Navigating HIV status disclosure: Adolescents' privacy management strategies in Rural South Africa.

We examined how adolescents living with HIV in rural KwaZulu-Natal, South Africa, navigate privacy boundaries in disclosure-related issues. We conducted small group discussions with 31 adolescents (aged 16-19 years) recruited from three HIV clinics. Reflexive thematic data analysis was conducted through the lens of communication privacy management theory. Three major themes emerged: (1) ownership and control of private information, in which adolescents demonstrated varying levels of autonomy over their HIV status information, often developing strategies to manage medication discreetly; (2) man

Implications of HIV status disclosure: Exploring the experiences of pregnant and postpartum women in Southwestern Uganda.

Disclosure of HIV status is critical for HIV prevention and control. However, although the benefits of HIV disclosure have received extensive documentation, the same cannot be said for its drawbacks. In the current study, we used a descriptive qualitative study design to highlight the implications of disclosure among 30 pregnant and postpartum women living with HIV in Uganda. We conducted a qualitative content analysis and found that healthcare professionals responded positively and supportively when HIV-positive pregnant and postpartum women disclosed their status, and that individuals who di

Sexual Abuse Disclosure Mediates the Effect of an Abuse Prevention Program on Substantiation.

Although abuse prevention programs have proliferated, little research has explored the direct effects of such programs on actual child sexual abuse disclosure rates, and no research has explored the effects of such programs on child sexual abuse substantiation. Employing a quasi-experimental design, the present research reflects an exploration of the effects of exposure to the Think First and Stay Safe™ abuse prevention program on abuse disclosure rates of 319 children who underwent a child forensic interview within 2015-2018 in a Midwestern child advocacy center. Disclosure of HIV Status Beyond Sexual Partners by People Living with HIV in France: A Call for Help? Results from the National Cross-Sectional Survey ANRS-VESPA2.

HIV status disclosure remains a complex issue for most people living with HIV (PLWH). We analyzed PLWH disclosure behaviors in France, where treatment is free and where the social image of HIV has improved in the general population. Analyses focused on disclosure to the social network excluding sexual partners (close family, other relatives, friends, colleagues). The study sample comprised 3016 participants from the nationally representative survey ANRS-VESPA2. Three PLWH clusters were identified using hierarchical classification ("high disclosure level": 28.2 %, "medium disclosure level": 27.

Practices and responses that help or hinder disclosures of child sexual abuse: Perspectives from victim survivors and practitioners.

BACKGROUND: Child sexual abuse (CSA) is a concerning social and public health problem, however there is limited knowledge around the practices and responses that best support safe disclosures of CSA from the perspective of victim survivors and practitioners. OBJECTIVE: The study explored practices and responses that enable or hinder disclosures of CSA, in order to enhance service system responses. PARTICIPANTS: Fifty-one adult victims of CSA and twenty-three practitioners supporting victim survivors in Australia. METHODS: Online in-depth interviews with survivors and Yarning sessions with p

Disclosure and Psychological Well-Being of Sexually Abused Adolescents in Hong Kong.

The role of disclosure on psychological well-being of adolescents with child sexual abuse experience was investigated in a subsample of 74 disclosers among 800 adolescents recruited in the community in Hong Kong. The results supported that CSA experiences have differential impact on adolescents' psychological well-being. Family characteristics of the disclosers accounted for only a small amount of the variance in an array of psychological well-being measures. CSA characteristics were robust predictors of disclosers' sexual eroticism and externalizing behavioral symptoms. Disclosers' cognitive

Breaking bad news and the cost of caring on physicians: a systematic scoping review.

BACKGROUND: Caring for dying patients and their distressed families can be traumatic. For physicians, these encounters shape the way they see, act and feel as professionals, influencing the inculcation and display of their professional expectations and values-otherwise known as professional identity formation (PIF). When inadequately supported, such changes in PIF can impact patient care, family support and teamwork. With recurrent, brief yet emotionally charged interactions with patients and families especially likely to predispose to moral distress, compassion fatigue and burnout, we propose

Caregiver disclosure of common early childhood pediatric urologic surgeries.

BACKGROUND: Certain pediatric urologic diagnoses can have serious long-term adverse health outcomes. As a result, it is important for a child to be aware of their diagnosis and a prior surgery. When children have surgery prior to the age of memory formation, it is incumbent upon their caregiver to disclose this surgery. When and how to disclose this information and even if this occurs, is not clear. OBJECTIVE: We developed a survey to assess caregiver plans to disclose early childhood pediatric urologic surgery and evaluate for predictors of disclosure and resources needed. METHODS: A questi

Severe early-onset fetal growth restriction: What do we tell the prospective parents?

