[Resection arthroplasty in the treatment of hallux valgus].

Sixty-six feet operated on for hallux valgus in 1984 were reviewed. Keller-Brandes' operation was performed in all cases, together with Lelièvre's "fibrous cerclage" and flexor hallucis longus tendon anchoring to the sesamoid pad, as described by A. Viladot. In most cases, satisfactory clinical correction was achieved and the patients could wear standard shoes. More than half of the cases presented partial limitation of movement at the first metatarsophalangeal joint; however, flexion power of the big toe was preserved, and the range of motion was wide enough to allow a painless and limp-free

Magnetic resonance imaging of the palmar aspect of the equine podotrochlear apparatus: normal appearance.

The purpose of this study was to describe the normal magnetic resonance (MR) imaging characteristics of the palmar structures of the equine podotrochlear apparatus by means of retrospective evaluation of MR imaging studies of 16 cadaver limbs. The articular aspect of the distal sesamoid bone was not evaluated in this study. Equine digits were imaged with a human knee radiofrequency coil in a 1.5 T magnetic field, using spin echo (SE) T1-weighted, turbo spin echo proton density (TSE PD)-weighted with and without fat saturation (FS), and FS TSE T2-weighted sequences. The limbs were dissected aft

Interobserver and intraobserver reproducibility of hallux valgus angular measurements and the study of a linear measurement.

BACKGROUND: The intermetatarsal angle (IMA), the hallux valgus angle (HVA), and the lateral sesamoid malalignment in relation to the first metatarsal are among the most commonly employed parameters to estimate the severity of the deformity. The aim of this study was to compare HV angular measurements and a linear measurement among 4 observers to determine its intraobserver and interobserver reliability, to find out whether this linear measurement technique is more reproducible than those already-described angular measurements. METHODS: The IMA, the HVA, and the distance between the lateral co

Hallux limitus and hallux rigidus. Clinical examination, radiographic findings, and natural history.

Numerous clinical features of hallux limitus/rigidus have been previously reported as isolated entities based on individual case review or myopic observations. Few attempts have been made to synthesize a comprehensive natural history which correlates the inter-relationship of these findings. Frequently unrecognized or overlooked subtle clinical findings, such as shoe-wear patterns, hyperkeratoses locations, and gait disturbances, precede significant radiographic changes or painful degenerative arthritis by months to years. Recognition of these subtle clinical features will aid in establishing

Osteonecrosis of the tibial and fibular sesamoids in an aerobics instructor.

Osteonecrosis of the sesamoids is a fairly uncommon clinical entity. The development of this condition involving both sesamoids has never been presented in the American literature. After extirpation of the sesamoids and interdigital fusion, the patient returned to her regular activities, including dance.

Differential diagnosis of carpal and tarsal ankylosis on dry bones: Example from the catacomb of Saints Peter and Marcellinus (Rome, 1st-3rd century AD).

Bone ankylosis is a pathological feature that may occur in many chronic diseases involving joints. In paleopathology, it is therefore challenging to attribute such a change to a specific condition. Here, we illustrate the differential diagnosis by discussing the lesions observed on an incomplete skeleton from the Roman period, recovered from the catacomb of Saints Peter and Marcellinus (Rome, Italy). The skeleton exhibits several bone changes, including ankylosis on both feet and the left hand. The right tarsal fusion is accompanied by soft tissue ossifications involving the plantar aponeurosi

An experimental approach to surgery of the equine fetlock and its associated structures.

An illustrated approach to the medial aspect of the equine fetlock joint is described, whereby the collateral ligaments are transected. This approach is suggested as a possible technique for metatarsal or metacarpal bone fracture repair by retrograde pinning, arthrodesis of the fetlock joint and other joint and sesamoid bone surgery. The histological evidence of satisfactory healing is presented and the advantages and disadvantages are discussed.

[Long term results of the Keller-Brandes method of hallux rigidus surgery].

Between the years 1971 and 1975 a surgical treatment of hallux rigidus according to Keller Brandes was performed on 293 patients. From 100 patients with 137 operated feet a clinical and roentgenological examination was evaluated 12 to 16 years after surgery. The subjective assessment showed in 92% of the feet very good or good results. Important for the success of the operation is the correct amount of bone removal of the proximal phalanx (between 1/3 and 1/2) and the resultant width of the nearthrotic joint space. The analysis of the podographic examination and the amount of retraction of the

Sesamoidectomy for hallux sesamoid fractures.

BACKGROUND: Hallux sesamoid fractures are challenging to treat. Symptomatic nonunion is a common problem after nonoperative treatment. Surgical fixation of the fracture can result in successful union, but is technically challenging and can be associated with prolonged return to activities (RTA). Sesamoidectomy is an alternative surgical option that may provide reliable outcomes and allow an earlier RTA in athletes. The purpose of this case-series study was to evaluate a cohort of athletic patients with a hallucal sesamoid fracture treated with sesamoidectomy. METHODS: A total of 24 patients w

Skeletal anomalies in trisomy 21 as an example of amplified developmental instability in chromosome disorders: a histological study of the feet of 21 mid-trimester fetuses with trisomy 21.

