Spinal Cord Leptomeningeal Enhancement as a Marker of Extensive Spinal Cord Involvement in Children With MOGAD.

BACKGROUND AND OBJECTIVES: Spinal cord leptomeningeal enhancement (LME) can be observed in children with myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) and with seronegative myelitis. We investigated whether the presence of spinal cord LME in MOGAD and seronegative myelitis is associated with distinct clinical, CSF, and MRI findings. METHODS: Study participants were identified among the 490 children and adolescents recruited to the Canadian Pediatric Demyelinating Disease study following an incident attack of CNS demyelination. Inclusion criteria for this study were:

Effect of lycopene on the blood-spinal cord barrier after spinal cord injury in mice.

The current study aimed to investigate the effect of lycopene on the blood-spinal cord barrier (BSCB) after spinal cord injury (SCI) in a mouse model. Lycopene inhibited lipid peroxidation and oxidative DNA damage as a highly efficient antioxidant and free radical scavenger. Lycopene (4 mg/kg/d) was administrated immediately following SCI. The permeability of the BSCB and water content in the spinal cord tissue were evaluated. Additionally, levels of expression of tight junction proteins and heme oxygenase-1 (HO-1) were determined with Western blotting. An enzyme-linked immunosorbent assay ana

Imaging of Spinal Cord Trauma.

This article outlines the evolving role of MRI in traumatic spinal cord injury (SCI) across acute to chronic stages. Conventional MRI remains indispensable for diagnosis and early prognosis, while quantitative morphometrics such as tissue bridges and spinal cord cross-sectional area provide robust and clinically applicable predictors of post-SCI recovery. Advanced techniques, including diffusion, myelin-sensitive, and functional MRI, enable the detection of microstructural damage, remote neurodegeneration, and functional reorganization. Ongoing advances in scan protocol standardization, automa

Pathobiology of Spinal Cord Compression.

Degenerative cervical myelopathy results from chronic spinal cord compression, yet clinical severity often poorly correlates with imaging, reflecting complex pathobiology. Neuroinflammation, driven by microglia, macrophages, reactive astrocytes, and peripheral immune cells, along with metabolic failure due to ischemia, and hypoxia, contributes to progression while also shaping maladaptive and compensatory neuroplasticity. Growing evidence implicates gut dysbiosis in modulating systemic and spinal cord inflammation via the gut-spinal cord axis, influencing symptoms development and recovery. Bio

Acute Traumatic Spinal Cord Injury.

Approximately 17,000 new spinal cord injuries occur each year in the United States, with motor vehicle collisions and falls being the most common causes. Even though 94% of patients survive the initial injury and corresponding hospitalization, their life expectancy is reduced secondary to the long-term complications that stem from their injury. Every patient should be approached in the same manner (i.e., as if they have a spinal cord injury) until proven otherwise to prevent additional injury. Important considerations that should take place in the emergency department include the patient's air

Spinal Cord Anatomy and Localization.

PURPOSE OF REVIEW: This article focuses on clinically relevant teaching points in spinal anatomy and localizing the lesion in myelopathy. RECENT FINDINGS: The principles underlying spinal cord lesion localization are well established, but improvements in MRI and the discovery of pathologic antibodies associated with causes of transverse myelitis distinct from multiple sclerosis, such as aquaporin-4 IgG and myelin oligodendrocyte glycoprotein IgG, have assisted in diagnosis. SUMMARY: The spinal cord has a highly organized neuroanatomy of ascending and descending tracts that convey sensory, mo

Acute Traumatic Spinal Cord Injury.

Traumatic spinal cord injury is a common neurologic insult worldwide that can result in severe disability. Early stabilization of the patient's airway, breathing, and circulation as well as cervical and thoracolumbar spinal immobilization is necessary to prevent additional injury and optimize outcomes. Computed tomography (CT) scan and magnetic resonance imaging (MRI) of the spinal column can assist with determining the extent of bony and ligamentous injury, which will guide surgical management. With or without surgical intervention, patients with spinal cord injury require intensive care unit

Metastatic epidural spinal cord compression.

