Multivariate Risk Adjustment of Primary Care Patient Panels in a Public Health Setting: A Comparison of Statistical Models.

We compared prospective risk adjustment models for adjusting patient panels at the San Francisco Department of Public Health. We used 4 statistical models (linear regression, two-part model, zero-inflated Poisson, and zero-inflated negative binomial) and 4 subsets of predictor variables (age/gender categories, chronic diagnoses, homelessness, and a loss to follow-up indicator) to predict primary care visit frequency. Predicted visit frequency was then used to calculate patient weights and adjusted panel sizes. The two-part model using all predictor variables performed best (R = 0.20). This mod

A methodological approach to identify external factors for indicator-based risk adjustment illustrated by a cataract surgery register.

BACKGROUND: Risk adjustment is crucial for comparison of outcome in medical care. Knowledge of the external factors that impact measured outcome but that cannot be influenced by the physician is a prerequisite for this adjustment. To date, a universal and reproducible method for identification of the relevant external factors has not been published. The selection of external factors in current quality assurance programmes is mainly based on expert opinion. We propose and demonstrate a methodology for identification of external factors requiring risk adjustment of outcome indicators and we appl

A comparison of three strategies for risk-adjustment of outcomes for AIDS patients hospitalized for Pneumocystis carinii pneumonia.

BACKGROUND: The need for risk-adjustment of patient outcomes has been driven by the competitive health care market and the subsequent increase in comparative outcome reporting among health care institutions, among managed care plans, and among individual providers for some procedures (e.g. coronary artery bypass graft surgery). However, if the outcomes reported do not take into account patient characteristics that can be considered dimensions of risk for poor clinical outcomes or increased utilization of services, there is the possibility that inaccurate conclusions will be drawn about the qua

Adjustment problems among Soviet immigrants at risk. Part I: Reaching out to members of the "1000 families" organization.

An outreach, preventive mental health project with Soviet immigrants implemented during and immediately following the Gulf War is described. One hundred and fifty immigrant families were visited by immigrant mental health professionals when the level and nature of their psychological distress was assessed. In the analyses of data, patterns of distress over time and factors which contributed to psychological distress were also examined. The main finding of the study concerned the subjects' well-being which was found to be marginally related to the Gulf War. Furthermore, no post-traumatic reacti

Family Functioning and Academic Adjustment in Medical Students: Coping Styles as Mediators and Differences in At-Risk Students.

OBJECTIVE: Academic adjustment is essential for medical students facing rigorous academic demands. While individual and instructional factors have been well-studied, the role of family functioning remains underexplored. This study examined the association between family functioning and academic adjustment, explored the mediating role of coping styles, and compared these pathways between at-risk students (those who had failed at least one final examination) and controls. METHODS: A cross-sectional study was conducted using validated scales for assessment of academic adjustment, family function

Eldecalcitol in older adults with kidney vulnerability: a narrative review of risk-based use, dose adjustment, and monitoring.

Eldecalcitol is an active vitamin D analogue used for osteoporosis in Japan. Its usual dose is 0.75 μg/day, but calcium-related adverse effects can become clinically important when reduced kidney reserve, frailty, low body weight, poor intake, dehydration risk, interacting medicines, or unreliable follow-up coexist. This narrative review synthesizes trial, post-marketing, pharmacovigilance, and real-world monitoring evidence to define a practical safety framework. Eldecalcitol reduces vertebral fractures compared with alfacalcidol, whereas hip-fracture protection has not been established; ther

Development and validation of risk adjustment models for long-term mortality and myocardial infarction following percutaneous coronary interventions.

BACKGROUND: Existing models for outcome after percutaneous coronary interventions (PCIs) lack assessment of long-term prognosis. Our goal was to derive 1- and 5-year mortality and mortality/myocardial infarction (MI) risk models for PCI outcomes from simple, easily obtainable clinical and laboratory variables. METHODS AND RESULTS: Using the Mayo Clinic registry, we analyzed long-term mortality and mortality/MI following PCIs on 9165 unique patients from January 1, 2001, through December 31, 2007. Cox proportional hazards regression was used to model the calculated risk score and major procedu

Failure of Marfan anatomic criteria to predict risk of aortic dissection in Turner syndrome: necessity of specific adjusted risk thresholds.

Patients with Turner syndrome are prompt to develop spontaneous acute aortic dissection following insidious aortic dilatation, with abnormal cardiovascular anatomy and consequently require specific guidelines for regular surveillance since they represent a subset of high-risk young patients. We report a rare and uncommon case of spontaneous acute aortic dissection in a 48-year old female patient with Turner syndrome who was not apparently eligible for a prophylactic surgery. A CT scan showed a Stanford type A aortic dissection and was urgently referred for surgical management. We operated on t

Inbreeding and Gallbladder Cancer Risk: Homozygosity Associations Adjusted for Indigenous American Ancestry, BMI, and Genetic Risk of Gallstone Disease.

