Spinal cord compression in Paget's disease.

Three cases of spinal cord compression secondary to Paget's disease of the spine are reported. In two of the cases a relatively short history with pain as a prominent feature suggested initially a diagnosis of extradural malignancy. The usual clinical features of this rare disorder are discussed and the characteristic radiological findings are emphasized.

Spinal-evoked potentials in dogs with acute compressive thoracolumbar spinal cord disease.

Spinal evoked potentials (SpEP) were recorded on an electromyograph from electrodes placed percutaneously in the ligamentum flava at the lumbosacral junction and between the 10th and 11th thoracic vertebrae following tibial nerve stimulation in 31 anesthetized dogs with acute compressive spinal cord injuries. The neurologic status of each dog was determined by clinical examination before SpEP recordings, and the neurologic status was monitored for 2 months in dogs that had surgical or conservative treatment. Two months after spinal injury, the response to treatment (outcome) of each dog was ev

Increasing Incidence of Degenerative Spinal Diseases in Japan during 25 Years: The Registration System of Spinal Surgery in Tohoku University Spine Society.

Spinal disorders affect mainly older people and cause pain, paralysis and/or deformities of the trunk and/or extremities, which could eventually disturb locomotive functions. For ensuring safe and high-quality treatment of spinal disorders, in 1987, the Tohoku University Spine Society (TUSS) was established by orthopedic departments in Tohoku University School of Medicine and its affiliated hospitals in and around Miyagi Prefecture. All spine surgeries have been enrolled in the TUSS Spine Registry since 1988. Using the data from this registration system between 1988 and 2012, we demonstrate he

[Emphasize functional magnetic resonance technology to enhance the clinical precise assessment of spinal diseases].

In the context of precision medicine, functional magnetic resonance imaging (fMRI) offers a key breakthrough in the diagnosis and treatment of spinal disorders, shifting the focus from morphological to functional assessment. Microstructural imaging techniques such as diffusion tensor imaging (DTI) of the spine enable early identification of pathological changes, such as demyelination and axonal degeneration, in the spinal cord and nerve roots, thereby facilitating ultra-early diagnosis and differential diagnosis. DTI can also assess the severity of neural injury, accurately localize the respon

Research waste in randomized clinical trials of degenerative spinal diseases: a cross-sectional study.

Evidence-based medicine continues to face the challenge of research waste (i.e., nonpublication, reporting limitations, high risk of bias) in randomized clinical trials (RCTs) of degenerative spinal diseases (DSDs). A search was conducted on ClinicalTrials.gov for RCTs registered before July 27, 2024 using the keywords DSDs and related terms. The major outcome was the percentage of trials exhibiting the characteristics of research waste on DSD RCTs. A total of 335 DSD RCTs were eligible, with a lower percentage of multicenter trials outside North America than in North America (33 [14.7%] vs. 5

Normal Pressure Hydrocephalus Overshadowed by Traumatic and Degenerative Spinal Diseases: A New Assessment Proposal.

AIM: To evaluate normal pressure hydrocephalus (NPH) patients who admitted to clinic with a walking disability, depressed mood, cognitive dysfunction, and compromised quality of life in order to create a system for minimizing misdiagnosis, and inaccurate treatment of NPH at the first instance. MATERIAL AND METHODS: In this study, patients diagnosed with NPH and registered in our clinical database from 2015 to May 2019 were retrospectively screened, and a ?Ste-P? formula was applied to devise a new diagnostic scoring system. Representative cases showed that some of the patients received inaccu

[Spinal cord protection during open and endovascular surgery in thoracic and thoracoabdominal aorta diseases].

In recent decades great advances have been made in surgical procedures for treating thoracic and thoracoabdominal aorta defects. Associated mortality and morbidity rates have dropped considerably, mainly in major reference centers, but nonetheless continue to be significant. The need for new strategies to reduce mortality and morbidity has made endovascular approaches an attractive alternative for high-risk surgical patients. The most feared complications of these procedures include paraparesis and paraplegia, which have devastating consequences on patients' quality of life. We provide an upda

[Preimplantation genetic diagnosis in prevention of genetic diseases --diagnostic of spinal muscular atrophy (SMA)].

INTRODUCTION: Preimplantation genetic diagnosis, PGD, is an established procedure allowing genetic research of the oocyte or embryo before implantation to the uterus. A neurodegenerative disorder known as spinal muscular atrophy (SMA) is the second most common lethal autosomal recessive disease in Caucasians, after cystic fibrosis. There are three clinically different types of the disease, with type I (Werding-Hoffman Disease) being the most severe. Around 98% of clinical cases of SMA are caused by the homozygous absence of a region of exons 7 and 8 of the telomeric copy of the SMN gene (SMN1)

MR diffusion tensor imaging and fiber tracking in inflammatory diseases of the spinal cord.

PURPOSE: Our aim was to study the fractional anisotropy (FA) variations and the fiber tracking (FT) patterns observed in patients with myelitis. MATERIAL AND METHODS: Fifteen patients with symptomatic myelitis and 11 healthy subjects were prospectively selected. We performed T2-weighted and diffusion tensor imaging on a 1.5T MR scanner. FA and apparent diffusion coefficient maps were computed in both healthy subjects and patients. In each patient, we performed FT to study pathologic aspects on this imaging method. FA data were analyzed by using z-scores. RESULTS: For the healthy subjects, av

Objective evaluation of pain in various spinal diseases: neuropeptide immunoreactivity in the cerebrospinal fluid.

A quantitative analysis was performed of substance P-like immunoreactivity (SPLI) and of beta-endorphin-like immunoreactivity (beta-ENDLI), in the cerebrospinal fluid (CSF) in various diseases. The results reported to date have not been consistent. The purpose of this study was to investigate whether or not the concentration of SPLI or that of beta-ENDLI in CSF demonstrated any potential for assessing the degree of subjective pain in various spinal diseases. SPLI in CSF was measured by radioimmunoassay in 158 patients with a spinal disease; involving 57 patients with a lumbar disc herniation (

Examination of spinal cord in diseases of the craniocervical junction and high cervical spine.

A simple necropsy technique for the removal of the craniocervical junction was devised: a relatively small specimen comprising part of the clivus, the foramen magnum, and cervical vertebral canal is removed in one piece with the medulla and spinal cord inside, and examined systematically after fixation. This method, used in a series of patients with chronic craniocervical instability, allows both good clinicopathological correlations to be made and histological changes in the lower medulla or upper cervical cord segments to be related to sites of extrinsic compression.

Diffuse Idiopathic Skeletal Hyperostosis (DISH): Potential Links Between Gut Microbiota and Spinal Degenerative Diseases.

Diffuse Idiopathic Skeletal Hyperostosis (DISH) is a systemic, non-inflammatory skeletal disorder characterized by progressive calcification and ossification of spinal ligaments and entheses. Its strong associations with aging, obesity, type 2 diabetes, and metabolic syndrome raise the possibility that metabolic and inflammatory pathways may contribute to disease development. The gut microbiota participates in nutrient metabolism, short-chain fatty acid production, bile-acid transformation, intestinal-barrier maintenance, and immune regulation; these functions provide a plausible framework for
