HMGA2::NCOR2 fusion-driven giant cell tumors: Diagnosis and treatment.

Giant cell-rich tumors encompass a heterogeneous group of bone and soft tissue neoplasms with overlapping morphologic features but distinct molecular drivers and clinical behaviors. Recent studies have identified a unique subset of giant cell-rich tumors characterized by recurrent HMGA2::NCOR2 gene fusions, variably described as xanthogranulomatous epithelial tumor or keratin-positive giant cell tumor. In this paper, we emphasize the clinicopathologic, histologic, and molecular features that distinguish these rare neoplasms from conventional giant cell tumors of bone, soft tissue giant cell tu

A Case of Primary Duodenal Liposarcoma.

Soft tissue sarcomas are common neoplasms accounting for 1% of all adult malignancies; however, soft tissue sarcomas infrequently arise from the abdominal viscera. Many case reports discuss gastric and esophageal neoplasms. In the group of gastrointestinal liposarcomas, primary duodenal liposarcomas are among the rarest, with only three previous cases reported in the literature. Herein, we discuss a case of primary duodenal liposarcoma. A 59-year-old woman presented with symptoms consistent with anemia raising suspicion for an upper gastrointestinal bleed. Upper endoscopy revealed an ulcerated

Giant cell tumor of soft tissue involving thyroid gland: a case report and review of the literature.

BACKGROUND: Giant cell tumor of soft tissue is a low malignant uncommon neoplasm, with histologic features and immunophenotype similar to its bone counterpart. Primary giant cell tumor of soft tissue in the thyroid gland is considered an exceedingly rare entity. CASE PRESENTATION: We describe a case of primary thyroid giant cell tumor of soft tissue in a 69-year-old Chinese female patient. Neck ultrasonography showed a 19 mm × 12 mm × 5 mm nodule with heterogeneous echo and clear boundary located within the left thyroid. Histopathological examination revealed that the neoplasm was composed of

Tumoral calcinosis of the foot: a radiologic case study.

Tumoral calcinosis is a disorder characterized by deposits of calcium phosphate in the subcutaneous tissues near large joints. While often resembling a neoplasm, the soft tissue deposits of tumoral calcinosis are benign in nature. The deposits can, however, cause significant morbidity to patients due to tissue pressure or impingement. The focus of this case report will include the presentation, radiographic examinations, clinical course, and treatment of a 90-year-old female patient presenting for evaluation of a painful plantar right foot soft tissue mass associated with chronic renal failure

Uncommon but intriguing: A pediatric primary histiocytic sarcoma case-Clinical insights and literature review.

Histiocytic sarcoma (HS) is a rare and aggressive malignancy derived from histiocytes, accounting for less than 1% of hematopoietic neoplasms. Pediatric soft tissue involvement is exceptionally rare, with limited data to guide management. We present the case of a 5-year-old girl with a painless mass on the right thigh. Imaging revealed a malignant intra-aponeurotic soft tissue lesion. Histopathological analysis confirmed HS with immunohistochemical positivity for CD68, CD31, LCA, and INI1, and a Ki-67 index of 40%. Staging PET-CT identified local lymph node involvement. Following R1 surgical r

Solitary extraosseous plasmacytoma.

Plasma cell neoplasms are characterized by a neoplastic plasma cell lineage which produces a monoclonal immunoglobulin. These neoplasms can present as a single lesion (solitary plasmacytoma) or as multiple lesions (multiple myeloma). Solitary plasmacytomas most frequently occur in bone (plasmacytomas of bone), but can also be found outside bone in soft tissues (extramedullary plasmacytomas).

[Soft tissue angiosarcoma in a child. Immunohistochemical and ultrastructural features].

This observation reports the case of a soft tissue tumor occurring in a 11 years-old boy. On standard histologic staining, this tumor corresponded to an undifferentiated sarcoma of high grade of malignancy. Immuno-histochemical features (laminin, vimentin, UEA I and Factor VIII R-ag) and ultrastructural analysis (presence of Weibel-Palade bodies) led us to the diagnosis of angiosarcoma. This tumor is rarely reported in children and its prognosis remains unknown. Soft tissue angiosarcoma must be differentiated from Kaposi's sarcoma, epithelioid hemangioendothelioma, hemangiopericytoma and spind

Ultrasonography of the popliteal fossa and lower extremities.

In this article, the numerous proven applications of diagnostic ultrasound to the assessment of disease processes affecting the popliteal fossa and lower extremities have been discussed. Popliteal cysts and aneurysms, hematomas, cellulitis, abscesses, soft tissue neoplasms, and other space-occupying fluid collections are readily identified using this technique. In addition, post-traumatic and inflammatory disorders affecting tendons, synovial thickening and effusion, derangement of articular cartilage, meniscal tears, and alterations in soft tissue thickness as well as foreign bodies are amena

Giant epidermal inclusion cyst of buttock.

Epidermal inclusion cyst is a common benign soft-tissue lesion of skin. It often presents as a small lesion, but rarely grows to a large mass. We are presenting a giant gluteal epidermal inclusion cyst that grew to an enormous size, masquerading as a large soft-tissue neoplasm. Magnetic resonance imaging not only displays the size and margins of these well-encapsulated lesions in three planes, but shows characteristic internal features that may suggest the preoperative diagnosis.

Malignant giant cell tumor of soft parts presenting as a skin tumor.

Malignant giant cell tumor of soft parts is a rare neoplasm that histologically resembles a giant cell tumor of bone. It has a distinctive multinodular growth pattern and is composed of numerous osteoclast-like giant cells, histiocytes, and fibroblasts. Although this tumor is usually found in deep soft tissues, a superficial form has been described in the subcutaneous tissue and fascia. The authors report two patients, aged 75 and 78, with malignant giant cell tumors presenting as ulcerating skin nodules of the arm and foot. The tumors were relatively small, measuring less than 3.5 cm in great

Multimodality Treatment in Ewing's Sarcoma Family Tumors of the Maxilla and Maxillary Sinus: Review of the Literature.

