[Preliminary results in the correction of the pectus excavatum with the Acastello modified Welch technique].

BACKGROUND: Congenital malformations of the chest wall are a heterogeneous group of diseases affecting the costal cartilage, ribs, sternum, scapula and clavicle. The pectus excavatum is characterized by a posterior depression of the sternum. Acastello-Welch technique consists in a partial resection of the costal cartilages adding some bars or plates unilaterally fixed to the sternum in each hemithorax. MATERIALS AND METHODS: From October 2008 to March 2011 we evaluated 108 patients with congenital malformations of the chest wall. Forty-seven patients (44%) had a pectus excavatum and 12 were t

Surgical treatment for chronic lung and thoracic cavity infection.

To avoid the complication of aspiration pneumonia, we attempted a new anterior approach of extrapleural pneumonectomy combined with thoracoplasty in four patients with chronic empyema, a bronchopleural fi stula, and severely destroyed lung. The thoracic cage was exposed by an L-shaped parasternal incision making a muscle-skin flap of the anterior chest. The anterior portions of ribs were removed at fi rst, and exfoliation was started in the mediastinal area. After detaching the lung from the mediastinum, an extrapleural pneumonectomy was completed. The backside ribs were then resected by way o

[Emergency pleuropneumonectomy via anterior approach to treat chronic hemorrhagic empyema due to massive hemoptysis].

A 67-year-old man with a history of surgical resection of the superior lobe of the left lung and thoracoplasty due to pulmonary tuberculosis occurring approximately 40 years previously, was admitted to the hospital due to recurrent hemoptysis. X-ray films and computed tomography (CT) scans of the chest showed the left thoracic cavity to be filled with empyema, compressing the inferior lobe downward. Since three unsuccessful attempts were made at bronchial artery embolization for hemostasis, yielding hemoptysis of approximately 1,000 ml, emergency surgery was performed. To prevent massive intra

Surgical Treatment of Complications 55 Years After Extraperiosteal Lucite Ball Plombage for Pulmonary Tuberculosis.

INTRODUCTION: In the 1930-50s, before the introduction of antimicrobial drugs and development of techniques of pulmonary resssection, collapse therapy was the mainstream of treatment for cavitary pulmonary tuberculosis. The methods to achieve the collapse included artificial pneumothorax with air refills, phrenic nerve crush, thoracoplasty and extrapleural plombage. The plombage involves creating a cavity surgically under the ribs in the upper chest wall and filling the space with inert material, such fat, paraffin wax, rubber ballons, oil and methyl-methacrylate (Lucite) balls. The theory beh

Complex space-filling procedures for intrathoracic infections - personal experience with 76 consecutive cases.

OBJECTIVE: The objective of our article is to analyse the results of complex space-filling procedures for chronic intrathoracic suppurations. METHODS: We performed a retrospective analysis of 76 consecutive patients operated in our unit between 1 January 2003 and 31 December 2008, who presented pleural and/or pulmonary suppurations not amenable to decortication or resection; 36 patients (47%) had tuberculosis (TB) lesions (28 with positive cultures at the moment of surgery, seven with multi-drug-resistant (MDR) infections), 13 patients (17%) had postoperative empyema, 18 patients (24%) presen

Tuberculous Bronchopleural Fistula: A Rare and Life-Threatening Disease.

Tuberculous bronchopleural fistula (BPF) is a rare and potentially life-threatening complication of pulmonary tuberculosis, in which abnormal connections form between the bronchial tree and the pleural space. These abnormal connections allow air and secretions to pass from the lungs into the pleural space, causing a range of symptoms from benign cough to acute tension pneumothorax. The management of tuberculous BPF requires an individualized approach based on the patient's condition and response to treatment. Anti-tuberculosis therapy is essential for controlling the active tuberculosis infect

Staged removal of artificial patches for thoracic empyema after extrapleural pneumonectomy for diffuse malignant pleural mesothelioma.

A 69-year-old man with occupational exposure to asbestos was referred to our hospital with right diffuse malignant pleural mesothelioma. He underwent extrapleural pneumonectomy with reconstruction of the pericardium and diaphragm using elongated polytetrafluoroethylene patches, followed by postoperative chemotherapy and chest wall irradiation. One year later, he was hospitalized because of a right empyema caused by Escherichia coli infection. As chest drainage and systemic antibiotics did not eliminate the abscess around the artificial patches, a Clagett window was created. To avoid mediastina

Exceptional life-threatening complication 19 years after Ravitch correction of pectus excavatum.

