OBJECTIVE: Sawtooth waves (STW) are a characteristic EEG feature of REM sleep but their source and function are unknown. We previously reported stereotypical properties of STW at stage REM onset, and alterations in bulbar postpolio syndrome. This study analyzes STW features throughout REM, in order to test the hypothesis that sawtooth wave activity may be predictable and have a consistent relationship across REM periods.
METHODS: Twenty polysomnographic recordings were scored for occurrence, duration, and frequency of STWs. STW density was calculated based on the number of bursts/min REM and

We investigated motor phenomena during rapid eye movement (REM) sleep in 13 patients with group A xeroderma pigmentosum aged from 11 to 39 months, and compared them with those obtained from 12 age-matched controls. At the time of sleep study, they had no abnormality on routine electrophysiological examinations. The amount of REM sleep and the incidence of motor phenomena during REM sleep in patients were similar to those in age-matched controls. However using the newly designated indices, we demonstrated disturbance on both the tonic motor inhibition occurring during the whole REM sleep period

A 24-year-old woman with a left pontine hematoma showed marked asymmetry in the EEG of REM sleep, suggesting that a unilateral pontine lesion is sufficient to disrupt normal REM sleep EEG in the ipsilateral hemisphere. Other REM sleep characteristics (rapid eye movements, muscle atonia) were unaffected by this lesion.

AIM: Analyse heart rate variability (HRV) of preterm neonates undergoing a polysomnography in relation to the occurrence of abnormal cardiorespiratory events on one hand and the type of sleep states on the other hand.
METHODS: To quantify nonlinear HRV, the numerical noise titration technique is used, adapted to neonatal heart rate data. HRV is calculated for 30 preterm neonates with mean post-conceptional age of 36.4weeks, divided into three groups according to the occurrence of abnormal events during the polysomnographies and the eventual home monitoring.
RESULTS: Periods of non-REM sleep

REM sleep is considered a 'hyperassociative' state enhancing the connections between weakly related concepts, thereby facilitating memory restructuring - a critical process proposed in creative problem-solving. We empirically explore this hypothesis by testing participants on a riddle before and after a 90-minute incubation period including rapid eye movement (REM) sleep, only non-REM sleep, or wakefulness. We quantified memory restructuring by computing differences in problem-related semantic memory network (SemNets) properties between pre- and post-incubation. For each sleep stage, we extrac

Rapid eye movement (REM) phases of sleep are consistently disrupted after trauma and in post-traumatic stress disorder (PTSD). REM disruption impairs cued fear learning, a core element of trauma processing linked to PTSD risk and symptom persistence. Configural threat learning depends on the hippocampus and may be influenced by REM sleep, though its role in humans remains unclear. Here, 55 healthy participants were randomly assigned to REM sleep fragmentation or a control group without sleep disruption for two nights. On the final day, participants completed a configural threat learning task w

Rapid-eye-movement (REM) sleep is generated in the brainstem, but the brainstem population dynamics that drive transitions to REM sleep remain largely unknown. Here, combining mouse Neuropixels recordings and dimensionality reduction, we found that population activity in the midbrain and pons is dominated by two components, one of which captures strong infraslow fluctuations in neural activity. During transitions from non-REM (NREM) to REM sleep, the population activity followed a stereotypic trajectory that was preceded by an increase in the infraslow component. Our analysis revealed-across a

PURPOSE: REM-related obstructive sleep apnea (OSA) presents a paradox where a low apnea-hypopnea index (AHI) may mask significant disease. We compared polysomnography, comorbidities, and oxygenation metrics across OSA phenotypes to determine if AHI-based severity classifications underestimate the nocturnal hypoxemia in REM-related OSA.
METHODS: Retrospectively, 378 OSA patients were classified as REM-related (AHIREM ≥2× AHINREM; AHINREM <5·h⁻¹, n = 30), REM-aggravated (AHIREM ≥2× AHINREM; AHINREM ≥5·h⁻¹, n = 63), or stage-independent (n = 285). We analyzed sleep architecture, AHI values, oxyg

Although growing evidence suggests that cholinergic basal forebrain degeneration is linked to cognitive impairment and axial motor symptoms in Lewy body disorders, the cholinergic contribution to their prodromal phase remains largely unknown. Herein, we aimed to address three important yet unresolved problems focusing on prodromal Lewy body disorders: (i) to examine whether and where basal forebrain degeneration begins; (ii) to determine how such alterations are related to other brain morphometric changes and monoaminergic deficits; and (iii) to investigate the extent to which basal forebrain

Background and Clinical Significance: Wilson's disease (WD) is an inherited, multisystem disorder of copper metabolism, resulting in pathological copper accumulation in various tissues (predominantly the liver and brain) and leading to secondary organ damage and corresponding clinical manifestations. Sleep disorders are frequent in neurodegenerative disorders, but remain underdiagnosed and poorly characterized in WD. Case presentation: We describe the case of a 51-year-old patient with WD presenting predominantly with neurological symptoms, who underwent routine video-polysomnography (v-PSG).

