Clients were divided into three mutually exclusive groups based on the type of harm reduction supplies they requested: 1) those who requested injection supplies only, 2) those

Host and primary tumor factors for the development of multiple cutaneous squamous cell carcinomas among a retrospective cohort in Rhode Island.

INTRODUCTION: Risk factors for a primary cutaneous squamous cell carcinoma (CSCC) are well-established; however, the host and primary tumor risk factors for subsequent CSCC have not been fully explored. METHODS: We performed a retrospective chart review of patients diagnosed with CSCC in an academic dermatology clinic in Rhode Island from 2016-2019. Logistic regression was used to evaluate the associations between host factors and multiple CSCC and between primary tumor characteristics and the risk of subsequent CSCC. Adjusted odds ratios (aORs) and 95% CIs were calculated. RESULTS: A total

A qualitative study among injection drug using women in Rhode Island: attitudes toward testing, treatment, and vaccination for hepatitis and HIV.

HIV and hepatitis C virus infection are serious and prevalent health conditions among many women who inject drugs. Qualitative interviews with 20 injection drug using women at a short term drug treatment center in Rhode Island revealed six primary barriers and facilitators for testing and receiving results and treatment for hepatitis and HIV, as well as for hepatitis vaccination. The primary barriers were prioritization of drug use; low level of disease-specific knowledge; stigmatization; accessibility of testing, results and treatment; and psychological factors. The primary facilitator was in

Point-Of-Care Drug-Checking: Assessing the Rhode Island Drug Supply Using FTIR Spectroscopy to Detect Fentanyl, Xylazine and Other Substances.

INTRODUCTION: The illicit drug supply constantly fluctuates, with new adulterants and unexpected mixtures creating uncertainty. Point-of-care drug-checking services are vital in reducing overdose risk and other harms by providing real-time analysis, harm reduction education and links to care. This study aimed to characterise the local drug supply using Fourier transform infrared (FTIR) spectroscopy, complemented by Liquid Chromatography-Quadrupole time-of-flight Mass Spectrometry (LC-QToF-MS) for additional context, from drug residues collected through a drug-checking program within a Rhode Is

Assessing Utility of 24-Hour Ambulatory Blood Pressure Monitoring to Distinguish Pediatric Populations Presenting with Elevated Blood Pressure in Rhode Island.

This retrospective study aimed to assess the value of 24-hour ambulatory blood pressure monitoring (ABPM) in distinguishing primary from secondary hypertension in pediatric patients. Our study was conducted on 293 patients referred to a pediatric nephrology clinic over 11 years. Various ABPM parameters were analyzed, including daytime and nighttime systolic and diastolic blood pressures, heart rate, and blood pressure load. Among the participants, 74% were normotensive (white-coat hypertension), 21.5% had primary hypertension, and 4.4% had secondary hypertension. There were no significant diff

Maternal ambient air pollution, preterm birth and markers of fetal growth in Rhode Island: results of a hospital-based linkage study.

BACKGROUND: Maternal exposure to ambient air pollution has been associated with higher risk of preterm birth and reduced fetal growth, but heterogeneity among prior studies suggests that additional studies are needed in diverse populations and settings. We examined the associations between maternal ambient air pollution levels, risk of preterm birth and markers of fetal growth in an urban population with relatively low exposure to air pollution. METHODS: We linked 61 640 mother-infant pairs who delivered at a single hospital in Providence, Rhode Island, from 2002 to 2012 to birth certificate

Structural and healthcare predictors of substance use-related death following release from incarceration: A retrospective cohort study in Rhode Island, 2018-2020.

BACKGROUND: Incarcerated individuals are at a high risk of substance use-related death after release. While prior research has demonstrated that this is partially due to loss of tolerance during incarceration and return to use after release, structural and healthcare use factors that may also impact overdose risk have been underexplored. We assessed the relationship between structural factors (e.g., housing services received) and health care use (e.g., emergency department use) and overdose risk at 12-months post-incarceration in Rhode Island in the United States. METHODS: We used a retrospec

Association between willingness to use an overdose prevention center and probation or parole status among people who use drugs in Rhode Island.

BACKGROUND: Overdose prevention centers (OPCs) are being implemented in the United States as a strategy to reduce drug-related mortality and morbidity. Previous studies have suggested that people who use drugs (PWUD) with a history of criminal legal system (CLS) involvement (e.g. current probation/parole) are at greater risk of overdose but may also encounter significant barriers to OPC use. The objective of this study was to explore the association between willingness to use an OPC and probation/parole status in a sample of PWUD in Rhode Island. METHODS: This study utilized data from the Rho

Using Timely Overdose Data to Address a Spike in Nonfatal Overdoses and Inform a Coordinated Community-Level Response in Rhode Island, 2019.

The Rhode Island Department of Health (RIDOH) uses emergency department data to monitor nonfatal opioid overdoses in Rhode Island. In April 2019, RIDOH detected an increase in nonfatal opioid overdoses in Woonsocket, Rhode Island, and sent an alert to state and local partners (eg, fire departments, emergency departments, faith leaders) with guidance on how to respond. To guide community-level, strategic response efforts, RIDOH analyzed surveillance data to identify overdose patterns, populations, and geographic areas most affected. During April-June 2019, nonfatal opioid overdoses in Woonsocke

Investigating heterogeneous effects of an expanded methadone access policy with opioid treatment program retention: a Rhode Island population-based retrospective cohort study.

Following federal regulatory changes during the COVID-19 pandemic, Rhode Island expanded methadone access for opioid treatment programs (OTPs) in March 2020. The policy, which permitted take-home dosing for patients, contrasted with longstanding restrictions on methadone. This study used patient-level OTP admission and discharge records to compare six-month retention before and after the policy change. We conducted a retrospective cohort study of 1248 patients newly admitted to OTPs between March 18 and June 30 of 2019 (pre-policy) and 2020 (post-policy). We used logistic regression to estimat

Primary care providers' and nurses' knowledge, attitudes, and skills regarding latent TB infection testing and treatment: A qualitative study from Rhode Island.

BACKGROUND: Untreated latent tuberculosis infection (LTBI) is a major source of active tuberculosis disease in the United States. In 2016, the United States Preventive Services Task Force (USPSTF) recommended that screening for latent tuberculosis infection among individuals at increased risk be performed as routine preventive care. Traditionally, LTBI management-including both testing and treatment-has been conducted by specialists in the United States. It is believed that knowledge gaps among primary care team members and discomfort with LTBI treatment are significant barriers to LTBI manage
