Trauma history screening in a community mental health center.

OBJECTIVE: This study assessed the lifetime prevalence of traumatic events among consumers of a community mental health center by using a brief trauma screening instrument. This study also examined the relationship between trauma exposure and physical and mental health sequelae and determined whether the routine administration of a trauma screening measure at intake would result in increased diagnoses of posttraumatic stress disorder (PTSD) and in changes in treatment planning in a practice setting. METHODS: A 13-item self-report trauma screening instrument, a shortened version of the Trauma

Representativeness of trauma center registries for farm injury surveillance.

Trauma center registries are the foundation for many surveillance systems that attempt to define the frequency and spectrum of various types of injuries. To assess the representativeness of trauma center-based farm injury surveillance, we evaluated data for 1986-1991 from the Marshfield Clinic/St Joseph's Hospital, a major trauma center located in Central Wisconsin. We compared the pattern of farm injuries seen in residents of the Marshfield Epidemiologic Surveillance Area (MESA), a geographically defined, population-based surveillance area, with those from outside MESA, a nonpopulation-based

[Hip tumor prosthesis--indication in a trauma center and results].

This is a report about six cases of replacement of the proximal femur by a cementless special prosthesis in modular system done in the Unfallkrankenhaus Wien-Meidling. One of the patients suffered by a lymphoma, three others had fractures caused by bone metastasis. One was a pertrochanteric fracture complicated by osteoporosis, and finally one fracture of the femur preceded by an insufficient implantation of a semitotal hip replacement. The hipsocket was supplied four times by a cementfree Polyäthylensocket, in one case a CLW-ring was implanted. Investigation showed that after a little more th

A Pragmatic Approach to Pancreatic Trauma: A Single-Center Experience From a Tertiary Care Center.

Introduction Pancreatic trauma is rare and is usually associated with adjacent organ and vascular injuries, which adds to the high morbidity and mortality. In the American Association for the Surgery of Trauma (AAST) pancreatic trauma (PT) grading system, the higher grades are a composite of less and more severe extents of injuries. We hereby present an observational study of PT with management based on an indigenous algorithmic approach. Our protocol incorporating both the extent of disruption of the main pancreatic duct (MPD) and its amenability to interventions (endoscopic, radiological, or

Typologies of hurricane trauma exposure: a person centered approach.

PURPOSE: Hurricane María (2017) and the subsequent large-scale migration of Puerto Ricans serve as a stark example of how extreme weather events, population displacement, and equity concerns impact the health of vulnerable populations. The aim of the present study is to examine the classes of hurricane-related experiences among Puerto Rican "María migrants" and to assess the relationship between different types of hurricane exposure and posttraumatic stress. METHODS: The present study was conducted with 319 Puerto Rican Hurricane María migrants on the U.S. mainland. Using a person-centered ap

Family-Centered Care for Children with Abusive Head Trauma.

Family adaptation to severe neurologic impairment often begins in hospital settings, where the medical care team begins to orient caregivers to alterations in the child's neurologic status. For children with abusive head trauma, there are added social dimensions that affect family adaptation throughout the inpatient stay and beyond. Principles of family-centered care are an important cornerstone of pediatric practice, yet family members whose children have severe neurologic impairment may struggle to adapt in outpatient and multidisciplinary settings, where they must assume a more active leade

Blast injuries: The experience of a level 1 trauma center.

BACKGROUND: There is a global surge in blast injuries, which are associated with high morbidity and mortality. To our knowledge, there are no guidelines for the management of blast injuries in the trauma bay. METHODS: This single-center retrospective cohort study utilized data on all patients admitted to our emergency department (ED) with terror- or combat-related injuries between October 7, 2023 (Gaza Iron Swords War onset) and February 4, 2024. The primary outcome was trauma severity indicated by either an injury severity score (ISS) >15 and/or need for acute care. We also analyzed the mech

Characteristics of traumatic major haemorrhage in a tertiary trauma center.

BACKGROUND: Major traumatic haemorrhage is potentially preventable with rapid haemorrhage control and improved resuscitation techniques. Although advances in prehospital trauma management, haemorrhage is still associated with high mortality. The aim of this study was to use a recent pragmatic transfusion-based definition of major bleeding to characterize patients at risk of major bleeding and associated outcomes in this cohort after trauma. METHODS: This was a retrospective cohort study including all trauma patients (n = 7020) admitted to a tertiary trauma center from January 2015 to June 202

Analysis of diagnoses of chest injuries in a Nigerian trauma center.

