Checkrein Deformity of the Hallux and Second Toe after Soft-Tissue Injury.

We report the case of a patient who developed a checkrein deformity of the hallux and of the second toe following a direct soft tissue trauma to his right leg, with no associated fracture. This dynamic deformity caused the patient significant trouble walking and prevented him from playing any sport as in the stance phase of gait the toes were forced into maximum plantar flexion and ended up trapped under the foot. An MRI study did not show any bone injury or tendinous tethering. So the cause could be a subclinical compartment syndrome. Treatment consisted of a z-plasty and application of a pul

Does Soft Tissue Injury Affect Intracapsular Condylar Fracture Healing?

PURPOSE: The soft tissue healing patterns of mandibular intracapsular condylar fracture (ICF) after closed treatment have not been well characterized. The purpose of the present study was to classify the injury and healing patterns in adult patients using magnetic resonance imaging (MRI) evaluation. PATIENTS AND METHODS: The present study represents a retrospective review of MRI examinations performed on patients treated with closed reduction of an ICF from 2010 to 2013. The MRI scans used for comparison were taken at 1 week and at least 3 months after the injury. These studies were used to i

Effects of soft tissue injury to the temporomandibular joint: report of 8 cases.

Our aim was to describe the effects of soft tissue injury to the temporomandibular joint (TMJ), to analyse possible reasons for it, and to evaluate the results of treatment. Eight patients (12 joints) who developed disorders of the TMJ after trauma to the mandible without fracture of the condyle were treated in our department from 2009 to 2010. Magnetic resonance imaging (MRI) and computed tomography (CT) were used to check the condition of the joint. Five patients had their joints explored to relieve pain and improve mouth opening. MRI showed all 12 joints had displaced discs. CT showed that

[Primary soft tissue and tenosynovial tuberculosis after needlestick injury in a surgeon].

We describe the case of a surgeon, who pricked himself with a needle used to drain a paravertebral abscess in a patient from Sudan. He lost this patient at follow up. Six weeks later, the surgeon developed oedema of his left hand and wrist. He started antibiotics, amoxicillin/clavulanate plus ciprofloxacin 2 weeks, without any improvement. He came to our centre for examination, and by chance his patient had been admitted to our ward the day before, and had died during the night of disseminated tuberculosis. The surgeon was treated with rifampin, isoniazid and pyrazinamide (3 drugs 2 months, fo

Aneurysm of the internal carotid artery following soft tissue penetration injury.

Aneurysms of the extracranial arteries are in most cases secondary to atherosclerosis but may also be due to degeneration, congenital abnormalities, trauma or unclear etiology. They present either with bulging in the lateral pharyngeal wall or the neck. Therefore, otolaryngologists are often among the first physicians to see the patient. In this report, we present a case of spontaneous oral bleeding that was caused by a pseudoaneurysm following 2 weeks after a soft tissue penetration injury in a child. The facial swelling of the child was initially diagnosed to be mumps by its pediatrician and

Pilot Study: Soft-tissue Healing After Augmented Closure of Traumatic Injuries With Synthetic Electrospun Fiber Matrix.

BACKGROUND: Primary closure of traumatic musculoskeletal soft-tissue wounds is often complicated by soft-tissue edema or compromised skin integrity. Tenuous primary skin closure is associated with an increased incidence of major wound complications. The aim of this retrospective case series was to evaluate soft-tissue healing outcomes following incorporation of a synthetic electrospun fiber matrix (SEFM) to provide soft-tissue reinforcement of primary surgical closure of musculoskeletal injuries. METHODS: A retrospective medical record review of patients treated at a single trauma site for mu

Research priorities in lower leg soft-tissue injuries in older adults: A modified Delphi study (PRIME).

BACKGROUND: Lower leg soft-tissue injuries in older adults are common. These injuries can be complicated by frailty, comorbidities, and impaired healing; however, there is limited high-quality evidence to guide optimal management. This study aimed to establish a consensus-driven research agenda for lower leg soft-tissue injuries in older adults using a modified Delphi approach. METHODS: An international three-round modified Delphi study was conducted, with clinicians involved in the management of lower leg soft-tissue invited to participate. In Round 1, participants submitted open-text respon

The Cambridge Knee Injury Tool (CamKIT): a clinical prediction tool for acute soft tissue knee injuries.

