The Unfinished Business Scale for Families: A Measure for Evaluating the Unfinished Business of Bereaved Family Members of Terminally Ill Patients with Cancer in Japan.

OBJECTIVES: Although unfinished business is associated with psychological states in bereaved families of patients with cancer, no evaluation tools have been developed for such families in Japan. This study aimed to develop and examine the validity and reliability of an evaluation tool for unfinished business among families of terminally ill patients with cancer in Japan. METHODS: In August 2020, a cross-sectional online survey consisting of the Unfinished Business Scale for Families, Unfinished Business in Bereavement Scale (UBBS), Brief Grief Questionnaire (BGQ), and Patient Health Questionn

Stopping aggressive treatment or hoping for a miracle: an analysis of spiritual care messages for terminally ill patients using the National Survey of religious leaders.

Religious and spiritual needs are important for seriously ill patients. Conflicting evidence for who provides this care and how it fits into the biomedical context remains. Quantitative analysis of the National Study of Religious Leaders (NSRL) survey data (N = 1600) resulted in three statistically significant logistic regression models. Conflicts with scientific beliefs, openness to changing beliefs based on scientific findings, and race/ethnicity were associated with religious leaders' recommendations related to stopping curative treatment. Multiple factors such as belief in miraculous heali

Association between baseline inflammation and the effectiveness of nutritional support among terminally ill patients with cancer: A secondary analysis of a multicenter prospective cohort study.

OBJECTIVES: Inflammatory status may influence the response to nutritional support. We aimed to determine associations between calorie intake and survival considering serum C-reactive protein (CRP) levels among advanced cancer patients. METHODS: Data on characteristics and CRP levels were obtained at baseline. The nutritional administration method and calorie intake during the first week were recorded. Patients were divided into three CRP groups: Low (<1 mg/dL), moderate (1-10 mg/dL), or high (≥10 mg/dL). Patients in each group were divided into two categories: low-calorie (<250 kcal/d) or hig

Job terminations among persons with severe mental illness participating in supported employment.

For persons with psychiatric disabilities, maintaining a job is often more difficult than acquiring a job. A large proportion of jobs end unsatisfactorily. This study explored job terminations among 63 persons with severe mental illness who participated in competitive jobs through supported employment programs. More than half of the job terminations were unsatisfactory, defined as the client quitting without having other job plans or being fired. Baseline ratings of demographic and clinical characteristics, preemployment skills training, and early ratings of job satisfaction and work environme

First generation migrants' experiences of terminal illness: a systematic review of diasporic dying.

BACKGROUND: Migration is an established global phenomenon. While many newly arrived migrants have better health than the general population of the country they have moved to, migrants also have their own healthcare needs and face particular issues when diagnosed with a terminal illness. First generation migrants are less likely to have social, financial, and medical supports when faced with a terminal illness. These factors make first generation migrants an important group to understand in order to inform service commissioning and delivery. METHODS: The systematic review was an international

The connections of physical and psychosocial symptoms among patients with terminal illnesses: A network analysis.

BACKGROUND: Patients with terminal illnesses have different symptoms. The goal of palliative care is to alleviate the suffering induced by these symptoms. Available research on symptoms has focused on their prevalence or identification of clusters. Little is known about the central symptoms contributing to the maintenance of the whole system of symptoms or the mechanism of influence between symptoms, particularly between physical and psychosocial symptoms. AIM: The study used network analysis to explore symptom clusters among patients with terminal illnesses and identify the central symptoms,

Impact of ascites on the prognostic performance of the palliative prognostic index in terminally ill cancer patients: a multicenter secondary analysis of the EASED study.

BACKGROUND: Ascites is common in patients with advanced cancer and is associated with systemic inflammation, malnutrition, and organ dysfunction. Although the Palliative Prognostic Index (PPI) is widely used to predict short-term survival in terminally ill cancer patients, ascites is not included in its components. We investigated whether ascites influences survival and modifies the prognostic performance of the PPI in terminally ill cancer patients. METHODS: This was a secondary analysis of the multicenter prospective the East-Asian cross-cultural collaborative Study to Elucidate the Dying p

Perceived non-ordinary end-of-life experiences witnessed and reported by a palliative care team accompanying terminally ill patients in a hospice: A qualitative study.

This study examines perceived non-ordinary end-of-life experiences (NOEoLEs) witnessed or reported by the hospice palliative care team and explores their influence on the accompaniment of dying patients, the team's work, and their attitudes toward death and afterlife. A qualitative research design was adopted, including 37 members of the interdisciplinary team from an Italian hospice. According to team members, patients and their relatives frequently report perceived non-ordinary end-of-life experiences (NOEoLEs). While patients typically respond positively to such experiences, relatives may e

Treatment of agitation in terminally ill patients with intranasal midazolam versus subcutaneous midazolam: study protocol for a randomised controlled open-label monocentric trial (MinTU Study).

