Delayed aortic injury following multiple rib fractures: a case report.

BACKGROUND: Delayed hemothorax due to aortic injury is rare, and its delayed diagnosis can be fatal. Herein, we report a case of delayed hemothorax owing to aortic injury following blunt chest trauma, which was successfully treated through emergency surgery. CASE PRESENTATION: A woman in her 70s fell on an escalator at a train station and was brought to our emergency department. Initial evaluation with computed tomography (CT) imaging revealed a thoracic vertebral fracture, bilateral lower rib fractures, and a right-sided hemothorax. She was admitted for observation and supportive care, and h

The burden of readmissions after rib fractures among older adults.

BACKGROUND: The index hospitalization morbidity and mortality of rib fractures among older adults (aged ≥65 years) is well-known, yet the burden and risks for readmissions after rib fractures in this vulnerable population remain understudied. We aimed to characterize the burdens and etiologies associated with 3-month readmissions among older adults who suffer rib fractures. We hypothesized that readmissions would be common and associated with modifiable etiologies. METHODS: This survey-weighted retrospective study using the 2017 and 2019 National Readmissions Database evaluated adults aged ≥6

Palliative Care Rounds: A Thanksgiving Break to Discuss Rib Fractures.

Rib fractures are common injuries, especially in the frail and elderly. They can happen in isolation or may be associated with significant concomitant morbidity, including but not limited to pain, pneumonia, or pneumothorax. In the palliative care population, rib fractures can be overlooked or attributed to other entities, which may lead to inappropriate treatment. The commonly accepted standards of care for the treatment of rib fractures are centered around early and adequate pain control, and stabilization of other complications. Accurate diagnosis and management demand a thorough history an

Management Pathways for Traumatic Rib Fractures-Importance of Surgical Stabilisation.

Rib fractures occur in almost half of blunt chest wall trauma victims in Australia. They are associated with a high rate of pulmonary complications, and consequently, with increased discomfort, disability, morbidity, and mortality. This article summarises thoracic cage anatomy and physiology, and chest wall trauma pathophysiology. Institutional clinical strategies and clinical pathway "bundles of care" are usually available to reduce mortality and morbidity in patients with chest wall injury. This article analyses multimodal clinical pathways and intervention strategies that include surgical s

Rib Fractures in Professional Baseball Pitchers: Mechanics, Epidemiology, and Management.

Pitching is a complex kinetic chain activity requiring the transfer of energy from the lower body, through the core and trunk, and finally through the arm to generate explosive acceleration of the baseball. As a result, large forces are generated in the trunk musculature and rib attachments from the late cocking phase of pitching through deceleration. The repetitive cumulative load and high pitch velocities put professional pitchers at risk of rib stress fracture. Given the potential for a prolonged recovery course and high rate of recurrence, early recognition of rib bone stress injury is cri

Surgical treatment ofpatients with severe non-flail chest rib fractures.

BACKGROUND: Many patients have inadequate long-term analgesia, respiratory distress, and hypoxemia due to a long-standing substantial smoking history or the presence of primary pulmonary diseases; analgesic treatment is not valid in these patients. Even if the imaging findings of rib fractures are relatively mild, rib fractures may cause severe position limitation, respiratory distress, and hypoxemia. AIM: To investigate the curative effect of surgical treatment for patients with severe non-flail chest rib fractures. METHODS: A total of 78 patients from our hospital with severe noncontinuous

Early Versus Late Tracheostomy in Trauma Patients With Rib Fractures.

BACKGROUND: Patients with blunt chest trauma with multiple rib fractures (RF) may require tracheostomy. The goal was to compare early (≤7 d) versus late (>7 d) tracheostomy patients and to analyze clinical outcomes, to determine which timing is more beneficial. METHODS: This retrospective review included 124 patients with RF admitted to trauma ICU at two level 1 trauma centers who underwent tracheostomy. Analyzed variables included age, gender, injury severity score, Glasgow Coma Scale, number of ribs fractured, total fractures of the ribs, prevalence of bilateral RF, flail chest, maxillofaci

Surgical plate fixation of multiple rib fractures: a case report.

BACKGROUND: The healthcare system in developing countries is limited; particularly, medical specialties such as emergency and trauma medicine are underdeveloped. Consequently, trauma injuries sustained in traffic accidents result in chronic morbidity more often than similar cases in developed countries. Multiple rib fractures induce significant patient morbidity. Current international guidelines recommend a multidisciplinary, surgery-based treatment approach to achieve optimal clinical benefit. CASE PRESENTATION: We admitted a 41-year-old Albanian man to our emergency department following a p

[Surgical stabilization of multiple rib fractures can shorten inpatient stay].

Flail chest is a common complication in patients with blunt chest wall traumas resulting in high mortality rates. In this case report a 43-year-old woman was stepped on by a horse, thereby receiving multiple rib fractures and pneumohaemothorax. She was not able to wean from epidural analgesia after ten days and maintained a "thoracic floating feeling". In opposition to the non-operative management previously preferred, a surgical stabilization was then performed in the patient, who was discharged only five days later. Thus, surgical fixation of multiple rib fractures may reduce inpatient stay.

