[Postoperative Atelectasis and Pneumonia after General Thoracic Surgery].

Atelectasis and pneumonia are common postoperative complications occurred in patients undergoing general thoracic surgery. Strategies to reduce the risk of postoperative pulmonary complications including these 2 disorders consist of smoking cessation, optimization of underlying chronic obstructive pulmonary disease( COPD), adequate pain control, intensive oral care, and so on. Preoperative treatments for patients with COPD are the same as those for COPD patients being not about to have surgery, which mainly consist of long-acting bronchodilators and pulmonary rehabilitation. Open aortic surgery after thoracic endovascular aortic repair.

In the last decade, thoracic endovascular aortic aneurysm repair (TEVAR) has emerged as an appealing alternative to the traditional open aortic aneurysm repair. This is largely due to generally improved early outcomes associated with TEVAR, including lower perioperative mortality and morbidity. However, it is relatively common for patients who undergo TEVAR to need a secondary intervention. In select circumstances, these secondary interventions are performed as an open procedure. Although it is difficult to assess the rate of open repairs after TEVAR, the rates in large series of TEVAR cases (

Cerebral Fat Embolism After Video-Assisted Thoracic Surgery.

Cerebral fat embolism (CFE) is an uncommon disease occurring mainly after traumatic lower limb long bone fractures. A 64-year-old woman with pneumonia and bilateral pleural effusion underwent video-assisted thoracic surgery (VATS). After 3 days, the patient had an acute decrease in consciousness level followed by the onset of seizures and right hemiparesis. Brain computed tomography and magnetic resonance imaging showed findings suggestive of CFE. CFE occurring after an uncommon nontraumatic cause of fat embolism, such as VATS, is a rare clinical event whose diagnosis could be challenging. Neu

Video-assisted thoracic surgery thymectomy: the better approach.

Minimally invasive video-assisted thoracic surgery (VATS) thymectomy has evolved significantly over the last decade. The most common indication for VATS thymectomy is the treatment of myasthenia gravis (MG). Video-assisted thoracic surgery thymectomy results in less postoperative pain, better preserved pulmonary function, and improved cosmesis, which can be particularly important to many young female MG patients. Results of VATS thymectomy, in terms of complete stable remission from MG and symptomatic improvement, as well as safety, are comparable with conventional surgical techniques. This mo

[Video-assisted thoracic surgery for metachronous lung cancer].

Of 1,391 patients who underwent operation for primary lung cancer between 2000 and 2009, 50 patients (3.6%) had a past history of pulmonary resection for lung cancer. Three patients underwent completion pneumonectomy by thoracotomy and in the other 47 patients video-assisted thoracic surgery (VATS) was performed. We considered 42 cases (3 of completion pneumonectomy and 39 of VATS) to be metachronous lung cancer and 8 cases of VATS to be recurrence by detailed histologic assessment. We examined 39 cases of metachronous lung cancer resected by VATS. The patients were aged 68 +/- 8 years and 4 p

Clinical variables of preoperative risk in thoracic surgery.

CONTEXT: Pulmonary complications are the most common forms of postoperative morbidity in thoracic surgery, especially atelectasis and pneumonia. The first step in avoiding these complications during the postoperative period is to detect the patients that may develop them. OBJECTIVE: To identify risk variables leading to early postoperative pulmonary complications in thoracic surgery. DESIGN: Prospective study. SETTING: Hospital das Clínicas, Faculdade de Ciências Médicas, Universidade Estadual de Campinas. PATIENTS: 145 patients submitted to elective surgery were classified as low, moderat

Video-assisted thoracic surgery--state of the art.

Video-assisted thoracic surgery (VATS) is one of the main medical revolutions of the past decade. For its satisfactory performance, the following prerequisites are essential: (1) knowledge and experience in thoracic surgery; (2) team of experienced anesthesiologists; (3) preoperative assessment of respiratory function; (4) adequate postoperative care; and (5) instruments specially designed for thoracoscopic surgery. VATS is routinely performed under general anesthesia with double lumen endotracheal intubation for separate control of each lung. Insufflation of carbon dioxide must not exceed 1-3

Thoracic computed tomography prior to redo coronary surgery.

