Introduction: surgical management of skull base meningiomas.

Meningiomas represent the most common primary intracranial neoplasm treated by neurosurgeons. Although multimodal treatment of meningiomas includes surgery, radiation-based treatments, and occasionally medical therapy, surgery remains the mainstay of treatment for most symptomatic meningiomas. Because of the intricate relationship of the dura mater and arachnoid mater with the central nervous system and cranial nerves, meningiomas can arise anywhere along the skull base or convexities, and occasionally even within the ventricular system, thereby mandating a catalog of surgical approaches that

Radiology of the nasopharynx: pictorial essay.

The nasopharynx, located just below the central skull base, is a difficult area to evaluate clinically. Endoscopic examination provides useful information but cannot delineate submucosal disease. Benign lesions of the nasopharynx are relatively uncommon. They include Thornwaldt cyst, juvenile angiofibroma, haemangioma, haemangiopericytoma, Kimura's disease, branchial cleft cyst, oncocytoma, amyloidoma, and non-ossifying fibromyxoid tumours. The most common malignant lesions are carcinomas and lymphomas. Other malignant neoplasms such as adenocystic carcinomas are rarely seen. The purpose of th

[Extracranial meningioma presenting as a polyp of the outer ear canal].

Meningiomas are the most frequent benign intracranial tumor. Up to 20% of these neoplasms may eventually extend beyond the skull, particularly when tumor spread affects the temporal bone. We report a clinicopathological observation of an extracranial meningothelial meningioma that was diagnosed morphologically using immunohistochemical techniques. The tumor presented as polyp in the ear canal of a 70-year-old woman who had a history of ear disease and seizures and was under medical treatment. The growth was associated with a right temporal lobe tumor involving the petrosal bone that had been d

Xanthoma of the temporal bone and skull base.

Xanthomata are soft tissue tumors composed of lipid-laden "foamy" histiocytes associated with cholesterol clefts and inflammation. They are considered to be specialized granuloma rather than true neoplasms and are usually associated with disorders of lipid metabolism, most commonly one of the hyperlipoproteinemia syndrome. A rare case of a xanthoma involving the temporal bone and causing extensive destruction of the skull base is presented. Otalgia and infection are the most common presenting symptoms of this disorder, with multiple cranial nerve palsies resulting in some cases. Treatment incl

A rare case of Maffucci's syndrome combined with tuberculum sellae enchondroma, pituitary adenoma and thyroid adenoma.

Maffucci's syndrome is a rare, congenital mesodermal dysplasia combined with dyschondroplasia and haemangiomatosis, and there are only about 150 reported cases. This syndrome is often combined with other neoplasms. Our case was associated with goitre, enchondroma of the tuberculum sellae and pituitary adenoma; the latter brought about disturbance of vision. Including ours, four cases of Maffucci's syndrome associated with pituitary adenoma are found in the literature. In the case of Maffucci's syndrome, it is necessary to examine associated disease as well as associated pituitary adenoma and s

Primary Meningeal Extraskeletal Osteosarcoma in a Domestic Shorthaired Cat: Case Report and Review of Comparative Pathology.

This case describes the presentation and diagnosis of an extraskeletal osteosarcoma arising in the cranial meninges of a domestic shorthaired cat. Clinical signs were compatible with a space-occupying cerebral lesion, and magnetic resonance imaging revealed a multilobular mass with extracranial and intracranial components, which asserted marked forebrain mass effect and apparently originated from the frontal and parietal bones. Due to clinical deterioration and poor prognosis, the cat was euthanized and a post-mortem examination performed. Gross lesions confirmed a solitary neoplasm, firmly at

Cancellous osteoma of the maxillary tuberosity: case report.

The osteoma is a benign, bone-forming lesion that is not considered a true neoplasm. It almost exclusively affects the flat bones of the skull and face. Jaw lesions may cause facial deformity and impair oral function. Multiple osteomas may be associated with Gardner's syndrome. Clinically and radiographically, osteomas may need to be differentiated from other bone-forming lesions. This article describes a cancellous osteoma of the left maxillary molar and tuberosity area and differentiates it from other benign and malignant lesions, including the parosteal osteosarcoma.

The incidence of vertebral body metastases.

Over 10 years, 842 consecutive autopsies on patients dying from a malignant neoplasm were evaluated microscopically to determine the incidence of spinal and skeletal metastases. Vertebral wedging, sclerosis and areas of osteolysis were defined as features of spinal metastases. The incidence was 30.6%, 65% of which arose from lung, breast or lymphoma and myeloma. The frequency of extraspinal metastases was 9.8% with 66% arising from the lung, liver, lymphoma or myeloma; they were mainly situated in the ribs, skull and clavicle.

Panhypopituitarism resulting from Hodgkin's disease of the nasopharynx.

Nasopharyngeal involvement by Hodgkin's Disease is rare and may result in deafness, headaches, nasal obstructions and other symptoms. Compression of the pituitary gland with resultant panhypopituitarism has not been reported in nasopharyngeal Hodgkin's Disease. This paper documents a patient in whom Hodgkin's Disease of the nasopharynx eroded the bony sella turcica and compressed the pituitary gland causing anterior pituitary insufficiency. The patient presented with a skull x-ray and history compatible with a pituitary neoplasm. The diagnosis was established only by surgical exploration.

Sinonasal glomangiopericytoma presenting as refractory epistaxis: imaging, embolisation and pathological correlation.

