Does the bone-anchored hearing aid have a complementary effect on audiological and subjective outcomes in patients with unilateral conductive hearing loss?

OBJECTIVES: To study the effect of a bone-anchored hearing aid (BAHA) in patients with unilateral conductive hearing loss. STUDY DESIGN: Prospective evaluation on 18 subjects. METHODS: Aided and unaided binaural hearing was assessed in the sound field using a sound localization test and a speech recognition in noise test with spatially separated sound and noise sources. The patients also filled out a disability-specific questionnaire. PATIENTS: 13 out of the 18 subjects had normal hearing on one side and acquired conductive hearing loss in the other ear. The remaining 5 patients had a unila

Hearing performance in single-sided deaf cochlear implant users after upgrade to a single-unit speech processor.

INTRODUCTION: Single-sided deaf (SSD) patients report multiple benefits after cochlear implantation (CI), such as tinnitus suppression, speech perception, and sound localization. The first single-unit speech processor, the RONDO, was launched recently. Both the RONDO and the well-known behind-the-ear (BTE) speech processor work on the same audio processor platform. However, in contrast to the BTE, the microphone placement on the RONDO is different. The aim of this study was to evaluate the hearing performances using the BTE speech processor versus using the single-unit speech processor. Subjec

Audiological performance and subjective satisfaction of the ADHEAR system in experienced pediatric users with unilateral microtia and aural atresia.

INTRODUCTION: Despite the reported auditory deficits and developmental challenges in children with unilateral microtia and aural atresia (UMAA), there remains a lack of consensus on early intervention with bone conduction hearing aids (BCHAs) to restore binaural hearing due to the uncertain clinical benefits and intolerability of the conventional devices. Previous studies investigating the auditory benefits under binaural hearing condition were limited and demonstrated controversial opinions in heterogenous patient groups with various devices. Our study aimed to evaluate the audiological perfo

Hyperacute directional hearing in a microscale auditory system.

The physics of sound propagation imposes fundamental constraints on sound localization: for a given frequency, the smaller the receiver, the smaller the available cues. Thus, the creation of nanoscale acoustic microphones with directional sensitivity is very difficult. The fly Ormia ochracea possesses an unusual 'ear' that largely overcomes these physical constraints; attempts to exploit principles derived from O. ochracea for improved hearing aids are now in progress. Here we report that O. ochracea can behaviourally localize a salient sound source with a precision equal to that of humans. De

Age-related myelin deficits in the auditory brain stem contribute to cocktail-party deficits.

Age-related hearing loss consists of both peripheral and central components and is an increasing global health concern. While peripheral hearing loss is well understood, central hearing loss- age-related changes in the central auditory pathways resulting in a listener's inability to process sound correctly -remains poorly understood. In this study, we focus on the pathway from the cochlear nucleus to the medial nucleus of the trapezoid body (MNTB), which depends on heavily myelinated axons for microsecond-level temporal precision required for sound localization. Using a combination of auditory

The anatomical basis of amphibious hearing in the American alligator (Alligator mississippiensis).

The different velocities of sound (pressure waves) in air and water make auditory source localization a challenge for amphibious animals. The American alligator (Alligator mississippiensis) has an extracolumellar cartilage that abuts the deep surface of the tympanic membrane, and then expands in size beyond the caudal margin of the tympanum. This extracolumellar expansion is the insertion site for two antagonistic skeletal muscles, the tensor tympani, and the depressor tympani. These muscles function to modulate the tension in the tympanic membrane, presumably as part of the well-developed sub

An automatic segmentation method for heart sounds.

BACKGROUND: There are two major challenges in automated heart sound analysis: segmentation and classification. An efficient segmentation is capable of providing valuable diagnostic information of patients. In addition, it is crucial for some feature-extraction based classification methods. Therefore, the segmentation of heart sound is of significant value. METHODS: This paper presents an automatic heart sound segmentation method that combines the time-domain analysis, frequency-domain analysis and time-frequency-domain analysis. Employing this method, the boundaries of heart sound components

Bone conductive implants in single-sided deafness.

CONCLUSION: Bone conduction implants (BCIs) have been shown to partially restore some of the functions lost when binaural hearing is missing, such as in subjects with single-sided deafness (SSD). The use of a single BCI needs to be recommended by a clinician based on thorough counselling with the SSD subject. OBJECTIVES: To perform an overview of the present capabilities of BCIs for SSD and to evaluate the reliability of the audiological evaluation for assessing speech recognition in noise and sound localization cues, which are major problems related to the loss of binaural hearing. METHODS:

What and where in the auditory systems of sighted and early blind individuals: Evidence from representational similarity analysis.

