Adjacent segment disease (ASD) was described after long-term follow-up of patients treated with cervical fusion. The term describes new-onset radiculopathy or myelopathy referable to a motion segment adjacent to previous arthrodesis and often attributed to alterations in the biomechanical environment after fusion. Evidence suggests that ASD affects between 2% and 3% of patients per year. Although prevention of ASD was one major impetus behind the development of motion-sparing surgery, the literature does not yet clearly distinguish a difference in the rate of ASD between fusion and disk replac

PURPOSE: To review outcomes of laminoplasty after anterior spinal fusion (ASF) in 8 patients with cervical spondylotic myelopathy (CSM).
METHODS: Records of 3 men and 5 women aged 49 to 80 (mean, 60) years who underwent laminoplasty after ASF for CSM were reviewed. Before and after ASF and laminoplasty, the causes of CSM, mechanical instability, the Pavlov Torg ratio, the numbers of levels of stenosis, myelomalacia, ASF, and laminoplasty, the modified Japanese Orthopaedic Association (JOA) score, and the Hirabayashi recovery rate were recorded in all the patients.
RESULTS: After ASF, the mea

BACKGROUND: The objective of this study is to provide a retrospective analysis using an NIS database to examine national trends in outcomes for CSM from 1993 to 2002.
METHODS: Data for CSM admissions (n = 138792) were extracted from the 1993 to 2002 NIS database to determine overall outcomes, as well as for those patients with CSM who underwent spinal fusion. Data from 1993 to 1997 (period 1) were compared with data from 1998 to 2002 (period 2).
RESULTS: The number of patients admitted with CSM increased 2-fold from 3.73 to 7.88 per 100000 US population. Approximately 10% of patients were ad

The use of biologic technologies to engender spinal fusion is at various stages of scientific and clinical development. Recombinant human bone morphogenetic protein 2 is now available for clinical use in anterior lumbar intervertebral fusion and has been extensively studied in other spinal fusion applications. Recombinant bone morphogenetic protein 7 is under examination in regulated clinical trials. Gene therapy technologies are in the preclinical stage of development. In time, biologic interventions will continue to capture greater roles in the treatment of spinal disease.

"Standard", noninstrumented, techniques of anterior interbody fusion are frequently followed by nonunion and collapse of the intervertebral space, probably because of persistent rocking movements, particularly in the sagittal plane. Elimination of these theoretical movements by supplementing an anterior interbody fusion with a posterior interspinous H-graft and a cerclage wire was considered to be biomechanically attractive without having the disadvantages associated with posterior instrumentation. In a prospective study a solid fusion was obtained at 16 of 17 operated levels, with a mediocre

Before any radiologic imaging modality is employed in an investigation study, its efficacy must be critically assessed. The purpose of this report is to demonstrate how the choice of computed tomography imaging parameters affects the information provided by a computed tomographic examination. It is apparent from these results that imaging parameters must be optimized before the results of an imaging technique can be compared to other modes of diagnostic evaluation, including surgical observations.

BACKGROUND: Adult spinal deformity (ASD) alters posture and gait, leading to greater functional impairment and lower quality of life than other chronic conditions [1, 2]. Coronal deformities (scoliosis) and sagittal imbalance impact gait differently [3]. Spinal fusion is associated with improvements in pain and physical function, yet little is known about its effects on gait [4-6].
RESEARCH QUESTION: How do spatiotemporal (SPT) parameters and joint kinematics differ in ASD patients with either coronal or sagittal imbalance compared to controls, and does spinal fusion improve SPT and joint kin

Spinal cord injury (SCI) is a severely disabling and catastrophic condition that poses significant global clinical challenges. The difficulty of SCI repair results from the distinctive pathophysiological mechanisms, which are characterised by limited regenerative capacity and inadequate neuroplasticity of the spinal cord. Additionally, the formation of cystic cavities and astrocytic scars after SCI further obstructs both the ascending and descending neural conduction pathways. Consequently, the urgent challenge in post-SCI recovery lies in repairing the damaged spinal cord to reconstruct a fun

BACKGROUND AND OBJECTIVES: Adipose-derived stem cells (ADSCs) offer a practical alternative to bone marrow-derived stem cells. They are often limited by poor cell survival following implantation into the hypoxic environment surrounding the fusion site. Dimethyloxalylglycine (DMOG), a stabilizer of hypoxia-inducible factor-1α, has been shown to boost both osteogenic and angiogenic functions of mesenchymal stem cells under low oxygen conditions. In this study, we investigated whether preconditioning ADSCs with DMOG and hypoxia could improve their bone-forming capacity and stimulate vascularizati

INTRODUCTION: Spinal deformity in adults is a major public health problem. After failure of conservative treatment, correction and fusion surgery leads to clinical and radiological improvement. However, mechanical complications and more particularly - proximal junctional kyphosis (PJK) - are common with an incidence of 10%-40% depending on the studies.
ANALYSIS: Several risk factors have been identified and can be grouped into three categories. Among the patient-related factors, advanced age, comorbidities, osteoporosis and sarcopenia play a determining role. Among the radiological factors, c

The established surgical procedure of spinal fusion, in which two or more spinal vertebrae are permanently connected, is constantly increasing worldwide due to new developments in fusion procedures and surgical techniques, as well as improved implants, even though there is still no consensus on whether fusion leads to subsequent degeneration of adjacent spinal segments. We outline the potential biomechanical impacts of mono-segmental and multi-segmental fusion on the spinal structures, especially on the adjacent functional spinal units (FSU), using MultiBody Simulation (MBS). In addition, we i

Sagittal balance is widely recognized as the primary determinant of optimal outcomes in adult spinal deformity. In adolescent idiopathic scoliosis (AIS), coronal correction risks being obtained at the expense of sagittal malalignment after posterior spinal fusion. Apical lordosis, often underestimated on two-dimensional imaging, is the primary deforming factor in AIS. Failure to restore thoracic kyphosis and lumbar lordosis during posterior spinal fusion contributes to problematic early surgical complications, including proximal or distal junctional kyphosis and failure. Although adolescent pa

RATIONALE: Facioscapulohumeral muscular dystrophy (FSHD) is the third most common muscular dystrophy, which is associated with facial, shoulder girdle, and paraspinal muscle atrophy. Most of the patients develop hypokyphosis and hyperlordosis in the course of the disease, to preserve standing posture. Corrective fusion is contraindicated in these patients as the surgery results with loss of compensatory hyperlordosis and leads to loss of trunk balance while standing. Although spinal fusion in neuromuscular scoliosis is a known treatment option, there are no studies in the literature on the spi

Approximately half a million spinal fusion procedures are performed annually in the US. It is estimated that up to one-third of arthrodesis constructs require revision surgeries. In this study the authors present endoscopic treatment strategies targeting 3 types of complications following arthrodesis surgery: 1) adjacent-level foraminal stenosis; 2) foraminal stenosis at an arthrodesis segment; and 3) stenosis caused by a displaced interbody cage. A retrospective chart review of 11 patients with a mean age of 68 ± 15 years was performed (continuous variables are shown as the mean ± SEM). All p

BACKGROUND: The children with congenital heart disease (CHD) have an increased risk of developing scoliosis. Many children with CHD may benefit from surgical intervention for scoliosis. However, there is uncertainty about the safety of spinal fusion in children with heart disease. The purpose of this study is to assess whether spinal fusion is safe and effective for children who have CHD.
METHODS: We reviewed all the cases of children with CHD undergoing first-time spinal fusion for spinal deformity at Mayo Clinic between August 1976 and June 2008. Data were abstracted regarding cardiac histo
