Developing a competency framework for operating room nurse specialist training instructors: a Delphi study.

OBJECTIVE: To develop a competency evaluation framework for OR nurse specialist training instructors. METHODS: Guided by the Iceberg Model and the Onion Model of competency, a preliminary indicator framework was developed through a literature review, behavioral event interviews, and expert group discussions. A two-round Delphi survey involving 31 operating room nursing experts was conducted to refine the indicators. The Analytic Hierarchy Process (AHP) was used to determine the weights of the final indicators. Expert authority, participation rate, and the degree of consensus were assessed. R

Efficacy of heartworm preventatives against ascarids and hookworms in client-owned dogs: a retrospective case control study.

The efficacy of heartworm preventatives against ascarids and hookworms was assessed in a retrospective analysis of 1329 dogs that received a fecal examination and were surveyed about heartworm preventative use upon presentation to the veterinary teaching hospital at the University of Pennsylvania. To remove confounding due to age, patients under 6 months old were analyzed separately from the remaining population. Although there were no reported cases of ascarids or hookworms in patients under 6 months old receiving monthly heartworm prevention, the prevalence reached 5.2% and 11.7%, respective

Braille literacy as a human right: A challenge to the "inefficiency" argument against braille instruction.

The United Nations Convention on the Rights of Persons with Disabilities (CRPD) affirms a right to education for disabled persons and aims to ensure braille instruction for blind individuals. However, there is evidence that braille instruction is often circumvented or abandoned early in CRPD nations because it is perceived as an inefficient learning medium for blind students. This perception persists despite insufficient empirical evidence and a lack of understanding of the efficiency of reading versus listening for learning in sighted individuals. We therefore investigated the efficiency of l

Large Language Model for Postoperative Clinical Decision Support in a Neurosurgery Ward in the Gambia: A Prospective Pilot Feasibility Study.

BACKGROUND AND OBJECTIVES: Access to specialty surgical care is growing in many low-income countries, but it remains unclear how hospital workforces can leverage technology to manage large numbers of increasingly complex patients. Large language models (LLMs) may be helpful for this type of clinical decision support, but their real-world performance and safety remain uncertain. The objective of this study was to evaluate feasibility, usability, and potential benefits and risks of an LLM-based assistant for postoperative neurosurgical care in the Gambia. METHODS: A prospective, single-arm impl

Defining the key elements of the Affolter Model® in a multiprofessional Delphi study: a first step toward evidence-based Tactual Interaction Therapy.

BACKGROUND: Despite the versatile application of the Affolter Model®, using Tactual Interaction Therapy as a treatment approach in health, social and educational care of people with congenital brain disorder or acquired brain injury, high quality studies with robust designs for efficacy are scarce. Evaluation of the effectiveness of the Tactual Interaction Therapy requires agreement and consensus among practitioners of what constitutes this approach. Such consensus has yet to be achieved. GOAL: To map the Affolter Model® by reaching consensus on its core domains among experienced practitioner

Primary tumour of the round ligament of the liver: a case presentation.

A 40-year-old Caucasian female patient presented to the outpatient General Surgery ward in "V. I. Lenin" Teaching Hospital complaining of a recurrent mesogastric pain that had lasted for 3 months. Physical examination showed a palpable mass confined to that area. She was then admitted with diagnosis of an abdominal tumour. Diagnostic work-up revealed that the process involved the round ligament of the liver, which is an exceptional localization, which motivated us to publish this case after surgical treatment by excision, having also taken into account the results of histopathology which revea

Where do we invest money to implement active learning on caries detection? An economic evaluation.

This trial-based economic evaluation aimed to estimate the incremental cost of implementing an active learning strategy (theoretical-practical workshop) to substitute the didactic lecture as the sole method for students training in caries detection. We also provided a budget impact analysis and explored the composition of costs related to the activity. Data from the coordinating centre of a multicentre randomized and controlled study (IuSTC01) was analyzed as the first part of our main economic analysis plan. The perspective of the educational provider (the institution implementing the activit

Increased Nursing Participation in Multidisciplinary Rounds to Enhance Communication, Patient Safety, and Parent Satisfaction.

