The effectiveness of spinal cord stimulation for control of spasticity was studied in 59 spinal cord injury patients. SCS was markedly or moderately effective in reducing spasticity in 63% of the patients. We found that control of spasticity by SCS was not correlated with the severity of spasticity, the type of spasticity (flexor or extensor), or the ability to ambulate. However, stimulation was more effective in patients with incomplete cervical lesions than in complete cervical lesions. Stimulation below the lesion was more effective than above. We conclude that SCS was effective when electr

BACKGROUND: Spinal cord injury (SCI) is frequently associated with orthostatic hypotension, defined by a sustained decrease in blood pressure upon assuming an upright posture due to impaired autonomic regulation. Cardiovascular spinal cord epidural stimulation (CV-scES) can regulate systolic blood pressure (SBP) in people with SCI, but stimulation paradigms are highly individualized. To make this treatment available to more patients, we developed an algorithm to tailor individualized CV-scES paradigms that closely mimic researcher-developed paradigms.
METHODS: We performed an offline analysis

IMPORTANCE: Individuals with spinal cord injury (SCI) have significant autonomic nervous system dysfunction. However, despite recent findings postulated to support that spinal cord stimulation improves dynamic autonomic regulation, limited scope of previous testing means the true effects remain unknown.
OBJECTIVE: To determine whether transcutaneous spinal cord stimulation improves dynamic autonomic regulation after SCI.
DESIGN: Single-blinded, randomized crossover trial with matched cohorts.
SETTING: Academic autonomic physiology laboratory.
PARTICIPANTS: Two pairs of well-matched individ

BACKGROUND: The recovery of autonomic functions and the ability to reproduce in particular is of the highest priority to individuals with spinal cord injury (SCI). The potential of epidural spinal cord stimulation (ESCS) for promoting recovery of sensorimotor functions in the chronic phase of SCI has long been studied. In recent years, several studies have emerged confirming the positive effect of ESCS also on the cardiovascular system and neurogenic bladder and bowel. However, the potential of ESCS in restoring sexual function, especially ejaculation, has not yet been addressed.

Spinal cord injury (SCI) is a severe central nervous system injury that can cause sensory or motor dysfunction. Although mortality rates for people with spinal cord injuries have dropped dramatically with advances in medicine, chronic long-term sequelae after SCI persist. The most bothersome problems reported by patients include pain, spasticity, urinary dysfunction, and loss of motor function. Thus, quality of life (QoL) is an essential issue in chronic SCI. Spinal cord stimulation (SCS) applies an adjustable, nondamaging electrical pulse that can reduce uncomfortable comorbidities and improv

Multisegmental muscle responses (MMR) are reflexes in the leg muscles evoked by transcutaneous electrical spinal cord stimulation over the Th11–Th12 vertebrae. We have used MMR to evaluate the excitability of lumbosacral motor neurons in individuals suffering lower paraplegia. Ten individuals were tested using H-reflexes and MMR bilaterally before (n 0 = 20) and during 4-weeks course of rehabilitation (n=76). The H-reflex and MMR of m. gastrocnemius lateralis were obtained in: 15 and 13 cases out of 20, respectively. Both reflexes were recorded in 11 and were absent in 3 cases, matched up to 7

BACKGROUND: Spinal cord stimulation (SCS) is an efficacious, safe, and well-documented procedure for treating chronic refractory pain syndromes. We report a rare case of spinal cord injury (SCI) after SCS implantation in a 54-year-old woman.
CASE REPORT: A 54-year-old woman underwent reimplantation of a new SCS following an acute L2 compression fracture that led to significant pain and dysesthesia. The intraoperative course was uncomplicated. Immediately postoperatively, the patient experienced an inability to move her right leg, with magnetic resonance imaging showing epidural fluid collecti

