[Polyvagal Theory and Its Clinical Potential: An Overview].

The polyvagal theory, proposed by Stephen Porges, describes the adaptive responses of the mammalian autonomic nervous system. According to this novel theory, the vagus nerve functions through two independent pathways, referred to as the ventral and the dorsal vagal pathways. The ventral vagus is a myelinated nerve that has newly emerged in mammals and in coordination with cranial nerves regulates the muscles of the face and head to form the ventral vagal complex, which enables social engagement via exchange of safety cues and downregulating sympathetic defense reaction. In a safe environment,

MRI of neonatal encephalopathy.

We present the magnetic resonance imaging (MRI) findings in neonatal encephalopathy, including hypoxic-ischaemic encephalopathy, perinatal/neonatal stroke, metabolic encephalopathy from inborn errors of metabolism, congenital central nervous system infections and birth trauma. The applications of advanced MRI techniques, such as diffusion-weighted imaging and magnetic resonance spectroscopy are emphasized.

Neurological episodes after minor head injury and trigeminovascular activation.

Children appear particularly susceptible to severe but reversible neurological symptoms and/or signs after minor head injury; these include headache, confusion, drowsiness, vomiting, hemiparesis, cortical blindness, or seizures. Significantly, these neurological episodes are not associated with any identifiable structural brain abnormality on neuro-imaging. We propose that the cause of this condition is a reactive hyperaemia, a 'benign hyperaemic encephalopathy' mediated via activation of the trigeminovascular system.

Models of bilirubin neurological damage: lessons learned and new challenges.

OBJECTIVE: Jaundice (icterus) is the visible manifestation of the accumulation of bilirubin in the tissue and is indicative of potential toxicity to the brain. Since its very first description more than 2000 years ago, many efforts have been undertaken to understand the molecular determinants of bilirubin toxicity to neuronal cells to reduce the risk of neurological sequelae through the use of available chemicals and in vitro, ex vivo, in vivo, and clinical models. Although several studies have been performed, important questions remain unanswered, such as the reasons for regional sensitivity

The relationship of trauma exposure to heart rate variability during wake and sleep in midlife women.

Traumatic experiences are common and linked to cardiovascular disease (CVD) risk, yet the mechanisms underlying these relationships is less well understood. Few studies have examined trauma exposure and its relation to autonomic influence over cardiac function, a potential pathway linking trauma exposure to CVD risk. Investigating autonomic influence over cardiac function during both wake and sleep is critical, given particular links of sleep autonomic function to cardiovascular health. Among midlife women, we tested whether trauma exposure would be related to lower high frequency heart rate v

Concepts in Neural Coaptation: Using the Facial Nerve as a Paradigm in Understanding Principles Surrounding Nerve Injury and Repair.

OBJECTIVES: Individuals with nerve transection face unpredictable outcomes, and microsurgical interventions have variable success. The facial nerve in particular is prone to traumatic transection and leads to debilitating sequelae. Surgeons have used multiple modalities of enhancing nerve regeneration and restoring premorbid functionality. The success of nerve regeneration is predicated on multiple physiologic factors. This article sought to collate the literature on factors influencing nerve damage and repair, using the facial nerve as a paradigm. As such, facial reanimation will also be brie

Musculocutaneous neuropathy after anaesthesia for thyroid surgery: a case report.

Postoperative isolated injury of the musculocutaneous nerve is a rare disorder and complication. Reported cases are claimed to present with loss of biceps and brachialis power without neuropathic pain. When injury occurs to one of the terminal branches of the brachial plexus, the lateral cutaneous nerve of the forearm, pain is the major symptom and it typically radiates along the radial aspect of the forearm. In the literature, isolated lesions of the musculocutaneous nerve have been attributed to repeated microtrauma, indirect trauma or direct trauma to the nerve. It may also occur due to str

Importance of the placenta and cord in the defense of neurologically impaired infant claims.

Verdicts in favor of the defense were delivered in all 12 cases tried to a conclusion in which the placenta and/or cord was available for examination and was found to be abnormal or diseased and in which testimony was provided by a placental pathologist. The St Paul (Minn) Fire and Marine Insurance Company recommends that placenta/cord specimens be retained in cases where the mother is at high risk, the fetus/newborn has certain high-risk characteristics, and/or the placenta appears to be grossly abnormal. If a claim is made against the health care providers, the specimens can subsequently be

Disclosure of traumas and psychosomatic processes.

