[Bilateral diaphragmatic palsy due to Lyme neuroborreliosis].

INTRODUCTION: Lyme disease is not uncommon and can sometimes progress to neurological complications. We report here an unusual case of bilateral diaphragmatic paralysis secondary to Lyme neuroborreliosis. CASE REPORT: A 79-year-old man was admitted to the intensive care unit for acute respiratory distress requiring intubation and the long-term use of nocturnal non-invasive ventilation. Three months beforehand he had been bitten by a tick and developed erythema migrans which was treated with Doxycycline for 10 days. This clinical presentation became complicated a few days later by the progress

[Bilateral tonic pupil as the only remaining ophthalmic sign of Lyme disease: case report].

Lyme disease is a multisystemic disorder caused by Borrelia burgdorferi spirochete, transmitted by ticks. Mainly described in the northern hemisphere and rarely in Brazil. The purpose of this report is to describe a patient with Lyme disease who developed bilateral tonic pupil as the only remaining sign. A 13-year-old female with Lyme disease, presented with bilateral peripheral facial and oculomotor paralysis. After recovery from neurological abnormalities the patient sustained anisocoria, reduced fotomotor reflex, less than normal amplitude of accommodation, tonic pupil constriction for near

[Development of cellular immunity in experimental tick-borne encephalitis].

The development of cellular immunity was investigated in mice infected experimentally with tick-borne encephalitis virus strains (TBE) isolated from patients. In the presence of strain variants of the response, two main trends in the dynamics of cellular immunity induced by different virus strains were established: a rapid increase in the level of sensitization of lymphoid cells in the first days after virus inoculation followed in the stage of paralysis by the desensitization phenomenon (the dynamics of the waning type), and a gradual increase in the level of cellular response in the course o

Borrelia burgdorferi: A Rare Cause of Stroke.

Lyme disease is the most common tick-borne zoonosis in Europe. It is a multisystemic infectious disease that can produce cerebrovascular events on rare occasions. Here, the authors describe a case of a 58-year-old female patient living in a rural area who presented to the hospital with dysarthria, right central facial paralysis, right hypoesthesia, and severe right hemiparesis. Imaging revealed an infarct in the left corona radiata, posterior limb of the internal capsule, posterior parietal region, and multiple microhemorrhages scattered throughout the cerebral hemispheres. She was hospitalize

Geographical distribution of arboviruses in Yugoslavia.

Studies of arboviruses started in Yugoslavia in 1953 following the isolation of TBE virus which caused a severe epidemic that year. Until now the following viruses have been proven to circulate in the country: tick-borne encephalitis (TBE), Crimean-Congo hemorrhagic fever (CCHF), Bhanja (BHA), sandfly fever (SF), Tahyna (TAH), Calovo (CVO), West Nile (WN), dengue (DEN), Jug Bogdanovac (JB), and Hantaviruses. TBE virus is endemic in the north-west part of the country, causing also epidemics in cyclical intervals. Its typical clinical picture is aseptic meningitis, but severe cases with paralysi

[Positive anti-Borrelia antibodies in patients with clinical manifestations compatible with neuroborreliosis].

The clinical features of 19 patients with neurological manifestations unexplained by another disease and positive serology for Borrelia burgdorferi were studied. ECM was present in only 11% of the cases and 32% referred tick bite. The characteristic features for suspicion of NB according to our series was the presence of polyneuritis in 84% of the cases specially in the form of multiple mononeuritis and involvement of the facial nerve (79%) leading to even greater suspicion with the association of V pair involvement. Seizures, sleep disorders, and higher mental dysfunction may be found in asso

Enterovirus type 71 infections: a varied clinical pattern sometimes mimicking paralytic poliomyelitis.

Not all poliomyelitis-like paralytic illnesses can be attributed to polioviruses. Among the most recently recognized agents able to cause severe central nervous system disease with persistent flaccid paralysis is enterovirus type 71. In the 1969-1973 California outbreaks during which strains of this type were first reported, meningitis predominated, but cases of encephalitis were also seen. Outbreaks in different regions of the world followed. In some, hand-foot-and-mouth syndrome predominated; in others, meningitis; and in some, the clinical patterns were mixed. In the 1975 Bulgarian outbreak

Venomous snake bites, scorpions, and spiders.

Neurologic dysfunction due to natural neurotoxins is an important, but neglected, public health hazard in many parts of the world, particularly in the tropics. These toxins are produced by or found among a variety of live forms that include venomous snakes, arthropods such as scorpions, spiders, centipedes, stinging insects (Hymenoptera), ticks, certain poisonous fish, shellfish, crabs, cone shells, skin secretions of dart-poison frogs, and bacterial poisons such as botulinum toxin. These toxins commonly act on neuromuscular transmission at the neuromuscular junction where acetylcholine is the

[Active vaccination against early summer meningoencephalitis].

