[Rotator cuff tear--an occupational disease? An epidemiological analysis].

BACKGROUND: In literature there are only few data which describe the influence of occupation on the development of rotator cuff disease. METHODS: In a retrospective study, 760 open rotator cuff repairs were analysed and related to the profession and occupational load. Exclusion criteria were traumatic tears and sports injuries. All male persons were statistically analysed and the data compared with occupational patterns of the region, obtained from the Federal Statistical State Office. RESULTS: Rotator cuff repairs were performed in 472 males who had no evidence for a traumatic origin. After

Mechanical properties of the different rotator cuff tendons in the rat are similarly and adversely affected by age.

Rotator cuff tendon tears and tendinopathies are common injuries affecting a large portion of the population and can result in pain and joint dysfunction. Incidence of rotator cuff tears significantly increases with advancing age, and up to 90% of these tears involve the supraspinatus. Previous literature has shown that aging can lead to inferior mechanics, altered composition, and changes in structural properties of the supraspinatus. However, there is little known about changes in supraspinatus mechanical properties in context of other rotator cuff tendons. Alterations in tendon mechanical p

The prevalence of rotator cuff weakness in patients with injured hands.

PURPOSE: The purpose of this study was to document bilateral rotator cuff strength in patients with unilateral hand or wrist disorders. METHODS: Fifty-seven patients who had suffered a unilateral injury or disorder isolated to their hand or wrist, distal to the distal radius, had their bilateral rotator cuff strength measured. RESULTS: A statistically significant decrease in strength was found in the ipsilateral shoulder for both elevation in the plane of the scapula (supraspinatus) and for elevated external rotation (infraspinatus). Younger patients and the nondominant extremity were affect

Acute Shoulder Injuries in Adults.

Acute shoulder pain lasting less than six months is a common presentation to the primary care office. Shoulder injuries can involve any of the four shoulder joints, rotator cuff, neurovascular structures, clavicle or humerus fractures, and contiguous anatomy. Most acute shoulder injuries are the result of a fall or direct trauma in contact and collision sports. The most common shoulder pathologies seen in primary care are acromioclavicular and glenohumeral joint disease and rotator cuff injury. It is important to conduct a comprehensive history and physical examination to identify the mechanis

Machine learning prediction for identifying individuals requiring rotator cuff repair: Shoulder muscle strength and questionnaires as the related factors.

BackgroundRotator cuff tears, a common cause of shoulder pain, often require surgery when conservative treatment fails. Arthroscopic repair is standard, though re-tear rates remain high.ObjectiveThis study aimed to identify the shoulder function questionnaires and strength tests that are significant factors for determining the necessity of rotator cuff repair and to build an effective machine learning model.MethodsThis study included 183 patients between January 2008 and December 2016. Among them, 89 patients underwent non-surgical treatment for rotator cuff injuries, whereas 94 underwent arth

Shoulder Cuff Tears in Athletes: Magnetic Resonance Imaging Findings.

Musculoskeletal injuries, particularly rotator cuff tears, are common in sport and can significantly impact athletes' performance and recovery. The rotator cuff stabilizes the glenohumeral joint, with the supraspinatus muscle the most susceptible to injury due to its central role in abduction and stabilization. Rotator cuff tears in athletes are in general the result of chronic microtrauma in an overused shoulder, but they can result after direct acute trauma. Sports-specific movements, such as overhead throwing, repetitive abduction, or heavy lifting, predispose athletes to different injury p

Arthroscopic rotator cuff repairs using suture anchors.

Arthroscopic rotator cuff repairs using titanium-alloy suture anchors are a new treatment option for active patients with shoulder injuries. Shoulder arthroscopy and arthroscopic repair procedures are alternative treatments to traditional open surgical procedures for Bankart lesions and rotator cuff tears. Distinct advantages of arthroscopic repair techniques include decreased patient tissue trauma and morbidity rates and shortened recovery and rehabilitation periods.

Conservative management of rotator cuff tear.

Ranking among the most prevalent of shoulder disorders, lesions involving shoulder rotator cuff strike million of patients in the United States at different points in their lives. Despite the fact that rotator cuff disease is a very common cause of pain and disability of the upper arm, a high proportion of patients is asymptomatic and unaware of performing daily living activities despite of a shoulder lesion. Given these drawbacks, surgeons are trying exploring whether conservative treatment is a viable option for the management of these injuries. This study presents a rationale to consider no

Arthroscopic subacromial decompression: a clinical review.

