Fetal permanent junctional reciprocating tachycardia with dilated cardiomyopathy, normal heart rate and transient fetal hydrops; a case report.

Permanent junctional reciprocating tachycardia (PJRT) is a rare form of congenital arrhythmia occurring predominantly in infants and children. Prenatal presentation is frequently characterized by incessant tachycardia leading to dilated cardiomyopathy (DCM). Some patients can have a normal heart rate which leads to a delayed diagnosis. We report a case of a neonate who was presented prenatally with DCM, fetal hydrops, and no signs of fetal arrhythmia. Diagnosis of PJRT was established after delivery with characteristic electrocardiographic patterns. Successful conversion to sinus rhythm with d

Long-term outcome of neonates and infants with permanent junctional reciprocating tachycardia. When cardiac ablation changes natural history.

UNLABELLED: Supraventricular tachycardias (SVT) are the most common arrhythmias in the perinatal period. Permanent junctional reciprocating tachycardia (PJRT) is a rare form of SVT, often incessant and refractory to pharmacological treatments. Our goal was to analyze the clinical features and treatment of PJRT in patients younger than 2 months and to describe their long-term outcomes. METHODS: Retrospective descriptive observational study of patients diagnosed between 2000 and 2015 in the NICU of a referral center for the treatment of pediatric arrhythmias. History of pregnancy, neonatal peri

Determining the Site of Accessory Pathways in Orthodromic Reciprocating Tachycardia by Using the Response to Right Ventricular Pacing.

BACKGROUND: Postpacing interval (PPI) after right ventricular (RV) pacing entrainment minus tachycardia cycle length (TCL) with a correction for atrioventricular (AV) node delay (corrected PPI-TCL) was useful to differentiate atrioventricular node reentrant tachycardia (AVNRT) from orthodromic reciprocating tachycardia (ORT). However, the value of corrected PPI-TCL in determining the site of the accessory pathway (AP) in ORT has not been investigated. The purpose of this study was to assess whether the corrected PPI-TCL is useful in differentiating ORT using a left-sided AP from a right-sided

Long-term follow-up in patients with the permanent form of junctional reciprocating tachycardia treated with radiofrequency ablation.

This study sought to determine the long-term follow-up, safety, and efficacy of radiofrequency catheter ablation of patients with the permanent form of junctional reciprocating tachycardia (PJRT). We assessed the reversibility of tachycardia induced LV dysfunction and we detailed the location and electrophysiological characteristics of these retrograde atrioventricular decremental pathways. PJRT is an infrequent form of reciprocating tachycardia, commonly incessant, and usually drug refractory. The ECG hallmarks include an RP interval > PR with inverted P waves in leads II, III, a VF, and V3-V

Three types of atrioventricular reciprocating tachycardia using bilateral accessory pathways in a patient with Wolff-Parkinson-White syndrome.

Electrophysiologic studies were performed on a patient with Wolff-Parkinson-White syndrome and recurrent supraventricular tachycardia. Bilateral accessory pathways capable of antegrade and retrograde conduction and three different types of atrioventricular (AV) reciprocating tachycardia were demonstrated. One type of narrow QRS tachycardia used the normal AV pathway for antegrade conduction and the left-sided accessory pathway for retrograde conduction. Two types of wide QRS tachycardia (one with right bundle branch block and one with left bundle branch block) used both accessory pathways for

[Atrial desynchronization induced by Valsalva's maneuver in a patient with reciprocating supraventricular tachycardia and Wolff-Parkinson-White syndrome].

A 22 years old man with ventricular preexcitation syndrome due to a left accessory pathway, was admitted because of orthodromic reciprocating tachycardia, 205 bpm in frequency. The patient was invited to perform a Valsalva Maneuvre, and at its ending, the tachycardia degenerated into atrial fibrillation (AF) associated with high ventricular rate, reverted to sinus rhythm by D.C. shock. The electrophysiologic study documented a left lateral by pass tract, with an anterograde refractory period of 230 msec. AF was inducible and had a minimal RR interval of 220 msec. Vagal stimulation, induced at

Recurrent preexcited reciprocating tachycardia, concealed ventricular preexcitation and conduction disturbances in the normal atrioventricular pathway in a newborn.

We describe a newborn infant with concealed ventricular preexcitation in sinus rhythm on the early ECGs, and who developed recurrent episodes of preexcited reciprocating tachycardia. Manifestation of the ventricular preexcitation and initiation of the tachycardias were dependent on the presence of severe conduction disturbances in the normal atrioventricular pathway, probably in the atrioventricular node.

Long-term control of reciprocating paroxysmal tachycardia by ventricular pacing in a case of Wolff-Parkinson-White syndrome.

