Systemic inflammatory response syndrome, organ failure, and outcome in critically ill obstetric patients treated in an ICU.

STUDY OBJECTIVES: To determine the incidence of systemic inflammatory response syndrome (SIRS) and organ failure and to describe the outcomes in critically ill obstetric patients who have been treated in medical ICUs. DESIGN: Retrospective review. SETTING: A multidisciplinary ICU at a tertiary-care institution. METHODS: We collected data on 74 obstetric patients who were admitted consecutively to the ICU from January 1991 through December 1998. Acute physiology and chronic health evaluation (APACHE) II scores were calculated. A p value < 0.05 was considered to be significant. MEASUREMENTS

Plasma bacterial and mitochondrial DNA distinguish bacterial sepsis from sterile systemic inflammatory response syndrome and quantify inflammatory tissue injury in nonhuman primates.

Systemic inflammatory response syndrome (SIRS) is a fundamental host response common to bacterial infection and sterile tissue injury. Systemic inflammatory response syndrome can cause organ dysfunction and death, but its mechanisms are incompletely understood. Moreover, SIRS can progress to organ failure or death despite being sterile or after control of the inciting infection. Biomarkers discriminating between sepsis, sterile SIRS, and postinfective SIRS would therefore help direct care. Circulating mitochondrial DNA (mtDNA) is a damage-associated molecular pattern reflecting cellular injury

Prognostic Value of the Systemic Inflammatory Response Index in Patients with Acute Coronary Syndrome and Obstructive Sleep Apnea.

Given the poor outcome of refractory and relapsing T-cell acute lymphoblastic leukemia (T-ALL), identifying prognostic markers is still challenging. Using single nucleotide polymorphism (SNP) array analysis, we provide a comprehensive analysis of genomic imbalances in a cohort of 317 newly diagnosed patients with T-ALL including 135 children and 182 adults with respect to clinical and biological features and outcomes. SNP array results identified at least 1 somatic genomic imbalance in virtually all patients with T-ALL (∼96%). Del(9)(p21) (∼70%) and UPD(9)p21)/CDKN2A/B (∼28%) were the most fre

Noninvasive Early Detection of Systemic Inflammatory Response Syndrome of COVID-19 Inpatients Using a Piezoelectric Respiratory Rates Sensor.

In 2020, 20% of patients with COVID-19 developed severe complications, including life-threatening pneumonia with systemic inflammatory response syndrome (SIRS). We developed a preliminary SIRS monitor that does not require blood sampling, is noninvasive, and can collect data 24 h per day. The proposed monitor comprises a piezoelectric respiratory sensor located beneath the patient's mattress and a fingertip pulse sensor that determines ultra-high accuracy respiratory rate (mode of a 40-min frequency distribution of respiratory rates (M40FD-RR)). We assessed the clinical performance of the M40F

Neutrophil-to-Lymphocyte Ratio Predicts Sepsis in Adult Patients Meeting Two or More Systemic Inflammatory Response Syndrome Criteria.

INTRODUCTION: Determining which patients who meet systemic inflammatory response syndrome (SIRS) criteria have bacterial sepsis is a difficult challenge for emergency physicians. We sought to determine whether the neutrophil-to-lymphocyte ratio (NLR) could be used to exclude bacterial sepsis in adult patients who meet ≥2 SIRS criteria and are being evaluated for sepsis. METHODS: Consenting adult patients meeting ≥2 SIRS criteria and undergoing evaluation for sepsis were enrolled. We recorded patient age, gender, vital signs, and laboratory results. We then later reviewed health records for cu

Ischemia-modified albumin and adenosine plasma concentrations are associated with severe systemic inflammatory response syndrome after cardiopulmonary bypass.

PURPOSE: Severe systemic inflammatory response syndrome (SIRS) occurring after cardiopulmonary bypass (CPB) is a common cause of mortality during cardiac surgery. These syndromes are characterized by vasoplegia and ischemia-reperfusion phenomenom. Adenosine is a strong endogenous vasodilating agent, which may be involved in blood pressure failure during CPB induced by severe SIRS. Ischemia-modified albumin (IMA) is considered as a sensitive marker of tissue ischemia. We examined whether the IMA or adenosine plasma concentrations (APCs) change during a severe SIRS-induced blood pressure failure

Comparison of the effects of different intravenous fat emulsions in patients with systemic inflammatory response syndrome and sepsis.

BACKGROUND: In this study, the authors aimed to compare the effects that a medium- and long-chain triglyceride (MCT/LCT) fat infusion and a fish oil-based (ω-3) fat infusion for parenteral nutrition (PN) had on systemic inflammation, cytokine response, and hepatic steatosis in mixed intensive care unit (ICU) patients. METHODS: This was a single-center, placebo-controlled, randomized clinical trial in a university hospital. Four patient groups, including systemic inflammatory response syndrome (SIRS) and sepsis patients, were assigned to receive PN employing the MCT/LCT fat infusion or the fis

Bilateral Keratouveitis and Secondary Glaucoma in a Young Dog With Immune-Mediated Polyarthritis and Systemic Inflammatory Response Syndrome.

A 7-month-old intact female mixed-breed dog presented emergently for inability to walk, lethargy, and vomiting. The patient was diagnosed with immune-mediated polyarthritis (IMPA) and secondary systemic inflammatory response syndrome (SIRS). Bilateral keratouveitis and secondary glaucoma developed within 24 h of presentation. Thorough diagnostic testing failed to identify an underlying etiology. Despite aggressive medical management, the patient developed rapid vision loss. Due to poor visual prognosis and anticipated need for long-term medical management, both eyes were enucleated. Electroretinographic measures and their correlation with retinal vascular calibers in bitches with pyometra-induced systemic inflammatory response syndrome.

Pyometra is characterized by a uterine inflammatory process that leads to systemic inflammatory response syndrome (SIRS). Bitches with pyometra-induced SIRS (P-SIRS) develop marked retinal microvascular alterations that may affect ERG findings. Therefore, this study aimed to assess potential changes in implicit times, wave amplitudes in pyometra-induced SIRS. Furthermore, we evaluated possible correlations between retinal vascular calibers and ERG parameters in the same patients. A prospective observational study was conducted on 14 bitches diagnosed with P-SIRS and 10 clinically healthy bitch

Interleukin-6-producing non-secreting cervical paraganglioma presenting with fever of unknown origin and systemic inflammatory response syndrome.

Pheochromocytomas and paragangliomas (PPGLs) are rare neuroendocrine tumours that usually present with symptoms related to catecholamine excess. However, a small subset may secrete cytokines such as interleukin-6 (IL-6), leading to atypical systemic manifestations and delayed recognition of a paraneoplastic inflammatory syndrome. We report the case of a middle-aged woman with a previously diagnosed non-secreting cervical paraganglioma who developed fever of unknown origin (FUO), anaemia and liver dysfunction 5 years after the initial diagnosis. Extensive investigations excluded infectious, aut

Predictors of postoperative systemic inflammatory response syndrome after scoliosis surgery in adolescents with cerebral palsy: A retrospective analysis.

BACKGROUND: Systemic inflammatory response syndrome (SIRS) is known to complicate postsurgical intensive care patients. We noticed that roughly half children with cerebral palsy who undergo posterior spinal fusion (PSF) for neuromuscular scoliosis developed SIRS in the intensive care unit. There is a paucity of literature detailing the impact of intraoperative causes of postoperative SIRS and downstream consequences in these patients. Study purpose was to understand the factors associated with SIRS in children who undergo PSF for neuromuscular scoliosis. METHODS: This retrospective, case cont
