Clinical interventions for tungiasis (sand flea disease): a systematic review.

Tungiasis (sand flea disease) is an epidermal parasitic skin disease occurring in resource-limited communities. There is no standard treatment for tungiasis, and available treatment options are scarce. To our knowledge, this is the first systematic review aimed to assess randomised controlled trials (RCTs) investigating interventions for tungiasis. We systematically searched databases including MEDLINE (EBSCOhost), CENTRAL, CINAHL, PubMed, Web of Science, SciELO, LILACS and Embase (Scopus) for RCTs in any language, from inception of the databases until June 12, 2021. RCTs exploring preventive

Disseminated tungiasis in a 78-year-old woman from Tanzania: a case report.

BACKGROUND: Tungiasis is one of the neglected tropical diseases; it affects up to 40% of individuals living in societies with poor housing and sanitation standards. In endemic areas, Tunga infestation, which predominantly affects the periungual areas of the lower limbs in humans, is associated with considerable morbidity and poor quality of life. CASE PRESENTATION: A 78-year-old woman of African descent presented with pain, inflammation, suppuration, ulceration, and deformation of digits of all four limbs. She had a total of 1146 embedded sand fleas: 812 in lower limbs and 334 in her hands. S

Flea parasitism in wild mammals in the metropolitan region of Sorocaba, São Paulo, Brazil.

Fleas are eurixenous ectoparasites that can parasitize different species of warm-blooded animals, including humans, with the possibility of causing irritation and blood spoliation. They are vectors responsible for the transmission of numerous pathogens and have a wide geographical distribution, more frequently in warm regions. Domestic animals (dogs and cats) are preferred hosts, but parasitism can also occur in wild hosts, with a greater variety of parasitic species and strong interaction between these siphonapters and their hosts. During the period from January 2021 to June 2023, flea specim

Initial clinical trials in 1980 showed that ivermectin was remarkably effective against Onchocerca volvulus. Some 25 years after more than 50 million people are treated annually with Mectizan mainly within the framework of the African Programme for Onchocerciasis Control (APOC). This success has been possible thanks to Merck Mectizan Donation Program and to distribution through a novel strategy based on the strong involvement of endemic communities. In the last few years Mectizan has been used in combination with albendazole to control lymphatic filariasis on a large-scale basis in African cou

Dermatologic Clues to Emerging Tropical Infections: A Narrative Review for Early Recognition and Differential Diagnosis.

Tropical infections often present with characteristic dermatologic manifestations, which can serve as early indicators for diagnosis. However, these signs are frequently misidentified or underreported in emerging and outbreak settings. This narrative review critically evaluates the diagnostic utility of dermatologic findings across over 25 tropical infections, spanning viral (e.g., dengue, Zika, chikungunya, Mayaro virus, O'nyong-nyong virus), bacterial (e.g., Bacillus anthracis, Mycobacterium ulcerans, Rickettsia spp.), parasitic/helminthic (e.g., Loa loa, Ancylostoma braziliense, Tunga penet

Assessing the potentials of two local topical ointments as affordable treatment against tungiasis infestation: A self-experimentation in Igbokoda, Nigeria.

Infestations caused by penetration of Tunga penetrans (Siphonaptera: Hectopsyllidae) in cutaneous sub-layers present morbidities and resulting mortalities if surgical treatment remains the obtainable. Considering the neglected nature of this infestation and the absence of marketable product, we report an observation on the use of grounded naphthalene in kerosene ointment, and powder of Piper guineense in coconut oil ointment as affordable treatment of embedded tungiasis flea in Igbokoda. A total of 80 individuals partitioned into two groups of 40 individuals each were assigned the locally made

Tungiasis in rural Haiti: a community-based response.

Tungiasis is a disease endemic to poor communities in Latin America, the Caribbean and Africa. It is caused by the female flea, Tunga penetrans, which burrows into the skin of its host to feed while producing and extruding eggs. Consequent lesions may be painful and even crippling with damage ranging from mild erythema and swelling to necrosis. Superinfection of lesions can be serious and may result in auto-amputation or death from tetanus. We describe an outbreak of tungiasis in rural Haiti and a community-based intervention used to address it. Of 177 patients assessed, 132 (47 female, 85 mal

Reduction of tungiasis prevalence, intensity, and morbidity during a two-year long community-based tungiasis control project in a hyperendemic region in Karamoja, Uganda.

