Perforation of the colon by invading recurrent gastrointestinal stromal tumors during sunitinib treatment.

The molecular targets of sunitinib are receptor tyrosine kinases (RTKs), and this drug has also been known to exert blocking effects on the activation of KIT, which is similar to the mechanism of action of imatinib. Moreover, sunitinib has an additional anti-angiogenic effect through its inhibition of the vascular endothelial growth factor receptor activation. We report here a 70-year-old patient diagnosed with a recurrent gastrointestinal stromal tumor (GIST), which invaded the transverse colon and led to a perforation during sunitinib treatment. A computed tomography scan and 3-dimensional r

Gastrointestinal stromal tumours: can we do better than sunitinib in the second line?

INTRODUCTION: Advances in understanding the molecular mechanisms have led to the development of targeted therapies, which transformed the treatment of gastrointestinal stromal tumor (GIST). Sunitinib is an oral, multi-targeted tyrosine kinase inhibitor (TKI) that suppresses the activity of VEGFR, PDGFR, KIT; and plays a significant role in the second-line treatment of patients with GIST. AREAS COVERED: This article provides an overview of data on treatment of patients with advanced GIST, with particular focus on second-line therapy. It discusses the mechanisms of resistance, the findings of k

Sunitinib induces immunogenic cell death through eIF2α phosphorylation to potentiate immunotherapy in HCC.

Hepatocellular carcinoma (HCC) prognosis remains poor due to its immunosuppressive tumor microenvironment. Inducing immunogenic cell death (ICD) represents a promising strategy to remodel this environment. Herein, we developed an ICD-related risk signature (ICDRS) to stratify patients and predict immunotherapy response. Through integrated database screening, sunitinib was repurposed as an ICD inducer. It elicited hallmark features of ICD, including calreticulin exposure, ATP release, and HMGB1 secretion, and effectively suppressed organoid growth. Sunitinib-pretreated HCC cells promoted dendri

Sunitinib-Induced Adrenal Crisis in a Patient with Pre-Existing Immunotherapy-Related Hypopituitarism.

Sunitinib is a tyrosine kinase inhibitor that is frequently used in the treatment of metastatic renal cell carcinoma (mRCC). As a multikinase inhibitor, numerous off-target side effects of this medication are widely recognized. More recently, endocrine side effects, including hypoadrenalism, are becoming more apparent. We report a case in which a 71-year-old female experienced recurrent adrenal crises when managed with sunitinib for mRCC on a background of immune-related hypopituitarism and hypoadrenalism as a result of previous treatment with immunotherapy. Clinicians should be aware of this

Autophagic degradation of CCN2 (cellular communication network factor 2) causes cardiotoxicity of sunitinib.

Excessive macroautophagy/autophagy is one of the causes of cardiomyocyte death induced by cardiovascular diseases or cancer therapy, yet the underlying mechanism remains unknown. We and other groups previously reported that autophagy might contribute to cardiomyocyte death caused by sunitinib, a tumor angiogenesis inhibitor that is widely used in clinic, which may help to understand the mechanism of autophagy-induced cardiomyocyte death. Here, we found that sunitinib-induced autophagy leads to apoptosis of cardiomyocyte and cardiac dysfunction as the cardiomyocyte-specific Atg7-/+ heterozygous

Effective treatment of advanced alveolar soft part sarcoma with sunitinib: A case report.

RATIONALE: Alveolar soft part sarcoma (ASPS) is a very rare soft tissue sarcoma. ASPS often occurs in deep soft tissues of the lower extremities, and develops into metastatic diseases. Chemotherapy is considered to be ineffective in patients with ASPS, which constitutes a very important clinical challenge. The strategy for ASPS treatment still requires clarification in order to improve survival outcome. Patients concerns: A 19-year-old female patient presented with a 5-year history of painless lump in the left knee and 4-day cough. Her previous medical history was unremarkable. Menstruation an

Therapy of Treatment-Related Hypertension in Metastatic Renal-Cell Cancer Patients Receiving Sunitinib.