TBE is a rather rare disease (incidence in Switzerland 0.46/1,000,000 population per year). Acquisition occurs only in well known endemic areas through bites of infected ticks. The course of the disease is not trivial: 0.5-2% of cases are fatal, 2.7% have persistent serious paralysis and some 33% a postencephalitic syndrome. There is no specific therapy. An active vaccine is commercially available which is reliably immunogenic (99% of vaccinated persons develop protective antibodies) and involve only a few mild side effects. Mass vaccination seems not to be cost-effective but would be epidemio

[Meningoencephalo-myeloradiculitis due to Flavivirus: bi-brachial paralysis and respiratory insufficiency].

3 patients developed rapid onset of fever and nuchal stiffness. Paresis of brachial muscles occurred within 4 days and all patients had respiratory failure that needed mechanical ventilation. At the peak of the disease there were bilateral asymmetrical severe atrophy of brachial, shoulder and neck muscles, cranial nerve pareses and absent or weak deep reflexes in the upper extremities. CSF analyses showed sterile lymphocytic pleocytosis. In 2 cases the patients suffered a tick bite in Switzerland and the third was probably bitten by an insect while opening a package received from Indonesia. Pa

Acute lower motor neuron tetraparesis.

Flaccid nonambulatory tetraparesis or tetraplegia is an infrequent neurologic presentation; it is characteristic of neuromuscular disease (lower motor neuron [LMN] disease) rather than spinal cord disease. Paresis beginning in the pelvic limbs and progressing to the thoracic limbs resulting in flaccid tetraparesis or tetraplegia within 24 to 72 hours is a common presentation of peripheral nerve or neuromuscular junction disease. Complete body flaccidity develops with severe decrease or complete loss of spinal reflexes in pelvic and thoracic limbs. Animals with acute generalized LMN tetraparesi

Isolation and characterization of a Far-Eastern strain of tick-borne encephalitis virus in China.

Tick-borne encephalitis virus (TBEV) is a leading cause of human neurological infection in many parts of Europe and Asia. Although several TBEV isolates have been reported, current understanding of the biological characteristics of a Chinese strain is limited. In this study, a Far-Eastern strain of TBEV designated WH2012 was isolated in northern China. Its genome has been sequenced and found to be closely related to other Chinese TBEV isolates. Human cell lines of neural origin exposed to WH2012 showed cytopathic effects and WH2012 replicated most efficiently in human neuroblastoma cells SK-N-

Guillain-Barré syndrome in children.

The Guillain-Barré syndrome is a pediatric neurologic emergency and the most common cause in children of rapidly evolving, usually flaccid, weakness with associated areflexia. Two treatable illnesses, namely tick paralysis and acute cord compression, demand immediate differential diagnosis. Rarely, poliomyelitis still mimics infantile Guillain-Barré syndrome. Specific precursor infections, such as Campylobacter jejuni, are now recognized to affect the clinical presentation of Guillain-Barré syndrome. Cerebrospinal fluid evaluation and electromyography are usually diagnostic; new modalities, su

Reporting of New tick-borne encephalitis virus strains isolated in Eastern Siberia (Russia) in 1960-2011 and explaining them in an evolutionary context using Bayesian phylogenetic inference.

Tick-borne encephalitis virus (TBEV) is one of the main tick-borne viral pathogens of humans. Infection may induce signs of meningitis, encephalitis, paralysis and high fever. TBEV is well studied by molecular phylogenetic methods. The present-day implementation of Bayesian phylogenetic models allows population dynamics to be tracked, providing changes in population size that were not directly observed. However, the description of the past population dynamics of TBEV is rare in the literature. In our investigation, we provide data on the dynamics of viral genetic diversity of TBEV in Zabaikals

Diagnosis and Treatment of Lower Motor Neuron Disease in Australian Dogs and Cats.

Diseases presenting with lower motor neuron (LMN) signs are frequently seen in small animal veterinary practice in Australia. In addition to the most common causes of LMN disease seen world-wide, such as idiopathic polyradiculoneuritis and myasthenia gravis, there are several conditions presenting with LMN signs that are peculiar to the continent of Australia. These include snake envenomation by tiger (Notechis spp.), brown (Pseudonaja spp.), and black snakes (Pseudechis spp.), tick paralysis associated with Ixodes holocyclus and Ixodes coronatus, and tetrodotoxins from marine animals such as
