Postmortem tryptase as a diagnostic marker in fatal anaphylaxis: A systematic review and meta-analysis.

Fatal anaphylaxis remains a diagnostically challenging condition in forensic practice, particularly in the postmortem setting where clinical information is often limited. Postmortem serum tryptase has been widely proposed as a marker of mast cell activation, but its quantitative significance and diagnostic performance have not been consistently defined. This systematic review and meta-analysis aimed to evaluate both the magnitude of tryptase elevation and its diagnostic accuracy in fatal anaphylaxis. Seven studies were included, comprising 105 anaphylactic deaths and 449 non-anaphylactic contr

Computational modeling of mast cell tryptase family informs selective inhibitor development.

Mast cell tryptases, a family of serine proteases involved in inflammatory responses and cancer development, present challenges in structural characterization and inhibitor development. We employed state-of-the-art protein structure prediction algorithms to model the three-dimensional structures of tryptases α, β, δ, γ, and ε with high accuracy. Computational docking identified potential substrates and inhibitors, suggesting overlapping yet distinct activities. Tryptases β, δ, and ε were predicted to act on phenolic compounds, with β and ε additionally hydrolyzing cyanides. Tryptase δ may poss

Prevalence and Variability of Allergen-Specific Immunoglobulin E in Patients with Elevated Tryptase Levels.

BACKGROUND In this paper we report on the link between increased tryptase levels and prevalence of allergen-specific immunoglobulin E (IgE) in patients with anaphylaxis. The main aim of the study was to assess the corelation between elevated tryptase levels and allergen-specific immunoglobulin E (IgE) in anaphylactic reactions, mastocytosis, and other types of allergic reactions. MATERIAL AND METHODS We enrolled 60 adult patients, aged 18-68 years (mean age 45±8 years). The subjects were divided into 3 groups (20 patients in each group) according to the serum tryptase changes: group 1 consiste

Elevated Basal Serum Tryptase: Disease Distribution and Variability in a Regional Health System.

BACKGROUND: Hereditary-alpha tryptasemia (HαT) is the most common etiology for elevated basal serum tryptase (BST). However, the utility of tryptase genotyping of individuals with elevated BST in general clinical practice remains undefined. Moreover, studies showing associations between elevated BST and chronic kidney disease (CKD), myelodysplastic syndrome (MDS), rheumatoid arthritis, or eosinophilic esophagitis did not include tryptase genotyping. OBJECTIVE: To determine the utility of tryptase genotyping among individuals with moderate elevations in BST at a regional health system. METHOD

Levels of haemolysis have no effect on femoral vein post-mortem tryptase levels.

Haemolysis is reported to be an artefact that may alter post-mortem tryptase levels. However, previous studies did not sample peripheral blood using newly standardised methods. Recent studies have shown that some previously recognised peri- and post-mortem confounders can be muted by careful sample collection with first clamping and then sampling the femoral vein. This prospective study investigated the relationship between the degree of haemolysis of the blood samples and femoral vein post-mortem tryptase levels when sampled using this recommended method. Seventy consecutive post-mortem trypt

Involvement of tryptase-positive mast cells and angiogenesis in the growth of inflammatory odontogenic cysts.

Inflammatory periapical lesions are characterized by infiltration of different immune cell types, the functions of which depend on an effective vascular network. This study aimed to evaluate the mast cells density (MCD) in inflamatory odontogenic cysts capsules concerning microvascular density (MVD), microvascular area (MVA), and microvascular perimeter (MVP), and correlate such findings with the type of lesion, intensity of the inflammatory infiltrate, and thickness of the epithelial lining. Twenty inflamatory dentigerous cysts (IDCs), twenty radicular cysts (RCs), and twenty residual radicul

Serum basal tryptase levels in healthy children: correlation between age and gender.

