Flow volume spirometry as a test for postoperative airway evaluation following arytenoidectomy.

The postoperative upper airway of nine patients who had undergone Woodman procedures for bilateral vocal cord paralysis were assessed by means of flow volume spirometry using the FIF-50 measurement expressed in percentage of normal. There appears to be a good correlation between the postoperative FIF-50 and the degree of postoperative subjective laryngeal obstruction.

Small airway dysfunction across alpha-1 antitrypsin deficiency genotypes with preserved spirometry.

PURPOSE: Alpha-1 antitrypsin deficiency (AATD) is a genetic disorder that generally predisposes to pulmonary emphysema with or without chronic obstructive pulmonary disease (COPD). Despite the lack of spirometric obstruction, early Small Airways Dysfunction (SAD) may occur in AATD patients preceding lung damage. METHODS: We conducted a retrospective, single-center study on 60 AATD patients without airflow obstruction (FEV₁/FVC ≥ 0.70). SAD was defined as: reduced spirometric flows (≥2 of FEF25-75, FEF50, FEF75 <65% predicted) and/or IOS R5-R20 value greater than 0.07 kPa•s•L⁻¹. Correlation an

Expiratory time constant patterns reveal obstructive lung mechanics in non-obstructive spirometry.

Based on the principle that increased airway resistance prolongs the expiratory time constant (TC), we investigated whether TC patterns could serve as a signature for obstruction. In this cross-sectional multicenter study of 19,937 validated spirometry records, we divided the descending expiratory flow-volume curve into five equal volume segments and derived consecutive TCs for each. A K-means algorithm trained on the normal subset defined data-driven patterns, which were then used to classify the entire cohort. We quantified pattern prevalence and used propensity score matching to assess spir

Online home spirometry in national pulmonary fibrosis care: insights from daily practice.

BACKGROUND: Monitoring lung function course in patients with pulmonary fibrosis is crucial to guide treatment decisions. Online home spirometry holds great potential for close monitoring and improving care access in times of pressured healthcare systems. However, little data is available on its large-scale use in daily practice. We evaluated the clinical applicability of online home spirometry in pulmonary fibrosis care. METHODS: We analysed data of a nationwide Dutch cohort of patients with idiopathic pulmonary fibrosis (IPF) and other forms of pulmonary fibrosis (PF) that used a home monito

Mechanism of spirometry associated gastro-esophageal reflux in individuals undergoing esophageal assessment.

Persistent variability observed during spirometry, even when technical and personal factors are controlled, has prompted interest in uncovering its underlying mechanisms. Notably, our prior investigations have unveiled that spirometry has the potential to trigger gastro-esophageal reflux in a susceptible population. This current study embarks on elucidating the intricate mechanisms orchestrating reflux induced by spirometry. To achieve this, we enlisted twenty-four (24) participants exhibiting reflux symptoms for esophageal assessment. These participants underwent two sets of spirometry sessio

Lung function in white children aged 4 to 19 years: I--Spirometry.

OBJECTIVE: A study was performed to produce reference standards for spirometric lung function in white children and to calculate standard deviation scores adjusted for gender and pubertal stage. METHODS: A cross sectional study was made of 772 white children aged 4.6 to 18.8 years (455 male) tested on an OHIO 840 spirometer and assessed anthropometrically and pubertally. RESULTS: Before puberty there was a linear increase in all lung function measurements with height. During puberty a sudden increase occurred, but subsequently the relationship was again linear. No simple single equation desc

Spirometry Monitoring and Asthma Responsibility in Kids (SMARt-Kids): A Mixed-Methods Study.

INTRODUCTION: Asthma affects 4.6 million U.S. children, and home spirometry offers a promising monitoring tool, yet research on its use in children is limited. This study explored feasibility, acceptability, usability, barriers, facilitators, and management responsibility of home spirometry in school-aged children with persistent asthma. METHODS: A convergent mixed-methods design was used to assess home spirometry feasibility, acceptability, usability, and management responsibility in 11 parent-child dyads (children aged 7-11), sampled from a completed RCT (NCT04908384). RESULTS: Both parent

Associations of cardiorespiratory fitness and body mass index with incident restrictive spirometry pattern.

