OBJECTIVE: Thyroid nodule ultrasound, conducted in France using the EU-TIRADS framework, is an essential tool for the management of these potentially malignant lesions. However, recent studies have suggested significant inter-reader variability in the description of sonographic features that form the foundation of EU-TIRADS scoring. To assess the impact of this variability in France, this work examines inter-reader differences among experienced French practitioners in assigning EU-TIRADS scores and sonographic feature labels to images acquired during routine clinical practice.

BACKGROUND: Automatic segmentation of ultrasound-based thyroid nodules can assist physicians in more efficiently and accurately assessing thyroid diseases. However, thyroid ultrasound imaging presents certain unique challenges, such as blurred boundaries of nodules, strong heterogeneity in tissue echoes, and limited contrast between the lesions and adjacent normal thyroid tissue, which hinder the precise identification of nodules in clinical assessment.
PURPOSE: To overcome these obstacles, we propose a deep hybrid convolutional network, named UTNseg, for precisely automatic segmentation of u

Ultrasound is widely used to monitor and diagnose thyroid nodules, but accurately segmenting these nodules in ultrasound images remains a challenge due to the presence of noise and artifacts, which often blur nodule boundaries. While several deep learning algorithms have been developed for this task, their performance is frequently suboptimal. In this study, we introduce the use of EfficientNet-B7 as the backbone for the DeepLabV3+ architecture in thyroid nodule segmentation, marking its first application in this area. We evaluated the proposed method using a dataset from the First Affiliated

Although using artificial intelligence (AI) to analyze ultrasound images is a promising approach to assessing thyroid nodule risks, traditional AI models lack transparency and interpretability. We developed a multimodal generative pre-trained transformer for thyroid nodules (ThyGPT), aiming to provide a transparent and interpretable AI copilot model for thyroid nodule risk assessment and management. Ultrasound data from 59,406 patients across nine hospitals were retrospectively collected to train and test the model. After training, ThyGPT was found to assist in reducing biopsy rates by more th

Thyroid nodules are common incidental findings, but only a small proportion of cases are malignant (4-6.5%) or symptomatic. Numerous follow-up examinations and invasive diagnostic procedures, such as fine-needle aspiration, fine-needle biopsies, and thyroidectomies, are performed, leading to potentially costly and time-consuming diagnostic procedures and overtreatment. Most experts and scientific societies (EFSUMB, WFUMB, etc.) encourage the use of multiparametric ultrasound evaluation (MPUS) to improve thyroid nodule characterization due to the continuous technological developments including

A 56-year-old woman presented for evaluation of a right thyroid nodule causing dysphagia. Thyroid function tests were normal. Ultrasound showed an isoechoic right thyroid nodule measuring 3.6 × 2.1 × 3.5 cm. After fine needle biopsy demonstrated benign pathology, ultrasound-guided radiofrequency ablation (RFA) of the nodule was performed. One day after the procedure, the patient developed a miotic, non-reactive right pupil, right upper eyelid and cheek swelling, right upper eyelid ptosis, and injection of the right eye. The patient was diagnosed with Horner syndrome (HS). RFA is a safe, minima

Immune checkpoint inhibitors, specifically programmed death-ligand 1 (PD-LI) inhibitors, are immune modifying medications that increasingly treat specific types of cancer. They are known to cause many side effects, including thyroid-related side effects. The use of PD-L1 inhibitors can cause hypothyroidism most commonly, while hyperthyroidism occurs less frequently. This case report describes a patient who developed a toxic thyroid nodule while taking the PD-L1 inhibitor, avelumab, for the treatment of Merkel cell carcinoma. It highlights the need for more research into the specific mechanisms

Laser thermal ablation (LTA) is an increasingly common procedure to treat benign and malignant thyroid nodules, allowing patients to avoid thyroidectomy. There are few reported postprocedural complications of LTA among patients with benign thyroid nodules. While vocal fold paralysis is a well-known potential complication after thyroidectomy, we present the first case report of vocal fold paralysis following LTA. A female in her 80s presented to an outside endocrinologist with symptoms of hyperthyroidism and benign thyroid nodules. The patient underwent a fine needle aspiration biopsy, radioiod

BACKGROUND Current medical technologies enable physicians to treat patients outside operating rooms using minimally invasive techniques. Non-operating room anesthesia (NORA) represents a growing field of medicine, with an increasing number of cases performed over the last decade. As a result, anesthesia providers will need to enhance their understanding of the resources, medical and paramedical staff, and environment outside the operating room. Patients undergoing such procedures under light conscious sedation still experience discomfort such as pain and anxiety, thus requiring the use of pain

Sixty-three patients with autonomous thyroid nodule were studied, thirty-five from an endemic goiter area (EA) and twenty-eight from a non-endemic goiter area (NEA). Clinical history, physical examination, ultrasonography, thyroid hormone levels, and fine needle aspiration (FNA), were utilized to investigate whether or not iodine deficiency determined differences in the latency of progression to toxicity, the seriousness of illness and thyroid hormone levels. No significant difference was observed in the age of onset of nodularity, while the latency of progression to toxicity was significantly

While few solitary thyroid nodules are carcinomatous, it is essential to identify and preferentially select those that are for surgery. Clinical, biochemical, serologic, radiographic, scintigraphic, sonographic, biopsy, and even therapeutic evaluation may be necessary to choose those patients with the greatest probability of malignancy. The benefits and limitations of each diagnostic modality are discussed, and the importance of fine-needle aspiration is stressed. After the operative confirmation of malignancy, the prognosis in any given case depends on 1) the histologic type of the neoplasm,

This description of 8 cases of thyroid nodules secondary to subacute thyroiditis serves to emphasise the relative prevalence of the circumscribed forms. These nodules, variable in size, firm and painful, developed after a period of cervical discomfort and pain. A contrast between a raised or normal blood hormone level and a very low uptake of I131 was found in five cases. It was all the more valuable when there was no iodine overload. Scintigraphy showed the nodule to be cold in 6 cases, isofixing in one and hot in one case. An essential diagnostic criterion is the rapidly favourable course: 4

This study aims to test computer-aided diagnosis (CAD) for thyroid nodules in clinical ultrasonography (US) practice with a focus towards identifying thyroid entities associated with CAD system misdiagnoses. Two-hundred patients referred to thyroid US were prospectively enrolled. An experienced radiologist evaluated the thyroid nodules and saved axial images for further offline blinded analysis using a commercially available CAD system. To represent clinical practice, not only true nodules, but mimicking lesions were also included. Fine needle aspiration biopsy (FNAB) was performed according t

BACKGROUND: Incidental thyroid nodules (ITNs) detected on nonthyroid imaging contribute to thyroid cancer overdiagnosis, largely through downstream evaluation initiated by thyroid ultrasound (TUS). However, TUS follow-up rates for ITNs and their drivers in routine practice remain unclear. We quantified TUS utilization after ITN detection and identified patient, imaging, nodule, and reporting factors associated with this first step in the diagnostic cascade.
METHODS: We conducted a retrospective cohort study of adults without prior thyroid disease who underwent imaging that incidentally captur
