Sacrum osteotomy to change pelvic parameters: Surgical technique and 3D modeling.

PURPOSE: Some clinical situations, such as great sagittal imbalance, high-grade isthmic spondylolisthesis or sacral malunion could require a sacral osteotomy to decrease pelvic parameters, horizontalize the sacrum or correct sacral malunion. Here is described a novel technique to perform a sacral osteotomy to decrease pelvic parameters with a lumbo-pelvic construct, with first a sacral slope decrease, then a pelvic tilt decrease. METHODS: Simulations have been performed using tridimensional reconstructions of the lumbar spine and pelvis, made from CT-scan images of a healthy individual. A cad

Nerve sheath tumors involving the sacrum. Case report and classification scheme.

BACKGROUND: Healing success for fifth metatarsal fracture nonunions is challenging and hindered by risk factors for nonunion. Pulsed electromagnetic field stimulation (PEMF) promotes bone healing and can help overcome biological deficiencies in patients at risk. OBJECTIVE: The current study assessed the impact of PEMF treatment to promote successful bone healing in metatarsal nonunion. METHODS: A multi-center, retrospective study was conducted to evaluate PEMF treatment for fifth metatarsal fracture nonunion. Subjects undergoing treatment and eligible for treatment with PEMF (PhysioStim™ dev

Unusual location of myxopapillary ependymoma in the sacrum: Case report and review of the literature.

Myxopapillary ependymoma, a rare variant of ependymoma, commonly occurs in the conus medullaris or filum terminale. The rarity of these tumors can make their diagnosis and treatment challenging. This case report presents an atypical occurrence of myxopapillary ependymoma within the sacrum in a 68-year-old patient presented with a 3-month history of persistent left-sided low back pain radiating to the legs and fecal dysfunction. The patient underwent a sacral laminectomy and subtotal excision of the tumor, followed by adjuvant radiotherapy with favorable outcomes. This report highlights the sig

ACCURACY OF RADIOGRAPHIC DETECTION OF THE CRANIAL MARGIN OF THE DORSAL LAMINA OF THE CANINE SACRUM.

An elongated sacral lamina has been described as one of the contributing factors for dogs with cauda equina syndrome due to degenerative lumbosacral stenosis (DLSS); however, published evidence is lacking on the accuracy of radiographic screening for the presence of this lesion. Objectives of this prospective, cross-sectional cadaver study were to describe the accuracy and repeatability of detection of the cranial sacral lamina margin on plain lateral radiographs of the lumbosacral junction in dogs. Twenty-five medium and large breed canine cadavers were radiographed before and after placement

First report of a solid variant of aneurysmal bone cyst in the os sacrum.

We report on a 19-year-old woman with a rapidly growing, solid variant of aneurysmal bone cyst (solid ABC) in the right part of the lateral mass of the sacrum. On admission, the magnetic resonance imaging (MRI) scan disclosed an inhomogeneous low intensity mass right of the centre of the os sacrum with a diameter of 38 x 64 mm and a height of 56 mm on T1 and T2 weighted images. The mass showed homogenous high signal intensity on Gd-enhanced images. Computed tomography demonstrated an expansile osteolytic lesion and (99m)Tc-methylene diphosphonate bone scintigraphy revealed uniform accumulation

Aneurysmal bone cysts of the sacrum: a report of ten cases and review of the literature.

Ten cases of aneurysmal bone cysts are presented. Patients ranged in age from five years to 64 years. Treatment was resection or curettage; four patients had preoperative embolization. Treatment results were excellent. Six patients had no recurrence while recurrences in the other four patients were successfully treated by recurretage, two with adjunctive CyberKnife therapy. All ten patients are currently disease free for at least two years. Only two patients have residual neurologic deficit in the form of bowel or bladder dysfunction.

[Fractures of the sacrum caused by bone insufficiency. Meta-analysis of 508 cases].

OBJECTIVES: Fractures of the sacrum due to bone deficiency are not extremely uncommon although this cause has been overlooked for many years. We performed a meta-analysis of reported cases in order to determine the specific characteristics. METHOD: The characteristics of 493 cases of sacral fractures reported in the literature and those of 15 personal cases were studied. RESULTS: Most of the fractures occurred in women over 60 years of age. No trauma was identified in two-thirds of the cases. Clinical expression was not specific (back pain, sciatica, pelvic pain). Standard x-ray showed a fra

A combined abdominoperineal approach for the resection of a large giant cell tumor of the sacrum.

