Between 1987 and 1992, one hundred eleven articular surface partial-thickness rotator cuff tears were diagnosed in 106 patients; 90 were men, and 16 were women. The average age was 42.5 years. Average follow-up was 32.3 months, ranging from 26 to 84 months. Patients were separated into three groups. Group 1 consisted of 85 shoulders in which impingement was believed to be the primary cause; these shoulders were treated with debridement of the tear and arthroscopic subacromial decompression. Group 2 consisted of 14 shoulders with instability treated with debridement of the partial tear and ante

Almost 100 years have passed since the first report of rotator cuff repair in 1898 by W. Müller. Even now new operative techniques are being developed. Various solutions for difficult extended forms of rotator cuff lesions are available besides the closed and semi-closed arthroscopic techniques. Anatomical reconstructive procedures such as local or distant tendon transfer compete with extra-anatomical procedures such as simple debridement, equatorial partial reconstruction or replacement with a delta flap. Hemiarthroplasty of the shoulder has proven to be more and more the treatment of choice

The results of rotator cuff repair through a lateral deltoid splitting approach combined with arthroscopic subacromial decompression are presented in 18 patients with an average followup of 46 months. Sixteen patients (88%) scored good or excellent on the University of California at Los Angeles shoulder rating scale. Pain scores improved from an average of 1.6 to 7.6. Function scores improved from 2.5 to 8.4. Two patients had poor results; both had workers' compensation cases pending. One patient with a poor result had 2 complications: superficial infection and failure of repair that required

This study analyzes the results of a previously unreported technique of reconstruction for the massive irreparable rotator cuff tear. The technique involves repair of the margins of the tear to restore the force couples and "suspension bridge" system of force transmission in the shoulder. Complete coverage of the defect was not considered to be essential as long as the normal mechanics of the shoulder were restored and the rotator cuff tear was converted to a "functional cuff tear." This procedure was performed on 14 patients. Improvement in function was dramatic. Active elevation (elevation d

To determine the pathophysiological condition of rotator cuff tear, 23 cases were observed using electromyogram of the shoulder girdle muscles during elevation of the arm. The correlation between the action potential of these muscles and the angle of the arm was analyzed using computer in order to identify the pattern of that. The pattern of the electromyogram of the affected side with the ability to elevate the arm above 90 degrees was similar to that of the sound side. Differences were seen between the affected side with the inability and the sound side in the electromyogram of trapezius, de

In terms of rotator cuff repair, there is a goal for complete repair and healing, as rotator cuff integrity correlates with clinical and functional results. Retear has been shown to have a significant influence on progression toward osteoarthritis, and patients with an intact supraspinatus show superior abduction and flexion strength. However, in cases where complete repair may not be possible and/or cost limitations may prohibit augmentation, partial repair can provide a respectable outcome. Furthermore, regardless of healing status, partial repair may provide satisfactory relief for patients

BACKGROUND: Few studies have investigated the correlation between shoulder kinematics and clinical outcomes in patients undergoing rotator cuff repair using dynamic analysis. This study assessed shoulder kinematics before and after surgical repair in patients with rotator cuff tears (RCTs) and determined the relationship among shoulder kinematics and between shoulder kinematics and clinical outcomes.
METHODS: Ten patients with large-to-massive RCTs and 10 control participants were included. In vivo shoulder kinematics during scapular plane abduction were measured preoperatively and 1 year pos

BACKGROUND: We compared the outcomes of patients who performed physical therapy versus those who did not in a longitudinal cohort of patients undergoing nonoperative treatment of rotator cuff tears. We also assessed whether there was a dose effect in which the pain and functional outcomes in patients performing physical therapy plateaued.
METHODS: From February 2011 to June 2015, a multicenter cohort of patients with rotator cuff tears undergoing nonoperative treatment completed a detailed health and demographic questionnaire and the Shoulder Pain and Disability Index (SPADI) at baseline and

The effect of humeral rotation on the appearance of the rotator cuff tendon in oblique coronal magnetic resonance images was evaluated in 70 such images. Internal rotation produced overlap of the supraspinatus and infraspinatus tendons with soft-tissue interposition or apparent discontinuity of the tendon in 22 cases, including 14 of 15 shoulders positioned in the first 30 degrees between extreme internal and external rotation. Imaging in internal rotation makes assessment of the tendon difficult and should be avoided.