A patient is described with a complex electrocardiographic pattern, including preexcitation, grade 1 atrioventricular block, reciprocal rhythm of atrial origin, and frequent attacks of reciprocating tachycardia at a rate of about 135 beats per minute. Long-term control of these attacks was obtained by synchronous ventricular pacing at 80 beats a minute, which was below the rate during the attacks of tachycardia and above the spontaneous heart rate between the attacks.

Mechanism of reciprocating tachycardia initiated during sinus rhythm in concealed Wolff-Parkinson-White syndrome: report of a case.

Reciprocating tachycardia in a patient with a leftsided atrioventricular accessory pathway (AP) (Kent bundle, type A) capable only of ventriculo-atrial (V-A) transmission is described. The V-A AP is established as an essential link of the tachycardia circuit, as evidenced by : 1) retrograde atrial activation of the left atrium (LA) 60 msec or more before the low and high right atrium during reciprocating tachycardia and during V-A conduction; 2) the absence of refractory-dependent delay in V-A conduction time with progressively premature ventricular stimulation, characteristic of retrograde co

[Provocation of the attack of supraventricular reciprocal paroxysmal tachycardia by frequent stimulation of the left atrium through the esophagus to determine its approximate range of reciprocal excitement].

The study involved 24 patients with supraventricular paroxysmal reciprocal tachycardia (SPRT) showing no ECG evidence of ventricular pre-excitation between the attacks. SPRT attacks could be provoked by frequent esophageal left-atrial stimulation in all patients, particularly with impulse frequencies in the 160 to 200 imp/min range, associated with first-degree atrioventricular block featuring Wenckebach's phenomenon. In most cases, induced SPRT was similar to a spontaneous attack. During an induced attack, the duration of Q--P interval was less than 70 ms in 15 (62.5%) suggesting that the sco

[Identification of slow anterograde conduction of excitation in patients with paroxysmal reciprocal atrioventricular nodal tachycardia].

Basing on results of examination of a cohort of patients with typical paroxysmal reciprocal atrioventricular tachycardia a group of patients with continuous curve of anterograde conduction was distinguished. The use of incremental doses of adenosine triphosphate (ATP) during transesophageal atrial pacing allowed in some cases to identify slow conduction through atrioventricular node and to initiate paroxysmal nodal tachycardia. The dose of ATP causing conduction block over fast pathway depended on the duration of effective refractory period.

['Uninterrupted' anterograde atrioventricular excitation conduction in patients with paroxysmal reciprocal atrioventricular nodal tachycardias].

Different variants of anterograde atrioventricular conduction of excitation were distinguished basing on data obtained from 150 patients with paroxysmal reciprocal atrioventricular nodal tachycardias. Excitation conduction during programmed and accelerating atrial transesophageal pacing allowed to assess characteristics of excitation conduction along fast and slow conduction pathways in atrioventricular node. Some parameters were suggested that allowed to predict possibility of detection of different variants of anterograde atrioventricular conduction of excitation. Electrophysiological mechan

Permanent junctional reciprocating tachycardia in a patient with an atypically located accessory pathway in the left lateral mitral annulus.

Permanent junctional reciprocating tachycardia (PJRT) is an uncommon form of atrioventricular reentrant tachycardia due to an accessory pathway characterized by slow and decremental retrograde conduction. The majority of accessory pathways in PJRT are located in the posteroseptal zone. Few cases of atypical location have been described. We report a case of PJRT in a 72-year-old woman in whom the accessory pathway was located in the left lateral region and treated by radiofrequency catheter ablation.

Spontaneous termination of reciprocating tachycardia owing to interaction of dual atrioventricular nodal pathways in patients with an accessory pathway.

Retrospective analysis of the electrophysiologic recordings from 125 consecutive patients with reciprocating tachycardia involving an accessory atrioventricular (AV) pathway suggested, by the mode of spontaneous termination of reciprocating tachycardia, the coexistence of dual AV nodal pathways in 7 patients. Three different modes of spontaneous tachycardia termination were observed. In 2 patients with antidromic tachycardia, termination was a result of AV nodal reentry, preceded by a decrease in retrograde AV nodal conduction. In 3 other patients with antidromic tachycardia, termination occur

Recurrent reciprocating tachycardias in the Wolff-Parkinson-White syndrome; control by the use of a scanning pacemaker.

Frequent episodes of arrhythmias in patients with the Wolff-Parkinson-White syndrome are sometimes unresponsive to medical management. Recent emphasis has been placed on the role of surgery as a means of effectively controlling these patients' tachyarrhythmias; however, in some patients the use of permanent pacing has been espoused as an effective means of controlling arrhythmias. This report documents the efficacy of a new pacemaker with rate-scanning features, which offers significant technical advantages. Subsequent follow-up has documented the efficacy of this unit in the management of a y
