Peripheral nerve tension is known to be related to the pathophysiology of neuropathy; however, assessing this tension is difficult in a clinical setting. In this study, we aimed to develop a deep learning algorithm for the automatic assessment of tibial nerve tension using B-mode ultrasound imaging. To develop the algorithm, we used 204 ultrasound images of the tibial nerve in three positions: the maximum dorsiflexion position and -10° and -20° plantar flexion from maximum dorsiflexion. The images were taken of 68 healthy volunteers who did not have any abnormalities in the lower limbs at the

Localized hypertrophic neuropathy (LHN) is a disorder of unknown cause that presents with progressive, predominantly motor weakness in the distribution of a single nerve, with the histological appearance of the "onion bulb" formation. Because of its rarity, the condition is often ignored in the differential diagnosis of mononeuropathy. We report a case of LHN involving the posterior tibial nerve and review the literature.

Distal tibial bone grafting is a safe and effective means of obtaining cancellous bone for hindfoot arthrodesis procedures, providing a sufficient quantity and quality of cancellous bone graft. Fusion rates using this bone graft are no less than those reported in the literature using autologous bone obtained from other sites. The 1% significant complication rate (fracture) of the donor site is low. Relative contraindications to the procedure include concomitant clinically significant peripheral neuropathy and severe osteopenia. Since cortical bone is not available from the distal tibia, proced

AIMS: We examined whether diabetic neuropathy is associated with altered muscle morphology, strength, and physical performance, and whether muscle characteristics relate to skeletal health.
METHODS: In this cross-sectional study, 123 individuals with type 2 diabetes were classified into +/- neuropathy based on peripheral and autonomic assessments. Tibial muscle morphology and bone microarchitecture were assessed using high-resolution peripheral quantitative computed tomography, while dual-energy X-ray absorptiometry assessed appendicular lean mass (ALM/height2) and femoral neck bone mineral d

Neuropathic ulcerations in diabetic patients are frequent causes of hospitalizations and morbidity. The plantar aspect of the first metatarsophalangeal joint is a common location for these ulcerations, because of the significant weightbearing forces generated through this joint and the presence of sensory and motor neuropathy. The authors describe 24 cases in which excision of the tibial sesamoid, fibular sesamoid, or both, was performed to resolve these lesions.

BACKGROUND: Many animal models can be used to study the underlying pathophysiological mechanisms of neuropathic pain. Most of these models rely on a partial denervation of the limb of the animal by ligating a selected nerve. In this study, we performed nerve lesions on three peripheral nerves supplying the plantar side of the rat hindpaw by differentially transecting the saphenous, the tibial, and the sural nerves alone or in paired combinations.
METHODS: The development of neuropathic pain symptoms at three different anatomical areas (medial, central, and lateral) of the glabrous skin of the

The effect of vincristine on regeneration of rat sural and tibial nerves following a crush lesion of the sciatic nerve was studied in the pinch test. Vincristine locally applied through an osmotic minipump at the site of the lesion dose-dependently retarded regeneration of the tibial and sural nerve at a threshold dose of 5 ng/day, whereas regeneration was blocked at a dose of 200 ng/day. Regeneration of the sural nerve was more sensitive to the retarding effects of vincristine than was regeneration of the tibial nerve. Systemic weekly administration (i.p.) of 1 mg/kg of vincristine for 7 week

Surgery for peripheral entrapment neuropathy aims to decompress the affected nerve and optimize the visualization of anatomical details during surgery. This paper describes our experience using the ORBEYE exoscope (Olympus) during surgery for tarsal tunnel syndrome (TTS). The patient was a 70-year-old male with complaints of bilateral pain and numbness on the plantar surface of the bilateral soles and medial halves of both lower limbs. He was diagnosed with idiopathic TTS with the American Orthopedic Foot and Ankle Score (AOFAS) of 20/100. Surgery for the right foot was performed under local a

INTRODUCTION AND HYPOTHESIS: The aim of the study was to describe effectiveness and safety of percutaneous tibial nerve stimulation (PTNS) at 2 years in women with overactive bladder (OAB) syndrome unresponsive to pharmacotherapy.
METHODS: Of 30 women who had initial positive response to PTNS, 23 continued to receive maintenance treatment and were reassessed at 2 years using bladder symptom diaries and the Incontinence Impact Questionnaire (IIQ-7). They were also questioned on acceptability and any adverse effects of long-term peripheral neuromodulative treatment.
RESULTS: Data from 23 women