We describe the case of a 24-year-old woman with a giant cell tumor originating from the distal part of the sacrum, which was almost completely filling the pelvis and extending to the umbilicus. At laparotomy, the tumor was separated from all of the pelvic structures including the ureters. The sigmoid colon was transected and the tumor was dissected down to the pelvic floor. A separate perineal incision was made, the rectovaginal septum was entered, and the posterior vaginal wall was bisected to give better access. It was possible to mobilize the tumor completely after dividing the rectum 5 cm

Unilateral S-1 root compression syndrome caused by fracture of the sacrum.

Unilateral S-1 nerve root compression after an S-1 sacral fracture was found in an 18-year-old man after a motor vehicle accident. The positive myelogram, myelogram-computed tomogram, and magnetic resonance studies led to surgical intervention. Marked bony callous formation contiguous with the S-1 alar fracture protruded into the canal and was responsible for tethering the S-1 nerve root. A right L-5 hemilaminectomy, an L-4, L-5 and L-5, S-1 medial facetectomy, and foraminotomy facilitated nerve root decompression. Postoperatively, the patient was markedly improved. The authors suggest a more

[Agenesis and dysgenesis of the os sacrum and the os coccygis: a contribution (author's transl)].

Agenesis and/or dysgenesis of the os sacrum and os coccygis is reported in seven children (4 boys and 3 girls between the ages of 2 and 15). Two of the children had diabetic mothers. The cause may be hereditary or even exogenous (vitamin A deficiency, O (2) deficiency, maternal toxoplasmosis, maternal diabetes). Our cases were compared with 160 cases presented in the literature. The symptomatic therapy, which must be determined on the basis of neurologic, muscular, and urologic findings, is discussed.

Implantable Electronic Stimulation Devices from Head to Sacrum: Imaging Features and Functions.

Electronic stimulation devices are implanted in various locations in the body to decrease pain, modulate nerve function, or stimulate various end organs. The authors describe these devices using a craniocaudal approach, first describing deep brain stimulation (DBS) devices and ending with sacral nerve stimulation (SNS) devices. The radiology-relevant background information for each device and its imaging appearance are also described. These devices have a common design theme and include the following components: (a) a pulse generator that houses the battery and control electronics, (b) an insu

Sacrum fracture following L5-S1 stand-alone interbody fusion for isthmic spondylolisthesis.

We report a 72-year-old man with a rare sacral fracture following stand-alone L5-S1 anterior lumbar interbody fusion for isthmic spondylolisthesis. The man underwent a minimally invasive management strategy using posterior percutaneous pedicle fixation and partial reduction of the deformity. We also discuss the current literature on fusion procedures for isthmic spondylolisthesis.

Giant Cell Tumor of the Sacrum: A Narrative Review of Management Challenges and Modalities.

Background/Objectives: Sacral giant cell tumor of bone (GCTB) is a rare, mostly benign but locally aggressive neoplasm that carries significant diagnostic and treatment challenges due to its anatomic complexity, proximity to sacral nerve roots as well as the blood vessels, and potential impact on bowel, bladder, sexual, and lumbopelvic function and stability. This narrative review aimed to synthesize current evidence on the epidemiology, clinical presentation, diagnostic evaluation, classification, management strategies, outcomes, and surveillance of sacral GCTB. Methods: A focused literature

A statistical shape model of the human female sacrum: An integrative analysis of form.

Sacral shape has been investigated in the context of many disciplines, such as anthropology, forensics and in clinical contexts. The female population is of special interest, given the complex interaction between locomotor, thermoregulatory and obstetric demands on the pelvis. However, interpretation of results between current studies is limited by the use of a diverse range of parameters, often offering an oversimplified and fragmented view on sacral shape. We investigated sacral shape in a young adult female population between 18 and 40 years old, using a statistical shape model based on 109

Sex differences in hip bone and sacrum morphology: a geometric morphometric analysis.

The pelvis plays a crucial role in sex estimation when reconstructing an individual's biological profile from skeletal remains. The present study aims to evaluate the sex differences in size and shape of the hip bones and sacrum using geometric morphometrics and to compare the degree of sexual dimorphism captured by size and shape variables within and between these skeletal elements. The study sample included the abdominal computed tomography scans of 255 adult Bulgarians (125 males and 130 females). Surface models of the pelvis were generated and used for landmark digitization. The three-dime

The first case of methotrexate-associated lymphoproliferative disorder in the sacrum: a case report.

Methotrexate (MTX) is a drug used for treating rheumatoid arthritis. Recently, the reported incidence of methotrexate-associated lymphoproliferative disease (MTX-LPD) has increased, especially in Japan. Extranodal involvement is observed in half of MTX-LPD cases. However, only a few spinal lesions have been reported, with none in the sacrum. Additionally, Epstein-Barr virus (EBV) infection has also been implicated in the pathogenesis of MTX-LPD. Herein, we describe the case of a 74-year-old woman with MTX-LPD in the sacral spine who complained of severe back pain and nocturnal pain. Radiograph
