Secondary intrachoroidal cavitation in a case of iridofundal coloboma.

Intrachoroidal cavitation is a morphological entity usually described in high myopic eyes. It is usually seen in peripapillary location, though macular location has also been reported. It has been associated with optic disc tilting and posterior staphyloma. We report a case of a 27 year old female who presented to us with recent onset scotoma in her visual field. On examination she had a fundal coloboma with intercalary membrane detachment with intrachoroidal cavitation at the edge of the coloboma. We hypothetise that this intrachoroidal cavitation is a secondary phenomenon and is responsible

Infliximab-induced demyelination causes visual disturbance mistaken for recurrence of HLA-B27-related uveitis.

PURPOSE: To report a patient treated with infliximab for ankylosing spondylitis and HLA-B27 uveitis presenting with a scotoma in his left good eye due to infliximab-induced occipital demyelination mistaken for a uveitis recurrence. DESIGN: Case report. METHODS: Review of clinical features, visual field, and investigational procedures. RESULTS: Loss of visual function could not be explained by mere ophthalmic examination. Magnetic resonance imaging showed several demyelinating lesions, explaining the patient's symptoms and signs. Infiliximab was discontinued and the scotoma disappeared. CON

Circumpapillary optical coherence tomography angiography differences in perimetrically affected and unaffected hemispheres in primary open-angle glaucoma and the preperimetric fellow eye.

PURPOSE: Evaluation of circumpapillary vessel density (VD) and perfusion density (PD) on optical coherence tomography angiography (OCTa) in mild-moderate glaucoma patients having unilateral visual field defects, with their fellow eyes and controls. METHODS: Both eyes of 24 patients having a definitive nasal step or arcuate scotoma in one hemisphere of one eye only, and 24 controls, underwent OCTa. RESULTS: In eyes with a superior field defect, the superior/inferior quadrant ratios, (SQ/IQ) of 3 mm scan of VD and PD were significantly higher in eyes with a superior arcuate scotoma than fellow

Visual field defects in optic neuritis and anterior ischemic optic neuropathy: distinctive features.

BACKGROUND: We analyzed the value of visual-field defects in the differential diagnosis of optic neuritis (ON) and non-arteritic anterior ischemic optic neuropathy (AION). METHODS: Ninety-nine consecutive patients with acute-onset optic neuropathy formed the basis for this study. Compressive and vasculitic neuropathies were excluded. Eighty-six patients fulfilled the criteria for either ON (50 patients): < or = 35 years, normal disk, recovery of visual function, or AION (36 patients): > or = 60 years, swelling of the disk, no recovery of visual function. Without knowledge of other clinical da

Microperimetry Reliability Assessed From Fixation Performance.

PURPOSE: Microperimetry provides an accurate assessment of central retinal sensitivity due to its fundus-tracking capability, but it has limited reliability indicators. One method currently employed, fixation loss, samples the optic nerve blind spot for positive responses; however, it is unclear if these responses arise from unintentional button presses or from tracking failure leading to stimuli misplacement. We investigated the relationship between blind spot scotoma positive responses (termed scotoma responses) and fixation. METHODS: Part 1 of the study involved a custom grid of 181 points

'I didn't see that coming': simulated visual fields and driving hazard perception test performance.

BACKGROUND: Evidence is limited regarding specific types of visual field loss associated with unsafe driving. We use novel gaze-contingent software to examine the effect of simulated visual field loss on computer-based driving hazard detection with the specific aim of testing the impact of scotomata located to the right and left of fixation. METHODS: The 'hazard perception test' is a component of the UK driving licence examination, which measures speed of detecting 15 different hazards in a series of real-life driving films. We have developed a novel eye-tracking and computer set up capable o

[Visual field defect in eyes with branch retinal artery occlusion].

There have been few clinical studies that analyzed quantitatively the visual field defects observed in eyes with branch retinal artery occlusion. In the current report we retrospectively studied Humphrey static perimetric results in 7 eyes and Goldmann kinetic perimetric results in 8 eyes with branch retinal occlusion. All but one eye showed altitudinal-like field defects. The static perimetric results demonstrated large difference between sensitivities in each pair of upper and lower measured points adjacent to the horizontal line nasal to Mariott's scotoma, which indicated a steep slope betw

Myopic Peripapillary Pits with Spatially Corresponding Localized Visual Field Defects: A Progressive Japanese and a Cross-Sectional European Case.

We report 2 peripapillary pit cases, in which the isolated visual field defects spatially correspond to the pit-related retinal nerve fiber layer and optical coherence tomography (OCT) angiography (OCTA) perfusion damage areas. A high myopic eye of a 39-year-old Japanese male patient, and a moderate myopic eye of a 47-years old Caucasian female patient were evaluated with OCT, OCTA, and visual field testing for peripapillary pits and spatially corresponding localized visual field defects. In the Japanese patient a temporal and in the Caucasian patient an inferotemporal peripapillary pit was co

Spatial agreement between Goldmann visual field defects and fundus autofluorescence in patients with birdshot chorioretinopathy.

BACKGROUND: The purpose of this paper is to study the spatial agreement between visual field defects and ultra-wide field (UWF) fundus autofluorescence (FAF) in patients with birdshot chorioretinopathy (BSCR). The study is a retrospective, cross-sectional analysis of a university uveitis practice. Eight (8) eyes of five (5) patients with BSCR were included. Inclusion criteria were ability to fixate reliably. Goldmann visual fields (GVF) and UWF FAF were obtained, digitalized, and standardized. Analysis was performed by measuring areas of overlap of hypo-autofluorescent FAF lesions and GVF scot

Early postlesional visuomotor capacity after V1 damage in the macaque.