Separated ventral and dorsal streams in auditory system have been proposed to process sound identification and localization respectively. Despite the popularity of the dual-pathway model, it remains controversial how much independence two neural pathways enjoy and whether visual experiences can influence the distinct cortical organizational scheme. In this study, representational similarity analysis (RSA) was used to explore the functional roles of distinct cortical regions that lay within either the ventral or dorsal auditory streams of sighted and early blind (EB) participants. We found func

Impact of processing-latency induced interaural delay and level discrepancy on sensitivity to interaural level differences in cochlear implant users.

PURPOSE: This study investigated whether an interaural delay, e.g. caused by the processing latency of a hearing device, can affect sensitivity to interaural level differences (ILDs) in normal hearing subjects or cochlear implant (CI) users with contralateral normal hearing (SSD-CI). METHODS: Sensitivity to ILD was measured in 10 SSD-CI subjects and in 24 normal hearing subjects. The stimulus was a noise burst presented via headphones and via a direct cable connection (CI). ILD sensitivity was measured for different interaural delays in the range induced by hearing devices. ILD sensitivity wa

Transmission of Binaural Cues by Bilateral Cochlear Implants: A Comparison of Spectral Peak-Picking and Interleaved Stimulation Strategies.

Cochlear implants (CIs) effectively restore speech understanding in quiet for many recipients. However, bilateral CI listeners continue to perform poorer than acoustic-hearing listeners on spatial-hearing tasks such as sound localization. One explanation is that stimulation strategies and the lack of coordination across sound processors degrade the fidelity of binaural cues. To provide further insight on this issue, this study compared the binaural cues transmitted by two CI stimulation strategies: a spectral peak-picking stimulation strategy and a constantly stimulating strategy. Two clinical

Audiological and Subjective Benefits in a Child with Microtia and Atresia After Sequential Bilateral Implantation with Active Bone Conduction Devices: A Case Study.

BACKGROUND: With bilateral hearing loss, the main problems for the patient are speech understanding in noise and, especially in asymmetrical hearing loss, an inability to correctly localize sound sources. There are multiple methods of treatment and rehabilitation for people with conductive hearing loss, and one of them is to use an active bone conduction implant. This case study is designed to evaluate the auditory benefits and sound localization accuracy with active bilateral bone conduction implants-in comparison to unilateral ones-in a patient with congenital bilateral conductive hearing lo

Analysis of cross-talk cancellation of bilateral bone conduction stimulation.

When presenting a stereo sound through bilateral stimulation by two bone conduction transducers (BTs), part of the sound at the left side leaks to the right side, and vice versa. The sound transmitted to the contralateral cochlea becomes cross-talk, which can affect space perception. The negative effects of the cross-talk can be mitigated by a cross-talk cancellation system (CCS). Here, a CCS is designed from individual bone conduction (BC) transfer functions using a fast deconvolution algorithm. The BC response functions (BCRFs) from the stimulation positions to the cochleae were obtained by

Evaluating Auditory Localization Capabilities in Young Patients with Single-Side Deafness.

BACKGROUND/OBJECTIVES: Unilateral hearing loss (UHL), particularly single-sided deafness (SSD), disrupts spatial hearing in children, leading to academic and social challenges. This study aimed to (1) compare azimuthal sound-localization accuracy and compensatory strategies between children with single-sided deafness (SSD) and their normal-hearing (NH) peers within a virtual reality environment, and (2) investigate sound-localization performance across various azimuths by contrasting left-SSD (L-SSD) and right-SSD (R-SSD) groups. METHODS: A cohort of 44 participants (20 NH, 24 SSD) performed

Ear-Specific Hemispheric Asymmetry in Unilateral Deafness Revealed by Auditory Cortical Activity.

Profound unilateral deafness reduces the ability to localize sounds achieved via binaural hearing. Furthermore, unilateral deafness promotes a substantial change in cortical processing to binaural stimulation, thereby leading to reorganization over the whole brain. Although distinct patterns in the hemispheric laterality depending on the side and duration of deafness have been suggested, the neurological mechanisms underlying the difference in relation to behavioral performance when detecting spatially varied cues remain unknown. To elucidate the mechanism, we compared N1/P2 auditory cortical

Bone conduction pathways confer directional cues to salamanders.

Sound and vibration are generated by mechanical disturbances within the environment, and the ability to detect and localize these acoustic cues is generally important for survival, as suggested by the early emergence of inherently directional otolithic ears in vertebrate evolutionary history. However, fossil evidence indicates that the water-adapted ear of early terrestrial tetrapods lacked specialized peripheral structures to transduce sound pressure (e.g. tympana). Therefore, early terrestrial hearing should have required nontympanic (or extratympanic) mechanisms for sound detection and loca