OBJECTIVES: Restrictive spirometry pattern (RSP) suggests an impairment of lung function associated with a significantly increased risk of premature mortality. We evaluated the independent and joint associations of cardiorespiratory fitness (CRF) and body mass index with incident RSP. METHODS: Data from the Aerobics Centre Longitudinal Study included 12 360 participants (18-82 years). CRF was assessed by maximal treadmill test and categorised into five groups. Body mass index was categorised into normal weight (<25.0 kg/m2), overweight (25.0-29.9 kg/m2) or obesity (≥30.0 kg/m2). RSP was defin

Impact of preheart transplant spirometry and DLCO measurement on post-transplant pulmonary outcomes.

BACKGROUND: Pulmonary function tests (PFT) are a frequent component of heart transplant evaluation. In cardiac surgery abnormal PFTs, especially reduced DLCO, have been associated with poor outcomes. We sought to evaluate the impact of pretransplant PFTs on post-transplant pulmonary outcomes and patient survival. METHODS: Among the 652 adult heart transplant recipients between January 1, 2010 and July 31, 2021, 462 had PFTs and constituted the patient cohort. Obstructive ventilatory defects (OVD), restrictive ventilatory defects (RVD), and reduced DLCO were defined according to established cr

Race-specific spirometry equations may overestimate asthma control in Black children and adolescents.

BACKGROUND: A growing body of evidence suggests that use of race terms in spirometry reference equations underestimates disease burden in Black populations, which may lead to disparities in pulmonary disease outcomes. Data on asthma-specific health consequences of using race-adjusted spirometry are lacking. METHODS: We performed a secondary analysis of 163 children from two observational asthma studies to determine the frequencies of participants with ppFEV1 < 80% (consistent with uncontrolled asthma) or ppFEV1 ≥ 80% using race-specific (GLI-African American or Caucasian) vs. race-neutral (GL

Initiating home spirometry for children during the COVID-19 pandemic - A practical guide.

The COVID-19 pandemic has led to a rapid escalation in use of home monitoring and video consultations in children with a variety of chronic respiratory conditions. Our department set up a home spirometry service from scratch once it became evident that we needed to keep patients away from hospital clinics whenever possible. We faced a number of challenges but now have around 400 children using home spirometers. There are a number of portable spirometers available, some with online platforms. The technology, particularly the software/apps interface, has been improved by the companies in respons

Association between blood pressure, inflammation and spirometry parameters in chronic obstructive pulmonary disease.

BACKGROUND/AIMS: Many systems including the cardiovascular system (ischemic heart diseases, heart failure, and hypertension) may act as comorbidities that can be seen during the course of chronic obstructive pulmonary disease (COPD). Comorbidities affect the severity and prognosis of COPD negatively. Nearly 25% of patients with COPD die due to cardiovascular diseases. In this study, we aimed to evaluate the relationship between the blood pressure, inflammation, hypoxia, hypercapnia, and the severity of airway obstruction. METHODS: We included 75 COPD patients in the study with 45 control case

Modern Spirometry Supports Anesthetic Management in Small Animal Clinical Practice: A Case Series.

Modern spirometry, like no other monitoring technique, allows insight into breath-to-breath respiratory mechanics. Spirometers continuously measure volume, airway pressure, and flow while calculating and continuously displaying respiratory system compliance and resistance in the form of loops. The aim of this case series is to show how observation of spirometric loops, similar to electrocardiogram or CO2 curve monitoring, can improve safety of anesthetic management in small animals. Spirometric monitoring cases described in this case series are based on use of the anaesthesia monitor Capnomac

Occupational associations of preserved ratio impaired spirometry (PRISm) in a population-based study.

INTRODUCTION: Preserved ratio impaired spirometry (PRISm) has never been characterized in Kazakhstan, and risk factors for this condition remain poorly described. Given that smoking was associated with PRISm, other sources of air pollution, including occupational exposures, surprisingly remained outside the scope of many studies. We, therefore, aimed to compute the prevalence and test the association of exposure to airborne pollutants in the workplace with PRISm in a cross-sectional study. METHODS: Subjects (N = 2,275, median age 41 (interquartile range (IQR) 29; 56) years, 54% men) were recr
