Surgical Considerations for an Awake Tracheotomy During the COVID-19 Pandemic.

Background: The current global COVID-19 pandemic is caused by the novel coronavirus Severe Acute Respiratory Syndrome coronavirus 2 (SARS-CoV-2). Given that SARS-CoV-2 is highly transmissible, surgical societies have recommended that procedures with a high risk of aerosolization be avoided or delayed. However, some high-risk procedures, such as those related to head and neck malignancies, cannot always be delayed. Care must be taken during aerosol-generating procedures to minimize viral transmission as much as possible. Preoperative testing for COVID-19, limited operating room personnel, adequ

Anesthetic Management of a Child With Jeune Syndrome for Tracheotomy: A Case Report.

Jeune syndrome is a rare autosomal-recessive skeletal disorder. Anesthetic management of these patients is often difficult because of thoracic and lung hypoplasia. A 5-month-old boy with Jeune syndrome was scheduled to undergo a tracheotomy. Despite 5-minute preoxygenation with continuous positive airway pressure, the patient's oxygen saturation rapidly dropped during the induction of anesthesia. The continuous positive airway pressure should have been titrated to effective tidal volume during preoxygenation to recruit the patient's functional residual capacity and to prevent desaturation. Dur

Protecting the airway and the physician: Aspects of litigation arising from tracheotomy.

BACKGROUND: In recent decades, medical malpractice costs have increased and have led to a change in the way physicians practice medicine. Tracheotomies are cases in which complications have a high risk of morbidity and mortality and the potential for litigation. METHODS: The Westlaw legal database was used to gather data on 43 jury verdicts and settlements from 1987 to 2013. Various factors included outcome, defendant specialty, and the reason for litigation. RESULTS: Median settlements were $500,000 and median verdict awards were $2,000,000. Postoperative negligence was alleged most often (

The effect of a speaking valve on laryngeal aspiration and penetration in children with tracheotomies.

OBJECTIVES/HYPOTHESIS: Alterations in swallowing can occur after tracheotomy and can result in varying degrees of aspiration. In tracheotomized adult patients, use of a Passy Muir Speaking Valve (PMV) has been shown to decrease laryngeal penetration and aspiration of foods and liquids. The objective of this study was to determine if the PMV has a similar effect on laryngeal penetration and aspiration in tracheotomized children. STUDY DESIGN: This is a prospective case-control study. METHODS: Pediatric patients with tracheotomies who were able to tolerate the PMV were identified. Modified bar

[Cervical hematoma after discectomy for spondylosis. An indication of urgent tracheotomy].

We report the clinical case of a hematoma after surgery of cervical spondylosis which required an urgent tracheotomy due to failure in the orotracheal intubation. This illness in advanced degree, so called cervical spondylotic myelopathy, is the most serious consequence of cervical intervertebral disc degeneration.

Complications of tracheotomy in patients with mucopolysaccharidoses type II (Hunter syndrome).

OBJECTIVE: To investigate the complication rate of tracheotomy in patients with mucopolysacchridoses (MPS) type II (Hunter syndrome). MATERIALS AND METHODS: From 2004 to 2005, seven tracheotomy procedures were performed for the airway management in three patients with MPS type II. The complications for each procedure were analyzed, which included the stomal narrowing, granulation formation, infrastomal tracheal stenosis, and wound infection. RESULTS: All tracheotomies in patients with MPS type II resulted in tracheotomy-related complications, though these procedures secured a safe airway. In

[Tracheotomy in children of the Pediatric ENT Department in years 1995-2005].

INTRODUCTION: In the nineteenth century tracheotomy was performed in case of diphtheria. In 1869 Trousseau presented data on 215 cases of this disease treated this way and it became an operation that was reserved for life threatening conditions. In 1907 Jackson presented his experience with tracheotomy and provided information on equipment and techniques that are still used nowadays. MATERIAL AND METHODS: A retrospective analysis was performed on 79 patients undergoing tracheotomy at Pediatric ENT Dept in Poznañ from 1995-2005. Charts were reviewed with respect to primary diagnosis and indica

The shift in the indications for the tracheotomy between 1940 and 1955: an historical review.

In the period between 1940 and 1955 the indications for tracheotomy were extended. For centuries tracheotomies were performed to treat obstructive diseases of the upper airway (hypopharynx, larynx, trachea). With the end of the second World War tracheotomy was indicated more and more for the therapy of lower airway disturbances. Medical empiricism is thought to be responsible for the gradual shift in the indications during that period from upper to lower airway disturbances. Knowledge about the underlying principles of respiratory physiology was, however, already present at the end of the 19th

A modified occlusal splint to avoid tracheotomy for total midface osteotomies.

A modified occlusal splint is described which may be used to avoid tracheostomy when a total midface osteotomy is required. We demonstrate the accuracy of the modified splint by using the technique in a control subject who underwent alginate impressions, dental casts, and a face-bow transfer. A modified splint was constructed on the articulated dental models and was found to be accurate within the range of rotational movement of the temporomandibular joint. The technique is demonstrated in a patient with Crouzon's syndrome undergoing cranial vault reshaping and a monobloc (midface) advancement

[Acute upper airway obstruction caused by a multinodular goitre after tracheotomy].

This case report illustrates how a multinodular goitre caused acute airway obstruction in a multitrauma 66-year-old male after tracheotomy. The patient had no symptoms of the goitre before the tracheotomy but developed subglottic airway obstruction at the time of decannulation. He underwent a subacute total thyroidectomy which removed the obstructive symptoms. A weakening of the tracheal tube followed by the tracheotomy possibly caused the tracheal stenosis in combination with the goitre.

[Acute bleeding from the innominate artery after removal of percutaneous tracheotomy].

Percutaneous tracheotomy is a standard procedure in intensive care units. Even though complications associated with this intervention have been described, acute massive bleeding from the innominate artery is rare and necessitates immediate intervention. We report a case with acute bleeding after removal of a percutaneous tracheotomy cannula.

[Local immunity following treatment with S-carboxymethylcysteine-lysine in tracheotomy patients].

Pathological modification in secretory IgA values as well as in circadian rhythms were found in tracheotomized patients in both nasal and tracheo-bronchial secretions. The protective role of the mucosal immune system in addition to the frequency of severe infectious respiratory diseases in laryngectomized patients justifies the efforts of clinicians to prevent and treat such modifications. Mucoregulatory drugs have a peculiar role in these therapeutical attempts. Forty tracheotomized subjects with neoplastic disease were studied. Twenty received SCMC-Lys as a mucoregulatory drug. Relative Factors Analysis of the Occurrence and Location of Intratracheal Granuloma Following Tracheotomy.

AIM: Tracheotomy has become more prevalent in clinical settings, and effectively managing postoperative complications plays a crucial role in determining patient outcomes. However, there is a scarcity of clinical research focusing on the development of intratracheal granuloma after tracheotomy, and there is insufficient theoretical support for early detection in clinical settings. This study investigates the relationship between clinical factors and the occurrence and location of intratracheal granuloma. METHODS: Clinical parameters from 872 patients who underwent tracheotomy between January