Periodic limb movements of sleep (PLMS) are involuntary, sleep-related phenomena characterized by periodic episodes of repetitive, stereotypical movements of the limbs. The presence PLMS increases with age but are frequently comorbid with other sleep disorders and other medical, neurologic, and psychiatric disorders, and can be triggered by medications and psychoactive substances. PLMS typically occur during non-rapid eye movement sleep and are generally absent during rapid eye movement (REM) sleep, with exception of patients with REM behavior disorder (RBD). However, even in the absence of RB

The formal identification and naming of rapid eye movement (REM) sleep behaviour disorder (RBD) in 1985-1987 is described; the historical background of RBD from 1966 to 1985 is briefly discussed; and RBD milestones are presented. Current knowledge on RBD is identified with reference to recent comprehensive reviews, allowing for a focus on research priorities for RBD: factors and predictors of neurodegenerative phenoconversion from isolated RBD and patient enrolment in neuroprotective trials; isolated RBD clinical research cohorts; epidemiology of RBD; traumatic brain injury, post-traumatic str

Patients with Parkinson's disease (PD) show a broad heterogeneity in clinical presentation, and subtypes may already arise in prodromal disease stages. Isolated REM sleep behaviour disorder (iRBD) is the most specific marker of prodromal PD, but data on clinical subtyping of patients with iRBD remain scarce. Therefore, this study aimed to identify iRBD subtypes. We conducted comprehensive clinical assessments in 66 patients with polysomnography-proven iRBD, including motor and non-motor evaluations, and applied a two-step cluster analysis. Besides, we compared iRBD clusters to matched healthy

BACKGROUND AND OBJECTIVES: Although orthostatic hypotension (OH) can be an early feature of autonomic dysfunction in isolated REM sleep behavior disorder (iRBD), no large-scale studies have examined the frequency of OH in iRBD. In this study, we prospectively evaluated the frequency of OH in a large multicenter iRBD cohort.
METHODS: Participants 18 years or older with video polysomnogram-confirmed iRBD were enrolled through the North American Prodromal Synucleinopathy consortium. All participants underwent 3-minute orthostatic stand testing to assess the frequency of OH, and a Δ heart rate/Δ

BACKGROUND/AIM: Sleep loss is proposed as a trigger for manic episodes in bipolar disorder in humans. It has been shown that sleep and wakefulness can affect changes in mitochondrial gene expression, oxidative phosphorylation (OXPHOS) activity, and morphology in the brain. In this study, we investigated alterations in mitochondrial bioenergetic function in the brain of rats after 72-h rapid eye movement sleep deprivation (REM-SD).
MATERIALS AND METHODS: Alterations in the mitochondrial DNA (mtDNA) copy number were detected in the prefrontal cortex and hippocampus through amplification of mito

Introduction: Fatigue is one of the most common and disabling symptoms of Parkinson’s Disease (PD). The occurrence and clinical features of fatigue in patients with prodromal PD remain largely elusive. This study aimed to investigate the prevalence and clinical characteristics of fatigue in patients with idiopathic/isolated REM sleep behavior disorders (iRBD). Methods: A total of 97 polysomnography-confirmed iRBD patients were enrolled in this study. A comprehensive neurological assessment (including motor and non-motor assessment) was performed. Fatigue was assessed using the Fatigue Severity

The rapid eye movement sleep behavioural disorder (RBD) population is an ideal study population for testing disease-modifying treatments for synucleinopathies, since RBD represents an early prodromal stage of synucleinopathy when neuropathology may be more responsive to treatment. While clonazepam and melatonin are most commonly used as symptomatic treatments for RBD, clinical trials of symptomatic treatments are also needed to identify evidence-based treatments. A comprehensive framework for both disease-modifying and symptomatic treatment trials in RBD is described, including potential treat