Damage to the primary visual cortex (V1) causes field defects in conscious vision, yet stimuli in the affected field may still guide saccades and other visuomotor responses ("blindsight"). Chronic blindsight is well documented, but early postlesional saccade profiles to scotoma stimuli remain poorly characterized. We mapped visuomotor functions in two macaques 3 to 4 weeks after a unilateral V1 lesion using a perimetry-like saccade assay up to 35° eccentricity, with analyses of saccade direction and catch-trial baselines. Targets had either unpredictable onset, as in clinical perimetry, or tem

Severe Linear Retinal Burn from a Handheld Laser Pointer in a Pediatric Patient: A Case Report.

INTRODUCTION: This paper presents a unique case of handheld laser-induced retinal injury associated with maculopathy and linear scotoma in a pediatric patient. RESULTS: A 12-year-old boy with no significant medical or ocular history presented with a scotoma in his right eye after using a handheld green laser pointer, labeled as 5 mW, 532 nm. Visual acuity was 20/20 in both eyes. Fundus examination revealed focal retinal damage at the macula with a linear burn extending from the 10 o'clock to 4 o'clock position, consistent with laser-induced phototoxicity. Fundus autofluorescence (FAF) demonst

[Modified threshold Amsler grid testing].

The conventional Amsler grid uses suprathreshold illumination, and hence may fail to detect early scotomas. When the grid is viewed through cross-polarizing filters to reduce luminance to visual threshold, the test becomes far more sensitive. The authors expanded the standard Amsler grid from 400 to 1600 squares and viewed under threshold luminance to examine the visual fields of glaucoma, retinitis, optic neuropathies and brain tumors; the modification was found much more sensitive for the detection of central and paracentral light scotomas.

Focal functional and microstructural changes of photoreceptors in eyes with acute zonal occult outer retinopathy.

PURPOSE: Acute zonal occult outer retinopathy (AZOOR) is characterized by an acute zonal loss of outer retinal function with minimal ophthalmoscopic changes in one or both eyes. We present a patient with AZOOR whose ultrastructural and functional findings were followed for 8 months. CASE: A 22-year-old woman developed an acute central scotoma in her right eye. Her best-corrected visual acuity (BCVA) was 0.5 OD and 1.2 OS. The ophthalmoscopic examinations, fluorescein angiography, and full-field electroretinograms (ERGs) were normal in both eyes. The amplitudes of the multifocal ERGs (mfERGs)

Investigation of visual field defects in strabismic and anisometropic amblyopes with the Octopus program G1.

Using the Octopus G1 program, we investigated the visual fields of amblyopic and healthy contralateral eyes in 21 patients with strabismic amblyopia and 14 patients with anisometropic amblyopia. All subjects had a visual acuity of 0.7-0.06. A central scotoma was detected in 85.7% of the patients with strabismic amblyopia and in 79% of those with anisometropic amblyopia. The mean maximal depth of central scotomas in subjects with strabismic amblyopia was 6.81 +/- 4.74 dB (mean +/- SD); in those with anisometropic amblyopia, it was 6.64 +/- 4.34 dB. This difference was statistically not signific

Branch retinal artery occlusion secondary to Bartonella henselae infection in a 13 year-old.

PURPOSE: To summarize the case of a 13 year-old boy diagnosed with a BRAO secondary to B. henselae infection. OBSERVATIONS: The patient presented with a sudden, unilateral, and painless scotoma. Fundoscopic findings and multimodal imaging were consistent with a BRAO with associated areas of intraretinal whitening along the involved artery. Upon further questioning, the patient reported having 15 cats at home. Antibodies were positive for B. henselae. The patient was treated with oral doxycycline 100 mg twice daily for 2 months with complete resolution of the retinal findings and the scotoma.

Localized bi-nasal macular edema in optic chiasmal syndrome.

A 28-year-old healthy male complaining of vision loss in his right eye was discovered to have localized bi-nasal macular edema in the presence of a pituitary adenoma. The presence of a junctional scotoma composed by a central scotoma in the right eye associated with superior temporal quadrantanopia in the fellow eye was seen. The pattern detected in the visual field suggested the presence of an expansive mass at the level of the optic chiasm. Optical coherence tomography findings also revealed subtle macular thickness beyond normal in the superior and nasal quadrants of both maculae. This repo

[Progression of early glaucomatous visual field defects].

We studied the mode of progression of visual field defects in glaucoma the using static computerized perimetry (OCTOPUS 201). As a result of analysis using Delta Program, the modes of progression of the visual field defects were divided into two groups, general depression and local depression. The cases of general depression were younger than those of local depression. In cases of local depression, we examined the direction of progression in isolated scotomas. Isolated scotomas located at the nasal visual field tended to progress toward the nasal and peripheral side.

Tunnel Vision Prismatic Field Expansion: Challenges and Requirements.

PURPOSE: No prismatic solution for peripheral field loss (PFL) has gained widespread acceptance. Field extended by prisms has a corresponding optical scotoma at the prism apices. True expansion can be achieved when each eye is given a different view (through visual confusion). We analyze the effects of apical scotomas and binocular visual confusion in different designs to identify constraints on any solution that is likely to meet acceptance. METHODS: Calculated perimetry diagrams were compared to perimetry with PFL patients wearing InWave channel prisms and Trifield spectacles. Percept diagr
