Does botulinum toxin injection in parotid glands interfere with the swallowing dynamics of Parkinson's disease patients?

BACKGROUND: During recent years, sialorrhea in Parkinson's disease (PD) has been treated with botulinum toxin type A (BT-A); however, there are no data describing its effects on swallowing dynamics in this group of patients. OBJECTIVE: To investigate whether injection of BT-A into the parotid glands interferes with the oropharyngeal swallowing dynamics of patients with Parkinson's disease. METHODS: Sixteen patients with diurnal sialorrhea were selected and evaluated during the "on" period by a standardized questionnaire and swallowing videofluoroscopy before and 30 days after ultrasound-guid

Diagnosis and treatment of speech disorders in children with Moebius syndrome.

BACKGROUND: Moebius syndrome (MS) is characterized by congenital bilateral paralysis of the facial and abducens nerves. Clinical features include feeding problems, dysarthria, dysphagia, sialorrhea, strabismus, and lack of facial expression. Patients with MS frequently present with dysphagia during infancy. Further on during childhood a severe speech disorder is a common feature. However, articulation deficits in patients with MS are scarcely reported in the related scientific literature. OBJECTIVE: The aim of this study is to describe speech deviations, intelligibility and sialorrhea in pati

Bilateral submandibular gland excision and parotid duct ligation.

Sialorrhea affects a significant number of children with cerebral palsy and other neuromuscular disorders. It can lead not only to social embarrassment but also to severe medical issues including chronic aspiration. There are numerous medical and surgical options, which include oral medications and transdermal patches, botulinum injection, and various forms of surgical ligation or excision of the major salivary glands. In this chapter, the authors describe the surgical management of sialorrhea, highlighting surgical pearls necessary for success.

A randomized trial of botulinum toxin A for treatment of drooling.

The authors compared the efficacy of three different doses (18.75, 37.5, and 75 MU per parotid gland) of botulinum toxin A (BTX-A; Dysport, Ipsen Pharma, Germany) injections vs vehicle in patients with sialorrhea (n = 32) using a single-center, prospective, double-blind, placebo-controlled dose-finding study. The primary endpoint was achieved with 75 MU BTX-A without treatment-related adverse events, suggesting BTX-A is a safe and effective treatment for patients with sialorrhea.

Herbal medicine and acupuncture relieved progressive bulbar palsy for more than 3 years: A case report.

RATIONALE: Progressive bulbar palsy (PBP) is a type of motor neuron disease (MND). The main symptoms include dysarthria, dysphagia, tongue muscle atrophy and fasciculations. This disease is generally severe and develops rapidly. Due to the lack of effective treatment, many patients with MND in China turn to traditional Chinese medicine treatment for help. We successfully relieved dysphagia and sialorrhea in a patient with PBP for 3 years with herbal medicine and acupuncture. PATIENT CONCERNS: The patient was a 68-years-old woman with PBP and suffered from severe dysphagia and sialorrhea. DIA

Successful treatment of drooling in children with neurological disorders with botulinum toxin A or B.

Drooling is a common and severe problem in children with neurological disorders and is caused by a disturbed coordination of orofacial and palatolingual muscles. Botulinum toxin could be a successful option to reduce excessive sialorrhea in children with neurological disorders. In 30 children with cerebral palsy or neurodegenerative disorder we injected under ultrasound guidance either botulinum toxin A or botulinum toxin B into the parotid and submandibular glands on both sides. All injections were well tolerated without general anaesthesia. Drooling severity at baseline and reduction of sial

Emerging drugs for autonomic dysfunction in Parkinson's disease.

INTRODUCTION: Autonomic dysfunction, including orthostatic hypotension (OH), sialorrhea, sexual dysfunction, urinary dysfunction and constipation is a common feature of Parkinson's disease (PD). Even though its treatment has been recognized as a major unmet need in PD, there is a paucity of clinical trials to assess their treatment. AREAS COVERED: Evidence about the efficacy and safety of available treatments for autonomic dysfunction is summarized. Potential targets for upcoming therapies are then discussed in light of what is currently known about the physiopathology of each disorder in PD.

Reduction of salivary flow with Scopoderm TTS.