OBJECTIVE: Median resternotomy in coronary redo surgery represents a specific risk for injury of adjacent retrosternal structures. Aiming at improved preoperative evaluation of retrosternal structures, computed tomography (CT) techniques were routinely applied in redo cases. METHODS: Of 99 patients undergoing coronary reoperations since April 93, thoracic CT scans were retrospectively analyzed for retrosternal vicinity of cardiovascular structures, condition of the ascending aorta and structural abnormalities of the sternum. RESULTS: The minimal median distance between the posterior sternum

Review: complications of surgery for thoracic disc disease.

BACKGROUND: Thoracic discectomy has evolved over the last 60 years from resection through standard laminectomy, to posterolateral procedures to open thoracotomy and finally thoracoscopy. Comparison of relative morbidity and mortality between these approaches is now possible. METHODS: Peer-reviewed publications reporting clinical data relating to thoracic discectomy, and which provided sufficient information to enable adequate assessment of mortality and morbidity were reviewed. These articles were determined via review of the results of MedLine searches and articles gathered through compilati

[Brain damage after surgery for thoracic aortic aneurysm].

We analyzed cases with brain damage after surgery for thoracic aortic aneurysm in our institution and investigated the causes, risk-factors and preventive measures for this disastrous postoperative complication. Irreversible brain damage was a complication in 25 out of 184 operative cases (13.6%) over a 21-year period. The cause of brain damage was determined to be embolism by manipulation of the aorta in six cases, clamping of the left subclavian artery in four cases, technical problems of separate cerebral perfusion (SCP) in four cases, severe shock in three cases, embolism unrelated to oper

[Blood preservation in thoracic surgery: feasibility and limitations].

The transfusion of homologous blood carries risks associated with reactions to transfusion and the transmission of diseases. Minimizing surgery-related blood use is therefore of utmost importance. However, the application of blood-saving methods in surgery is restricted by a number of factors. Thoracic surgery poses particular problems in this regard due to the type of diseases treated, patients' general status and the use of neoadjuvant chemotherapy protocols. The aim of this study was to analyse the feasibility of a blood conservation program for thoracic surgery. A detailed analysis of bloo

Viral pulmonary infections in thoracic and cardiovascular surgery.

Viral respiratory infections are uncommon causes of pulmonary infiltrates in immunocompetent patients who undergo cardiothoracic surgery. In winter months, however, influenza can be acquired in the community preoperatively or in the hospital setting. The recognition of influenza cases is essential to prevent nosocomial transmission. Respiratory syncytial virus is also an important pulmonary pathogen in pediatric patients who undergo cardiothoracic surgery and may produce serious disease in children with underlying pulmonary or congenital heart disease. Viral infections of the respiratory tract

Video-assisted thoracic surgery: experience with 126 cases.

VATS was performed in 126 patients at the Medical Center of Delaware from December 1991 to August 1993, with no major complications and no mortality. A definitive diagnosis was made in all cases. Results with VATS therapeutic procedures appear to equal those of the standard open technique. Operating time was comparable to that with the open technique. Length of stay and pain and suffering were dramatically reduced when compared with those associated with the open technique. We now consider VATS to be the preferred procedure in cases of: 1. Undiagnosed pulmonary infiltrate in the nonventilator-

Delayed thoracic closure after cardiac surgery in infants.

We report 4 cases of delayed thoracic closure after cardiac surgery in infants: 3 cases with transposition of the great arteries and one with hypoplastic left heart syndrome. Sternal and skin closure in the primary operation sometimes causes haemodynamic cardiac compression. Postoperative correction of haemodynamic and haemostatic functions are allowed by delayed sternal and skin closure. No complications were encountered.

[Epidural analgesia after thoracic surgery: morphine versus buprenorphine].

A double-blind study was carried out to assess the efficiency and possible side-effects of a single epidural injection of either morphine or buprenorphine at equipotent doses after elective thoracic surgery. The series included 24 patients aged 53.7 +/- 11.4 years; 13 underwent a lobectomy and 11 a pneumonectomy. 6 h after the last intravenous injection of fentanyl, the patients were randomly allocated to one of three equal groups. They received an epidural injection at T8-9 or T9-10 level of either 100 micrograms.kg-1 morphine (group M) or 6.6 micrograms.kg-1 buprenorphine (group B) or a subc