INTRODUCTION: While spinal cord stimulator implant is an increasingly safe procedure, one of the most feared complications is spinal cord injury. Still, literature regarding its incidence remains highly variable. This retrospective analysis aims to evaluate the incidence of spinal cord injury after spinal cord stimulator implant using a large-scale claims database.
METHODS: The PearlDiver-Mariner database of national all payer claims was used to identify patients who underwent spinal cord stimulator implant (percutaneous or paddle) and developed subsequent spinal cord injury within 45 days. T

OBJECTIVES: This study estimates the economic and organizational impact of adopting the All-in-One Spinal Cord Stimulation (SCS) implantation strategy compared to the traditional two-stage approach for chronic pain patients in Spain. We assess savings, resource utilization, and the success rate threshold above which it would be more profitable to perform the procedure in a single phase. METHODS: A retrospective study analyzed Real-World Data (RWD) from chronic pain patients treated with SCS between 2012 and 2023. Data included socio-demographics, resource use, and post-implant procedures. A he

OBJECTIVE: To determine whether psychological variables independently predict spinal cord stimulation success or instead moderate pain-response dynamics over time.
METHODS: This retrospective cohort included 217 patients undergoing SCS implantation, with available follow-up in 153 at 6 months and 155 at 12 months. Treatment success was defined as a ≥30% reduction in Numeric Rating Scale (NRS) pain from baseline. Baseline psychological measures included Beck Depression Inventory (BDI) categories (Minimal/Mild/Moderate/Severe), Pain Disability Index (PDI) categories (Low/High), and 10 binary Pe

Spinal cord stimulation (SCS) is an established neuromodulatory therapy for refractory chronic pain; however, the timing of SCS initiation has received relatively little attention. In some patients requiring spine surgery, the indication for SCS is already present at the time of surgery. Percutaneous leads combine reversibility with low invasiveness, allowing the timing of SCS introduction to be matched to clinical need. We describe a technical approach for simultaneous percutaneous SCS lead placement during spine surgery and present a retrospective case series of five illustrative cases. This

UNLABELLED: Hereditary spastic paraplegia is a diverse group of inherited neurodegenerative diseases characterized by spasticity and weakness in the lower extremities. Treatment is comprehensive and includes pharmacotherapy and physical therapy. If these approaches are ineffective, neurosurgical methods such as rhizotomy or intrathecal baclofen therapy are applied. Epidural spinal cord stimulation is still poorly studied.
OBJECTIVE: To evaluate safety and efficacy of epidural spinal cord stimulation in patients with hereditary spastic paraplegia.
MATERIAL AND METHODS: The study (2015-2024) i

Cervical spinal cord stimulation (SCS) improves arm function in people with chronic post-stroke hemiparesis, yet its mechanism remains unclear. The leading hypothesis is that SCS enhances motor drive to agonist muscles. To test this, we developed a proportional-derivative (PD) control model of agonist-antagonist interactions and estimated feedback control gains in 12 healthy and 5 stroke participants. In healthy individuals, gains followed a stereotypical structure that we showed was optimal for generating smooth, stable movements. In stroke participants, this structure was disrupted but shift

Spinal cord injury (SCI) is a central nervous system (CNS) disease with a high disability rate, and reconstructing motor function after SCI remains a global challenge. Recent advancements in rehabilitation and regenerative medicine offer new approaches to SCI repair. Electrical stimulation has been shown to alter cell membrane charge distribution, generating action potentials, and affecting cell behavior. This method aids axon regeneration and neurotrophic factor upregulation, crucial for nerve repair. Biomaterials, used as scaffolds or coatings in cell culture and tissue engineering, enhance

BACKGROUND: Chemotherapy-induced peripheral neuropathy (CIPN) is one of the most common persistent adverse effects of chemotherapy affecting cancer survivors. Other than the use of duloxetine, no other intervention has a proven benefit for these patients. Here, a patient with multiple myeloma who developed CIPN experienced a significant reduction in their symptoms after the implantation of a spinal cord stimulator.
CASE DESCRIPTION: A 64-year-old male with a history of multiple myeloma presented with chronic pain 4 years after chemotherapy, followed by an autologous stem cell transplant. As t
