[Incomplete partition type III revisited-long-term results following cochlear implant. German version].

BACKGROUND: Incomplete partition type III (IP III) is defined by a missing lamina cribrosa between the cochlea and the internal auditory canal (IAC). Cochlear implantation (CI) may result in an insertion of the electrode array into the IAC. The aim of this study is to evaluate CI surgery protocols, long-term audiological outcome, mapping and electrophysiological data after CI in IP III patients. MATERIALS AND METHODS: Nine IP III patients were implanted with perimodiolar electrode arrays between 1999 and 2014; eight of them were included in this study. We evaluated mapping data, stapedius ref

[Correlation of subjective and objective methods of evaluating hearing threshold].

INTRODUCTION: Objective evaluation of hearing threshold and detection of hearing impairment are the basic prerequisites for hearing and speech rehabilitation. In infants and early pre-school children tonal audiometry is not possible. Though subjective, tonal audiometry is the most reliable method for evaluation of hearing threshold. Therefore, in children of this age, hearing threshold is evaluated by objective electroacoustic methods: early brainstem auditory evoked potentials (BAEP) and acoustic stapedius reflex. MATERIAL AND METHODS: In 60 adult examinees (120 ears) with normal hearing we

Diagnosis of Menière's disease.

Stapedius-reflex measurement and the glycerol-test in combination with pure tone audiogram and speech-audiogram are reliable methods for an exact diagnosis of Menière's disease. For differential diagnosis one should know the so called "tensor tympani syndrome" which is discussed in this paper.

Stapedial reflex in amyotrophic lateral sclerosis.

OBJECTIVE: To examine mechanisms controlling the stapedial reflex in patients with amyotrophic sclerosis (ALS). METHODS: The stapedial reflex was examined using impedance audiometry in 38 patients with sporadic ALS and in 25 age matched controls. RESULTS: All patients showed normal reflex decay test results. There were no significant differences between patients with ALS and control subjects in reflex threshold, latency, amplitude, or contraction time (C50). Although each reflex variable in the patients with classic or progressive muscular atrophy types of ALS showed no significant differenc

[Menière's disease and sudden deafness (author's transl)].

The question is, is there any relation between sudden deafness and Menière's disease? Total hearing loss without recovery is typical of viral infection. In cases of sudden deafness some hearing rest generally remains. It can be the first symptom of Menière's disease, like the first attacks of vertigo which may occur without deafness. Measurements of stapedius reflex threshold and Békésy audiogram, especially the difference between the impulse tone and continuous tone indicate an inner hair cell damage and are helpful in the diagnosis of sudden deafness. But pathological adaptations are seen in

Dental malocclusion stimulates neuromuscular circuits associated with temporomandibular disorders.

Unilateral anterior crossbite (UAC) has been demonstrated to cause masseter hyperactivity via the periodontal trigeminal mesencephalic nucleus (Vme)-trigeminal motor nucleus circuit. Here, we studied activation of motor neurons of the facial nucleus (VII), hypoglossal nucleus (XII), nucleus ambiguus (Amb), and spinal nucleus of the accessory nerve (SNA) in rats with UAC via their similar connections with Vme. An anterograde tracer, biotinylated dextran amine (BDA), was injected into the Vme to identify the central axon terminals around the motor neurons of VII, XII, Amb, and SNA. The expressio

Prevalence of hearing loss in patients with late-onset Pompe disease: Audiological and otological consequences.

OBJECTIVE: The metabolic disorder Pompe disease mainly affects the skeletal muscle in adults. The hearing impairment due to stapedius muscle involvement in adult patients is not known. DESIGN: The frequency, severity, and type of hearing impairment was characterized prospectively using pure-tone audiometry, tympanometry, stapedial reflexes, otoacoustic emissions, and brainstem-evoked response audiometry in adult patients on enzyme replacement therapy for genetically confirmed Pompe disease. STUDY SAMPLE: 11 adult patients (median age: 47 years, range: 22-71). RESULTS: Four patients complain

Familial stapes superstructure fixation.

Congenital stapes superstructure fixation was found in a mother in both ears and her daughter in one ear. The head of the stapes was fixed to the pyramid by a bony bar which lay immediately superior to a normal stapedius muscle. Removal of the bony bar resulted in normal hearing, which has been maintained. We believe that these are the first reported cases of this abnormality.

Oculostapedial synkinesis following Bell's palsy.

A case of oculostapedial synkinesis occurring after Bell's palsy is described. This rare phenomenon has not previously been reported following Bell's palsy. The authors discuss the method of objectively proving the diagnosis, which can be difficult. The patient was successfully treated by stapedius tendon section under local anaesthesia.

[Impedance measurements and otosclerosis (author's transl)].