Latin Americans have a rich genetic make-up that translates into heterogeneous fractions of the autosomal genome in runs of homozygosity (FROH) and heterogeneous types and proportions of indigenous American ancestry. While autozygosity has been linked to several human diseases, very little is known about the relationship between inbreeding, genetic ancestry, and cancer risk in Latin Americans. Chile has one of the highest incidences of gallbladder cancer (GBC) in the world, and we investigated the association between inbreeding, GBC, gallstone disease (GSD), and body mass index (BMI) in 4029 g

Risk and protective factors, stressors, and symptoms of adjustment disorder during the COVID-19 pandemic - First results of the ESTSS COVID-19 pan-European ADJUST study.

BACKGROUND: The COVID-19 pandemic exposes individuals to multiple stressors, such as quarantine, physical distancing, job loss, risk of infection, and loss of loved ones. Such a complex array of stressors potentially lead to symptoms of adjustment disorder. OBJECTIVE: This cross-sectional exploratory study examined relationships between risk and protective factors, stressors, and symptoms of adjustment disorder during the first year of the COVID-19 pandemic. METHODS: Data from the first wave of the European Society of Traumatic Stress Studies (ESTSS) longitudinal ADJUST Study were used. N =

Suboptimal decision making by children with ADHD in the face of risk: Poor risk adjustment and delay aversion rather than general proneness to taking risks.

OBJECTIVE: Suboptimal decision making in the face of risk (DMR) in children with attention-deficit hyperactivity disorder (ADHD) may be mediated by deficits in a number of different neuropsychological processes. We investigated DMR in children with ADHD using the Cambridge Gambling Task (CGT) to distinguish difficulties in adjusting to changing probabilities of choice outcomes (so-called risk adjustment) from general risk proneness, and to distinguish these 2 processes from delay aversion (the tendency to choose the least delayed option) and impairments in the ability to reflect on choice opti

Accounting for urgency: Implications of risk adjustment for surgical site infection after emergency colon surgery at safety net hospitals.

BACKGROUND: Colon operations performed for urgent indications or trauma are associated with a higher risk of surgical site infection. Prior models for surgical site infection surveillance did not account for this urgency, potentially yielding inadequate risk adjustment for surgical site infection at safety net hospitals that perform a disproportionate number of these emergency operations. METHODS: We used data from the 2021 Healthcare Cost and Utilization Project State Inpatient and Emergency Department Databases from California and Florida. We evaluated index encounters during which colon su

Development of a risk adjustment scoring system for patients treated with cytoreductive surgery and hyperthermic intraperitoneal chemotherapy: HIPEC-POSSUM.

BACKGROUND: The rate of morbidity and mortality in patients with peritoneal metastasis(PM) has decreased due to advancements in effective perioperative chemotherapy and in complex surgical techniques [Cytoreductive surgery and hyperthermic intraperitoneal chemotherapy(CRS-HIPEC)], and preoperative optimization of patients. Despite this, PM, an apex form of advanced and complex oncological surgery platform, continues to present significant morbidity and mortality challenges. This study aimed to analyze morbidity and mortality and final oncologic outcomes with a novel hybrid HIPEC-POSSUM(H-POSSU

Development and Assessment of a New Framework for Disease Surveillance, Prediction, and Risk Adjustment: The Diagnostic Items Classification System.

IMPORTANCE: Current disease risk-adjustment formulas in the US rely on diagnostic classification frameworks that predate the International Classification of Diseases, Tenth Revision, Clinical Modification (ICD-10-CM). OBJECTIVE: To develop an ICD-10-CM-based classification framework for predicting diverse health care payment, quality, and performance outcomes. DESIGN SETTING AND PARTICIPANTS: Physician teams mapped all ICD-10-CM diagnoses into 3 types of diagnostic items (DXIs): main effect DXIs that specify diseases; modifiers, such as laterality, timing, and acuity; and scaled variables, s

Current diagnosis-related group-based bundling for upper-extremity arthroplasty: a case of insufficient risk adjustment and misaligned incentives.

BACKGROUND: The current Centers for Medicare & Medicaid Services diagnosis-related group (DRG) bundled-payment model for upper-extremity arthroplasty does not differentiate between the type of arthroplasty (anatomic total shoulder arthroplasty [ATSA] vs. reverse total shoulder arthroplasty vs. total elbow arthroplasty [TEA] vs. total wrist arthroplasty) or the diagnosis and indication for surgery (fracture vs. degenerative osteoarthritis vs. inflammatory arthritis). Strength of Fibroblast Growth Factor 23 as a Cardiovascular Risk Predictor in Chronic Kidney Disease Weaken by ProBNP Adjustment.

BACKGROUND: Various epidemiological studies linked high fibroblast growth factor 23 (FGF23) levels with cardiovascular events in chronic kidney disease (CKD). It remains enigmatic whether high FGF23 exerts adverse cardiovascular effects, or whether it reflects detrimental effects of residual confounders. Earlier studies adjusted for CKD-mineral bone disease (CKD-MBD) regulators of FGF23 rather than for recently discovered non-CKD-MBD regulators, among which iron deficiency and heart failure are of particular importance. Moreover, they used c-terminal FGF23 (cFGF23) assays rather than more spec