INTRODUCTION: Treatment-related hypertension (tHTN) is frequent during sunitinib treatment. However, data on risk factors and treatment of tHTN remain scarce. PATIENTS AND METHODS: Patients with metastatic renal-cell carcinoma treated with sunitinib from June 2004 to December 2011 were included. Medical records were retrospectively analyzed for tHTN risk factors and antihypertensive treatments (AHT). Descriptive statistics, Cox regression, and competitive risk models were applied. RESULTS: A total of 51 (70.8%) of 72 patients developed tHTN after a median sunitinib treatment of 28 days. Mean

Impaired endothelium-dependent vasodilation does not initiate the development of sunitinib-associated hypertension.

BACKGROUND: Tyrosine kinase inhibitors targeting angiogenesis have become an important part of the treatment of patients with several types of cancer. One of the most reported side effects of vascular endothelial growth factor receptor (VEGFR)-targeted therapies is hypertension. In this study, we hypothesized that the development of hypertension in patients treated with sunitinib, a multitargeted tyrosine kinase inhibitor, is preceded by reduced endothelium-dependent vasodilation. Moreover, we hypothesized that this endothelial dysfunction is a result of impaired nitric oxide release. METHOD:

Phase II open-label study of sunitinib in patients with advanced breast cancer.

This multicenter, open-label phase II study was conducted to evaluate sunitinib monotherapy in patients with either metastatic or locoregionally recurrent advanced breast cancer. Patients received sunitinib 37.5 mg on a continuous daily dosing schedule. The primary endpoint was objective response rate (ORR); the predefined target ORR was 25 %. All 83 patients enrolled into the study received study treatment. The majority of patients (90 %) had metastatic disease; 92 % had received prior systemic therapies, and 60 % had received two or more regimens for early and/or advanced disease. The ORR wa

Long-term response with everolimus for metastatic renal cell carcinoma refractory to sunitinib.

A 70-year-old man with metastatic renal cell carcinoma developed progressive liver metastases after 8 weeks of treatment with the multitargeted tyrosine kinase inhibitor (TKI) sunitinib. He then participated in the phase III placebo-controlled clinical trial of the oral mammalian target of rapamycin (mTOR) inhibitor everolimus, initially randomized to placebo (but had disease progression after 3 months) and crossed over to everolimus at time of unblinding. The patient had stable disease after 8 weeks (two cycles) of everolimus that was maintained until 28 months of therapy, at which time the p

Synergistic induction of ferroptosis by paclitaxel and sunitinib is mediated through SLC7A11 in lung cancer.

The combination of paclitaxel (PTX) and sunitinib (SUN) exhibits synergistic antitumor activity against lung cancer, but the underlying mechanism is unclear. We show that PTX and SUN co-treatment synergistically inhibits tumor growth in murine allograft models and induces ferroptosis in lung cancer cells. Mechanistically, the combination concurrently downregulates ferroptosis suppressors (FTH1, GPX4, SLC7A11) and upregulates the pro-ferroptotic enzyme ACSL4, leading to iron accumulation, glutathione depletion, and lethal lipid peroxidation. Genetic studies identify SLC7A11 as a critical mediat

Genetic polymorphisms of VEGFR2 and FGFR2 genes are associated with exposure to sunitinib and cardiovascular toxicity.

PURPOSE: Sunitinib is a multikinase inhibitor used to treat metastatic renal cell carcinoma (mRCC), with an inter-individual variability of pharmacokinetics and toxicity. Our goal was to assess associations between the pharmacogenetics, pharmacokinetics and toxicity of sunitinib. METHODS: In this multi-center prospective study, 42 patients with mRCC were included. An NGS panel was used to identify variations in 19 genes involved in sunitinib pharmacokinetics and pharmacodynamics. Residual concentration of sunitinib and N-desethyl-sunitinib were used to estimate a composite AUC at steady-state
