Performance considerations of prosthetic actuators for round-window stimulation.

Round-window (RW) stimulation has improved speech perception in patients with mixed hearing loss. In cadaveric temporal bones, we recently showed that RW stimulation with an active prosthesis produced differential pressure across the cochlear partition (a measure related to cochlear transduction) similar to normal forward sound stimulation above 1 kHz, when contact area between the prosthesis and RW is secured. However, there is large variability in the hearing improvement in patients implanted with existing modified prosthesis. This is likely because the middle-ear prosthesis used for RW stim

Intracochlear pressure measurements during acoustic shock wave exposure.

INTRODUCTION: Injuries to the peripheral auditory system are among the most common results of high intensity impulsive acoustic exposure. Prior studies of high intensity sound transmission by the ossicular chain have relied upon measurements in animal models, measurements at more moderate sound levels (i.e. < 130 dB SPL), and/or measured responses to steady-state noise. Here, we directly measure intracochlear pressure in human cadaveric temporal bones, with fiber optic pressure sensors placed in scala vestibuli (SV) and tympani (ST), during exposure to shock waves with peak positive pressures

Inner ear inflammation and round window otosclerosis.

Recently, it has been suggested that otosclerosis represents the host's ongoing immunologic response to measles or other viral antigens. Documentation of past inflammation within the inner ear would serve as further evidence that this mechanism may be at play in the pathogenesis of the disease. Among the characteristic signs of prior inflammation in the inner ear is the presence of lamellar bone at the site of inflammation. This has been described in the temporal bone of a patient with immune-mediated deafness and with the temporal bones of experimental models of immune-mediated inner ear dise

Effects of cyclophosphamide on the pathogenesis of cytomegalovirus-induced labyrinthitis.

Cyclophosphamide was used in this study to define the contribution of the inflammatory response relative to direct cytopathic effects of guinea pig cytomegalovirus (GPCMV) in inducing sensorineural hearing loss in the guinea pig. The eighth nerve compound action potential (CAP) threshold on day 7 after inoculation of GPCMV into the scala tympani was an average of 35 dB greater for control animals than for those that were immunosuppressed with daily intraperitoneal injections of cyclophosphamide (20 mg/kg body weight). The amount of GPCMV antigen in the cochlea, detected immunohistochemically d

The round window membrane: otological observations.

The membrane of the round window serves as a barrier between the middle and inner ear and has a well-established role in cochlear physiology. It is important in many otological conditions: otitis media, otosclerosis, trauma, sequelae to drugs reaching the middle ear, tumors, congenital dysplasia, sudden deafness. We here describe the first electron microscopic findings in round window membranes from adult humans, and various conditions in reference to these findings. Since surgical lesions are made in this membrane during cochlear implantation, and by some otologists in treating vertigo, preli

Efficacy of intracochlear administration of betamethasone on peripheral vestibular disorder in the guinea pig.

We evaluated the effect of steroid hormone on vestibular function in a guinea pig of peripheral vestibular disorder. The right lateral semicircular canal was surgically damaged, and after surgery, animals were treated with 0.1 mg/ml of betamethasone in saline, 1 mg/ml of betamethasone in saline, or saline only, which was administrated directly into the scala tympani by osmotic pump. Rotation tests were performed, and the post-rotatory nystagmus (PRN) ratio (PRN number after counterclockwise rotation/PRN number after clockwise rotation) was calculated. The PRN ratio was recovered to normal at 5

The effects of pressure on cochlear microphonics in experimentally induced hydropic ears in the guinea pig.

The effect of perilymphatic pressure on the cochlear microphonics (CM) threshold was examined in guinea pigs with experimentally induced hydrops. When a pressure of 300 mm H2O was applied to the scala tympani of the basal turn, the 500 Hz CM threshold shifts in ears during the 1st week following obliteration of the endolymphatic sac and duct were significantly smaller when compared with shifts in normal ears. Larger threshold shifts were then regained at 4-12 weeks after surgery. When acetazolamide was administered to hydropic animals, the effect of the increase of perilymphatic pressure on th

Cochlear changes after herpes simplex virus infection.

After direct inoculation of herpes simplex virus (HSV) into the scala tympani of the guinea pig, the tectorial membrane showed various morphological changes: atrophy, roll-up and dot formation. Immunofluorescent and electronmicroscopic studies revealed that the changes were due to HSV infection. The findings were compared with those observed in the temporal bones of a 77-year-old patient who suffered from sudden deafness. The tectorial membranes of both temporal bones showed various changes identical with those observed in experimental viral labyrinthitis. This supports the view that sudden de

Various Surgical Techniques for Cochlear Implantation in an Ossified Cochlea: A Systematic Review.

BACKGROUND: Cochlear ossification, otherwise known as labyrinthitis ossificans, is not an uncommon encounter in candidates for cochlear implantation (CI) that may necessitate alternative surgical approaches due to the insertion difficulties. We aimed to review the literature for the documented approaches for CI in cases with cochlear ossification. METHODS: This review was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines on articles that described their surgical approach for CI in cases with cochlear ossification. A comprehensive

Case report of mite infestation of the external ear in Carollia perspicillata: Histological features of the ear.