The Ewing sarcoma family of tumors (ESFT) encompasses a group of highly aggressive, morphologically similar, malignant neoplasms sharing a common spontaneous genetic translocation that affect mostly children and young adults. These predominantly characteristic, small round-cell tumors include Ewing's sarcoma of the bone and soft tissue, as well as primitive neuroectodermal tumors (PNETs) involving the bone, soft tissue, and thoracopulmonary region (Askin's tumor). Extraosseous ESFTs are extremely rare, especially in the head and neck region, where literature to date consists of sporadic case r

Chondromyxoid fibroma of the femur: a case report with intra-cortical location.

Chondromyxoid fibroma (CMF) is the least common benign cartilaginous tumor, comprising less than 0.5 to 1% of all skeletal neoplasms. This subject was a 16-year-old female with a three-year history of pain involving the distal femoral metaphysis. This case showed an unusual feature: it was intracortical in location. Radiologic differential diagnosis included metaphyseal fibrous defect, periosteal chondroma, simple or aneurysmal bone cyst, and cortical abscess. On operation, the lesion filled the intracortical defect with whitish myxoid soft tissue, bulging into the adjacent soft tissue. Micros

Extraskeletal Ewing's sarcoma.

Ewing sarcoma (E.S.) is a rare malignant tumour of the bone, dubious in origin and uncertain in the direction of its differentiation [2,3,7,10]. Recent reports have presented description of primary neoplasms of soft tissues which revealed no differences from E.S. in light and electron-microscopy but had no connection with the skeleton [1,4,5,6,8,9]. These lesions, found in subcutaneous tissue of the trunk and limbs, in the retroperitoneum and in paravertebral regions were called extraskeletal Ewing sarcoma (E.E.S.) or soft tissue Ewing sarcoma. As tumours of this type are infrequent and may ca

Angiomatoid fibrous histiocytoma: primary intracranial lesion with thoracic spine metastasis and a malignant course. Illustrative case.

BACKGROUND: Angiomatoid fibrous histiocytoma (AFH) is an exceptionally rare soft tissue neoplasm. This tumor primarily presents as a benign soft tissue lesion in children with an average age of 14 years. The standard treatment regimen is wide local excision with interval follow-up. However, newer reports have demonstrated malignant potential with the possibility of intracranial metastasis. OBSERVATIONS: A 45-year-old male with no soft tissue primary tumor presented with a primary intracranial lesion and thoracic spine metastasis refractory to chemotherapy and radiation treatment. LESSONS: Th

Proximal-type epithelioid sarcoma of the mons pubis: report of a case.

INTRODUCTION: Proximal-type epithelioid sarcoma (PES) represents an extremely rare and aggressive form of soft tissue neoplasm, typically presenting as a painless subcutaneous nodule in the trunk often located in the genital area. CASE REPORT: A 46-year-old female was subjected to the excision of a growing soft tissue mass in the mons pubis that, at histology, was identified as PES. The tumor showed an extreme aggressiveness involving subsequently adjoining structures and lymph nodes despite subsequent wide surgical resections during the following months. DISCUSSION: Gynecologists should pay

The marriage of cytology and cytogenetics.

The past 20 years have witnessed extraordinary advances in the field of cytogenetics, with the discovery that a multitude of neoplasms is characterized by identifiable chromosomal changes. The ability of Cytogenetics to aid in the identification and precise classification of a variety of neoplasms has not gone unnoticed by Cytology. In particular, Cytology has recognized Cytogenetics as a welcome companion in the evaluation of soft tissue tumors, lymphomas, renal and urothelial tumors, and mesothelioma. This relationship requires a good understanding of the proper handling of specimens for opt

Tophaceous pseudogout: a pitfall in the diagnosis of chondrosarcoma.

Tumoral calcium pyrophosphate dihydrate deposition disease (TCPPD, tumoral or tophaceous pseudogout) is a rare nonneoplastic entity which mimics soft-tissue or skeletal malignancy. We present here the fine-needle aspiration cytology findings of a unique case of TCPPD in a 76-yr-old woman, with a large paraischial soft-tissue mass diagnosed as a malignant neoplasm. The difficulty in diagnosing such lesions by fine-needle aspirates is discussed and reviewed in the context of known cases from the literature.

Oral alveolar soft part sarcoma in childhood and adolescence: report of two cases and review of literature.

Alveolar soft part sarcoma (ASPS) constitutes a rare soft tissue malignant neoplasm comprising less than 1 % of all soft tissue sarcomas. ASPS demonstrates a strong predilection for adolescents and young adults, with a female predominance reported. The head and neck region is the most commonly affected region in pediatric patients with the tongue and orbit affected most commonly. Herein we present the clinical, radiographic, histopathologic, immunohistochemical and molecular features of two examples of ASPS affecting the oral cavity of 4 and 13 year-old boys, along with a focused review of the

PLAG1-Driven Lipoblastomatosis: Diagnostic Insights from a Case Report and Literature Review.

Lipoblastomatosis is a rare benign adipocytic tumour of infancy and early childhood, characterized by rapid growth. Diagnosis is challenging, owing to tumour heterogeneity and overlap with other pediatric soft-tissue neoplasms. PLAG1 rearrangement is the defining molecular hallmark, with an expanding list of fusion partners. We report a case of a 1-year-old child with a firm, nonmobile mass in the right thigh, which on CECT was deep-seated and heterogeneously enhanced. Gross evaluation of the excision specimen demonstrated an ill-circumscribed soft tissue mass. Histopathologically, the tumour
