Early modulation of Angiopoietin-2 plasma levels predicts benefit from regorafenib in patients with metastatic colorectal cancer.

BACKGROUND: No biomarkers are currently available to predict the efficacy of trifluridine/tipiracil (FTD/TPI) in chemorefractory metastatic colorectal cancer. The multicohort REGOLAND study aims at exploring and validating circulating markers potentially able to predict benefit from regorafenib in this setting. MATERIAL AND METHODS: In the retrospective 'regorafenib exploratory cohort', including 105 patients treated with regorafenib, baseline (d1) plasma levels of angiogenesis-related biomarkers and their early modulation after 15 days (d15) of treatment were investigated for correlation wit

Viral Keratitis, Surgical Intervention in Viral Keratitis, Challenges in Diagnosis and Treatment of Viral Keratitis, HSV, HZV.

Viral keratitis is a significant cause of ocular morbidity and visual impairment worldwide. In recent years, there has been a growing understanding of the pathogenesis, clinical manifestations, and diagnostic modalities for viral keratitis. The most common viral pathogens associated with this condition are adenovirus, herpes simplex (HSV), and varicella-zoster virus (VZV). However, emerging viruses such as cytomegalovirus (CMV), Epstein-Barr virus (EBV), and Vaccinia virus can also cause keratitis. Non-surgical interventions are the mainstay of treatment for viral keratitis. Antiviral agents s

Late-line Systemic Therapy in Metastatic Colorectal Cancer: A Narrative Review and Australian Expert Perspective.

Metastatic colorectal cancer (mCRC) remains a major clinical challenge, particularly for patients with disease progression after multiple lines of therapy. Advances in molecular profiling have transformed the therapeutic landscape, enabling personalized treatment selection and expanding third- and later-line options. Targeted therapies such as BRAF inhibitors, anti-HER2 agents, and KRASG12C inhibitors are utilized in patients whose tumors harbor specific molecular alterations, with their use increasingly being explored across various lines of therapy. Agents such as trifluridine/tipiracil, reg

Managing refractory herpes simplex keratitis.

Herpes simplex keratitis (HSK) caused by herpes simplex virus type 1 (HSV-1) is a major cause of infectious corneal blindness in developed countries. The disease reflects a variable combination of viral replication, immune-mediated inflammation and neurotrophic dysfunction, whose respective contributions determine both the clinical presentation and therapeutic strategy. Standard management relies on antiviral agents to suppress viral replication, corticosteroids to control immune stromal involvement and ocular surface optimisation to address chronic inflammation and neurotrophic impairment. Efficacy and safety of TAS-102 followed by regorafenib or reverse sequence in later-line treatment of metastatic colorectal cancer: a systematic review and meta-analysis.

BACKGROUND: Trifluridine/tipiracil (TAS-102) and regorafenib are approved and widely utilized as later-line treatment options for patients with metastatic colorectal cancer (mCRC). However, the optimal sequence between these agents remains unclear. We conducted a systematic review and meta-analysis to compare the efficacy and safety of two treatment sequences: TAS-102 followed by regorafenib (TR) versus regorafenib followed by TAS-102 (RT) in patients with mCRC who had previously failed chemotherapy. METHODS: This study was registered with PROSPERO (CRD42024622437). We systematically searched

Multikinase inhibitors in refractory metastatic colorectal cancer: an optimal sequence?

Despite major advances in management strategies, metastatic colorectal cancer remains an important clinical challenge because most patients experience progression after standard first- and secondline treatments. In the setting of refractory disease, defined as disease that progresses after 2 or more lines of treatment, the therapeutic landscape is growing. Options include regorafenib, trifluridine plus tipiracil (FTD/TPI) with or without bevacizumab, and fruquintinib, all of which received approval from the US Food and Drug Administration after showing modest survival benefits in phase 3 trial

Recent Advances in the Prevention and Management of Monkeypox Viral Infection in Humans.

BACKGROUND: There have been several neglected infectious pathogens that have reemerged in the last few decades, including the monkeypox virus, a virus from the orthopoxviral genus that causes monkeypox and is transmitted between animals and humans. The human monkeypox outbreak has spread to several different countries. Because of the outbreak's unusually high case count and lack of connections to endemic nations, there are concerns that the monkeypox transmission pattern may have changed. OBJECTIVE: The current study aimed to provide recent advancements in the prevention and management of the

Treatment decisions in metastatic colorectal cancer - Beyond first and second line combination therapies.

Median overall survival (OS) of patients with metastatic colorectal cancer (mCRC) has reached up to 30months in recent clinical trials of first line therapies. Following disease progression after the standard in both, 1st and 2nd line, combination chemotherapy with monoclonal antibodies, many patients maintain a good performance status and a significant proportion is motivated to undergo further therapy. Choices of treatment beyond the second line setting for mCRC are therefore becoming increasingly important. New options have entered the therapeutic field recently: Regorafenib is a multikinas

Combination of TAS-102 and bevacizumab as third-line treatment for metastatic colorectal cancer: TAS-CC3 study.

BACKGROUND: TAS-102 improved the overall survival of metastatic colorectal cancer (CRC) patients with a median progression-free survival (PFS) in the RECOURSE trial. Subsequently, the combination of TAS-102 and bevacizumab was shown to extend the median PFS (C-TASK FORCE study). However, the study included patients who received second- and third-line treatment. Our study exclusively examined patients receiving this combination as a third-line treatment to investigate the clinical impact beyond cytotoxic doublets.