Impedance measurements (model Madsen ZO 72) were performed on 158 patients with otosclerosis and compared with the operative findings. It was found that otosclerosis shows in 95% of the cases a symmetrical tympanogram, type A. Its amplitude does not differ from the standard values: that means that otosclerosis does not correlate with a flattened amplitude of the tympanogram. In addition there is no correlation between the intraoperative finding and the ampitude. The stapedius reflex could not be elicitated by acoustic stimuli.

Vestibular neuritis: a follow-up study.

Seven to eight years after a disease period of vestibular neuritis 19 patients previously examined at our department were given a questionnaire about whether they had experienced any audiovestibular symptoms since they first fell ill. All 19 patients responded and 18 consented to participate in the long-term follow-up study. The patients were subjected to a clinical evaluation, a bithermal caloric test and audiological examinations with stapedius reflex measurements with use of the same technique as at the first examinations. Ten of the 18 patients had experienced recurrent vestibular symptoms

[Middle ear function in children with cleft palate].

Middle ear problems in patients with cleft palate have for a long time received insufficient attention. The present study was designed to evaluate middle ear function in 56 children with cleft palate and in a control group of 50 children without cleft palate. The age ranging between 5 and 14 years was similar in both groups. All children underwent routine otoscopic examinations of ear, nose and throat, and were also examined using the Madsen acoustic impedance audiometer. Four major parameters (static compliance, middle ear pressure, acoustic stapedius reflex and Eustachian tube function) were

The Efficacy of Acupuncture and Bee Venom Injections in Improving Hearing in a Patient with Idiopathic Conductive Hearing Loss: a case report.

Conductive hearing loss (CHL) is commonly caused by tympanic membrane perforation or otitis media. Congenital ossicular anomalies and otosclerosis are the primary considerations in patients with normal tympanic membranes and external auditory canals, whereas stapedius tendon abnormalities are less frequently observed. Acupuncture and bee venom (BV) injections are frequently used for treating otological disorders; however, their effectiveness in treating CHL remains unexplored. Therefore, this case report describes a 22-year-old man with idiopathic left-sided CHL that persisted for 101 days. Wh

[Acoustic reflexometry in presbyacusis (author's transl)].

The authors have studied the relations between crossed and uncrossed acoustic stapedius reflex thresholds in presbyacusic subjects in order to verify whether ageing produces brain stem auditory lesions. The results showed in 55% of the presbyacusics a brain stem disorder characterized by the absence of one, several or all the acoustic reflexes; very often there was only an abnormal difference between crossed and uncrossed acoustic reflex thresholds.

Drop attacks secondary to superior canal dehiscence syndrome.

Two patients with unprovoked drop attacks were found to have dehiscence of the superior semicircular canal on CT of the temporal bone. Both had conductive hearing loss, preservation of stapedius reflex, and abnormal vestibular evoked myogenic potentials. Neither had sound- or pressure-induced nystagmus. Repair of the dehiscence in one case stopped the drop attacks, supporting a causal relationship between the dehiscence and the drop attacks.

Developmental anatomy, angiography, and clinical implications of orbital arterial variations involving the stapedial artery.

Classical anatomists have provided detailed description of the arterial collateral pathways found in the head and neck. The small branches building this intricate network are difficult to access. The arterial map inherited from the anatomists has been put to the test with detailed high-resolution vascular imaging. Superselective angiography has helped rediscover the complexity of the craniocervical arterial network. The concept of dangerous collaterals or dangerous anastomoses was born with the advent of endovascular therapy. Although dangerous anastomoses of the skull base are described in th

[Pathological changes of the pyramidal eminence in chronic suppurative otitis media].

Temporal bones of patients with chronic suppurative otitis media were used for evaluation of possibility of penetration of the infection from the cavum tympani into the facial nerve via the pyramidal eminence. The infection can spread to the facial nerve in two ways: via the natural opening on the pyramidal eminence through which tendon of the musculus stapedius is passing, without bone lesions of the eminence and via the sites of bone destruction on the eminence.

The acoustic reflex at low sensation levels.

The acoustic stapedial reflex, which normally occurs at a hearing level of about 80 dB, can be detected at lower levels by means of preactivation. Two techniques are presented, which allow the detection of reflex activity at intensities from 30 to 50 dB HL. It was the authors' intention to find a way to determine the hearing threshold from the stapedius reflex threshold even in patients, in whom the hearing threshold was not horizontal.

Otospongiosis as a genetic disease. Early detection, medical management, and prevention.

Extensive research into the inmost mechanism of otospongiotic disease and an extended study of otosclerotic patients' ancestry have led the authors to a plan for prevention of this disease, which appears to be a genetic deafness with autosomal dominant inheritance and about 40 percent penetrance of genes. Progress in impedance audiometry has permitted early detection of stapedial fixation by means of systematic audiometric investigations, particularly the elicitation of stapedius reflex showing a very special pattern called the "on-off effect." The evidence supporting an enzymatic origin of th
