Adolescent females and hormonal contraception: a retrospective study in primary care.

PURPOSE: The aim of this study was to assess change in the number of adolescent females prescribed hormonal contraception in primary care following the publication in the United Kingdom of the Social Exclusion Unit report on Teenage Pregnancy. METHODS: We conducted a retrospective observational study of 320 primary care practices in Scotland. Hormonal contraceptive prescribing to girls aged <16 years and those aged 16-19 years was assessed for April 1 to March 31 for the study years 2000-2001 to 2005-2006 from Scottish primary care practice data. RESULTS: Between 2000-2001 and 2005-2006, the

Wearing many hats: The art of addiction psychiatry in private practice.

ObjectiveTo argue that contemporary addiction psychiatry, particularly in private and mixed-sector practice, is best understood as an integrative clinical craft rather than a narrow detoxification or prescribing role.ConclusionsAddiction psychiatrists increasingly work in rooms where trauma, ambivalence, psychiatric comorbidity, physical illness, family distress, medication complexity, social adversity, and crisis arrive together. Good care requires more than technical knowledge. It requires trauma-informed engagement, careful listening, psychotherapy fluency, formulation, medical competence,

Provider barriers to prescribing HAART to medically-eligible HIV-infected drug users.

We aimed to identify factors associated with a medical provider's resistance to prescribing HAART to medically-eligible HIV-infected illicit drug users. In four US cities, a mailed, self-administered survey queried 420 HIV care providers about patients' characteristics and barriers to care. Adjusted odds ratios (AOR) and 95% confidence intervals (CI) were estimated using logistic regression. Providers identified as resistant to prescribing HAART to medically-eligible HIV-infected illicit drug users were more likely to be non-physicians (AOR=1.89, 95% CI: 1.04-3.46), to work in populations with

Identifying provider, patient and practice factors that shape long-term opioid prescribing for cancer pain: a qualitative study of American and Australian providers.

INTRODUCTION: Prescribing long-term opioid therapy is a nuanced clinical decision requiring careful consideration of risks versus benefits. Our goal is to understand patient, provider and context factors that impact the decision to prescribe opioids in patients with cancer. METHODS: We conducted a secondary analysis of the raw semistructured interview data gathered from 42 prescribers who participated in one of two aligned concurrent qualitative studies in the USA and Australia. We conducted a two-part analysis of the interview: first identifying all factors influencing long-term prescribing

Manipulating the sensation of feeling fat: The role of alexithymia, interoceptive sensibility and perfectionism.

OBJECTIVE: Feeling fat reflects difficulties in processing emotions and is an important aspect of body image and eating disorders. The current study aimed to develop a novel social comparison manipulation to induce feeling fat and to explore personality traits that may increase an individual's vulnerability. METHODS: At time 1, 254 healthy females (24.14 years, BMI = 23.77) completed the feeling fat subscale of the Body Attitudes Questionnaire, as well as self-report measures of alexithymia, interoceptive sensibility, physical appearance comparison and perfectionism online. At time 2, a subse

'So, this feels very different and, you know, kindly': a mixed methods study of arts on prescription in secondary care.

Arts on Prescription (AoP) programmes occur within primary care and community settings. The aim of this study was to examine the impact of AoP delivered in secondary care, on patient wellbeing, social connection, and experiences of care. A mixed-methods design was used. Quantitative data were collected from 61 patients referred to AoP through secondary care outpatient services for cancer, chronic pain, weight management, breathlessness, or neuromuscular conditions. Wellbeing and loneliness were assessed pre- and post-participation in the AoP programme. Mood and loneliness were assessed before

Health risks of airplane boarding methods with apron buses when some passengers disregard safe social distancing.

Many airlines instituted social distancing practices to keep passengers safe during the pandemic. The practices include keeping the middle seats empty, reducing the number of passengers taking an apron bus from the terminal to the airplane, and prescribing that passengers maintain 1 m social distance of separation from other passengers in the aisle while advancing to their seats. However, not all passengers comply with a prescribed 1 m aisle social distance. Through agent-based simulations of passenger boarding when apron buses are used, we examine boarding policies adapted for the pandemic wh

An Instrument for Evaluating the Self: The Self-Discrepancies Scale in Non-Clinical Participants.

OBJECTIVE: The Self-Discrepancies Scale (S-DS) is a multiple-subscore instrument designed to assess discrepancies between mental representations of the self: the actual self on one hand, and the ideal and socially prescribed selves on the other. Its idiographic subscores rely on the endorsement of self-descriptive traits, while its abstract subscores form an overall judgement of felt self-discrepancies. The objectives of the present study were to (i) evaluate the S-DS in a new non-clinical population, (ii) expand the body of data on the S-DS's convergent validity, (iii) study the idiographic d

Improving physician prescribing patterns to treat rhinopharyngitis. Intervention strategies in two health systems of Mexico.