BACKGROUND/AIM: This study focuses on the development of the Cambridge Knee Injury Tool (CamKIT), a clinical prediction tool developed as a 12-point scoring tool based on a modified e-Delphi study. METHODS: A retrospective cohort evaluation was conducted involving 229 patients presenting to a Major Trauma Centre with acute knee pain over 3 months. The evaluation extracted data on the 12 scoring tool variables as well as diagnostic and management pathway outcomes. CamKIT scores for the injured and non-injured cohorts were then calculated and evaluated. RESULTS: The CamKIT yielded a median sco

Diagnostic impacts on management of soft tissue injuries associated with tibial plateau fractures: A narrative review.

INTRODUCTION: Currently there is no consensus on the need for investigating knee ligamentous and meniscal injuries in a patient with a tibial plateau fracture. Consequently, many soft tissue injuries are likely undiagnosed and therefore untreated. The impact this has on long term knee outcomes is not well defined. We aimed to identify the impacts of various diagnostic methods on the management of meniscal injuries associated with tibial plateau fractures and evaluate the clinical outcomes. MATERIALS AND METHODS: We performed a systematic review using Pubmed, Medline, Embase, CINAHL and Cochra

Effects of Perioperative Venous Thromboembolism on Outcomes in Soft Tissue Reconstruction of Traumatic Lower Extremity Injuries.

BACKGROUND: Venous thromboembolism (VTE), including deep vein thrombosis and pulmonary embolism, represents a significant perioperative complication. However, the outcomes of lower extremity salvage in the setting of perioperative VTE are not well reported. METHODS: A retrospective review of lower extremity trauma patients requiring soft tissue reconstruction between July 2007 and December 2015 at an urban trauma center was performed. Patients with clinically apparent VTE during inpatient stay were identified. Outcomes compared included success of limb salvage, flap survival, and flap complic

Soft tissue reconstruction with omental free flap in complex upper extremity injuries: report of 13 cases.

Although there is a wide list of free flaps options for soft tissue reconstruction of complex upper extremity injuries, the omental flap has some useful anatomical and biochemical advantages. We report 13 patients who underwent hand or upper extremity reconstruction with omental free flaps. Nine patients had extensive tissue damage, resulting with digital cyanosis and hypothermia, and some of them with areas of cutaneous necrosis, or avulsed tissues with tendons and bones exposed or infected. The remaining four patients had minor extensive tissue damage without circulatory problems. Patient's

Management of soft tissue extremity degloving injuries with full-thickness grafts obtained from the avulsed flap.

BACKGROUND: A "degloving injury" is referred to as seperation of cutaneous tissue from the deeper structures of the body. Although many methods have been defined to reform the tissue integrity; defatting and readaptation of the avulsed flap still comprises one of the most effective methods. METHODS: From 2000-2012, we treated a total of nine patients with avulsed extremities with defatting and readaptation of the same flap. The fat compartment of the flaps was removed and the skin was meshed. The patients were followed-up with for an average of 12 months (range: 8-18 months). RESULTS: Total

Bilateral disruption of soft tissue extensor mechanism of knee: functional outcome and comparison to unilateral injuries.

BACKGROUND: Previous studies suggest that Dupuytren's disease is caused by fibroblast and myofibroblast contractility. Cell contractility in smooth muscle cells is caused by calcium-dependent and calcium-independent signaling mechanisms. In the calcium-dependent pathway, calcium/calmodulin activates myosin light chain kinase (MLCK). In this study, the effects of calcium/calmodulin inhibition with the FDA-approved drug fluphenazine on Dupuytren's fibroblast contractility and MLCK expression were tested. METHODS: Fibroblast lines from the palmar fascia of patients with Dupuytren's disease were

MR imaging assessment of temporomandibular joint soft tissue injuries in dislocated and nondislocated mandibular condylar fractures.

BACKGROUND AND PURPOSE: Evaluation of temporomandibular joint (TMJ) soft tissue injuries after condylar fractures remains a challenge with use of conventional radiography and CT. The aim of this study was to explore MR imaging in the assessment of TMJ soft tissue injuries after condylar fractures. MATERIALS AND METHODS: Eighty subjects (118 TMJs) with condylar fractures were examined with sagittal and coronal MR imaging. Proton attenuation and T2-weighted sequences were the key sequences in our imaging protocol. All of the condylar fractures were classified into condylar fractures with disloc

Reconstruction of skin and soft-tissue defects in crush-injuries of the lower leg in children.

Severe crush injuries with open comminuted fractures of the lower extremity often result in extensive tissue loss. In such a situation early free microsurgical tissue transplantation offers most effective possibilities of treatment. Among others, two of the most eminent advantages of free microsurgical tissue transplantation are: 1. The possibility of tissue replacement which cannot be achieved by traditional procedures such as functional muscle transplantation or the replacement of extremely extensive tissue loss; moreover immediate or early bone coverage by well vascularized soft-tissue is m