Rib fractures in trauma patients: does operative fixation improve outcome?

PURPOSE OF REVIEW: Renewed interest in surgical fixation of rib fractures has emerged. However, conservative treatment is still preferred at most surgical departments. We wanted to evaluate whether operative treatment of rib fractures may benefit severely injured patients. RECENT FINDINGS: Several studies report a reduction in mechanical ventilation time, ICU length of stay (LOS), hospital LOS, pneumonia, need for tracheostomy, pain and costs in operatively treated patients with multiple rib fractures compared with patients treated nonoperatively. Although patient selection and timing of the

Long-term patient outcomes after surgical stabilization of rib fractures.

BACKGROUND: Rib fractures are common, and can be disabling. Recently, there has been increased interest in surgical stabilization of rib fractures (SSRF). It is difficult to define long-term benefits of the procedure. This is a descriptive study of patient outcomes after SSRF. METHODS: SSRF patients between April 2010 and August 2012 at a Level I trauma center were identified. Data were collected from the medical records. A telephone survey asking about pain, satisfaction, and employment was administered to patients after hospital discharge. RESULTS: One hundred-one patients met inclusion cr

Two consecutive rib stress fractures in a female competitive swimmer.

We present a case of two consecutive stress fractures in a female swimmer. The diagnosis of the present stress fracture of the ninth rib was based on clinical history and examination and on a new fracture line and callus formation seen in consequent conventional radiographs. Based on the clinical history and radiography, the patient had suffered another rib stress fracture in the fifth rib 15 months earlier. No external trauma had preceded either of the fractures, and no secondary cause of stress fracture was found. Her anatomical and biomechanical characteristics and training errors seem to h

Is there an association between fractures of the cervical spine and first- and second-rib fractures?

OBJECTIVE: To investigate a potential association between cervical spine injury and first- and second-rib fractures. METHODS: Retrospective review of the radiologic and medical records of 28 consecutive patients admitted to an acute spinal injury unit. RESULTS: A total of 10 (36%) of the patients with cervical spine trauma also had fractures of either the first or second ribs. Eight of these 10 patients had a fracture or fracture-subluxation of the seventh cervical vertebra or the C6/7 segment. CONCLUSION: Almost one-third of patients with traumatic cervical spine injury have an associated

Efficacy of percutaneous cryoneurolysis for pain management of rib fractures.

Rib fractures are a common injury with several etiologies. Various techniques for managing pain with rib fractures have evolved over the past few decades. Pain control is key in reducing further complications of this traumatic injury. In this regard, cryoneurolysis is a form of interventional pain management that involves applying freezing temperatures to disrupt the signaling pathways of nerves, hence reducing pain. Alternative forms of pain management, such as cryoneurolysis, are being studied to find less invasive techniques and to reduce dependence on opioids and other potentially addictiv

Isolated First Rib Fractures in Adolescent Athletes: An Epidemiologic Review.

BACKGROUND: First rib fractures are infrequently encountered injuries, especially in the young athlete. The incidence and natural history of first rib fractures sustained during sport are poorly elucidated in the literature, contributing to the variability of treatment of this injury. Thus, the purpose of this study was to characterize first rib fractures and their treatment outcomes, specifically among a case series of young athletes. METHODS: A retrospective review of patient charts identified through electronic health record diagnosis code query from 2 tertiary children's hospitals was per

Impact of Trauma Team Census on Patients With Rib Fractures.

BACKGROUND: Workloads of healthcare professionals have continued to increase in recent years. With increased workload, some studies have shown increased length of stay for patients, higher cost, and higher risk of patient safety events. Trauma teams are a group that is particularly vulnerable to changes in patient census. This study aims to evaluate the impact of high versus low trauma team census on outcomes. METHODS: A retrospective review of all rib fracture patients at a level I trauma center from 2017 to 2022 was conducted. The average daily census was determined, and high or low census

Surgical stabilization of posterior rib fractures involving the costotransverse joint.

PURPOSE: Surgical stabilization of posterior rib fractures involving the costotransverse joint is debated, with most patients managed non-operatively. However, surgery may be indicated for respiratory failure due to altered mechanics or severe pain leading to functional limitations. Long-term outcomes related to quality of life and pulmonary function in these patients are underreported. This study aimed to describe our unique experience with the surgical stabilization of posterior rib fractures involving the costotransverse joint and to report the associated outpatient clinical outcomes. Predictors of Discharge Disposition Following Surgical Stabilization of Rib Fractures.

Introduction: Patients are commonly discharged to either home health care (HHC), skilled nursing facility (SNF), or inpatient rehabilitation facility (IRF) following surgical stabilization of rib fractures (SSRF) to assist with recovery. This study explores demographic and surgical factors that may be associated with discharge disposition to a transitional care setting (HHC, SNF, and IRF) relative to routine discharge (self-care) following SSRF. Methods: Patients who underwent SSRF between 2017 and 2021 were included using retrospective data from the National Trauma Data Bank. Patients were st