Bats host a diverse assemblage of ectoparasites that occupy specific anatomical microhabitats, yet the histological effects of these associations remain poorly documented. Here, we report a case of mite infestation of the external ear in the Neotropical bats species Carollia perspicillata and Carollia brevicauda, with a detailed histological characterisation of the auricle and inner ear of C. perspicillata. Bats were captured during biodiversity monitoring programmes in Antioquia, Colombia, and mites were collected and identified morphologically. Histological and histochemical analyses of ear

The evaluation of a slim perimodiolar electrode: surgical technique in relation to intracochlear position and cochlear implant outcomes.

PURPOSE: In cochlear implantation (CI), the two factors that are determined by the surgeon with a potential significant impact on the position of the electrode within the cochlea and the potential outcome, are the surgical technique and electrode type. The objective of this prospective study was to evaluate the position of the slim, perimodiolar electrode (SPE), and to study the influence of the SPE position on CI outcome. METHODS: Twenty-three consecutively implanted, adult SPE candidates were included in this prospective cohort study conducted between December 2016 and April 2019. Mean age

Intracochlear Drug Delivery Through the Oval Window in Fresh Cadaveric Human Temporal Bones.

HYPOTHESIS: Drug delivered to the oval window can diffuse to the apex of the human cochlea. BACKGROUND: We have previously demonstrated that zoledronate, a nitrogen-containing bisphosphonate, can arrest the sensorineural hearing loss in cochlear otosclerosis. We have also shown that, in animals, delivery of bisphosphonate into the cochlea can dramatically increase delivery efficiency. Intracochlear drug delivery has the potential to increase local concentration of drug while decreasing the risk of systemic toxicity. In the present study, a fluorescently labeled bisphosphonate compound (6-FAM-

Cochlear microperfusion: experimental evaluation of a potential new therapy for severe hearing loss caused by inflammation.

HYPOTHESIS: Cochlear microperfusion will be a useful treatment of severe sensorineural hearing loss caused by inflammation. BACKGROUND: Viruses, bacteria, and autoimmunity can initiate inflammation in the inner ear. The acute phase is associated with elevations in cytokines, nitrous oxide, and cellular infiltrates and the breakdown of the blood-labyrinthine barrier. The chronic phase leads to irreversible ossification of the labyrinth. METHODS: The authors developed cochlear microperfusion to facilitate removal of inflammatory cells and their byproducts during the acute phase of inflammation

Ganglion cell populations in labyrinthitis ossificans.

Ossification of the perilymphatic spaces of the inner ear is frequently found in patients with profound deafness who may be candidates for cochlear implantation. The ossification may be a hindrance in electrode insertion and has previously been considered a relative contraindication to implantation. The quantity of peripheral auditory elements remaining in a series of patients with labyrinthitis ossificans has not been reviewed. We performed a histopathologic study of 16 temporal bones with partial cochlear ossification of multiple etiologies. The organ of Corti and spiral ganglion were graphi

A lumped-element model of the chinchilla middle ear.

An air-conduction circuit model was developed for the chinchilla middle ear and cochlea. The lumped-element model is based on the classic Zwislocki model of the same structures in human. Model parameters were fit to various measurements of chinchilla middle-ear transfer functions and impedances, using a combination of error-minimization-driven computer-automated and manual fitting methods. The measurements used to fit the model comprise a newer, more-extensive data set than previously used, and include measurements of stapes velocity and inner-ear sound pressure within the vestibule and the sc

Superparamagnetic Nanoparticle Delivery to the Cochlea Through Round Window by External Magnetic Field: Feasibility and Toxicity.

Introduction. The objective of this study was to evaluate the feasibility and toxicity of superparamagnetic iron oxide nanoparticles (SPIONs) administered into the cochlea through the round window (RW) by an external magnetic field. Materials and Methods. In 5 Wistar rats, the left RW was punctured. SPIONs suspended in hyaluronic gel (5 mg/mL) were applied in the RW niche and covered by a muscle graft. The nanoparticles were mobilized using a rare earth magnet (0.54 T) held in 4 consecutive positions around the head. The right ear served as control. Hearing function was monitored by auditory b

Clinicopathologic study of leptomeningeal carcinomatosis involving the temporal bone.

The temporal bone pathology of a 71-year-old man with bilateral sensorineural hearing loss and facial paralysis caused by diffuse metastatic leptomeningeal carcinomatosis is described. The origin of this malignant disease was an extremely rare entity, a transitional cell carcinoma of the renal pelvis. Histopathologic study of the temporal bone demonstrated that tumor cells filled the internal auditory meatus, infiltrated into the Rosenthal's canals, and reached the scala tympani of the basal turn of the bilateral cochleas. The vestibulocochlear nerve and facial nerve trunks in the internal aud

[An experimental study of hearing loss caused by mumps virus in guinea pigs].

Guinea pigs were inoculated with mumps virus into the scala tympani to establish an experimental animal model of viral labyrinthitis. Electrophysiological reactions of the inner ears were assessed by ECoG and histopathological changes of the cochleas were investigated under light and electron microscopes. Viral lesions or/and proliferation were demonstrated in 19 out of 25 cochleas. Compound action potential (CAP)N1 thresholds for the nineteen cochleas showed very significant elevations (P < 0.001) after inoculation with mumps virus. This study revealed that viral labyrinthitis caused by mumps