BACKGROUND: Chest injury is one of the most common types of trauma-related injuries accounting for about 10% of trauma admissions in emergency services. The injuries may be immediate, life-threatening, or potentially life-threatening based on diagnosis after initial assessment. This study is intended to analyze the pattern of diagnosis found in chest-injured patients in a trauma center of a developing country. METHODS: This is a retrospective study carried out in the trauma center of the National Hospital Abuja, Nigeria, between January 2015 and December 2017, as an analysis of the existing t

Reliability adjustment: a necessity for trauma center ranking and benchmarking.

BACKGROUND: Currently, trauma center quality benchmarking is based on risk adjusted observed-expected (O/E) mortality ratios. However, failure to account for number of patients has been recently shown to produce unreliable mortality estimates, especially for low-volume centers. This study explores the effect of reliability adjustment (RA), a statistical technique developed to eliminate bias introduced by low volume on risk-adjusted trauma center benchmarking. METHODS: Analysis of the National Trauma Data Bank 2010 was performed. Patients 16 years or older with blunt or penetrating trauma and

Pattern of mandibular fractures in an urban major trauma center.

PURPOSE: Many studies focusing on the pattern of presentation of mandibular fractures are inconclusive. This is largely due to a multifactorial involvement of associated factors and the role of anatomic factors. PATIENTS AND METHODS: We reviewed a 10-year retrospective analysis of the pattern of presentation of mandibular fractures at an urban level I trauma center in Washington, DC, and the significance of the variables in association with the fractures. These include mechanism of action, age group, illicit drug use, seasonal variation, and the incidence of multiple fracture sites. We also l

The role of a substance abuse consultation team in a trauma center.

OBJECTIVE: Trauma is a leading cause of morbidity and mortality in the U.S., and substance abuse makes an enormous contribution to this problem as well as to the increased cost of health care. A substance abuse consultation (SAC) team was developed to evaluate and intervene with trauma victims who are suspected of having problems with alcohol and/or other drugs. This study was undertaken to determine the effectiveness of this service. METHOD: This study is a retrospective review of 100 consecutive trauma admissions who were felt to be at high risk for substance abuse and were referred to the

Nationwide Evaluation of the Validity of the Trauma and Injury Severity Score Method in Korean Regional Trauma Centers Using Multi-Institutional Large-Scale Data.

BACKGROUND: The Trauma and Injury Severity Score (TRISS) method is a widely used tool for assessing patient severity and predicting survival probability in trauma care. However, its accuracy and applicability in the Korean context, particularly for neurotrauma patients, have not been thoroughly validated yet. Previous studies at a single institution have identified significant discrepancies between TRISS predictions and actual outcomes, particularly in severe neurotrauma cases. This study aimed to evaluate the accuracy of the TRISS method on a national scale using data from multiple regional t

Contemporary management of rectal injuries at Level I trauma centers: The results of an American Association for the Surgery of Trauma multi-institutional study.

INTRODUCTION: Rectal injuries have been historically treated with a combination of modalities including direct repair, resection, proximal diversion, presacral drainage, and distal rectal washout. We hypothesized that intraperitoneal rectal injuries may be selectively managed without diversion and the addition of distal rectal washout and presacral drainage in the management of extraperitoneal injuries are not beneficial. METHODS: This is an American Association for the Surgery of Trauma multi-institutional retrospective study from 2004 to 2015 of all patients who sustained a traumatic rectal

The implications of alcohol intoxication and the Uniform Policy Provision Law on trauma centers; a national trauma data bank analysis of minimally injured patients.

BACKGROUND: Alcohol intoxication may confound the initial assessment of trauma patients, resulting in increased use of diagnostic and therapeutic procedures, thereby increasing hospital costs. The Uniform Policy Provision Law (UPPL) exists in many states and allows insurance companies to deny payment for medical treatment for alcohol-related injuries. If intoxication increases resource utilization, these denials compound the financial burden of alcohol use on trauma centers. We hypothesized that patients injured while under the influence of alcohol require more diagnostic tests, procedures, an
