A Novel Artificial-Intelligence-Based Reverberation-Reduction Algorithm for Cochlear Implants Enhances Speech Intelligibility and User Experience.

OBJECTIVES: Cochlear implantation is the standard of care for severe-to-profound hearing loss in the Netherlands. Cochlear implants (CIs) generally perform well in quiet conditions, but speech understanding in an environment with reverberations remains difficult. This study aimed to minimize the amount of reverberation present in a speech signal by using novel artificial-intelligence-based algorithms created for use in CIs. DESIGN: A prospective crossover study was performed, which included 15 CI users, with each participant being their own control. Two versions of the algorithm were tested:

Contributions of Speech Timing and Articulatory Precision to Listener Perceptions of Intelligibility and Naturalness in Parkinson's Disease.

PURPOSE: Parkinson's disease (PD) results in hypokinetic dysarthria in as many as 90% of cases. Among the most distinctive features of hypokinetic dysarthria are atypical timing and articulatory imprecision in speech production. Here, we examined the contributions of perceived speech timing typicality and articulatory precision, both on their own and while controlling for the other, on intelligibility and naturalness in speakers with PD. METHOD: Twenty speakers with PD and four healthy older adults read aloud the first paragraph of the Rainbow Passage. Twenty inexperienced listeners with typi

Validation and cross-linguistic adaptation of the Frenchay Dysarthria Assessment (FDA-2) speech intelligibility tests: Hebrew version.

'Dysarthria' is a group of motor speech disorders resulting from a disturbance in neuromuscular control. Most individuals with dysarthria cope with communicative restrictions due to speech impairments and reduced intelligibility. Thus, language-sensitive measurements of intelligibility are important in dysarthria neurological assessment. The Frenchay Dysarthria Assessment, 2nd edition (FDA-2), is a validated tool for the identification of the nature and patterns of oro-motor movements associated with different types of dysarthria. The current study conducted a careful culture- and linguistic-s

Chronic Conductive Hearing Loss Is Associated With Speech Intelligibility Deficits in Patients With Normal Bone Conduction Thresholds.

OBJECTIVES: The main objective of this study is to determine whether chronic sound deprivation leads to poorer speech discrimination in humans. DESIGN: We reviewed the audiologic profile of 240 patients presenting normal and symmetrical bone conduction thresholds bilaterally, associated with either an acute or chronic unilateral conductive hearing loss of different etiologies. RESULTS: Patients with chronic conductive impairment and a moderate, to moderately severe, hearing loss had lower speech recognition scores on the side of the pathology when compared with the healthy side. The degree o

Hybridizing conversational and clear speech to investigate the source of increased intelligibility in speakers with Parkinson's disease.

PURPOSE: A speech analysis-resynthesis paradigm was used to investigate segmental and suprasegmental acoustic variables explaining intelligibility variation for 2 speakers with Parkinson's disease (PD). METHOD: Sentences were read in conversational and clear styles. Acoustic characteristics from clear sentences were extracted and applied to conversational sentences, yielding 6 hybridized versions of sentences in which segment durations, short-term spectrum, energy characteristics, or fundamental frequency characteristics for clear productions were applied individually or in combination to con

Comparing loudness normalization (IHAFF) with speech intelligibility maximization (NAL-NL1) when implemented in a two-channel device.

OBJECTIVE: At least two rationales are available for fitting wide dynamic range compression hearing aids. The goal of one rationale is to normalize loudness, and the goal of the second rationale is to maximize speech intelligibility. Neither rationale has been validated against other fitting rationales for the range of input levels common to the hearing aid user in the real world. The goal of the study was to compare the two rationales when implemented in a 2-channel compression hearing aid. DESIGN: Loudness normalization and speech intelligibility maximization were implemented using the Inde

Effect of botulinum toxin injection on speech outcomes among patients with synkinetic facial nerve palsy: An assessment of speech intelligibility and quality of life.

BACKGROUND: Facial nerve palsy (FNP) considerably impairs functional outcomes and quality of life. For patients with synkinetic FNP, physiotherapy and botulinum toxin A (BTX) are commonly used to reduce hyperkinesis and improve facial expression. This study assessed the effect of BTX administration on oral competence in patients with FNP. METHODS: Patients referred for BTX for management of their synkinetic FNP were recruited from the Sydney Facial Nerve Service. BTX was administered by a single surgeon. Their oral competence was measured using clinician and patient-rated outcome measures bef

Characterization and Prediction of Speech Intelligibility at the Output of Hearing Aids in a Noisy Working Environment.

OBJECTIVE: Hearing aids are more and more technically advanced, but do not necessarily guarantee the reproduction of useful signals in all working environments. This is particularly the case for speech intelligibility. This study focuses on the prediction of hearing aid performance in the case of a moderate deafness setting, in service and industrial work environments. To improve intelligibility, hearing aids propose signal processing options such as noise reduction and compression. These processes can transform hearing aids into nonlinear systems. The aim of this study is to develop a nonline

Spontaneous speech intelligibility: early cochlear implanted children versus their normally hearing peers at seven years of age.

Speaking intelligibly is an important achievement in children's language development. How far do congenitally severe-to-profound hearing-impaired children who received a cochlear implant (CI) in the first two years of their life advance on the path to intelligibility in comparison to children with typical hearing (NH)? Spontaneous speech samples of children with CI and children with NH were orthographically transcribed by naïve transcribers. The entropy of the transcriptions was computed to analyze their degree of uniformity. The same samples were also rated on a continuous rating scale by ano

Intelligibility assessment of cleft lip and palate speech using Gaussian posteriograms based on joint spectro-temporal features.

