Comparing the impact of standard and abbreviated treatment in a therapeutic community. Findings from the district of Columbia treatment initiative experiment.

This study examines the efficacy of providing Enhanced Abbreviated or Standard Inpatient treatment and Outpatient treatment to drug-abusing clients. The experiment randomly assigned 412 clients to two therapeutic community programs, which differed primarily in planned duration. This study addressed limitations of prior research, as it used random assignment of clients to treatment programs, achieved high follow-up rates and used objective measures of drug use and criminal history. Self-reports and objective measures of criminal activity and substance abuse were collected at pre- and posttreatm

Beyond the green myth: a scoping review of spatial justice and structural barriers to therapeutic urban agriculture for vulnerable communities.

INTRODUCTION: Motivated by the urgent need to address urban health inequities, this scoping review critiques the mainstream public health discourse that champions therapeutic urban agriculture (UA) as an accessible mental health intervention. This narrative often obscures the systemic spatial inequities and structural barriers marginalized populations face. To address this gap, we systematically mapped the physical, economic, and institutional mechanisms impeding therapeutic UA for vulnerable communities. METHODS: Adhering to JBI and PRISMA-ScR methodologies, we executed a systematic retrieva

In-Treatment Changes in Quality of Life-Related Variables in Therapeutic Communities for Cocaine Abusers: Are These Changes Associated with Clinical Outcomes?

BACKGROUND: Few studies have explored changes in quality of life during the first three months of admission to a therapeutic community for addictions. The objective of this study was to determine the relationship between these changes and treatment outcomes at discharge. METHODS: We undertook a prospective longitudinal study of 142 cocaine-dependent patients treated at a therapeutics community. All of these patients reached the 3-month evaluation and were subsequently followed until discharge. All participants completed the following measures: Health-Related Quality of Life for Drug Abusers T

Current use of therapeutic groups by Australian occupational therapists servicing community-dwellers with acquired brain injury: a qualitative interpretive descriptive study.

PURPOSE: Therapeutic groups are recognised as an effective intervention modality to support community-dwellers with acquired brain injury (ABI), but little is known about their current use by occupational therapists. This study aimed to understand the use of groupwork by Australian occupational therapists servicing community-dwellers with ABI. Barriers and facilitators for using groups were also explored. METHODS: A qualitative interpretative descriptive approach was used. Participants were private practice occupational therapists who provided services to community-dwellers with ABI. Data wer

Intramuscular therapeutic doses of enrofloxacin affect microbial community structure but not the relative abundance of fluoroquinolones resistance genes in swine manure.

Livestock manure is a major source of veterinary antibiotics and antibiotic resistance genes (ARGs). Elucidation of the residual characteristics of ARGs in livestock manure following the administration of veterinary antibiotics is critical to assess their ecotoxicological effects and environmental contamination risks. Here, we investigated the effects of enrofloxacin (ENR), a fluoroquinolone antibiotic commonly used as a therapeutic drug in animal husbandry, on the characteristics of ARGs, mobile genetic elements, and microbial community structure in swine manure following its intramuscular ad

The Effect of Positive Therapeutic Communication on Pain (POPAIN) and Anxiety During Arterial Blood Gas Standardized Procedures in the Emergency Department Compared to Traditional Communication: Protocol for a Monocentric Randomized Controlled Trial.

BACKGROUND: In the emergency department (ED), medical procedures, such as arterial blood gas (ABG) testing, can cause pain and high stress levels. However, ABG testing is a routine procedure assessing the severity of the patient's condition. To reduce the pain of ABG, several methods have been investigated without significant difference in pain perception. Communication, a key element of care, has shown a significant effect on pain perception. A positive communication strategy, including positive, kind, or reassuring words, can reduce pain perception, while negative words can raise this percep

Therapeutic Target and Cell-signal Communication of Chlorpromazine and Promethazine in Attenuating Blood-Brain Barrier Disruption after Ischemic Stroke.