Arthroscopic subacromial decompression has become a popular technique supplanting the open Neer acromioplasty in many instances of chronic rotator cuff disease. A review of 61 consecutive decompressions with a minimum follow-up of 12 months was undertaken to evaluate preoperative criteria and surgical outcomes. Of the 61 patients, 53 patients with an average follow-up of 23 months were available for review. Thirty-four men and 19 women with an average age of 47 years comprised the study group. Eleven (21%) had full-thickness tears, 35 (66%) had partial-thickness injuries, and 7 (13%) had norma

Anterior Shoulder Instability in the Aging Population: MRI Injury Pattern and Management.

BACKGROUND. Literature on glenohumeral dislocations has focused on younger patient populations because of high recurrence rates. However, the spectrum of injuries sustained in younger versus older patient populations is reported to be quite different. OBJECTIVE. The purpose of this article is to describe MRI findings and management of anterior shoulder instability in the aging (≥ 60 years) population. METHODS. Shoulder MRI examinations of anterior glenohumeral dislocations in patients 40 years old and older were subdivided into groups younger than 60 years old or 60 years old and older and rev

Imaging of triathlon injuries.

Injuries in triathletes are common and are mostly overuse injuries. Rotator cuff tendinitis is the most common complaint from swimming, but the incidence of tendinopathy and rotator cuff tears on magnetic resonance imaging is comparable in triathletes without and with shoulder pain. Cycling injuries are mainly to the knee, including patellar tendinosis, iliotibial band syndrome, and patellofemoral stress syndrome, and to the Achilles tendon and the cervical and lumbar spine. Running is associated with most injuries in triathletes, during both training and racing, causing the athlete to discont

MRI of trauma and sports-related injuries of tendons and ligaments. Part I: Spine and upper extremities.

MRI is valuable in evaluation of tendons and ligaments in sports-related injuries. Tendons and ligaments show dark signal intensity on all MR sequences. Alteration in size, shape, continuity, and signal intensity of these structures can be well visualized with MRI. Impingement syndrome is the leading cause of atrophy and degeneration of the rotator cuff. Shape of the acromion, presence of subacromial spur, and involvement in certain sport activities that lead to shoulder instability may cause impingement of the rotator cuff. This article systematically reviews the injuries of tendons and ligam

Concomitant Coracoid Process Fracture and Massive Rotator Cuff Tear: A Rare Case Report and Surgical Management Approach.

Coracoid process fractures are rare injuries that can be overlooked, especially when they occur in isolation or alongside soft tissue damage. This case report describes a unique clinical presentation involving a displaced coracoid process fracture with a concomitant massive rotator cuff tear following high-energy trauma. The diagnostic process required a multimodal imaging approach to accurately assess both bony and soft tissue structures, which was critical in formulating an effective treatment plan. Surgical management involved open reduction and internal fixation of the coracoid fracture th

Compensatory mechanisms to maintain glenohumeral joint stability in rotator cuff tears of differing severity during activities of daily living: A musculoskeletal model simulation study.

Rotator cuff tears decrease glenohumeral joint stability; however, surrounding uninjured muscles could compensate for the reduced function of injured muscles to maintain joint stability. This study set out to analyze compensatory mechanisms that could maintain glenohumeral joint stability following rotator cuff tears of varying severity during normal daily life, using a musculoskeletal model simulation of rotator cuff tears. Fifteen healthy males performed 6 activities including shoulder flexion and abduction, contralateral shoulder reaching, head reaching, and lifting a 1 kg object placed on

Rehabilitation of overuse injuries of the shoulder.

The most common overuse injuries in sports medicine are rotator cuff tendinitis and shoulder impingement. Anatomic and biomechanical analysis of the shoulder complex demonstrates the likelihood of overuse injury to this region. Total rehabilitation includes relief of inflammation, restoration of normal joint arthrokinematics, and rotator cuff strengthening. The reduction of force overload to the shoulder in sport-specific activities and total arm strengthening are also emphasized in a comprehensive rehabilitation program.

Imaging Review of Subscapularis Tendon and Rotator Interval Pathology.

As the largest rotator cuff muscle, the subscapularis plays a major role in stabilizing the glenohumeral joint, in conjunction with surrounding rotator cuff structures. Injury to the subscapularis tendon can be isolated, but more commonly is seen in conjunction with supraspinatus tendon pathology. Injury can be associated with biceps pulley instability, superior labral anterior-posterior (SLAP) tears, humeral head subluxation, and anterosuperior and coracoid impingements. The involvement of the rotator interval can lead to what is called "the hidden lesion," due to its difficulty to diagnose d