In a previous radiographic study on the feet of 71 adults with trisomy 21 we found, in comparison to control individuals, an increased prevalence of biphalangeal toes and metatarsophalangeal sesamoid bones. The present histological study on the feet of 21 mid-trimester fetuses with prenatally diagnosed trisomy 21 confirms results of the earlier study. At both stages of development these minor bone anomalies have about the same frequency, thus suggesting 1) that they are selectively neutral, and 2) that they reflect a basic (innate) failure of ordered morphogenesis. Our observation that the nor

Fetal development of external nose cartilages.

There seems to be no information on when and how the nasal capsule cartilage is divided into the lateral nasal cartilage (LNC) and greater alar cartilage (GAC). Likewise, there is no information of timing and sequence of development of the medial crus of GAC. We examined histological sections from 22 human fetuses with 45-350 mm of crown-rump length (approximately 9-40 weeks of gestational age). The LNC and GAC were separated from the antero-inferior marginal part of the nasal capsule cartilage at 10-12 weeks. The initial sesamoid was a wavy bar and it did not accompany a joint cavity. The LNC

Endoscopic Plantar Partial Sesamoidectomy for the Management of Metatarsalgia of the First Metatarsal Head After Arthrodesis of the First Metatarsophalangeal Joint.

First metatarsophalangeal arthrodesis has been used to treat various pathologies of the great toe with good clinical outcome. First metatarsal head metatarsalgia can occur after first metatarsophalangeal arthrodesis. Surgery is indicated if conservative measures cannot alleviate the symptoms. If there is isolated excessive plantar pressure of the first metatarsophalangeal joint without significant sagittal deformity of the first metatarsophalangeal joint, plantar partial ostectomy of the sesamoid bone(s) is indicated. The purpose of this technical note is to describe the technical details of e

Ossicles of knee menisci. Report of seven cases.

Seven cases of ossicles in the posterior horn of the medial meniscus revealed that: trauma is not necessarily an antecedent; some knees with ossicles are asymptomatic; some knees may be treated conservatively and without surgery; ossicles can be distinguished from loose bodies under fluoroscopic examination. Microscopically the ossicles are living bone. Although the etiology is not proven, perhaps merit should be given to the theory of a vestigial or sesamoid-like bone.

Sesamoid disorders of the first metatarsophalangeal joint.

The sesamoid complex is located centrally and plantar to the first metatarsal head, where they are imbedded within the plantar plate, which transmits 50% of body weight and more than 300% during push-off, is susceptible to numerous pathologies. These pathologies include sesamoiditis, stress fracture, avascular necrosis, osteochondral fractures, and chondromalacia, and are secondary to these large weight-bearing loads. This article discusses sesamoid conditions and their relationship with hallux limitus, and reviews the conditions that predispose the first metatarsophalangeal joint to osteoarth

Objective assessment of lameness in cattle after foot surgery.

Assessment of lameness in cattle after foot surgery is important to monitor the recovery period, to improve the long-term success and the cows` welfare. This longitudinal multicenter retrospective study was carried out to evaluate the usefulness of automated tools of weight bearing and gait analysis following foot surgery to support the clinician to monitor lameness in cattle. For this purpose, the effect of involvement of different anatomical structures and the use of different surgery methods on gait parameters of post-operative recovery was assessed. The study consisted of 2 experiments and

Radiological Anatomy of the Pelvis and Pelvic Limb of the Greater Cane Rat (Thryonomys swinderianus).

Greater cane rats (Thryonomys swinderianus) are now being captive reared and domesticated in the sub-Saharan Africa because of increase in their demand for biomedical research and traditional medicine and as a source of meat. This research was performed to provide the normal radiological anatomy of the pelvis and pelvic limb in greater cane rats for a reference in biomedical research, in anatomical studies, and in clinical use. Radiological examination of the pelvis and right pelvic limb was done in five greater cane rats. Radiological results were correlated with bones of the pelvis and right

Numeric and morphological studies of the African lion (Panthera leo leo) pectoral limb.

BACKGROUND: The Importance of the appendicular skeleton in the conformation, shape and physiology of wild animals especially carnivores for adaptation and survival cannot be overemphasized, as limited and obsolete information on the bones of the forelimb necessitated this study. Matured adult African lions (Male and female) that weighed 173 kg and 112 kg were obtained at different times after post mortem examinations of their carcasses. Bone preparation was achieved via cold water maceration after proper skin and muscle removal to a barest minimum. RESULTS: The acromion process of the scapula

Correction of Recurred Hallux Valgus Deformity by Endoscopic Distal Soft Tissue Procedure.

The underlying reason for recurrence of hallux valgus deformity after bunion surgery is multifactorial and includes surgeon-based and patient-based factors as well as original components of deformity initially unaddressed at the index procedure. Surgical treatment of a recurred hallux valgus deformity should be undertaken using the same guidelines for correction of a primary hallux valgus deformity. It requires correction of bony alignment, restoration of joint congruity, and achievement of soft tissue balance. The purpose of this Technical Note is to describe the details of endoscopic soft ti