Results from a series of studies are summarized in support of a general theory of inhibition and psychosomatics. According to this view, to inhibit thoughts, feelings, or behaviors is associated with physiological work. In the short term, inhibition results in increased autonomic nervous system activity. Over time, inhibition serves as a cumulative stressor that increases the probability of psychosomatic disease. Actively avoiding thoughts and feelings surrounding a trauma and/or not discussing a trauma is a particularly insidious form of inhibition. The results from recent surveys and experim

Electric shock, Part II: Nature and mechanisms of injury.

Virtually every part of the body can be injured by electric current. The extent of injury to any given tissue will depend on many factors, including the nature of the tissue and the amount and duration of the electric current. In addition, cardiac and respiratory arrest can be induced by a number of mechanisms with little or no immediate tissue damage. Burns can be caused by the heating of tissue by electric current and by other mechanisms. Secondary trauma may result from falls, explosions, and other events initiated by electric forces. Nervous tissue has the least resistance to current flow

Impact of hypnotic safety on disorders of gut-brain interaction: A pilot study.

Functional gastrointestinal disorders, i.e., abdominal conditions without identifiable structural etiologies, are seen frequently in primary care and specialty practices. As subtle physiological processes have been identified as potential contributing factors to these functional disorders, these disorders have been recently relabeled, Disorders of Gut-Brain Interaction (DGBI). Moreover, some of these processes, e.g., sympathetic nervous system activity and inflammation, are being increasingly related to psychosocial factors such as situational stress and histories of trauma, abuse, and neglect

An unusual etiology of post-traumatic meningitis.

Clostridium perfringens and related species are well-known culprits of post-traumatic soft tissue and bone infection. However, central nervous system involvement with these organisms spontaneously or subsequent to trauma is unusual. The authors reported a case of post-traumatic meningitis in which both C perfringens and Enterococcus faecium were isolated.

The pathogenesis of peripheral aneurysms of the central nervous system: a subject review from the AFIP.

Most central nervous system aneurysms occur around the circle of Willis, and are congenital or arteriosclerotic in origin when in that location. Peripherally located aneurysms are either idiopathic or secondary to infection, tumor embolus (from choriocarcinoma and cardiac myxoma), Moyamoya disease, or trauma. The pathophysiologic features of these aneurysms are discussed.

Beyond the Breath: Improving Interoceptive Awareness & Autonomic Regulation through Trauma-Informed Biofeedback for Veterans.

Posttraumatic stress disorder (PTSD) may be conceptualized as a condition of autonomic nervous system (ANS) imbalance and hyperreactivity, as multiple symptoms are indicative of ANS imbalance (e.g., hypervigilance, exaggerated startle response, etc.). Several meta-analyses found hyperreactivity of the sympathetic nervous system (SNS), reduced basal parasympathetic nervous system (PNS) activity, and exaggerated PNS withdrawal. Additionally, evidence-based psychotherapies (EBPs) for PTSD do not always improve this ANS imbalance, nor the negative health consequences associated with it (i.e., hype

The neuro-glial coagulonome: the thrombin receptor and coagulation pathways as major players in neurological diseases.

The neuro-glial interface extends far beyond mechanical support alone and includes interactions throu-gh coagulation cascade proteins. Here, we systematically review the evidence indicating that synaptic and node of Ranvier glia cell components modulate synaptic transmission and axonal conduction by a coagulation cascade protein system, leading us to propose the concept of the neuro-glial coagulonome. In the peripheral nervous system, the main thrombin receptor protease activated receptor 1 (PAR1) is located on the Schwann microvilli at the node of Ranvier and at the neuromuscular junction. PA

Intramuscular cysticercosis--the solitary reaper.

Occurrence of solitary intramuscular cysticercosis without involvement of the central nervous system is rare. We report a case of solitary cysticercosis of the brachioradialis muscle in a 35-year-old woman who presented with discomfort and pain in the right elbow and arm after trivial trauma. There were no systemic or neurological features.

Horner's syndrome following internal jugular vein cannulation.

We present two cases of Horner's syndrome occurring following uncomplicated internal jugular venous cannulation. An awareness of this potential complication will reduce confusion over the aetiology of anisocoria in critically ill patients. This consideration is important, since lesions in the central nervous system or carotid dissection following trauma might otherwise be suspected.
