Whole Genome Profiling of Lung Microbiome in Solid Organ Transplant Recipients Reveals Virus Involved Microecology May Worsen Prognosis.

Solid organ transplantation (SOT) is the final therapeutic option for recipients with end-stage organ failure, and its long-term success is limited by infections and chronic allograft dysfunction. Viral infection in SOT recipients is considered an important factor affecting prognosis. In this study, we retrospectively analyzed 43 cases of respiratory infections in SOT recipients using metagenomic next-generation sequencing (mNGS) for bronchoalveolar lavage fluid (BALF). At least one virus was detected in 26 (60.5%) recipients, while 17 (39.5%) were virus-negative. Among virus-positive recipien

Exploratory study of Torque teno sus viruses in pulmonary inflammatory lesions in pigs.

The pathogenic role of Torque teno sus viruses 1 (TTSuV1) and 2 (TTSuV2), and their capacity to induce lesions are controversial. TTSuVs have been linked to porcine circovirus diseases (PCVDs) and described as cause of mild respiratory lesions in gnotobiotic pigs; moreover, an increased TTSuV prevalence has been found in porcine respiratory disease complex (PRDC) affected pigs. In the present study, TTSuV1 and TTSuV2 loads and their prevalence were evaluated in formalin-fixed, paraffin-embedded (FFPE) lung samples displaying different types of inflammatory lesions. Such measurements were made

Probable transfusion-transmitted Zika virus in Brazil.

BACKGROUND: Zika virus (ZIKV) is an emerging arthropod-borne flavivirus transmitted by Aedes mosquitoes. Recent commentaries regarding ZIKV routes of transmission describe a potential transmission by transfusion. Herein, we report a probable case of transfusion-transmitted ZIKV infection through a platelet transfusion that was detected from postdonation information. CASE REPORT: A blood donor made a voluntary telephone report to the blood donor facility 3 days after donation and informed the facility of a febrile illness (fever, malaise, and headaches). Due to the ongoing dengue epidemic, the

Predictive Utility of ViroFind Detection of Blood and CSF Virome for Viral Presence in Human Brain Tissue.

Viral presence in the brain may contribute to chronic neurologic diseases. However, investigating these associations is limited by the difficulty of directly sampling brain tissue in living individuals. Here, we evaluated whether peripheral viral detection using unbiased target-enrichment next-generation sequencing could inform viral presence in the brain across a diverse set of viral taxa. We applied ViroFind to matched brain, blood (peripheral blood mononuclear cells, spleen, and/or lymph node), and cerebrospinal fluid (CSF) to assess the predictive utility of viral detection in blood and CS

Sustained allogeneic kidney graft operational tolerance despite discontinued conventional immunosuppression after CD19-CAR-T cell therapy for relapsed/refractory posttransplant lymphoproliferative disorder.

Management of immunosuppression after solid organ transplantation in context of chimeric antigen receptor T cell therapy (CART) is challenging. Although required to prevent graft rejection, systemic immunosuppression can interfere with biological functions of apheresis products and adoptively transferred T cells. We treated a 33-year-old kidney transplant recipient who developed relapsed/refractory posttransplant lymphoproliferative disorder with CD19-directed CART as a fourth-line therapy. Immunosuppression was discontinued before leukapheresis for CART and not reinitiated ever since. Althoug

Predictive models for graft rejection and infections in renal transplant patients based on TTV viral load.

BACKGROUND: Graft rejection and opportunistic infections are among the main causes of graft dysfunction and loss in renal transplant recipients. Torque Teno Virus (TTV) is a highly prevalent, non-pathogenic virus that has been postulated as an immunosuppression biomarker in renal transplant patients. This study aimed to develop predictive models for these adverse events based on TTV viremia. METHODS: From 130 patients, 871 plasmas were tested for TTV viral load. Patients' follow-up was documented up to 3 years post-transplantation. To develop predictive logistic regression models, ROC curve a

TT virus detection in oral lichen planus lesions.

Epidemiological studies have demonstrated a correlation between oral lichen planus and different liver diseases. The new virus termed TT virus (TTV) is highly prevalent in patients with chronic hepatitis of different etiology and it may be speculated that TT virus may be involved in the pathogenesis of oral lichen planus. This study examined the presence of TT virus DNA in serum by PCR and in oral mucosa biopsies by in situ hybridization from 20 patients with oral lichen planus (13 with chronic hepatitis and seven without liver disease). Serum and oral mucosa biopsies from six patients all wit

Efficacy of Different Testing Scenarios in Reducing Transfusion-Transmitted Hepatitis B Virus (TT-HBV) Infection Risk.

The efficacy of different screening scenarios in reducing hepatitis B virus (HBV) transmission risk as compared to the risk without screening was modeled in 9,337,110 donations from four geographical regions that had been subjected to hepatitis B surface antigen (HBsAg) and individual donation nucleic acid amplification testing (ID-NAT). We used the Weusten models for estimating infectivity risk for Red Blood Cell (RBC) transfusions in eight HBV infection stages and then evaluated multiple screening strategies based on minipool (MP) and ID-NAT options of different sensitivity for their efficac

Human genetic polymorphisms and risk of viral infection after solid organ transplantation.

The immune system plays a key role in the host defense against viral pathogens. A signaling cascade is activated upon infection involving a variety of molecules such as pattern-recognition receptors (PRRs), interleukins or antiviral interferons. Long-term immunosuppression after solid organ transplantation (SOT) mainly abrogates adaptive T-cell-mediated responses, thus highlighting the relative contribution of innate immunity. Single-nucleotide polymorphisms (SNPs) within genes coding for PRRs or soluble mediators have been associated with differential susceptibility to viral infections among

TTV and CMV viral load dynamics: Which emerges first during immunosuppression?

Novel biomarkers reflecting the degree of immunosuppression in transplant patients are required to ensure eventual personalized equilibrium between rejection and infection risks. With the above aim, Torque Teno Virus (TTV) viremia was precisely examined in a large cohort of transplanted immunocompromised patients (192 hematological and 60 solid organ transplant recipients) being monitored for Cytomegalovirus reactivation. TTV load was measured in 2612 plasma samples from 448 patients. The results revealed a significant increase in TTV viral load approximately 14 days following CMV reactivation

Survey of Viral Reactivations in Elite Athletes: A Case-Control Study.

Exercise-induced immune perturbations have been proposed to increase susceptibility to viral infections. We investigated the replication of persisting viruses as indicators of immune function in elite cross-country skiers after ten months of sustained high-performance exercise. The viruses evaluated, nine human herpesviruses (HHVs) and torque teno virus (TTV), are typically restrained in health but replicate actively in immunosuppressed individuals. We collected sera from 27 Finnish elite cross-country skiers at the end of the competition's season and 27 matched controls who perform moderate e