Ischemic stroke destroys blood-brain barrier (BBB) integrity. There are currently no effective treatments available in the clinical setting. Post-ischemia treatment with phenothiazine drugs [combined chlorpromazine and promethazine (C+P)] has been shown to be neuroprotective in stroke. The present study determined the effect of C+P in BBB integrity. Sprague-Dawley rats were divided into the following groups ( n=8 each): (1) stroke, (2) stroke treated by C+P with temperature control, and (3) stroke treated by C+P without temperature control. Infarct volume and neurological deficits were measure

The Montreal Cognitive Assessment (MoCA) is Sensitive to Head Injury and Cognitive Impairment in a Residential Alcohol and Other Drug Therapeutic Community.

INTRODUCTION: Retaining clients in residential alcohol and other drug (AOD) treatment is difficult and cognitive impairment has been identified as a significant predictor of treatment dropout. The application of extensive screening for cognitive impairment is cost-prohibitive for most AOD treatment services. The current study aimed to explore cognitive functioning and impairment-associated factors in a typical sample of residential AOD clients using a free brief screening tool that could be utilised by front-line AOD services. METHODS: Residents of an AOD therapeutic community (n=128) and a n

A modified therapeutic community for homeless persons with co-occurring disorders of substance abuse and mental illness in a shelter: an outcome study.

This article reports on a study conducted to determine the effectiveness of a modified therapeutic community (MTC) shelter on client outcomes. The seven-study hypotheses focused on whether greater effectiveness in the MTC would be demonstrated in longer periods of sobriety, fewer days of psychiatric hospitalization, shorter lengths of stay in a shelter, positive discharge from the shelter, medication compliance, housing placement within the first year, and appropriate housing placement according to level of functioning. The study utilized a quasi-experimental design with two groups: 1) an expe

[Therapeutic results obtained from women with functional sterility. First communication: combined oestrogen-gestagen therapy (author's transl)].

Combined oestrogen-gestagen preparations were applied to 169 women who had been sterile for an average period of 2.5 years, between 1975 and 1979. Their uncorrected pregnancy rate was 19 per cent. Pregnancy rates were 25 per cent for women who desired to have children up to two years or six per cent for those who tried longer. Oestrogen-gestagen therapy was successful in 20 per cent of women up to 30, but only in nine per cent of those beyond that age limit. Best therapeutic results were 45 per cent in cases of secondary amenorrhoea and 35 per cent in cases of genuine corpus luteum insufficien

Estimation of therapeutic threshold for tuberculosis using adapted nominal group technique and clinical vignettes in clinical and community settings in Southern Africa.

BACKGROUND: When confronted with diagnostic uncertainty and a decision on whether to start treatment or not, clinicians consider the potential harm and benefit of offering versus withholding treatment. Treatment can be offered if the probability of tuberculosis (TB) in the patient is above the "therapeutic threshold" (ThT): the probability of disease at which the expected utility of treating and not treating is the same. We estimated ThT for TB in clinical and community settings in Southern Africa using two methods: an adapted nominal group technique (aNGT), and decisions made based on clinica

Community-directed treatment with ivermectin in Maridi, South Sudan: Impact of an onchocerciasis awareness campaign and bi-annual treatment on therapeutic coverage.

BACKGROUND: A high onchocerciasis disease burden and a low coverage of community-directed treatment with ivermectin (CDTI) have been observed in many parts of South Sudan. In the Maridi County, CDTI was re-introduced in 2017 and various interventions implemented to improve coverage. METHODS: Through successive community-based surveys, we investigated whether an onchocerciasis awareness campaign and a switch from annual to bi-annual distribution of ivermectin in Maridi County increased CDTI coverage. We also reviewed the evolution of ivermectin distribution in Maridi since 2017 and identified

The Dual Roles of Microglia- and Astrocyte-Derived Exosomes in Cerebral Ischemia-Reperfusion Injury: from Intercellular Communication to Therapeutic Prospects.

Cerebral ischemia-reperfusion injury (CIRI) is a complex pathological process characterized by metabolic dysfunction, oxidative stress, neuroinflammation, and structural and functional alterations of the neurovascular unit (NVU). Across different studies, CIRI has been reported to be associated, to varying degrees, with neuronal injury and neurological dysfunction. Increasing evidence suggests that exosomes (EXOs) derived from glial cells, particularly microglia and astrocytes, play critical roles in mediating intercellular communication and regulating injury progression in CIRI. This review s