Scopoderm transdermal therapeutic system (TTS) is a form of application permitting programmed release of scopolamine through the skin into the bloodstream. Scopoderm TTS is indicated for prevention of nausea and vomiting associated with motion sickness. The most common side effect associated with its use is dryness of the mouth. We have been using Scopoderm TTS for reducing salivary flow in patients with sialorrhea or with difficulties in swallowing normal amounts of salivary secretions. Good results were achieved with minimal discomfort from adverse effects. We recommend the use of Scopoderm

Clinical and radiographic follow-up after the Wilkie procedure at 28 years: A case report.

Rerouting of the parotid ducts posteriorly to drain into the tonsillar fossae (Wilkie procedure) was initially designed to address drooling in patients with cerebral palsy. This procedure was subsequently modified to include bilateral submandibular gland excision and extended to apply to other etiologies of sialorrhea. Our literature review failed to identify report of long-term follow-up beyond 2 decades following this procedure. We describe a 33-year-old female with spastic cerebral palsy who underwent the Wilkie procedure to treat sialorrhea and, 25 years later, developed right-sided facial

Incidence of emesis induced by low-dose intravenous medetomidine administration in dogs: a retrospective study of 155 cases.

This study aimed to determine the incidence of emesis in dogs receiving a low dose of intravenous medetomidine. Anesthetic records of dogs that received ≤3 µg/kg medetomidine intravenously before anesthesia induction were retrospectively reviewed. Data included demographics, doses, and the presence of emesis, retching, sialorrhea, or lip licking. A total of 155 cases met the criteria. The administered dose of medetomidine was 3.0 (0.5-3.0) µg/kg (median [range]). Emesis occurred in 3 of 155 dogs (1.9%), while retching, sialorrhea, and lip licking occurred in two, two, and five dogs, respective

Can clozapine be used by dental practitioners to increase salivary flow in patients with dry mouth? A scoping review.

OBJECTIVES: Clozapine, an atypical antipsychotic used to treat people with schizophrenia, has been proposed as a possible treatment for salivary gland hypofunction. This scoping review investigated the available literature on clozapine's impact on salivary flow, in order to determine whether it could be used by dental practitioners in low doses as a treatment for dry mouth. DATA SOURCES: An electronic search was completed using Ovid MEDLINE (1996 to Nov 2021). Key MeSH search terms included "clozapine," "Clozaril," "salivation," "salivary flow rate," "sialorrhea," "hypersalivation," and "droo

A Case Report of Systemic Intoxication Following Onabotulinum Toxin A Injections Into the Salivary Glands in a Patient With Spinal Muscular Atrophy Type 1.

BACKGROUND: Sialorrhea in spinal muscular atrophy (SMA) is caused by bulbar weakness, which is aggravated by low oromotor tone rather than saliva overproduction. Botulinum toxin (BTX) reduces sialorrhea by preventing acetylcholine release from the presynaptic secretory parasympathetic nerve terminals. An important adverse effect of BTX, as highlighted in its black box warning, is a systemic spread of BTX leading to botulismlike symptoms including dysphagia, muscle weakness, and death. These symptoms may be more pronounced in peripheral motor neuropathic disorder population such as SMA, whose n

Postoperative technetium scanning in patients with submandibular duct diversion.

Submandibular duct diversion is a common procedure for refractory sialorrhea in children. The procedure reroutes Wharton's ducts from the floor of the mouth to the tongue base. As the majority of saliva in the resting state is produced by the submandibular glands, rerouting markedly decreases sialorrhea. However, the procedure has been criticized in that diversion may cause fibrosis and stricture of the ducts. The gland would atrophy, and the physiologic functions of saliva would be lost. Glandular function of six patients with cerebral palsy (mean age, 14.7 years) was evaluated by technetium

Clinical pharmacokinetics of atropine oral gel formulation in healthy volunteers.

Sialorrhea or drooling is a common problem in children and adults with neurodevelopmental disorders. It can negatively impact the quality of life due to its physical and psychological manifestations. Providers commonly prescribe atropine eye drops for topical administration to the oral mucosa, as an off-label treatment to manage sialorrhea. However, the off-label use of atropine eye drops can be associated with medication and dosing errors and systemic side effects. To address these limitations of treatment, we developed a mucoadhesive topical oral gel formulation of atropine as an alternative