To improve prescribing practices for rhinopharyngitis, an interactive educational intervention and a managerial intervention were carried out in 18 primary care facilities in metropolitan Mexico City. Four family medicine clinics of the Mexican Social Security Institute (IMSS) and 14 health centres of the Ministry of Health (SSA) were included. A quasi-experimental design was employed. One hundred and nineteen physicians (IMSS 68, SSA 51) participated. Sixty-five physicians (IMSS 32, SSA 33) were in the study group, while 54 were in the control group (IMSS 36, SSA 18). The study had four stage

A Closer Look at Racism and Heterosexism in Medical Students' Clinical Decision-Making Related to HIV Pre-Exposure Prophylaxis (PrEP): Implications for PrEP Education.

Social biases among healthcare providers could limit PrEP access. In this survey study of 115 US medical students, we examined associations between biases (racism and heterosexism) and PrEP clinical decision-making and explored prior PrEP education as a potential buffer. After viewing a vignette about a PrEP-seeking MSM patient, participants reported anticipated patient behavior (condomless sex, extra-relational sex, and adherence), intention to prescribe PrEP to the patient, biases, and background characteristics. Minimal evidence for racism affecting clinical decision-making emerged. In unad

A Critical Review of the Social and Behavioral Contributions to the Overdose Epidemic.

More than 750,000 people in the United States died from an overdose between 1999 and 2018; two-thirds of those deaths involved an opioid. In this review, we present trends in opioid overdose rates during this period and discuss how the proliferation of opioid prescribing to treat chronic pain, changes in the heroin and illegally manufactured opioid synthetics markets, and social factors, including deindustrialization and concentrated poverty, contributed to the rise of the overdose epidemic. We also examine how current policies implemented to address the overdose epidemic may have contributed

Reducing the risk of adverse drug events in older adults.

Adverse drug events occur in 15 percent or more of older patients presenting to offices, hospitals, and extended care facilities. These events are potentially preventable up to 50 percent of the time. Common serious manifestations include falls, orthostatic hypotension, heart failure, and delirium. The most common causes of death are gastrointestinal or intracranial bleeding and renal failure. Antithrombotic and antidiabetic medications, diuretics, and nonsteroidal anti-inflammatory drugs cause most of the preventable hospital admissions due to adverse drug events. Strategies to reduce the ris

A latent class analysis of clinical complexity: Secondary analysis of the collaboration leading to addiction treatment and recovery from other stresses (CLARO) randomized trial.

BACKGROUND AND AIMS: Individuals with opioid use disorder (OUD) frequently present with co-occurring mental health conditions such as depression and posttraumatic stress disorder (PTSD), along with other mental health substance use disorders, physical health conditions and social determinants that together comprise 'clinical complexity.' Collaborative care (CC), a primary care-based behavioral health integration model, aims to improve outcomes through coordinated, patient-centered treatment. This study examined heterogeneity in baseline clinical complexity among participants in the CLARO (Coll

[Decision-making in gender reassignment surgery].

The prescribing of hormones and the performing of gender reassignment surgery on a healthy body are dangerous and irreversible acts. To ensure patients' safety, endocrinologists and surgeons must use the skills of experienced psychiatrists to create a differential diagnosis, evaluate any frailties and weigh up the risk-benefit ratio. Therapeutic decisions are made as part of multidisciplinary consultation meetings. A minimum two-year observation period must be respected for the treatment to be covered by social security.

Assessment of Knowledge and General Attitudes of Primary Care Physicians Toward Dipeptidyl Peptidase-4 (DPP-4) Inhibitor Drugs in the Management of Type 2 Diabetes Mellitus in the Qassim Region.

BACKGROUND: Dipeptidyl peptidase-4 (DPP-4) inhibitors play a significant role in type 2 diabetes mellitus (T2DM) management due to their efficacy and favorable safety profiles. Despite their growing importance, there is limited information on the knowledge and prescribing behaviors of primary care physicians in Saudi Arabia, particularly in the Qassim region, regarding the use of DPP-4 inhibitors. This study sought to assess the knowledge, attitudes, and prescribing patterns of primary care physicians concerning DPP-4 inhibitors in the management of T2DM. METHODS: A cross-sectional study was

Association between antibiotic prescribing in pregnancy and cerebral palsy or epilepsy in children born at term: a cohort study using the health improvement network.

BACKGROUND: Between 19%-44% pregnant women are prescribed antibiotics during pregnancy. A single, large randomised-controlled-trial (ORACLE Childhood Study II) found an increased risk of childhood cerebral palsy and possibly epilepsy following prophylactic antibiotic use in pregnant women with spontaneous preterm labour. We ascertained whether this outcome could be reproduced across the population of babies delivered at term and prospectively followed in primary-care using data from The Health Improvement Network. METHODS: We determined the risk of cerebral palsy or epilepsy in children whose

Capability, opportunity and motivation for shared decision-making about valproate as an antiseizure medication treatment for epilepsy in women with pregnancy potential: A qualitative study of patient perspectives.

OBJECTIVES: Valproate is a highly effective antiseizure medication but carries significant teratogenic and neurodevelopmental risks to offspring if used during pregnancy. A shared decision-making (SDM) approach is recommended to guide clinician/patient discussions on valproate suitability for women with pregnancy potential. This study applied the Capability, Opportunity, Motivation-Behaviour (COM-B) theoretical framework to explore barriers and facilitators to SDM in valproate prescribing from the perspectives of women with epilepsy who have pregnancy potential. DESIGN: Qualitative study usin