Up-to-date management for metastatic epidural spinal cord compression is based on appropriate scoring systems. In this case, separation surgery and stereotactic radiosurgery achieved an optimized outcome. A 75-year-old man had thoracic band-like pain, left lower limb weakness and difficulty ambulating for 6 weeks. Spinal images showed a T4 metastatic epidural spinal cord compression with a pathological fracture of T4 and T10 and L3 lytic lesions. There was no other neoplastic lesion. The metastasized T4 was decompressed by a posterolateral right costotransversectomy, vertebrectomy and insertio

HIV and spinal cord disease.

The epidemiology of spinal cord disease in human immunodeficiency virus (HIV) infection is largely unknown due to a paucity of data since combination antiretroviral therapy (cART). HIV mediates spinal cord injury indirectly, by immune modulation, degeneration, or associated infections and neoplasms. The pathologies vary and range from cytotoxic necrosis to demyelination and vasculitis. Control of HIV determines the differential for all neurologic presentations in infected individuals. Primary HIV-associated acute transverse myelitis, an acute inflammatory condition with pathologic similarities

Spinal Cord Stereotaxy: An Overview.

The origin of spinal cord stereotaxy can be traced back to the 19th-century work of Woroshiloff, the pioneer of brain stereotaxy. The development of clinical brain stereotaxy began in the mid-20th century, but spinal cord stereotaxy lagged behind. The first stereotactic spinal cord surgery was successfully performed by Hitchcock for pain treatment in the 1960s, and surgery for urinary bladder hyperspasticity performed by Nádvorník followed several years later. Other stereotactic surgeries of the spinal cord movement system could not be considered until Slovak anatomist Čierny used animal exper

Adiposity and spinal cord injury.

The drastic changes in body composition following spinal cord injury (SCI) have been shown to play a significant role in cardiovascular and metabolic health. The pattern of storage and distribution of different types of adipose tissue may impact metabolic health variables similar to carbohydrate, lipid and bone metabolism. The use of magnetic resonance imaging provides insights on the interplay among different regional adipose tissue compartments and their role in developing chronic diseases. Regional adipose tissue can be either distributed centrally or peripherally into subcutaneous and ecto

Spinal cord: motor neuron diseases.

Spinal cord motor neuron diseases affect lower motor neurons in the ventral horn. This article focuses on the most common spinal cord motor neuron disease, amyotrophic lateral sclerosis, which also affects upper motor neurons. Also discussed are other motor neuron diseases that only affect the lower motor neurons. Despite the identification of several genes associated with familial amyotrophic lateral sclerosis, the pathogenesis of this complex disease remains elusive.

Spinal cord compression in pseudohypoparathyroidism.

BACKGROUND CONTEXT: Spinal cord compression associated with pseudohypoparathyroidism (PHP) is an increasingly reported sequelae of the underlying metabolic syndrome. The association of neurologic dysfunction with PHP is not well appreciated. We believe this to be secondary to a combination of underlying congenital stenosis, manifest by short pedicles secondary to premature physeal closure, and hypertrophic ossification of the vertebral bony and ligamentous complexes. PURPOSE: The purpose of this case report is to review the case of spinal stenosis in a child with PHP Type Ia. We are aware of

Intramedullary spinal cord tumor resection.

The authors present a case of a 27-year-old patient who presented with spastic gait and worsening difficulty walking over a 6 month period. Spinal MR imaging revealed a heterogeneously enhancing intramedullary spinal cord tumor (IMSCT) with associated syrinx in the cervical spine. The lesion was resected through posterior en bloc laminotomy, durotomy, and microscopic resection of the intramedullary component followed by laminoplasty reconstruction. Surgical resections with a goal of gross total resection can significantly improve overall survival and progression free survival in patients with
