Background: The trapeziometacarpal joint (TMCJ) is the most common hand joint affected by osteoarthritis (OA), and trapezium implant arthroplasty is a potential treatment for recalcitrant OA. This meta-analysis aimed to investigate the efficacy and safety of various trapezium implants as an interventional option for TMCJ OA. Methods: Web of Science, PubMed, Scopus, Google Scholar, and Cochrane library databases were searched for relevant studies up to May 28, 2022. Preferred Reported Items for Systematic Review and Meta-Analysis guidelines were adhered to, and the protocol was registered in PR

INTRODUCTION: Trapeziometacarpal osteoarthritis sometimes results in hyperextension of the thumb metacarpophalangeal (MCP) joint, which could negatively impact outcomes following trapeziectomy with ligament reconstruction and tendon interposition (LRTI) arthroplasty. Although algorithms on performing trapeziectomy with LRTI for the management of this deformity are available, they lack clear evidence. Here, we investigate the function of the thumb MCP joint after trapeziectomy with LTRI and whether this procedure alone corrects preoperative MCP hyperextension, and also analyze clinical factors

PURPOSE: Advanced-stage trapeziometacarpal (TMC) osteoarthritis of the thumb often presents with concomitant hyperextension deformity of the metacarpophalangeal (MCP) joint. Although several studies have reported simultaneous procedures to correct this deformity, the indication for these procedures remains controversial. The purpose of this study was to evaluate changes in the range of motion (ROM) of the thumb MCP joint before and after TMC arthrodesis.
METHODS: We evaluated the functional flexion and extension and functional ROM of the MCP joints during the performance of 10 activities of d

There are a number of possible assessments of functional outcomes for thumb carpometacarpal joint arthritis. The most important and easiest to measure is pain, but it is not the only material outcome. Functional scores for measuring the outcome of the treatment of thumb carpometacarpal joint arthritis have been recommended for some time, but are still not widely used even in published studies. It is also unclear which functional scores are the most valuable. Easily used scores, such as the Disabilities of the Arm Shoulder and Hand questionnaire, are freely available, but may not be sensitive e

UNLABELLED: A prospective randomised study on 46 patients was performed to evaluate postoperative analgesia after trapezectomy. We compare the efficacy of an in situ catheter 777 (ISC) positioned by the surgeon in the space of the trapezectomy (26 patients) with an axilliary perineural catheter (APC) positioned preoperatively by the anaesthetist (20 patients). The patients were followed-up postoperatively for 48 hours and assessed at 3, 7, 24 and 48 hours, recording pain (VAS), consumption of intravenous rescue analgesia and recovery of sensory and motor function.
RESULTS: The mean time to si

BACKGROUND: Measurable changes in patients with progression of thumb carpometacarpal (CMC) osteoarthritis (OA) include joint space narrowing, osteophyte formation, subluxation, and adjacent-tissue changes. Subluxation, an indication of mechanical instability, is postulated as an early biomechanical indicator of progressing CMC OA. Various radiographic views and hand postures have been proposed to best assess CMC subluxation, but 3D measurements derived from CT images serve as the optimal metric. However, we do not know which thumb pose yields subluxation that most indicates OA progression.

BACKGROUND: The incidence of wrong-bone excision in hand surgery is not known. The wrist offers a unique challenge in that a single surgical incision yields access to multiple potential surgical sites, thus rendering site marking less effective. Mistaken identification of structures in the wrist is a real and potentially troublesome phenomenon. This study is designed to evaluate the occurrence of wrong-bone excision and to identify any risk factors and preventative measures that may help to prevent this complication in the future.
METHODS: We designed an anonymous online survey to determine t

PURPOSE: To evaluate the diagnostic value of magnetic resonance (MR) imaging in patients with clinical suspicion of scaphoid fractures and normal initial plain radiographs.
MATERIALS AND METHODS: MR imaging was performed within 7 days after trauma in 42 patients with clinical suspicion of scaphoid fractures and normal plain radiographs. T1-weighted spin-echo, T2*-weighted gradient-echo, and short inversion time inversion-recovery (STIR) sequences were performed. MR images were evaluated independently by two radiologists. Six-week follow-up radiographs were used as a standard to diagnose fract

Trapezium fractures are unusual; however, they represent the third most frequent fracture of the carpal bones. As they usually follow a high-energy trauma, they are associated with distal radius, Bennett, or Rolando fractures in 80% of cases. Traditional treatment options include, closed reduction and percutaneous pinning, or open reduction and internal fixation. To minimize the additional surgical trauma, an arthroscopic technique has been developed for safe, minimally invasive management of complex injuries of the first carpo-metacarpal joint. Intra-articular dislocated fracture fragments ar