Tungiasis is a widespread and debilitating zoonotic Neglected Tropical Disease (NTD). Manual extraction of the sand fleas with non-sterile sharp instruments is the most common but unsafe treatment method in affected communities. Topical application of a dimeticone oil formula (NYDA) has previously been shown to be a safe and effective method of killing embedded sand fleas. The objective of this study was to evaluate a two-year long humanitarian One Health tungiasis control project in 17 villages in Napak district, Karamoja region, Northeastern Uganda. The community-based intervention included

Bioecology of fleas with a focus on the plague vector Xenopsylla Brasiliensis in Mandritsara district, Northern Madagascar.

The rat flea Xenopsylla brasiliensis, a known plague vector present in East and South Africa, has recently been identified in Mandritsara, Madagascar, a plague-endemic area. However, its national distribution remains poorly documented, and studies on other flea vectors and their small mammal hosts in this district are limited. We surveyed flea species, their mammalian hosts, and flea abundance, with a particular focus on X. brasiliensis, in four sites of Mandritsara District between 2021 and 2022. Small mammals were trapped for three consecutive nights per site, inside and outside houses, off-

Chronic ulcers and myasis as ports of entry for Clostridium tetani.

Evaluating tetanus immune status is not yet the usual clinical practice regarding patients with chronic ulcers or myasis. However, of 858 tetanus patients at Hospital Couto Maia (Salvador, Bahia, Brazil) aged 1 year or above, 2 had pressure ulcers and 17 had chronic ulceration of the lower limbs where these skin lesions were the ports of entry for Clostridium tetani. In these 19 cases, the following predisposing factors were described: venous insufficiency (n=6), sickle cell anemia (n=2), Hansen s disease (n=1), malnutrition (n=1), diabetes mellitus (n=1), trauma (n=1) and unknown factors (n=7

Tungiasis among five communities in south-western Trinidad, West Indies.

The prevalences of Tunga penetrans Linn. within the five townships of Granville, Fullerton, Icacos, Coromandel and Cedros in south-western Trinidad, West Indies, as determined by physical examination of all inhabitants present during the survey, were 17.0% (38/223 subjects), 15.7% (36/237), 31.4% (102/325), 17.4% (42/242) and 17.9% (50/280), respectively. The rate in Icacos was significantly higher than in the other four sites (P < 0.001). Not only were males more likely to be infected than females in all five study sites (P < 0.0007) but chigoe-flea burdens were also higher in males than in f

A Randomised, Parallel-Group Study to Compare the Efficacy of 3% Phenothrin-Containing Jigger Lotion Versus Potassium Permanganate for Treatment of Tungiasis in Vihiga County, Kenya.

Tungiasis, caused by the sand flea Tunga penetrans, results in itching and pain. Effective treatments, such as dimeticones, are often unaffordable. A 3% phenothrin lotion has shown safety and efficacy in Kenyan trials. This study compared the cure rate and safety of 3% phenothrin lotion (as the intervention) and 0.05% potassium permanganate (KMnO4; as the standard-care comparator) over 14 days. This parallel-group, three-arm, non-blinded, randomised comparative trial was conducted in Vihiga County, Kenya. Participants aged ≥2 years with ≥1 viable flea on each foot were allocated (2:1:1) to KMn

Fleas and lesions in armadillo osteoderms.

Armadillos are bitten by several species of flea. Females of the genus Tunga penetrate the epidermis and when in place are fertilised by males, after which the abdomen swells enormously to form a 'neosome'. Within the penetrans group, T. perforans, makes lesions that perforate the osteoderms within the integument to form ~3 mm diameter cavities occupied by a discoid neosome. We examined these lesions in carapace material from animals which had died in the wild to see whether we could recruit evidence as to how they may be generated, either by the insect or by the host. We studied one species w

Parasites of importance for human health in Nigerian dogs: high prevalence and limited knowledge of pet owners.

BACKGROUND: Dogs are the most common pet animals worldwide. They may harbour a wide range of parasites with zoonotic potential, thus causing a health risk to humans. In Nigeria, epidemiological knowledge on these parasites is limited. METHODS: In a community-based study, we examined 396 dogs in urban and rural areas of Ilorin (Kwara State, Central Nigeria) for ectoparasites and intestinal helminths. In addition, a questionnaire regarding knowledge and practices was applied to pet owners. RESULTS: Nine ectoparasite species belonging to four taxa and six intestinal helminth species were identi