We describe a case of sinonasal glomangiopericytoma (GPC), a rare perivascular tumour accounting for less than 0.5% of sinonasal neoplasms, presenting with refractory unilateral epistaxis. An anticoagulated male in his 60s required admission, nasal packing and unilateral left internal maxillary artery embolisation to achieve haemostasis.Following stabilisation, nasoendoscopy identified a vascular polypoid lesion arising high in the left nasal cavity near the olfactory cleft. MRI demonstrated a contrast-enhancing mass abutting the cribriform plate without orbital or skull-base invasion, which h

Intracranial Extracranial Huge Meningioma: Report of a Case.

Meningioma accounts for 15% to 20% of all intracranial neoplasms. Intracranial meningioma is a common brain tumor but huge meningiomas with extracranial extensions are extremely rare. Various symptoms, including neurological deficits and epileptic seizures are very frequent in these cases. Surgery still remains the principal form of the treatment and must be preceded by appropriate preoperative diagnostics. We report an unusual case of a huge meningioma of the brain (intracranial-extracranial extension) invading and destroying the skull bone and subcutaneous and cutaneous tissue. A 44-year old

Rare case of a primary intracranial chondrosarcoma.

BACKGROUND: Intracranial chondrosarcomas comprise 6% of all skull base neoplasms and account for 0.15% of all intracranial tumors. They are potentially fatal, invading the brain and elevating intracranial pressure by progressive enlargement. Diplopia and headache are the most common clinical symptoms. Previous data indicate a particularly aggressive behavior of intracranial chondrosarcomas. CASE REPORT: A 17-year-old female patient presented to the hospital with focal seizures. A magnetic resonance imaging (MRI) scan revealed a brain tumor located in the right meso-temporal region. Total tumo

Inferior mastoidectomy-hypotympanic approach for surgical removal of glomus jugulare tumors: an anatomical and radiologic study emphasizing distances between critical structures.

The technique of inferior mastoidectomy-hypotympanic dissection, with preservation of the middle and inner ear structures, has been established for radical surgical removal of glomus tumors involving the skull base. In order to study the gross anatomic differences and correlate with the computed tomography (CT) scan, 20 human temporal bones were dissected and accurate measurements between vital structures were made. This study revealed a wide variation in distances between the neurovascular structures, whereas the distances between inner ear structures were not statistically different. This or

Monocular visual loss in a disseminated colorectal malignancy-A case report of a rare skull base metastasis.

Neoplasms of the orbit may be primary, secondary (infiltration from the adjacent structures), or metastatic (from distant structures). It can be divided into 3 histologic categories: benign, benign but locally aggressive, and malignant. Primary and secondary orbital tumors, including intra-orbital and optic nerve tumors are uncommon observations in daily medical practice. Orbital tumors represent approximately 0.1% of all tumors and approximately 18% of all orbital diseases. We report a case of a 42 year old male patient with colorectal malignancy with basal skull metastasis involving the orbi

Juvenile nasopharyngeal angiofibroma.

Juvenile nasopharyngeal angiofibromas (JNAs) are rare, benign, highly vascular, locally aggressive tumors that primarily affect male adolescents. Historical treatment of these neoplasms has been primarily surgical. In the past decade, endoscopic resection of JNAs has become a viable and promising surgical treatment option. Endoscopic resection has many advantages over traditional open techniques, including better cosmesis, decreased blood loss, shortened hospital stays, and equivalent or improved recurrence rates. Emerging endoscopic technology continues to push the boundaries of resection of

[The nodule of the small intestine--a rare case of benign metastasizing leiomyoma].

The diagnosis of benign metastasizing leiomyoma BML is a casuic case. The term is used to describe a entity of neoplasms characterized by a proliferation of smooth muscle cells with low mitotic index, usually located outside female genital organs. In the world literature there are reports of benign metastasizing leiomyomas in the lungs, skull base and spine, the lymph nodes of the small pelvis and the heart. In the following study the authors analyzed a case of a 72-year-old woman in which, during hysterectomy due to uterine leiomyomas, a nodule of the small intestine was found, confirmed hist

Osteoma of the tongue.

Osteomas are benign bony neoplasms commonly found in long bones and in the skull and facial bones. Osseous growths within soft tissue structures of the head and neck are rare. Review of the English literature reveals only 31 cases of osteomas within the soft tissues of the tongue and 2 in the buccal mucosa. We report on a patient with a bony hard asymptomatic mass at the base of the tongue, which was found to be an osteoma by biopsy. Complete removal was declined by the patient. There was no progression during a six-month period of follow-up.

Spindle-cell tumours and hypoglycaemia.

The light microscopy and clinical features of two patients with extrapancreatic tumours and hypoglycaemic episodes are described-together with the electron microscopy findings in one patient. The primary tumour of one patient arose within the skull and later metastasized to the liver, while the other patient had a locally recurrent intrathoracic tumour. Neither the intracranial origin nor the ultrastructural features support the concept of a mesothelial origin for these tumours, and they should be referred to as spindle-cell tumours associated with hypoglycaemia. There are ultrastructural simi

Auricular complications in parotid, temporal bone, infratemporal fossa, and lateral skull base surgery.

Neoplasms located in the parotid region, temporal bone, infratemporal fossa, and lateral skull base represent a challenge due to their difficult anatomic location and surrounding neurovascular structures. A variety of surgical approaches are appropriate to access this area, although several of them can place the auricular blood supply in danger. If the auricular blood supply is compromised, ischemia and, eventually, avascular necrosis of the auricle can occur. Auricular necrosis often can cause patients a delay in adjuvant radiation therapy and result in the need for additional reconstructive