Tryptase is used as a marker in many clinical conditions such as mast cell activation and systemic anaphylaxis. Normal levels of the serum basal tryptase (sBT) are determined in adulthood; however, data about nonallergic healthy children is limited. This study was designed to determine the normal sBT levels in healthy children. Total sBT levels were measured in 113 sera from healthy nonallergic children, for routine follow-up or diagnosis of illnesses that are not known to induce changes in serum tryptase levels. One hundred thirteen children aged 0.16-9.91 years (male/female subjects, 68/45 [

Tryptase activation of immortalized human urothelial cell mitogen-activated protein kinase.

The pathogenesis of interstitial cystitis/painful bladder syndrome (IC/PBS) is multifactorial, but likely involves urothelial cell dysfunction and mast cell accumulation in the bladder wall. Activated mast cells in the bladder wall release several inflammatory mediators, including histamine and tryptase. We determined whether mitogen-activated protein (MAP) kinases are activated in response to tryptase stimulation of urothelial cells derived from human normal and IC/PBS bladders. Tryptase stimulation of normal urothelial cells resulted in a 2.5-fold increase in extracellular signal regulated k

Immunohistochemical evaluation of angiogenesis and tryptase-positive mast cell infiltration in periapical lesions.

INTRODUCTION: Several studies have linked mast cells (MCs) with angiogenesis. The aim of this study was to correlate angiogenesis with MCs in radicular cyst (RC) and periapical granuloma (PG) cases. METHODS: Forty-eight samples of periapical lesions, diagnosed as RC (n = 24) and PG (n = 24), were included. The microvessel density and microvessel area measured through the immunoexpression of CD105 and CD34 and the MC density measured through the immunoexpression of tryptase were performed. RESULTS: MCs were detected in all RCs and PGs (P = .888), mainly in perivascular location and within fib

Subjects with diarrhea-predominant IBS have increased rectal permeability responsive to tryptase.

BACKGROUND AND AIMS: Patients with diarrhea-predominant irritable bowel syndrome (IBS-D) appear to have increased intestinal permeability; it has been suggested that activation of protease-activated receptor-2 (PAR-2) receptors is responsible for this alteration. The aims of this study are to evaluate (1) if rectal (large bowel) permeability is increased in IBS-D and (2) if tryptase plays a critical role in the altered permeability. METHODS: Rectal biopsies from 20 patients with IBS-D and 30 subjects without the condition (normal controls) were assessed for macromolecular permeability using h

A quantitative and morphometric study of tryptase-positive mast cells in cutaneous leprosy lesions.

While mast cells are known to induce differences in the matrix structures, microvascular patterns, and immune responses in a number of diseases, the possible role of mast cells in these same processes over the spectrum of leprosy has not yet been investigated. Thus, ascertaining the possible influence of mast cells in the outcome of the anti-leprosy response to Mycobacterium leprae is of major importance. In this study, 51 cutaneous biopsies of leprosy patients were stained with anti-tryptase antibody in order to quantify mast cells in leprosy lesions and compare the number and size of these c

cDNA cloning and substrate specificity of equine tryptase, a possible mediator in equine heaves.

BACKGROUND: Mast cell mediators are believed to play a central role in inflammatory lung disorders such as human allergic and occupational asthma. Equine heaves is characterized by reversible neutrophilic airway inflammation and airway obstruction, primarily due to bronchospasm and mucus hypersecretion, following exposure of susceptible horses to organic stable dusts. As such, heaves shares many similarities with human occupational dust-induced asthma and therefore it is proposed that mast cells may also be implicated in the pathogenesis of heaves. Tryptase, a mast cell-specific proteinase, ca

Modulation of tryptase release from human tonsil mast cells by protease inhibitors.

AIM: To examine the influence of protease inhibitors on tryptase release, and as a comparison the influence of the inhibitors on histamine secretion was assessed. METHODS: Enzymatically dispersed cells from human tonsil were challenged with anti-IgE or calcium ionophore A23187 (CI) in the absence or presence of the tryptase and chymase inhibitors, and tryptase and histamine release was determined. RESULTS: IgE-dependent tryptase release from dispersed tonsil mast cells was inhibited by a maximum of approximately 35.5% and 35.7% by N-alpha-p-tosyl-L-lysine chloromethyl ketone (TLCK) and N-tos

Serum tryptase levels in sudden infant death syndrome in forensic autopsy cases.