Fetal growth restriction (FGR) is a common complication of pregnancy, associated with higher risk of perinatal mortality and adverse health and developmental outcomes for surviving infants. True FGR relates to a pathological restriction of fetal growth resulting from complex interactions between maternal, placental, fetal, and environmental factors. Early-onset FGR (onset <32 weeks' gestation) is often first suspected at routine mid-trimester sonographic assessment of fetal morphology, or identified as part of the placental syndrome, commonly maternal pre-eclampsia. Prenatal investigations may

Disclosure of mental illness to prospective employers: Clinical, psychosocial, and work correlates in persons receiving supported employment.

Deciding whether to disclose one's psychiatric disorder to a prospective employer is a complex decision for people with severe mental illness seeking to return to work, with potential advantages and disadvantages. The present study examined the rates, patterns, and correlates of disclosure in 51 participants (74.5% schizophrenia or schizoaffective disorder) receiving high fidelity Individual Placement Support (IPS) who obtained competitive work over a two-year study period. Most participants (64.7%) disclosed their psychiatric disorder in their first job, and there was a tendency for those wit

Plagued by doubt and viral misinformation: the need for evidence-based use of historical disease images.

The digitisation of historical disease images and their widespread availability on the internet have been a boon to education and research, but with unintended consequences, including the misrepresentation of infectious diseases in the past and the viral spread of misinformation. Many medieval images containing scenes of infectious disease come from non-medical sources and are not meant to convey any medical meaning. Erroneous modern captions have led to the publication of several historical images labelled as depictions of the plague, although artistic and textual evidence shows that they are

Disclosures of sexual abuse: what do you do next?

BACKGROUND: Sexual abuse, especially in childhood, often has severe and long-lasting clinical consequences, both physical and psychological. OBJECTIVE: This article outlines an approach to use when patients disclose a history of sexual abuse, and provides some resources to call on in those circumstances. DISCUSSION: This article addresses questions one might ask to uncover a history of sexual abuse, possible responses to disclosures of sexual abuse, and documentation of these disclosures.

"Notify your partners--it's the law": HIV providers and mandatory disclosure.

HIV care providers in the United States must counsel clients about disclosure to sexual partners and report anyone who is suspected of noncompliance. This study compared provider attitudes and practices in relation to counseling clients about mandatory disclosure in North Carolina and Alabama, the 2 states with similar HIV epidemiology but different laws for HIV control. Personal interviews were conducted with 20 providers in each state (n = 40). The results were analyzed in a qualitative, cross-comparison method to identify patterns of convergence or difference. Providers in both states belie

Expert testimony influences juror decisions in criminal trials involving recovered memories of childhood sexual abuse.

We examined the impact of expert witness orientation (researcher or clinical practitioner) and type of testimony (testimony for the prosecution, defense, both prosecution and defense, or no testimony) on mock jurors' decisions in a sexual abuse trial. Participants acted as mock jurors on a sexual abuse criminal trial based on recovered memory that included expert witness testimony. Results showed that expert witness testimony provided by a researcher did not impact mock jurors' guilt ratings any differently than the expert witness testimony provided by a clinical practitioner. However, type of

Donor conception, secrecy and the search for information.

Donor conception has historically been shrouded in secrecy. Such secrecy has been underpinned by social views and legal issues concemrning the adults involved in the process--the donor, the recipient parent(s), and, at times, the doctor. However, there is increasing recognition of the need to focus upon donor-conceived people's interests and rights to have identifying and non-identifying information about their donors. This editorial examines issues raised in relation to information release, while also introducing some of the arguments presented by other authors in this Special Issue of the JL

Consent in surgery.

A review of consent for surgery is timely. As the length of surgeons' training diminishes, despite the increasing interest in the content of the surgical curriculum, the law governing the process of gaining consent has been given scant attention. The advent of non-medically qualified surgical practitioners raises questions about the breadth of knowledge that is required to ensure that valid consent is obtained. Consent is as fundamental as any other basic principle on which surgical practice relies, and its use in patient care is a clinical skill. The 'traditional' approach to consent containe

Family secrecy: a comparative study of juvenile sex offenders and youth with conduct disorders.

The reported research was designed to compare adjudicated male juvenile sexual offenders and youth with conduct disorders on five aspects of family secrecy and deception. Twenty-nine male juvenile sex offenders and 32 comparison youth from three child welfare agencies in New York State participated in the study. Research assistants, blind to the hypotheses of the study and status of the youth, coded agency records for five variables identified a priori as a basis of comparison. Analyses revealed that the two groups were different on three of the five and on the total number of items scored. Fa