BACKGROUND: Stiffness and retear are 2 common complications of rotator cuff repair. McNamara et al found that postoperative stiffness was associated with lower retear rates at 6 months. This study aimed to determine if stiffness after rotator cuff repair protects the individual from retear up to 9 years after surgery.
MATERIALS AND METHODS: Two groups of patients, 69 with stiff and 63 with nonstiff shoulder, who underwent arthroscopic rotator cuff repair were created based on external rotation measurements at 6 weeks postoperatively. Patients had regular follow-up assessments at 6, 12, and 24

OBJECTIVE: To evaluate the clinical effect of arthroscopic 360° capsular release with concomitant rotator cuff repair for patients with rotator cuff tears in global stiff shoulder.
METHODS: From December 2016 to December 2018, 247 patients full-thickness rotator cuff tear were treated with arthroscopic rotator cuff repair. Among them, 42 patients (17%) with global limitation of shoulder movement were treated with arthroscopic 360° capsular release operation at the same time of rotator cuff repair. The function of shoulder was evaluated by Constant-Murley score and visual analogue scales (VAS)

BACKGROUND: The Western Ontario Rotator Cuff Index (WORC) is a joint specific outcome tool that assesses the quality of life in patients with various rotator cuff problems. Our purpose was to evaluate selected psychometric characteristics (internal consistency, validity, reliability and agreement) of the Polish version of WORC in patients undergoing rotator cuff repair.
METHODS: Sixty-nine subjects took part in the study with a mean age 55.5 (range 40-65). All had undergone arthroscopic rotator cuff repair in 2015-2016. Data from 57 patients in whom symptoms in the shoulder joint had not chan

BACKGROUND: Previous biomechanical studies regarding deltoid function during glenohumeral abduction have primarily used static testing protocols.
HYPOTHESES: (1) Deltoid forces required for scapular plane abduction increase as simulated rotator cuff tears become larger, and (2) maximal abduction decreases despite increased deltoid forces.
STUDY DESIGN: Controlled laboratory study.
METHODS: Twelve fresh-frozen cadaveric shoulders with a mean age of 67 years (range, 64-74 years) were used. The supraspinatus and anterior, middle, and posterior deltoid tendons were attached to individual should

Rheumatic diseases, particularly in the context of rheumatoid arthritis (RA), frequently affect the shoulder joint and lead to lesions of the rotator cuffs, which are associated with chronic pain and restricted movement. Systemic inflammation endangers the integrity of the rotator cuff, while pharmacotherapy can impair tissue repair; however, the function of rotator cuffs is decisive for the selection of diagnostic and therapeutic measures. In cases of failure of conservative treatment surgical reconstruction represents an option in order to relieve pain and restore the biomechanical function.

»: Rotator cuff tear arthropathy (RCTA) describes a pattern of glenohumeral degenerative changes following chronic rotator cuff tears that is characterized by superior humeral head migration, erosion of the greater tuberosity of the humeral head, contouring of the coracoacromial arch to create a socket for the humeral head, and eventual glenohumeral arthritis.
»: Acute and chronic inflammatory changes following rotator cuff tears are thought to contribute to cartilage damage, muscle fibrosis, and fatty infiltration in the glenohumeral joint.

Arthroscopic rotator cuff repair (RCR) has been reported to have good clinical results but high retear rates by ultrasound. We prospectively assessed postoperative cuff integrity and outcome after arthroscopic RCR (40 patients) and compared these results with open RCR (32 patients). Evaluation preoperatively and at 1 year included a physical examination and magnetic resonance imaging. American Shoulder and Elbow Surgeons and Constant scores improved significantly in both groups (P < .0001). Overall, 69% of repairs in the open group and 53% in the arthroscopic group were intact by magnetic reso

BackgroundPartial-thickness rotator cuff tears are a commonly underreported shoulder condition that can lead to pain, functional impairment, and limited range of motion. These tears affect both young, active individuals and the elderly. If left untreated, partial-thickness tears may progress to full-thickness tears over time.Classification and diagnosisPartial-thickness tears can occur on the bursal side, articular side, or within the tendon itself (intratendinous). The severity of pain and the risk of tear progression depend on the tear's location. Diagnosis often involves physical examinatio

In its natural state, the shoulder is unbalanced in both the vertical and horizontal planes because the deltoid is stronger than the rotator cuff muscles and the internal rotator muscles are stronger than the external rotator muscles. With aging, this muscle imbalance can become worse, leading to tendon wear, irreversible fatty infiltration of the rotator cuff muscles, and upward migration of the humeral head. Most shoulders with tendon wear are functional and asymptomatic. A traumatic event (such as a fall onto the upper limb) can lead to rotator cuff tearing and a shoulder that becomes sympt

PURPOSE: The treatment of massive anterior L-shaped rotator cuff tears (RCTs) remains challenging. This study focused on restoration of humeral head coverage by superiorly repositioning the rotator interval tissue. The study aimed (1) to present the short-term clinical and radiological outcomes of arthroscopic partial repair combined with rotator interval shift and (2) to identify risk factors associated with failure in restoring superior humeral head coverage.
METHOD: A retrospective review was conducted on arthroscopic rotator cuff repairs performed by a single surgeon between January 2018