BACKGROUND: Ravitch technique of chest correction has been considered, although invasive, as a safe and efficacious surgical method. CASE: We describe a case of a 35-year-old woman with cardiac tamponade and in cardiogenic shock due to exceptional late complication after pectus excavatum reconstruction by means of classic Ravitch technique 19 years earlier. This very late adverse event was caused by a broken metal sternal wire that injured the wall of the ascending aorta. The patient underwent salvage repair of this segment of the aorta in cardiopulmonary bypass. Postoperative course and post

Correction of pectus excavatum using the modified Nuss procedure, ad modum Pilegaard.

Pectus excavatum is the most common chest wall anomaly; it is often present at birth and may worsen during adulthood. In 1998 the minimally invasive Nuss procedure for correcting pectus excavatum was introduced. It uses curved Lorenz bars to push the sternum into a normal position and is an alternative to the Ravitch technique, where costal cartilages are removed using a sternal midline incision.  Since 2001 pectus excavatum corrections at Aarhus University Hospital have been performed using the modified Nuss procedure ad modum Pilegaard, which calls for a short asymmetrical pectus bar and no

[Modified Ravitch operation in the treatment of patients with funnel chest].

The characteristic traits of funnel chest have been presented and the clinical results of surgical treatment in 13 patients by the modified Ravitch method have been discussed. Nine results were excellent and four results were good. A marked improvement of general physical function of the patients due to the increase of respiratory system efficiency and a good cosmetic effect were found. The modification of the operation consists in additional longitudinal osteotomy of the sternum, suturing of the manubrium to the sternum shaft, non-suturing of the rib cartilages after resection, filling of the

Simultaneous costal cartilage-sparing modified Ravitch procedure and latissimus dorsi transfer for chest wall deformity repair in Poland's syndrome.

Poland's syndrome is a constellation of rare congenital anomalies that include hypoplasia of breast and underlying subcutaneous tissue, absence of the costosternal portion of the pectoralis major muscle, deformity or absence of ribs, absence of axillary hair, and syndactyly. Various surgical techniques have been described to repair such chest wall defects. We report a case of simultaneous Fonkalsrud procedure (costal cartilage-sparing version of the modified Ravitch procedure) and latissimus dorsi transfer in a 15-year-old boy with Poland's syndrome. The Fonkalsrud procedure has been used in t

[Various possibilities of reconstruction of the thoracic wall in radiation defects].

Reconstruction of the thoracic wall is often necessary after cancer surgery of the breast or irradiation. The resulting large defects often cause an instable thoracic wall. To reconstruct stability, we can use fascia or dura mater. The large defects can be closed either by latissimus flap, or, if the thoracodorsal artery is destroyed, which is often the case by rotating flaps according to the technique of Schrudde or Bohmert, in very rare cases by distant flaps from the neighbouring upper arm.

Recurrent Myxoid Liposarcoma of the Upper Thoracic Spine: A Case Report.

INTRODUCTION: Liposarcoma, one of the most common soft tissue sarcomas, originates from primitive mesenchymal cells. However, spinal involvement of liposarcoma, whether primary or metastatic, is rare. According to literature reports, myxoid/round cell liposarcomas are the most common types affecting the spine. Spinal liposarcomas, whether primary or metastatic, have only been sporadically reported. CASE PRESENTATION: The author presents an unusual case of recurrent myxoid liposarcoma originating from the erector spinae muscle, infiltrating the upper thoracic spine in a 65-year-old female. The

[Immunologic disorders in children with developmental thoracic defects].

As the result of immunological examination of 21 children with developmental defects of the chest and analysis of the course of the postoperative period in 136 children, among which 36 had hereditary syndromes of systemic connective-tissue dyshistogenesis, it was found that suppurative complications of thoracoplasty, which are encountered in 15% of children with isolated developmental chest defects and in 33.3% of those with the above mentioned syndromes, were caused to a great measure by disorders of the immune status. The most serious immunological deviations were encountered in the Marfan s

Apical vertebral derotation in the posterior treatment of adolescent idiopathic scoliosis: myth or reality?

INTRODUCTION: Direct apical vertebral rotation represents an important goal of posterior surgery for thoracic adolescent idiopathic scoliosis (AIS), so as to obtain a better cosmetic effect and to avoid posterior thoracoplasty. However, the real effectiveness in correction of vertebral rotation, using posterior only procedures, is still open to debate. The aim of the present study is to compare the correction of axial apical rotation obtained with direct rotation procedure versus simple concave rod rotation, in patients treated by posterior fusion for thoracic AIS using pedicle screw-only cons