The tarsal tunnel syndrome is a complex of symptoms affecting the foot produced by compression neuropathy of the posterior tibial nerve on the medial aspect of the ankle, within the fibrous osseous "tunnel" that has the posteromedial aspect of the tibia as its floor and the flexor retinaculum as its roof. Keck first drew attention to this entity in 1962, and was followed by Lam in the same year. Despite sporadic reports following these documentations, the clinical recognition of the syndrome is often delayed. It is still frequently misdiagnosed as acute foot strain or plantar fasciitis at its

BACKGROUND: Tarsal tunnel syndrome (TTS) describes a compression neuropathy of the tibial nerve or its branches as they pass under the flexor retinaculum of the ankle. TTS is a clinical diagnosis, but often involves needle electromyography, nerve conduction studies (NCS), and magnetic resonance imaging. To our knowledge, the use of ultrasound to diagnose tibial nerve compression from synovial cysts (SCs) in the tarsal tunnel has been sparsely reported upon.
CASE REPORT: A 50-year-old male patient suffering from TTS whose diagnosis of SC-related compression was confirmed via ultrasound. Patien

PURPOSE OF REVIEW: Single-organ vasculitis of the peripheral nervous system (PNS) is often designated nonsystemic vasculitic neuropathy (NSVN). Several variants or subtypes have been distinguished, including migratory sensory neuropathy, postsurgical inflammatory neuropathy, diabetic radiculoplexus neuropathies, skin-nerve vasculitides, and, arguably, neuralgic amyotrophy. NSVN often presents as nondiabetic lumbosacral radiculoplexus neuropathy (LRPN). This review updates classification, clinical features, epidemiology, and imaging of these disorders.
RECENT FINDINGS: A recent study showed th

The authors present an unusual case study of a Charcot joint of the first metatarsophalangeal articulation as well as a successful tibial sesamoidectomy for an associated chronic ulceration. Serial radiographs document the destructive disease process. The authors also address the etiologies of peripheral neuropathy, diagnostic tests to differentiate between Charcot joint and osteomyelitis, and both conservative and surgical management of Charcot joints.

Tarsal tunnel syndrome (TTS) is an entrapment neuropathy caused by compression of the posterior tibial nerve beneath the ankle flexor retinaculum. Treatment of TTS consists of surgical release of the retinaculum. The failure rate is 10%-20%. Magnetic resonance (MR) imaging was used to evaluate a patient with an unsatisfactory response. MR imaging demonstrated incompleteness of the surgical release of the flexor retinaculum.

BACKGROUND: Infection with COVID-19 has evolved from a novel health crisis into an entity with a myriad of increasingly recognized sequelae. Among these, peripheral neuropathy is often an underrecognized and underdiagnosed complication. Neuropathic pain can be challenging to treat with many cases being refractory to conservative management, often requiring advanced techniques. Neuromodulation has become an important tool for the treatment of the refractory cases.
CASE REPORT: We present a case of a 43-year-old woman with refractory right tibial mononeuropathy secondary to COVID-19 infection w

The tibial nerve injury model is a novel, surgically uncomplicated, rat model of neuropathic pain based on a unilateral transection (neurotomy) of the tibial branch of the sciatic nerve. The aim of the present study was to describe some behavioral and molecular features of the model, and to test its sensitivity to a number of drugs which are currently used for the treatment of neuropathic pain. The model was characterized by a pronounced mechanical allodynia which was present in all subjects and a less robust thermal hyperalgesia. Mechanical allodynia developed within 2 weeks post-surgery and

INTRODUCTION: Distal hereditary motor neuropathy (dHMN) is characterized by isolated distal muscle atrophy without sensory deficit. Nevertheless, clinical sensory loss has been reported despite preserved sensory nerve conduction in a few patients, thus differentiating these cases from the classical type 2 Charcot-Marie-Tooth disease (CMT2).
METHODS: We report 4 patients who presented with clinical sensory and motor neuropathy and normal peripheral sensory nerve conduction studies and were investigated with complete electrophysiological studies, including somatosensory evoked potentials (SEP).