BACKGROUND: Intranasal (i.n.) drug application is a widely known and low-invasive route of administration that may be able to achieve rapid symptom control in terminally ill patients. According to the German S3 guideline "Palliative care for patients with incurable cancer", benzodiazepines, such as midazolam, are recommended for the treatment of terminal agitation. To the best of our knowledge there is no evidence for i.n. midazolam in terminally ill patients. We aim to assess the use of i.n. midazolam as an alternative to subcutaneous administration of the drug. METHODS: In this monocentric,

Current practice of pharmacological treatment for hyperactive delirium in terminally ill cancer patients: results of a nationwide survey of Japanese palliative care physicians and liaison psychiatrists.

OBJECTIVE: The objective of this survey was to identify areas where doctors have divergent practices in pharmacological treatment for hyperactive delirium in terminally ill patients with cancer. METHODS: We conducted a survey of Japanese palliative care physicians and liaison psychiatrists. Inquiries were made regarding: (i) choice of drug class in the first-line treatment, (ii) administration methods of the first-line antipsychotic treatment, (iii) starting dose of antipsychotics in the first line treatment and maximum dose of antipsychotics in refractory delirium, and (iv) choice of treatme

Pharmacological Interventions for Anxiety and Depression in Terminal Illness: A Scoping Review of Randomized Controlled Trials.

Anxiety and depression are highly prevalent among patients with chronic and progressive illnesses. At the terminal stage, these symptoms are closely linked with existential distress and can worsen quality of life. A scoping review of randomized controlled trials (RCTs) reporting on pharmacological interventions for anxiety and depression in terminal illnesses was conducted. MEDLINE, Embase, and CENTRAL databases were searched from inception to September 2024. Two reviewers independently extracted data on study design, population, interventions, and outcomes. Thirteen RCTs involving 621 partici

Euthanasia and end-of-life decisions in terminal illness situations: Exploring attitudes of patients and relatives.

Although euthanasia is an increasingly debated issue, there is little information on the beliefs held by patients with advanced diseases and their relatives on it. As part of the ILive project, our research team aimed at filling that gap by conducting a study on the degree of agreement among Argentine patients with terminal illnesses and their relatives on end-of-life medical decisions using the Attitudes toward Euthanasia (ATE) scale. Data analysis showed that 77 of 153 patients (50.3%) and 73 of 139 patients' relatives (52.5%) scored higher than 30 on the scale. Agreement with performing eut

The effects of home nursing care for patients during terminal illness on the bereaved's psychological distress.

BACKGROUND: Although some studies have examined the effects of terminal illness care models such as hospice care on survivor outcomes, no studies could be found that examined whether nursing care affected such outcomes. OBJECTIVE: To determine whether specialized oncology home care services provided to terminally ill patients with lung cancer positively influenced bereavement psychological distress among survivors, compared with other models of care. METHODS: A secondary analysis was performed to test the effects of home nursing care for terminally ill patients on spousal psychological distr

An augmented illness-death model for semi-competing risks with clinically immediate terminal events.

Preeclampsia is a pregnancy-associated condition posing risks of both fetal and maternal mortality and morbidity that can only resolve following delivery and removal of the placenta. Because in its typical form preeclampsia can arise before delivery, but not after, these two events exemplify the time-to-event setting of "semi-competing risks" in which a non-terminal event of interest is subject to the occurrence of a terminal event of interest. The semi-competing risks framework presents a valuable opportunity to simultaneously address two clinically meaningful risk modeling tasks: (i) charact

Antibiotics in the end-of-life phase in pediatric oncological patients with a diagnosis of terminal illness: a dilemma.

BACKGROUND: Pediatric cancer patients in the final phase of life receive antibiotics empirically. The decision to start, maintain, or stop the antibiotic administration as part of care at this stage is a dilemma. METHODS: We conducted a retrospective, descriptive, cross-sectional study including cancer patients in the final phase of life, hospitalized during the last 5 to 7 days of life. We included demographic variables, diagnoses, days of hospitalization, cultures, antibiotics used, prevalent symptoms in the last week of life, and principal diagnosis at the time of death, and performed desc

John Paul Jones: An Overlooked Autopsy Finding that May Explain His Terminal Illness.

A finding in the autopsy of John Paul Jones, the American Revolutionary War naval hero, may explain his terminal illness. During his last 2 years, he had a persistent productive cough and dyspnea. Ten days before death, he developed rapidly progressive dependent edema and ascites. He died in France in 1792. His body, preserved in alcohol in a lead coffin, was, in 1905, removed to the United States. Glomerulonephritis was noted on an autopsy, performed in France, but there was no comment then or since about ventricular wall thickness being the same in both ventricles at 5-6 mm. Hypertrophy and