An elevated serum tryptase concentration is considered to be a specific marker for systemic mast-cell activation, a central feature of anaphylaxis, which has been observed in some cases of sudden infant death syndrome (SIDS). However, it is still unclear whether anaphylaxis is involved in the etiology for SIDS. In the present study, we measured serum tryptase levels in 21 infants with SIDS, and 14 control infants from forensic autopsy cases by Uni-CAP TRYPTASE Fluoroenzyme immunoassay system, which detects both alpha- and beta-tryptase. The assay did not show any significant elevation of trypt

A fatal case of amniotic fluid embolism with elevation of serum mast cell tryptase.

A case of a 40-year-old female who died of amniotic fluid embolism is presented. This case showed typical histological findings of this syndrome. Postmortem serum of this case showed an elevated tryptase level (67.2ng/ml, normal levels <10ng/ml). Tryptase is a neutral protease of mast cells, and an important indicator of mast cell activation and degranulation. Thus, mast cell activation, a central feature of anaphylaxis, may have been involved in the pathogenetic mechanism of this case.

Co-localization of matrix metalloproteinase-1 and mast cell tryptase in the human uterus.

The endometrium displays characteristic cyclical changes involving proliferation and differentiation. The differentiation that takes place requires major tissue remodelling involving the matrix metalloproteinase (MMP) family as key enzymes in this process. Mast cells, containing the tryptase and chymase enzymes that are capable of stimulating the MMP cascade, have been identified in the endometrium, but their role is still unclear. In this study, we observed that the majority of mast cells in the uterus reside in the myometrium and that they co-express mast cell tryptase and MMP-1 in the same

Development of a new, more sensitive immunoassay for human tryptase: use in systemic anaphylaxis.

Tryptase, a neutral protease, is selectively concentrated in the secretory granules of human mast cells, and its release into the circulation serves as a clinical marker of mast cell activation. The current study describes a new, more sensitive ELISA utilizing a newly developed, mouse monoclonal IgG1 antibody for capture called B12 and capable of detecting tryptase in normal plasma and serum. The greater sensitivity of the new immunoassay results in part from a greater portion of tryptase being detected. Mean levels of tryptase in serum from normal subjects from Richmond, Virginia (4.9 ng/ml;

Elevated serum tryptase level in a case of intraoperative anaphylaxis caused by latex allergy.

We measured serial serum tryptase levels in a case of intraoperative anaphylaxis caused by allergy to latex. The serum tryptase level was elevated 7 hours after the hypotensive episode and returned to normal after recovery 4 months later. Both skin tests to latex and serum IgE to latex confirmed latex allergy in this patient. To our knowledge, this is the first report of an elevated serum tryptase level in intraoperative anaphylaxis caused by latex allergy. This observation confirms the role of mast cell degranulation in anaphylaxis caused by allergy to latex.

Substance P and vasoactive intestinal peptide degradation by mast cell tryptase and chymase.

The peptides substance P (SP) and vasoactive intestinal peptide (VIP) released from peptidergic neurons have potent effects on gland secretion and on smooth muscle tone. Because mast cells release proteases during degranulation, and are located in many of the same tissue microenvironments into which SP and VIP are released, we wished to examine whether mast cell proteases, by cleaving and thus inactivating these peptides, could modulate their effects. We used active site-titrated preparations of the two major neutral proteases of mast cell granules, tryptase and chymase, to determine the sites

Serum baseline tryptase levels across chronic kidney disease stages: A retrospective report.

BACKGROUND: Chronic kidney disease (CKD) has been associated with elevated serum baseline tryptase (sBT) levels, which may exceed the standard reference value of less than 11.5 ng/mL as kidney function declines. OBJECTIVE: This study aimed to characterize sBT levels across different CKD stages in a United States-based cohort. METHODS: We conducted a retrospective chart review of consecutive nephrology outpatients across the Mayo Clinic enterprise from January 2020 to July 2025. Patients with a confirmed CKD diagnosis (International Classification of Diseases, Revision 10, codes N18.1-N18.5)