Intelligibility is considered as one of the primary measures for speech rehabilitation of individuals with a cleft lip and palate (CLP). Currently, speech processing and machine-learning-based objective methods are gaining more research interest as a way to quantify speech intelligibility. In this work, joint spectro-temporal features computed from a time-frequency representation of speech are explored to derive speech representations based on Gaussian posteriograms. A comparative framework using dynamic time warping (DTW) is used to quantify the intelligibility of child CLP speech. The DTW di

A physiologically-inspired model reproducing the speech intelligibility benefit in cochlear implant listeners with residual acoustic hearing.

This study introduces a speech intelligibility model for cochlear implant users with ipsilateral preserved acoustic hearing that aims at simulating the observed speech-in-noise intelligibility benefit when receiving simultaneous electric and acoustic stimulation (EA-benefit). The model simulates the auditory nerve spiking in response to electric and/or acoustic stimulation. The temporally and spatially integrated spiking patterns were used as the final internal representation of noisy speech. Speech reception thresholds (SRTs) in stationary noise were predicted for a sentence test using an aut

Sentence intelligibility during segmental interruption and masking by speech-modulated noise: Effects of age and hearing loss.

This study investigated how single-talker modulated noise impacts consonant and vowel cues to sentence intelligibility. Younger normal-hearing, older normal-hearing, and older hearing-impaired listeners completed speech recognition tests. All listeners received spectrally shaped speech matched to their individual audiometric thresholds to ensure sufficient audibility with the exception of a second younger listener group who received spectral shaping that matched the mean audiogram of the hearing-impaired listeners. Results demonstrated minimal declines in intelligibility for older listeners wi

Preparing an E-learning-based Speech Therapy (EST) efficacy study: Identifying suitable outcome measures to detect within-subject changes of speech intelligibility in dysarthric speakers.

We explored the suitability of perceptual and acoustic outcome measures to prepare E-learning based Speech Therapy (EST) efficacy tests regarding speech intelligibility in dysarthric speakers. Eight speakers with stroke (n=3), Parkinson's disease (n=4) and traumatic brain injury (n=1) participated in a 4 weeks EST trial. A repeated measures design was employed. Perceptual measures were (a) scale ratings for "ease of intelligibility" and "pleasantness" in continuous speech and (b) orthographic transcription scores of semantically unpredictable sentences. Acoustic measures were (c) "intensity du

The Influence of Speech Material, Intelligibility, and Masker Type on the Pupil Dilation Response in Cochlear Implant Users.

OBJECTIVES: The pupil dilation response is often used as a physiological measure of listening effort. One way to manipulate listening effort is to manipulate intelligibility. Only a few studies assessed the relationship between intelligibility and the pupil dilation response in cochlear implant (CI) users. Those studies did not consistently replicate the effects of intelligibility on the pupil dilation response evoked by sentence recognition in noise as observed in individuals with typical hearing or hard of hearing. Therefore, this study examined the effects of intelligibility on the pupil di

Computer-based voice familiarization, delivered remotely using an online platform, improves speech intelligibility for older and younger adults.

Understanding speech in noisy environments is often challenging but is easier if we are listening to someone familiar-for example, naturally familiar people (e.g., friends, partners) or voices that have been familiarized artificially in the lab. Thus, familiarizing people with voices they regularly encounter (e.g., new friends and colleagues) could improve speech intelligibility in everyday life, which might be particularly useful for people who struggle to comprehend speech in noisy environments, such as older adults. Yet, we do not currently understand whether computer-based voice familiariz

A comparison of speech intelligibility and subjective quality with hearing-aid processing in older adults with hearing loss.

OBJECTIVE: This study characterised the relationship between speech intelligibility and quality in listeners with hearing loss for a range of hearing-aid processing settings and acoustic conditions. DESIGN: Binaural speech intelligibility scores and quality ratings were measured for sentences presented in babble noise and processed through a hearing-aid simulation. The intelligibility-quality relationship was investigated by (1) assessing the effects of experimental conditions on each task; (2) directly comparing intelligibility scores and quality ratings for each participant across the range

Intelligibility of conversational and clear speech in young and older talkers as perceived by young and older listeners.

A related paper [Hazan, Tuomainen, Tu, Kim, Davis, Brungart, and Sheffield. (2018b). Hear. Res. 369, 33-41] showed that, for young adult listeners, speech produced by older adults was less intelligible than the speech of young adults but both talker groups improved the intelligibility of their speech via clear speech modifications. Here, this study was extended to include older listeners with/without presbycusis. The results showed that for older listeners, speech produced by older adults was less intelligible than the speech of young adults and scores improved in the clear speech condition. T

Effects of simulated spectral holes on speech intelligibility and spatial release from masking under binaural and monaural listening.

The possibility that "dead regions" or "spectral holes" can account for some differences in performance between bilateral cochlear implant (CI) users and normal-hearing listeners was explored. Using a 20-band noise-excited vocoder to simulate CI processing, this study examined effects of spectral holes on speech reception thresholds (SRTs) and spatial release from masking (SRM) in difficult listening conditions. Prior to processing, stimuli were convolved through head-related transfer-functions to provide listeners with free-field directional cues. Processed stimuli were presented over headpho