We report a rare case of major trapezometacarpal instability observed in a 31-year-old woman with congenital hyperlaxity. The patient presented major functional impotency of both thumbs, closure of the first commissure, and absence of effective opposition except between the lateral borders of the long fingers due to severe pain in the trapezometacarpal joint attributed to osteoarthritis. The patient underwent bilateral trapezectomy with ligamentoplasty. The gross examination of the trapezeal specimens revealed major cartilage destruction on the medial portion of the trapezeal joint surface. Th

The anatomical snuff box is bounded laterally by the Abductor pollicis longus (APL) and the Extensor pollicis brevis (EPB) tendons. The variation in insertion of extensor tendons at the forearm and wrist is an interesting phenomenon. Therefore, a sound knowledge of such variations in extensor tendons is essential to know the consequence of tendon injury during implementation of its repair. Hence, we studied 48 formaldehyde fixed forearms of Indian-origin cadavers with age groups ranging from 33 to 67 years from the Central Indian population at Nagpur, Maharashtra, India. Variation in the inser

At the transitional zone from the forearm to the hand the insertion tendon of the m.flexor carpi radialis (FCR) glides on a fibrous and fatty cushion, which is connected dorsally with the joint capsule of the radiocarpal articulation. The tendon distally crosses the palmar side of the scaphoid tubercle and enters the dorsally curved rim of the trapezoid tubercle. At the level of the wrist joint the narrow tendon sheath begins, which extends to the insertion at the metacarpus. Immediately after entering the gliding tunnel the tendon branches off radially as a rule with an accessory fibre strand

BACKGROUND: Trapeziometacarpal osteoarthritis (TMC OA) is a common degenerative condition, characterized by pain and functional limitations, predominantly affecting older women. While conservative treatments are the primary approach, functional outcomes are highly variable, and the predictive factors influencing these outcomes remain uncertain.
PURPOSE: This systematic review aims to identify demographic, clinical, and psychological factors that predict functional outcomes following conservative management of TMC OA.
STUDY DESIGN: Systematic Review.

Isolated trapezium fractures are a rare type of injury, accounting for approximately 4% of all carpal fractures. Due to their nonspecific symptoms and difficulty in detection with conventional radiography, these fractures are often underdiagnosed. This case report details a 22-year-old male who sustained a trapezium fracture after a motorcycle accident. The patient presented with wrist pain and mild edema, but radiographs were normal. However, computed tomography (CT) revealed a small fracture in the dorsal portion of the trapezium. Conservative treatment with wrist immobilization was administ

STUDY OBJECTIVE: To evaluate the accuracy of day 4 bone scans in predicting the presence or absence of fracture in patients with "clinical scaphoid fracture."
DESIGN: Prospective sensitivity study of ED patients with clinical scaphoid fractures. Each patient was immobilized in a thumb spica cast and had day 4 bone scans of both wrists and hands. Blinded day 4 bone scan results were ultimately compared with the diagnosis on day 14 when patients returned for repeat clinical examination and radiographs. In cases of equivocal radiographic or clinical examination results, a day 14 bone scan was pe

Subcutaneous rupture of the flexor carpi radialis (FCR) tendon from scapho-trapezio-trapezoid (STT) osteoarthritis is a rarely reported and diagnosed condition. A 65-year-old female presented in the clinic with wrist pain aggravated with movement. On physical examination, there was a 2-cm soft elastic mass at the radial side of the volar wrist. There was no tenderness, but pain was elicited with range of motion. Computed tomography confirmed an osteoarthritic STT with large spurs. Magnetic resonance imaging (MRI) showed impingement on the tract of the FCR associated with synovitis. Complete ru

Thumb carpometacarpal implant arthroplasty aims to preserve thumb length and motion and to provide pain relief and functional recovery after a short postoperative time. For several decades, implant arthroplasty has been performed with total trapeziometacarpal joint prosthesis using the concept of "ball-and-socket" joint. More recently, pyrocarbon implants used as hemiarthroplasty or interposition arthroplasty have been proposed. Whatever the type of arthroplasty used, the surgical technique must be precise and may require a learning curve. Implant arthroplasty has proven that in the medium and

Trapeziectomy has been regaining popularity in treatment of thumb carpometacarpal (CMC). Current approaches to trapeziectomy use Kirschner wire fixation to prevent subsidence of the thumb metacarpal into the newly created trapeziectomy space. This fixation method requires at least 4 weeks of strict immobilization of the thumb. A technique using a suture-button device to suspend the thumb allows for earlier mobilization of the thumb and potentially leads to a faster overall recovery. Preliminary results are promising, but further studies must be conducted to confirm the long-term efficacy of th

The purpose of this study was to assess the effect of limb elevation on hand swelling after surgery. We prospectively randomized 113 patients undergoing fasciectomy or a trapeziectomy into one of two groups. The first group had the hand elevated postoperatively and the hands of the second group were not elevated. Hand swelling was assessed using a volumetric method. Although the swelling was less in the elevated group this did not reach statistical significance. Subgroup analysis revealed no further significant differences. There were no complications in patients who did not have limb elevatio