Effective communication among health care team members is a mainstay of patient safety, especially in a neonatal ICU (NICU), given small errors can have serious and life-threating consequences. Ineffective communication with families of hospitalized children can lead to decreased satisfaction and trust in the health care team. To enhance communication, the NICU nursing staff at the Johns Hopkins Children's Center spearheaded an initiative to create an enhanced nursing role in multidisciplinary patient rounds. Education of the nursing staff and other team members and the development of a roundi

Hemophagocytic lymphohistiocytosis: an unusual complication of leprosy.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a rare condition of chaotic uncontrolled immune system stimulation and not fully understood pathophysiology. Most reported cases of hemophagocytic syndrome in patients with mycobacterial infections have been associated with Mycobacterium tuberculosis. As far as we could ascertain, to date, no established HLH case complicating leprosy has been published in the medical literature. CASE REPORT: We describe here a new case of Hansen's disease in a 58-year-old Tunisian man with an unusual complicated clinical course documented as hemophagocyt

The management of isolated meniscal tears in skeletally immature children. An international expert consensus.

PURPOSE: The prevalence and appreciation of meniscal tears in children have increased in both number and complexity. There is currently a paucity of high-quality evidence that can guide surgeons in treating skeletally immature patients with meniscal injuries. The aim of this study was to develop comprehensive recommendations for the management of isolated meniscal tears in skeletally immature children. METHODS: An international, two-round, modified Delphi consensus was completed. Included 'experts' were identified as having an established adult knee practice, including children and either: (1

Reaching an initial consensus on the non-pharmacological management of post-stroke emotionalism using the Delphi Method and mini-focus groups.

We aimed to identify global healthcare staff and researcher opinions regarding the perceived helpfulness and accessibility of non-pharmacological Post-Stroke Emotionalism (PSE) interventions using the Delphi Method and mini focus groups. Thirty-eight healthcare professionals completed the first round of the Delphi Method and 19 completed the second round. Eight participants then completed one of three mini-focus groups. Data were summarized in line with Delphi Methods including content analysis of the mini-focus group data. "Ask the patient to take a deep breath," "Provide education for patien

Developing a competency model for clinical nurse educators to support nursing career ladder advancement: A grounded theory study.

BACKGROUND: Clinical nurse educators greatly impact the development of high-quality clinical learning experiences. However, globally, the role remains inconsistently defined, and competency frameworks are rarely integrated with structured career ladder systems, limiting professional development and workforce sustainability. OBJECTIVE: This study aimed to develop an empirically grounded competency model for clinical nurse educators integrated with a structured career ladder to support progressive professional development in clinical education settings. METHODS: A qualitative study using a Str

Generating a consensus on an undergraduate curriculum for paediatric caries management: a protocol for a Delphi and nominal groups study.

BACKGROUND: Untreated dental caries is the most common condition to affect children globally, and significantly affects their oral health-related quality of life. Our understanding of caries and its management has progressed significantly over the past two decades, though a number of Dental Schools in the United Kingdom (UK) continue to teach traditional techniques, which no longer align with the evidence base. There is a clear need for an evidence-based curriculum for paediatric caries management for UK Dental Schools. This protocol details a study to generate consensus on what should be incl

Dysgerminoma in an Arabian filly.

A yearling Arabian filly was referred to the Veterinary Medical Teaching Hospital with a history of weight loss, profound anemia, and peritoneal effusion. At necropsy, a large, soft, mottled tan and red neoplastic mass was at the pelvic inlet replacing the left ovary. Additional tumor nodules of various sizes were disseminated throughout the mesentery, diaphragm, and serosal surfaces of the abdominal viscera. Histologically, the neoplasm had sheets of large round to polygonal cells separated into lobules by fibrous connective tissue with multifocal areas of necrosis. Tumor cells stained strong
