Snoring surgery: which one is best for you?

Excessive snoring is a common problem that is frequently treated surgically. In the early 1980s, uvulopalatopharyngoplasty was introduced to the United States as the first surgical treatment for excessive snoring. It remains in common use, but its limitations created an incentive to develop a procedure that is as effective, but safer and more economical. Several other surgical procedures for snoring were developed, including laser-assisted uvulopalatoplasty, palatal stiffening operations, and radiofrequency ablation. Each of these procedures has its own advantages and limitations; which proced

Effect of Breathe Right nasal strip on snoring.

Snoring is a significant problem both for the patient and for the bedpartner. It is well known that nasal stuffiness can contribute to snoring, and sleep quality may deteriorate because of the snoring. Nasal dilation can reduce snoring and improve sleep. Thirty-five habitual snorers (18 female, 17 male) and their bedpartners participated in an open label study. The patients were diagnosed as heavy snorers after they underwent overnight polysomnography showing that their apnoea indexes were below 5, thus sleep apnoea patients were not included in the study. The participants and their partners f

The differentiation of snoring mechanisms using sound analysis.

Ten subjects known to suffer from heavy snoring but not obstructive sleep apnoea were studied using the technique of sleep nasendoscopy. The mechanism of snoring was noted for each and sound recordings of the snoring noise were made. Six subjects were observed to snore using their soft palate only, three snored using only their tongue base and one snored using a combination of palate and tongue base. The sound recordings were subjected to computer analysis of waveform and frequency. Palatal flutter snoring and tongue base snoring appear to have distinct waveform and frequency patterns which al

[Diurnal hypersomnolence and chronic snoring: an epidemiological study].

Excessive daytime somnolence (EDS) is highly correlated to the presence of obstructive sleep apnea syndrome (OSAS). It appears that interest in such disorders may also be extended to extraneurological fields. The Authors report the results of a study on EDS in 1,146 adult subjects seen consecutively by different physicians. Excessive Somnolence during normal activity was reported in 4.5% of the subjects studied. In a subgroup of habitual snorers, EDS was observed in 8.3% of the subjects and in 2.0% in a group of non-snorers. The subjects with EDS has a mean age and body mass index (BMI) greate

Obstructive sleep apnoea in adults presenting with snoring.

Snoring is a common disorder, and may be associated with obstructive sleep apnoea, although there is little published information on the incidence of apnoea in snorers. This study aimed to assess the upper airway and to relate the findings to sleep study data in a population of patients referred by their general practitioners with loud snoring. Each patient had a full history, weight and height measurements, nasal examination, rhinomanometry, peroral grading of the oropharyngeal features, and fibreoptic pharyngoscopy with a modified Muller manoeuvre, followed by a sleep study. The results in o

Digital signal analysis of snoring sounds in children.

Snoring sounds of 5 children with sleep-dependent compromised breathing were recorded. Underlying diseases were large adenoids, adeno-tonsillar hyperplasia, hemangioma of the tongue-base and Rubinstein-Taybi's syndrome associated with micrognathia. For comparison snoring sounds of a 42-year-old chronic snorer were analyzed. Time series of the power spectral density of the breath sounds were calculated as well as the mean power spectra of the recordings. The results show that the frequency spectrum reflects the pathomechanism responsible for the production of the abnormal breath sound. In adult

Nocturnal asthma: snoring, small pharynx and nasal CPAP.

We studied two populations of patients who snored and had frequent nocturnal asthma attacks: ten overweight men presenting with typical obstructive sleep apnoea syndrome, and a group of five adolescents with regular snoring and an increase in negative inspiratory oesophageal pressure during stage II non-rapid eye movement (NREM) and rapid eye movement (REM) sleep. All subjects presented cranio-mandibular abnormalities at cephalometric evaluation, with a narrow space behind the base of the tongue. Both populations were treated with nasal continuous positive airway pressure (CPAP) during sleep.

[Surgical treatment of chronic snoring. Description and indications].

Chronic rhonchopathy groups all clinical forms of a disease having a common functional sign: snoring. Surgical treatments form part of a therapeutic strategy which includes medical treatment, essentially reduction in body weight, and nocturnal respiratory prosthetic treatments, essentially continuous positive pressure respiration. First intention surgery is an uvulopalatopharyngoplasty (UPP) with or without tonsillectomy. In the severe forms of chronic rhonchopathy, after failure of UPP, major surgery:protrusion mandibular osteotomy or reduction glossectomy can be associated with the other tre

When and why to treat the child who snores?

Obstructive sleep-disordered breathing (SDB) can result in cardiovascular and neurocognitive morbidity as well as adversely affect behavior, growth, quality of life, and nocturnal continence. This article summarizes the latest evidence regarding the morbidity related to obstructive SDB, commenting on the impact of severity of obstruction, that is, the difference in effects seen of moderate to severe obstructive sleep apnea syndrome (OSAS) compared to those of mild OSAS or primary snoring. The impact of therapy is discussed, focusing on which children are likely to benefit from treatment interv

Combined radiofrequency assisted uvulopalatoplasty in the treatment of snoring.

Despite its minimally invasive character and superiority over placebo, the clinical efficacy of interstitial radiofrequency surgery (RF) of the soft palate in primary snoring is limited. In particular, excessive soft tissue at the palatal arches (webbing) and uvula hyperplasia are common phenomena, but cannot be addressed with interstitial RF surgery. Aim of this study was to assess the efficacy and morbidity of combined RF assisted uvulopalatoplasty (RF-UPP) in the treatment of snoring. Twenty-two patients with primary snoring (AHI < 15, BMI < 32) were included in the prospective clinical tri

[Clinical evaluation of the patient with a chronic snoring disease].

INTRODUCTION: Sleep disordered breathing patients usually undergo an ENT clinical examination before any therapeutic decision is taker. This clinical examination should be predictive about the occurrence of OSAS, reproductible and should determine the sites of obstruction in the upper airways. We have evaluated the clinical ENT examination and compared it with the bibliography in order to standardize it. MATERIALS AND METHODS: We have done the same clinical ENT examination to 181 patients. 127 had a standar polysomnography. RESULTS: The patient's mean age was 49.6 years, the mean BMI was 29.

Snoring, sleep apnoea and the role of dental appliances.

This article describes the problems of snoring and obstructive sleep apnoea, together with an outline of treatment options. The Glasgow approach, whereby patients are investigated at a sleep clinic and a custom-made mandibular advancement device is made in semi-soft material, is also described. We have demonstrated the acceptability and effectiveness of a simple appliance in patients with varying dental states, some with simple snoring and some with mild to moderate sleep apnoea. Our experience relates to around 260 patients, extending over a period of 4 years with good success. The simple int

Treatments for snoring: a comparison of somnoplasty and an oral appliance.

OBJECTIVES: Objectives of the study are 1) to test the effectiveness of somnoplasty (radiofrequency volumetric tissue reduction of the soft palate) for the control of loud, socially disruptive, snoring; 2) to test the long-term efficacy of this treatment by spouse report; and 3) to compare the effectiveness of somnoplasty with another treatment. STUDY DESIGN: Twenty patients with complaints of loud snoring and a respiratory disturbance index no greater than 15 respiratory events per hour with sleep-related episodes of oxygen desaturation no lower than 80% were offered an experimental treatmen

[Surgical options for treatment of snoring and sleep apnea].

Simple snoring and obstructive sleep apnea are part of a complex disease process with a high prevalence. Surgical treatment options have been developed by otolaryngologists over the last decades. Knowledge of different surgical procedures is required to address the various findings during preoperative assessment. Patients with nasal airway blockage due to septal deviation, large turbinates or polyps may benefit from endonasal surgery. Obstructive tonsils, palatal webbing, and an enlarged uvula require surgical approaches including the soft palate and tonsils. The controversy on indications and

From snoring to sleep apnea in a Singapore population.

We have no information on snoring and obstructive sleep apnea (OSA) in our population, which is predominantly Chinese. Our perception is that sleep apnea syndrome is more common than the 2-4% prevalence (Young et al., 1993) often quoted, judging from the experience in our sleep disorder unit. We studied the snorers in an adult population in Singapore and then went on to see how many snorers suffer pathological apnea and sleep apnea syndrome. Room partners, 220 of them aged 30-60 years, were interviewed for their observation of snoring among each other. 106 consecutive habitual loud snorers of
